Claims (1)

Способ оперативного лечения сужения уретры, включающий разрез в промежностной области, доступ к бульбозной мышце, вскрытие уретры, введение в мочевой пузырь проводника, иссечение склерозированной части уретры, заготовку трансплантата и замещение зоны дефекта уретры трансплантатом, отличающийся тем, что разрез выполняют по средней линии промежности длиной 4-5 см, отступив от ануса вверх 2 см, затем формируют доступ в бульбо-спонгиозной мышце к белочной оболочке бульбы, после чего выполняют уретротомный доступ через спонгиозную ткань бульбы в зоне изгиба уретры дистальнее ее сужения для визуализации суженной части уретры, иссекают рубцовую ткань интрауретрально на всем протяжении сужения уретры, после разведения рассеченных краев уретры формируют ромбовидной формы площадку под трансплантат на всем протяжении сужения до здоровой части уретры, затем заготавливают трансплантат из слизистой оболочки ротовой полости в соответствии с формой и размерами сформированной ромбовидной формы площадки, трансплантат с вершинами треугольной формы укладывают на здоровые участки уретры и фиксируют швами, сначала в проксимальной части подготовленной площадки, после наложения швов в мочевой пузырь вводят уретральный катетер диаметром 6-8 мм, баллон которого раздувают до 5-10 мл, аналогично прошивают дистальную часть трансплантата к уретре без натяжения, после фиксации краев трансплантата выполняют сквозное прошивание его центральной части к подлежащим структурам, рану послойно ушивают, уретральный катетер удаляют через 10-14 дней.A method for surgical treatment of urethral narrowing, including an incision in the perineal region, access to the bulbous muscle, opening of the urethra, insertion of a conductor into the bladder, excision of the sclerosed part of the urethra, transplant harvesting and replacement of the urethral defect with a graft, characterized in that the incision is made along the midline of the perineum 4-5 cm long, backing up from the anus up to 2 cm, then they form an access in the bulbo-spongy muscle to the protein membrane of the bulb, after which urethrotomic access through the spongy tissue of the bulb is performed In the bend zone of the urethra distal to its narrowing to visualize the narrowed part of the urethra, the scar tissue is intra-urethrally dissected throughout the narrowing of the urethra, after the dissection of the dissected urethral edges, a diamond-shaped platform is formed for the transplant along the entire narrowing to the healthy part of the urethra, then the transplant from the mucous membrane is harvested from the mucous membrane the oral cavity in accordance with the shape and size of the formed rhomboid shape of the site, the graft with the vertices of a triangular shape is placed on healthy areas of ur they are fixed with sutures, first in the proximal part of the prepared site, after suturing, the urethral catheter with a diameter of 6-8 mm is inserted into the bladder, the balloon is inflated to 5-10 ml, the distal part of the graft is also sutured to the urethra without tension, after fixing the edges of the graft perform through flashing of its central part to the underlying structures, the wound is sutured in layers, the urethral catheter is removed after 10-14 days.
