Abstract
Способ оценки степени тяжести состояния и прогноза исхода заболевания пациентов с ургентной хирургической патологией органов брюшной полости, включающий оценку состояния органов брюшной полости по шкале Мангеймского индекса перитонита, тяжесть состояня по шкале SAPS II, определение абсолютного количества в мкл периферической крови CD3+, CD4+, CD8+, CD16+, CD20+ лимфоцитов, определение отношений лимфоцитов: CD38+/CD95+, CD25+/CD38+, CD25+/CD95+,отличающийся тем, что дополнительно используют показатели МИП, SAPS II, абсолютное число CD20+ лимфоцитов, отношения CD38+/CD95+, CD25+/CD38+, CD25+/CD95+, а также дистанцию от центра кластера иммунной системы для рассчета тяжести состояния пациента по формулеТС=0,090+0,108·b+0,130·b+0,086·b+0,068·b+0,133·b0,076·b-0,001·ДК,где ТС - тяжесть состояния пациента,b- балльная оценка состояния органов брюшной полости на основе шкалы Мангеймского индекса перитонита,b- балльная оценка состояния тяжести пациента на основе шкалы SAPS II,b- балльная оценка количества CD20лимфоцитовb- балльная оценка отношения CD38+ и CD95+ лимфоцитов,b- балльная оценка отношения CD25+ и CD38+ лимфоцитов,b- балльная оценка отношения CD25+ и CD95+ лимфоцитов,ДК - дистанция от центра кластера иммунного статуса;индивидуальный рассчет вероятности летального исхода в процентах проводят по формуле степенной функцииВЛИ=7,446·ТС-75,551·ТС+266,261·ТС-379,511·ТС+229,489·ТС-49,025,где ВЛИ - вероятность летального исхода, выраженная в процентах;ТС - тяжесть состояния пациента.A method for assessing the severity of the condition and predicting the outcome of a disease in patients with urgent surgical pathology of the abdominal organs, including evaluating the condition of the abdominal organs using the Mannheim Peritonitis Index scale, severity of the condition using the SAPS II scale, determining the absolute amount of peripheral blood in CDL +, CD4 +, CD8 +, CD16 +, CD20 + lymphocytes, determination of the ratio of lymphocytes: CD38 + / CD95 +, CD25 + / CD38 +, CD25 + / CD95 +, characterized in that they additionally use indicators MIP, SAPS II, the absolute number of CD20 + lymphocytes, ratios CD38 + / CD95 +, CD25 + / CD38 + , CD25 + / CD95 +, as well as the distance from the center of the cluster of the immune system to calculate the severity of the patient's condition according to the formula TTS = 0.090 + 0.108 · b + 0.130 · b + 0.086 · b + 0.068 · b + 0.133 · b0.076 · b-0.001 where TS is the severity of the patient’s condition, b is the score for the state of the abdominal organs based on the Mannheim index of peritonitis, b is the score for the severity of the patient based on the SAPS II scale, b is the score for the number of CD20 lymphocytes b is the score for the ratio of CD38 + and CD95 + lymphocytes , b is a score for the ratio of CD25 + and CD38 + lymphocytes, b is a score for the ratio of CD25 + and CD38 + lymphocytes, DK is the distance from the center of the cluster of immune status; the individual calculation of the probability of death in percent is carried out according to the formula of the power function VLI = 7.446 · TS-75.551 · TS + 266.261 · TS-379.511 · TS + 229.489 · TS-49.025, where VLI is the probability of death expressed as a percentage; TS - the severity of the patient's condition.
