Abstract
Description
The invention relates to medicine, namely to surgery.
A known method of applying a double-row intestinal suture. The method is as follows. Anastomosis is formed in two rows, the first row of nodal sutures with nodules in the anastomotic lumen is applied to the submucosa and the base of the mucous layer at the edge of the incision, and when the second row of sutures is applied, the thread is passed through the serous, muscle and submucosal layers with a puncture on the surface of the submucous layer of one side anastomosis and a puncture from the surface of the submucosal layer through the submucosa, muscle and serous layers of the other side of the anastomosis.
The disadvantage of this method is poor healing due to inaccurate comparison of homogeneous layers of the intestinal wall due to the presence of loosely contacted area between the first and second row of sutures. The disadvantage is also the lack of physical tightness in case of insolvency of the first or second row of seams.
There is a method of applying a double-row intestinal suture, which consists in applying knotted sutures and matching anastomosed intestinal walls, while the first row of sutures passes through all layers and is tied in the lumen of the intestine, the second row of sutures passes through the serous-muscular-submucous layers.
The disadvantage of this method is poor healing due to inaccurate comparison of homogeneous layers of the intestinal wall. There may be a violation of intestinal patency due to the presence of a not tightly contacted area between the first and second row of sutures, which, as a result of the formation of the anastomosis, plunges into the thickness of the anastomosis site, while reducing the intestinal lumen. In addition, in the case of failure of the first row of sutures, a sharp violation of the associated homogeneous layers of the intestinal wall occurs, as a result of which physical integrity is violated.
The aim of the invention is to improve healing by accurately matching homogeneous layers of the intestinal wall, improve patency of the lumen of the intestine by eliminating not tightly touching the area between the first and second row of sutures, improve physical tightness.
This goal is achieved in that a double-row intestinal suture is formed by passing the filament through the serous, muscular and submucosal layers along the edges of the intestinal wall on the one hand and the submucous, muscular, serous layer on the other hand. Then, closer to the edge of the wound, a through suture is applied, which is tied into a knot in the lumen of the intestine, after which a serous-muscular-submucosal knot is tied outside.
Thus, we consider the new signs to be significant, since only with their introduction the goal is achieved - to improve healing, improve patency of the intestinal lumen, and improve physical tightness.
The method is as follows: first, the thread using a needle, backing 0.4-0.6 cm from the edge of the intestinal wound, suture the serous-muscular-submucous layers of one side of the incision, then the submucosal-muscular-serous layers are sutured with the same needle the other side of the incision, with a puncture of 0.4-0.6 cm, from the edge of the wound. Another needle with a thread produces a through jab, stepping 0.2 cm from one and the other edge inward. A through seam is tied into a knot in the intestinal lumen; when a through seam is tied, an exact comparison of the edges and layers of the intestinal wall occurs. After this, the serous-muscular-submucous knot is tied, as a result of which the incision line is immersed in the wound edges with a wide gray-serous layer, and the removal of the not tightly adjoining section of the intestinal wall between the first and second row of sutures.
П р и м е р. PRI me R. A dog weighing 6 kg under morphine-hexenal anesthesia with a mid-laparotomy incision opened the abdominal cavity in layers. A loop of the small intestine was taken out into the wound, it was resected and an anastomosis of the end-to-end type was applied using the proposed double-row intestinal suture. The edges of the intestinal wound are well adapted, the implementation of the method is technically not difficult. As a suture material used chrome catgut N 2. The abdominal cavity was sutured in layers tightly. The postoperative period was uneventful. After 21 days, the animal was feeling fine. The wound healed by first intention Macroscopically: the epithelium in the anastomosis is completely restored, a delicate scar is determined.
