Abstract
Способ реконструкции фронтального отдела верхней челюсти при помощи стереолитографической модели, характеризующийся тем, что предварительно пациенту делают дентальную объемную томографию и по ее результатам создают стереолитографическую модель для планирования костной пластики; в области дефекта на стереомодели восполняют объем кости из воска, для возможности определения точного размера костного аутотрансплантата; после определения размера костного аутотрансплантата и установления донорской области, под местной инфильтрационной анестезией производят забор костного аутотрансплантата с помощью хирургического пъезона с донорской области подбородочного отдела или с ветви нижней челюсти; далее, полученный костный аутотрансплантат припасовывают на стерильной стереолитографической модели, добиваясь его плотного прилегания, при этом для улучшения фиксации аутотрансплантата в нем образовывают, по меньшей мере, одно, предпочтительно два отверстия под винты; после получения точной посадки костного аутотрансплантата, его извлекают и помещают в 0,9% раствор NaCl на 5-10 мин; далее для последующей фиксации костного аутотрансплантата во фронтальном отделе верхней челюсти в реципиентной области, под инфильтрационной анестезией, формируют слизисто-надкостничный лоскут, а для большего контакта кортикальной пластинки и костного аутотрансплантата между собой, в каждой части образуют 8-10 дренажных, равномерно распределенных по площади, перфорационных отверстий диаметром 1,5 мм, на расстоянии 4,5-5 мм между их осями, после чего костный аутотрансплантат извлекают из раствора NaCl и фиксируют в реципиентной области с пA method for reconstruction of the anterior maxilla using a stereolithographic model, characterized in that the patient is first subjected to dental volumetric tomography and, based on its results, a stereolithographic model is created for planning bone grafting; in the area of the defect on the stereo model fill the volume of the bone from the wax, in order to determine the exact size of the bone autograft; after determining the size of the bone autograft and establishing the donor region, under local infiltration anesthesia, a bone autograft is taken using a surgical piezone from the donor region of the chin or from the lower jaw branch; further, the obtained bone autograft is fitted on a sterile stereolithographic model, achieving a snug fit, while at least one, preferably two screw holes are formed in it to improve the fixation of the autograft; after receiving the exact landing of the bone autograft, it is removed and placed in a 0.9% NaCl solution for 5-10 minutes; further, for subsequent fixation of the bone autograft in the frontal part of the upper jaw in the recipient region, under the infiltration anesthesia, a mucoperiosteal flap is formed, and for greater contact of the cortical plate and the bone autograft with each other, in each part they form 8-10 drainage uniformly distributed over the area , perforations with a diameter of 1.5 mm, at a distance of 4.5-5 mm between their axes, after which the bone autograft is removed from the NaCl solution and fixed in the recipient region with
