Abstract
Способ оценки вентиляционных и перфузионных нарушений в раннем послеоперационном периоде при радикальных вмешательствах на легких, путем ингаляции радиофармпрепарата (РФП) и проведения последующей перфузионной сцинтиграфии легких с оценкой альвеолярной проницаемости, отличающийся тем, что исследование проводят дважды: до операции и в раннем послеоперационном периоде на 5 сутки, альвеолярную проницаемость определяют из всего легкого или остаточной части легочной ткани, в заднее-передней проекции в статическом режиме и при проницаемости ингалируемого РФП из пораженного легкого менее 25% через 30 мин после ингаляции и наличии субсегментарных перфузионных дефектов накопления РФП в капиллярном русле легких диагностируют воспалительные изменения обусловленные ателектазированием легочной ткани, а также тромбоэмболию мелких ветвей легочной артерии высокой вероятности.A method for assessing ventilation and perfusion disorders in the early postoperative period with radical interventions on the lungs, by inhalation of a radiopharmaceutical (radiopharmaceutical) and subsequent perfusion lung scintigraphy with an assessment of alveolar permeability, characterized in that the study is performed twice: before surgery and in the early postoperative period by 5 day, alveolar permeability is determined from the entire lung or residual part of the lung tissue, in the posterior-anterior projection in static mode and with permeable Inhaled radiopharmaceuticals from the affected lung less than 25% 30 minutes after inhalation and the presence of subsegmental perfusion defects in radiopharmaceutical accumulation in the capillary bed of the lungs are diagnosed with inflammatory changes due to atelectasis of the lung tissue, as well as high probability thromboembolism of small branches of the pulmonary artery.
