Abstract
Способ оценки клинической эффективности антиагрегантной терапии у больных острым коронарным синдромом, заключающийся в заборе венозной крови, стабилизации ее, определении агрегационной активности тромбоцитов импедансным методом, определении амплитуд агрегации тромбоцитов, отличающийся тем, что венозную кровь разделяют на два образца цельной крови, один из которых стабилизируют раствором цитрата натрия, а второй - этилендиаминтетраацетатом, в первый образец цельной крови вносят в качестве индуктора агрегации тромбоцитов аденозиндифосфат, определяют в нем максимальную амплитуду агрегации тромбоцитов и максимальную амплитуду кривой высвобождения аденозинтрифосфата, во втором образце определяют долю объема, занимаемую тромбоцитами, и долю объема крови, приходящуюся на форменные элементы, определяют индекс тромбоцитарного агрегационного потенциала крови и при значении индекса менее 0,5% судят о низкой эффективности антиагрегантной терапии, а при значении индекса 1,5-2,5% - о ее высокой эффективности, при этом индекс тромбоцитарного агрегационного потенциала цельной крови определяют по формулегде L- максимальная амплитуда кривой высвобождения аденозинтрифосфата,Ω- максимальная амплитуда агрегации тромбоцитов,РСТ - доля объема цельной крови, занимаемая тромбоцитами,НТС - доля объема крови, приходящаяся на форменные элементы.A method for evaluating the clinical efficacy of antiplatelet therapy in patients with acute coronary syndrome, which consists in taking venous blood, stabilizing it, determining platelet aggregation activity by the impedance method, determining platelet aggregation amplitudes, characterized in that the venous blood is divided into two whole blood samples, one of which is stabilized a solution of sodium citrate, and the second - ethylenediaminetetraacetate, in the first sample of whole blood make as an inducer of platelet aggregation adenosindi osfate, determine in it the maximum amplitude of platelet aggregation and the maximum amplitude of the adenosine triphosphate release curve, in the second sample, the fraction of the volume occupied by platelets and the proportion of blood volume per formed elements are determined, the index of platelet aggregation potential of blood is determined and if the index value is less than 0.5 % judge the low effectiveness of antiplatelet therapy, and with a value of the index of 1.5-2.5% - about its high efficiency, while the platelet aggregation potential index is integral to JVI determined according to the formula: L- maximum amplitude curve release adenosine triphosphate, Ω- maximum amplitude platelet aggregation PCT - the proportion of whole blood volume occupied by platelets NTS - the proportion of blood volume per shaped elements.
