Известен способ прогнозирования течения псориаза путем исследования крови больного, основанный на определении цитотоксической активности лимфоцитов по лизису бараньих эритроцитов в период выраженного обострения псориатического процесса. Поставленная цель достигается исследованием активности фермента фосфолипазы А2 в гемолизате эритроцитов и сыворотке крови больного и анализом полученных показателей. При превышении показателей в 2 раза и более относительно нормы прогнозируют ранний рецидив заболевания (в пределах 4-8 мес. ). Производится забор крови из локтевой вены в количестве 5 мл, затем добавляют 0,5 мл разведенного в 10 раз (1 : 10 физиологического раствора) гепарина. Активность фермента ФЛА2 в гемолизате эритроцитов и плазме крови определяют по методике Тужилина С. А. , Салуэнья А. И. Сыворотку инкубируют 15 мин при 60оС, затем к 1 мл добавляют 1 мл 1% -ного раствора трипсина и выдерживают 1 ч при температуре 0оС. К 1 мл инкубационной смеси добавляют 2 мл рабочего раствора субстрата и выдерживают при 37оС в течение 1 ч. Затем добавляют 1 мл раствора хлористого кальция для прекращения реакции и титруют 0,1н. NaOH в присутствии фенолфталеина. Процедура занимает примерно 3 ч. Расчет производят по формуле - 2 × 1 ед фосфолипазы А2 в 1 мл сыворотки крови. Определение данного показателя проводится до и по окончании курса лечения, а также в процессе диспансерного наблюдения за больными. Через 3 недели на фоне явного регресса псориатического процесса после курса фотохимиотерарии (ФХТ) показатели составляли соответственно 0,22 и 1,71. Период ремиссии длится 18 мес.PRI me R 1.
