Способ резекции печени, заключающийся в укрытии ее раневой поверхности, отличающийся тем, что на основание предполагаемой линии иссечения мобилизованной левой доли печени, по периметру, накладывают полоску полипропиленового сетчатого материала (ПСМ) шириной не менее 1 см, чтобы она охватывала долю в виде муфты, концы которой сшивают отдельными узловыми швами у нижнего края органа и фиксируют полоску ПСМ отдельными узловыми швами к ткани печени у верхнего его края, после чего длинной прямой тупоконечной иглой с ушком на конце накладывают “гирляндный” шов с прошиванием переднего и заднего листка полоски ПСМ и всей толщи печени, причем первым проколом у верхней трети органа иглой в обратном направлении проводят двойную лигатуру, первую нить перевязывают у верхнего края печени формируя первое звено “гирляндного” шва, вторая нить при этом остается временно свободной, отступя на 2-3 см таким же образом вторым проколом проводят в обратном направлении свободный конец второй нити и следующую, третью нить, вторую нить завязывают на вентральной поверхности органа формируя, второе звено “гирляндного” шва, а третью нить перевязывают у нижнего края органа, формируя следующее звено “гирляндного” шва, затем электроножом по свободному дистальному краю полоски ПСМ производят резекцию левой доли печени и коагуляцию отдельных кровоточащих участков раневой поверхности органа, после чего соответственно размеру культи печени выкраивают ПСМ, которым укрывают ее по периметру и подшивают отдельными узловыми швами с захватом печеночной ткани и дистальных краев наложенной полоски ПСМ, завершая фиксацию ПСМ к ткани печени по центру культи отдельными узловыми швами.The method of resection of the liver, which consists in sheltering its wound surface, characterized in that a strip of polypropylene mesh material (PSM) of at least 1 cm wide is applied to the base of the proposed excision line of the mobilized left lobe of the liver so that it covers the lobe in the form of a sleeve, the ends of which are sutured with separate interrupted sutures at the lower edge of the organ and the PSM strip is fixed with individual interrupted sutures to the liver tissue at its upper edge, after which a long straight blunt-pointed needle with an eye is placed at the end garland ”suture with the front and back sheet of PSM strip and the entire thickness of the liver pierced, with the first puncture at the upper third of the organ using a needle in the opposite direction, double ligature is performed, the first thread is ligated at the upper edge of the liver forming the first link of the“ garland ”suture, the second thread it remains temporarily free, backing off 2-3 cm in the same way, the second puncture is carried out in the opposite direction, the free end of the second thread and the next, third thread, the second thread are tied on the ventral surface of the organ forming, the second the link of the “garland” suture, and the third thread is ligated at the lower edge of the organ, forming the next link of the “garland” suture, then, using the electric knife on the free distal edge of the PSM strip, the left lobe of the liver is resected and coagulation of individual bleeding sections of the wound surface of the organ is performed, after which, according to the size of the stump PSM are cut out of the liver, which cover it around the perimeter and hemmed with separate interrupted sutures with the capture of the liver tissue and the distal edges of the applied PSM strip, completing the fixation of PSM to the liver tissue On the center of the stump separate interrupted sutures.
