Description
The invention relates to the field of medicine, namely traumatology and orthopedics.
A known wound dressing made of a two-layer polymer material with an antiseptic.
However, the known dressing does not allow to stimulate the regeneration of wounds in soft tissues.
The goal is to provide stimulation of wound regeneration in soft tissues.
A wound dressing consists of a drug portion and a bioinert gas exchange bilayer material with fasteners at the ends. The medicinal part is made in the form of a dense jelly-like briquette of any geometric configuration (round, oval, square, triangular, etc.) depending on the specific shape of the wound. The dimensions of the drug part are also commensurate with the size of the wound, and the thickness varies from 0.2 to 10 mm, depending on the anatomotopographic location of the wound.
При этом не об зательно в пов зку вводить сразу все перечисленные компоненты: они подбираютс индивидуально, выборочно, Вс эта смесь доведена до достаточно плотной желеобразной консистенции и размещенной между двум герметично скрепленными по кра м сло ми биоинертного материала, который в свою очередь, состоит из наружной пленки и внутреннего сетчатого сло .The medicinal part consists of a demineralized bone matrix, ground to a powder state and treated with antiseptic agents (bactericidal ions of copper and silver), but can also be treated with protein preparations (blood plasma, albumin, salcoseryl, etc.), including gelatin, biostimulants and sorbent. At the same time, it is not necessary to introduce all the listed components into the dressing at once: they are selected individually, selectively. outer film and inner mesh layer.
The mesh layer on the periphery contains a rim. The rim is made of elastic, thin (25-40 microns) bioinert gas exchange material - from the same material

2 and is intended for tightly bonding the film and the net layer to each other after preliminary placement of the drug part between them. The mesh layer is made quite loose. The mesh size of the mesh layer is, for example, 0.8-0.8 mm; 1.0-1.0 mm. The diameter of the mesh fibers is 4-0.1-0.4 mm. The mesh layer is tightly stretched over the drug portion.
The outer film is made of a flexible, flexible, gaseous polymeric material, for example, a polycarbonate-siloxane film with a thickness of 20 to 60 microns or more. If the dressing is intended to treat a wound on a finger of the hand, then it is made of a thinner film; if for the treatment of a wound on the lower legs - from a thicker one. The film is not traumatic, does not macerate the skin, does not inhibit its gas and vapor permeability. The rough surface of the film in the area opposite the mesh layer helps to keep the drug part on the surface of the film. This rough surface, moderately irritating the mechanoreceptors and baroreceptors of the wound surface, improves the microcirculation of the wound and thereby increases its resistance. By means of a fastener made, for example, of Wilcro of the Repay type, the ends of the film are fixed to each other.
The ratio of components in the drug part depends on the specific features of the wound; therefore, the bandage can be made of several types with the corresponding marking. The proportion of the main therapeutic component, the powdered demineralized bone matrix, ranges from 1/3 to 3/4 of the total mass and is selected depending on the characteristics of the wound process, the severity of the reparative processes in it: the more reparative processes occur in the wound and in the wake, the more more should be the content of the powder matrix, and
The remainder are protein preparations and other ingredients.
Antiseptic agents, including silver and copper ions, are used either prophylactically in case of clean wounds or in bactericidal concentrations (with severe microbial contamination of the wound).
A dressing is used as follows.
The wound is preliminarily cleaned with conventional dressings using necrolytic and antiseptic preparations, taking into account the infused microflora. Further, after sufficient cleaning of the wound, a bandage is applied to the wound and fixed to the extremities, over the brine with fasteners. The medicinal part of its superficially should completely cover the surface of the wound. The applied dressing is maintained on the wound surface for the entire duration of the treatment. Only replace it if necessary. After treatment is complete, upon epithelialization of the wound surface, the dressing is removed.
The dressing makes it possible to stimulate the reparative regeneration of long-term non-healing wounds by uniformly and continuously influencing the inductive factor of the demineralized bone matrix on fibroblasts and other cellular elements of the connective tissue, which contributes to epithelization and subsequent scarring of the wound.
