Способ панкреатикоеюностомии при кистозных формах кальцифицирующего и воспалительного хронического панкреатита с локализацией кист в теле, перешейке и хвосте поджелудочной железы, отличающийся тем, что производят формирование анастомоза однорядным П-образным «матрасным» швом передней и задней губы анастомоза после выполненной продольной вирсунготомии и клиновидного иссечения ткани ПЖ по всему периметру анастомоза, после вирсунготомии и иссечения ткани ПЖ накладывают гемостатические швы из нерассасывающегося шовного материала размером 3\0 или 4\0, концы которых завязывают; затем с использованием этих швов формируют переднюю и заднюю губы анастомоза П-образными швами, при этом используют для шва анастомоза оставленные нити гемостатических швов, при этом формирование задней губы производят путем прошивания тонкой кишки одной из нитей предыдущего, а затем и последующего гемостатического шва от подслизистого слоя к серозному покрову; после наложения швов на заднюю губу их последовательно завязывают, тем самым формируя узлы швов задней губы анастомоза кнаружи просвета анастомоза, и таким же образом формируют переднюю губу анастомоза, а формирование межкишечного анастомоза производят ниже панкреатического на 60-70 см; для дренирования кист до 5 см при их расположении по задней поверхности поджелудочной железы используют один ПХВ дренаж диаметром до 5 мм, а при кистах большего размера используют дренирование спаренными дренажами; в случае расположения кисты в теле или хвосте поджелудочной железы, прилежащей к переднему, верхнему, нижнему краям поджелудочной железы, выполняют максимальную резекцию кисты с освобождением передней поверхности поджелудочной железы непосредственно перед этапом продольной панкреатикотомии, при этом панкреатикоеюноанастомоз накладывают таким образом, что в зону анастомоза включают оставшуюся после резекции часть кисты.The method of pancreaticojejunostomy for cystic forms of calcifying and inflammatory chronic pancreatitis with localization of cysts in the body, isthmus and tail of the pancreas, characterized in that the anastomosis is formed by a single-row U-shaped "mattress" suture of the anterior and posterior lips of the anastomosis after the performed longitudinal tissue of the viruniformis The pancreas along the entire perimeter of the anastomosis, after virsungotomy and excision of the pancreas tissue, hemostatic sutures made of non-absorbable suture material 3 \ 0 or 4 \ 0 are applied, the ends of which are tied; then, using these sutures, the anterior and posterior lips of the anastomosis are formed with U-shaped sutures, while the left threads of the hemostatic sutures are used for the suture of the anastomosis, while the formation of the posterior lip is performed by stitching the small intestine with one of the threads of the previous and then the subsequent hemostatic suture from the submucosal layer to the serous cover; after suturing the posterior lip, they are sequentially tied, thereby forming the nodes of the sutures of the posterior lip of the anastomosis outside the lumen of the anastomosis, and in the same way the anterior lip of the anastomosis is formed, and the formation of the interintestinal anastomosis is performed below the pancreatic one by 60-70 cm; for drainage of cysts up to 5 cm when they are located on the posterior surface of the pancreas, one PVC drainage with a diameter of up to 5 mm is used, and for larger cysts, drainage with paired drains is used; in the case of the location of the cyst in the body or tail of the pancreas, adjacent to the anterior, upper, lower edges of the pancreas, the maximum resection of the cyst is performed with the release of the anterior surface of the pancreas immediately before the stage of longitudinal pancreaticotomy, while the pancreaticojejunostomy is applied in such a way that the anastomosis is included in the anastomosis area the part of the cyst remaining after resection.
