Description
The invention relates to medicine, namely to surgery.
The aim of the invention is to reduce the treatment of purulent wounds.
The method is carried out as follows.
20 ml of sterile distilled water is introduced into a vial containing 10 mg mg of dry papain. The top cap of the vials is removed and a direct laser beam from the LG-75 apparatus is directed through the hole in the vial onto the solution surface. The power density of the laser light energy on the surface of the solution is 80 mW / cm2. Irradiation is carried out fractionally in one second twenty times with an interval of 6 seconds. Then, by adding sterile distilled water, the papain solution is adjusted to 0.1% and applied to the purulent wound 15-20 minutes after irradiation.
After opening the abscess, evacuating free-flowing pus, necrotic masses and diluting the edges of the purulent wound, it is introduced into the cavity sterile, moistened with an irradiated papain solution, and friable
Во врем перев зки удал ютс турунды, которые, как правило, свободно отход т вместе с большим количеством некротических тканей, фибрина, После полного очищени раны от патологического содержимого примен ютс антисептические мазевые пов зки по традиционной методике .gauze turundas so that the wound cavity and its pockets are softly filled. The sizes, thickness and number of turundas are selected individually according to the volume, depth and shape of the wound cavity. The wound is closed with sterile wipes. The thickness of the upper dressing is determined in accordance with the expected amount of pathological detachment. Within 1–2 hours after ligation, a large quantity of liquid pus is absorbed and impregnated with the dressing. In connection with this, it is often necessary to lay an additional layer of absorbent cotton. After 6-12 hours, ligation is performed. During the ligation, turundas are removed, which, as a rule, freely depart with a large number of necrotic tissues, fibrin. After the wound is completely cleansed of the pathological contents, antiseptic ointment dressings are applied according to the traditional method.
Таким образом, первый период раневого процесса-очйщение раны сокращаетс на 2 + 0,8 дней, чем сокращаетс сроки лече (/)Thus, the first period of the wound healing process - wound cleansing is reduced by 2 + 0.8 days, which reduces the treatment time (/)

Examples. B. K., 50 years old. Diagnosis at admission: Postinjection abscess of the left throat. Under an intravenous narcosis, an abscess was opened, up to 600 cm of pus was evacuated. The wound is 7 cm long and 6-8 cm deep. The abscess cavity contains a large number of necrotic tissues, the walls of the cavity are a cliff of necrotic muscle tissue, fascia. The abscess cavity was treated with a solution of hydrogen peroxide and furacilin, drained with gauze drainage moistened with furacilin. Over the next 3 days, ligation was performed with furacilin solution. A large amount of pus was isolated, but there was no tendency to cleanse the wound from necrotic masses. On the fourth day after one-oration, drainage was introduced into the abscess cavity, moistened with a 0.5% papain solution, which was previously (in 20 min) irradiated in a penicillin vial with a helium-neon laser fractionally 1 time 20 times with an interval between irradiations in 6 sec. The power density of the beam on the surface of the solution is 80 mW / cm2. After 24 hours, the drainage bandage was removed. The abscess cavity was cleansed of pus and necrotic tissue. Granulation appeared. An ointment dressing was applied, which was applied for the next three days, after which secondary sutures were applied to the wound.
B. T., 21 years old. Diagnosis at admission: .Abscess of the anterior abdominal wall in the right iliac region. An abscess was opened under intravenous anesthesia. Stand out up to 50 cm pus. The abscess cavity is 6x8 cm. A dressing with a hypertonic solution is applied. A day after the operation, the cavity of the abscess and its wall contains a large number of necrotic tissues and pus. Gauze drainage was introduced into the abscess cavity; moistened with a 0.1% solution of papain, previously irradiated with helium-neon laser beams in a penicillin vial, in fractions of 1 every twenty times with an interval between irradiations with a beam power density of 80 mW / cm2 on the solution surface. After 12 hours, a new ligation was performed with a freshly irradiated 0.1% papain solution. After another 12 hours, the drainage was removed. The wound is clean, islands of granulation are visible. The swelling of the surrounding tissues has disappeared, the skin has acquired a normal color, the body temperature has returned to normal. A dressing was applied, and the next day - secondary sutures on the wound. Thus, the proposed method allows to reduce the treatment time for purulent wounds by 5 + 0.6 days.
