Abstract
Description
The invention relates to medicine, in particular to ophthalmology, and can be used in the treatment of optic atrophy of various origins.
A known method of treating a partially atrophied optic nerve (ChASN) by electrical stimulation. At the same time, the use of wire electrodes in a polymer shell is provided. The electrodes are implanted directly into the optic nerve canal under the control of an electron-optical transducer and craniography in three projections, after which electrophysiological observations are carried out, the least damaged portion of the optic nerve is detected, it is advanced to it and the electrode is fixed, then electrostimulation is performed. The implantation technique of the electrodes in the known method is quite traumatic and requires the use of special equipment. The manufacture of electrodes involves applying a polymer coating to the wire, which can only be done at a specialized enterprise.
The essence of this invention lies in the fact that in the treatment of ChASN, the juxtabulbar segment of the optic nerve is electrostimulated, for which two wire electrodes in the sheath of the hemostatic collagen sponge are implanted diametrically opposite at the exit of the optic nerve from the eyeball, after which electrostimulation is performed. This solves the problem of improving the visual functions of the eye.
The technical result achieved by the implementation of the proposed method is to reduce the invasiveness of the operation by eliminating the introduction of the electrode into the channel of the optic nerve and simplifying the technique of operation.
The method is as follows.
The conjunctiva and tenon capsule are cut at a distance of 8 mm from the limbus in the upper inner and lower outer quadrants of the eyeball in the intermuscular spaces, then using a spatula through the formed “tenon” space, two wire electrodes in the shell of the hemostatic collagen sponge are brought to the exit point of the visual nerve from the eyeball. The outer part of each electrode is brought out through the formed subconjunctival "tunnel" and then through the thickness of the lower eyelid, where it is fixed with a single nodal suture and to the skin of the eyelid.
Electrical stimulation is carried out in a known manner with bipolar pulses. At the end of the course of electrical stimulation, the inner part of the electrode with the hemostatic sponge membrane remains in the tenon space, and the outer part is crossed at the conjunctival incision and removed. In contrast to the known treatment method, special monitoring equipment and instruments are not required for implantation of the electrodes, the electrodes do not injure the optic nerve, they are fixed in the surgical field due to the swelling of the hemostatic collagen sponge used as an electrode sheath, which makes it possible to simplify the operation technique and reduce its invasiveness.
