Abstract
Usage: in medicine, namely in otorhinolaryngology. The goal is to reduce the number of relapses. The essence of the proposal is that the cricoid cartilage is cut into S-shapes, then a triangular shape graft is formed from the larynx of the larynx and placed inside the epigastrium at the incision site of the thyroid and upper cricoid cartilage. The advantage of the proposal is to improve functional results.
Description
The invention relates to medicine, namely to otorhinolaryngology, and can be used in the treatment of chronic cicatricial stenosis of the larynx
The purpose of the invention is to reduce the number of relapses of the disease.
The method is carried out as follows.
During surgery, a patient with chronic cicatricial stenosis of the larynx is made a bordering incision of the skin and soft tissues around the tracheostomy with its extension up and down in the midline. Skin edges are mobilized. In front of the trachea, scarred skin and soft tissues are removed. The anterior wall of the trachea is cut in the midline up to the cricoid cartilage and 1-2 cm downward. If there is granulation or scar tissue in the presence and clearance of the trachea, the latter are removed. The edges of the trachea are sutured to the skin with knotted sutures on both sides and thereby form a new tracheostomy. Then the section of the anterior wall of the cricoid cartilage is S-shaped so that it
После рассечени перстневидного и щитовидногоends at the thyroid cartilage in the midline, after which the lower third of the thyroid cartilage is cut in the midline (the anterior wall of the larynx. Then, a triangular plate on the pedicle is cut out on the anterolateral wall of the cricoid cartilage, which is implanted at the site of the incision of the thyroid and upper part of the anterior Moreover, when the specified plate is taken, the epigastrium on the inner wall of the larynx is kept intact, and when implanted, the lamina is laid on the outer surface covered with the epithelium, As a result of the above stages of the operation, an expansion of the lumen of the larynx is achieved, which is especially important with the congenital narrowness of the latter, as well as preservation of the anterior wall of the cricoid cartilage. The latter is achieved by an S-shaped incision, which results in two plates (upper and lower). cartilage. The implantable plate is fixed by means of catgut. After dissection of the cricoid and thyroid

From the lumen of the larynx with obligatory control with direct laryngoscopy. If necessary, removal of scars from the larynx is carried out both through a laryngotracheostomy and through the oral cavity with direct hanging laryngoscopy.
Subsequently, the larynx is sutured in layers, and a tampon-dilator is inserted into its lumen through the tracheostomy. Change of the latter is carried out in 1-2 days. Dilation of the larynx is carried out for 3 months. After which the patients are decanulated.
Example 1. Sick П-к Yu.M., 4 years old, was admitted to the hospital with complaints of carriage, impossibility of breathing through the natural airways.
The child suffered from a congenital stridor. Repeatedly ill ARVI. At the age of 1.5, he fell ill with acute stenosing laryngotracheobronchitis. During the treatment, an upper bronchoscopy was performed, after which the child was on prolonged intubation for 4 days, and then a tracheostomy was performed. Deanul qi failed. Subsequently, he was repeatedly treated in the thoracic ward, where larynx was performed, cryodestruction of the scars of the larynx, prolonged intubation for 2 weeks. Deanul qi baby failed.
Upon admission to the ENT clinic on the anterior surface of the neck in the middle third of the tracheostomy, in which there is plastic tracheostomy tube No. 00. For direct laryngoscopy: the vocal fold on the right is scarred (the scar on the fold protrudes into the lumen of the larynx). In the sub-vocal cavity, the lumen of the larynx is narrowed at the level of the cricoid cartilage due to the cicatricial process. Deformation of the trachea above the tracheostomy has been detected.
Ultrasound examination of the larynx showed an increase in echo signals in the sub-vocal cavity on the echogram, more at the level of the cricoid cartilage. Echoes from the vocal folds are mobile, biased during phonation, and are located at the same level.
Other ENT organs did not reveal any pathology.
In the lungs, hard breathing, dry rales.
Diagnosis: chronic cicatricial stenosis of the larynx, cannula r. Tracheal deformity above the tracheostomy. Chronical bronchitis.
Treatment .. The patient underwent surgery - plastic surgery of the larynx and trachea. Under the endotracheal anesthesia, a bordering incision of the skin and subcutaneous
