Description
Foreliggende oppfinnelse anvendelse av et diagnostisk middel ved forbedring av hysteroskopi. Ved hysteroskopi forstås den endoskopiske inspeksjon av limorhulrommet med et spesialendoskop (hysteroskop). Hysteroskopien er en essensiell diagnostisk fremgangsmåte til bedømmelse av diverse livmorsykdommer f.eks. myomerhhv. endometrium hyperplasi.
Fra Gynaecol.-Endoscopy 4:4 259-264 1995 er det kjent at GnRH-analog, her triptorelin blir anvendt før den laparoskopiske myomektomi. Ytterligere ble det med LH-RH agonisten typtorelinen gjennomført en behandling av endometrium hyperplasi ved hysteroskopisk og diagnostisk kurettasje (Contraception-Fertilite-Sexualite 23,9 516-523, (1995)). Fra J. Gynaecol. Surg, 11:2,65-70, (1995) er det kjent, å bedømme effekten av fortynnelsen av endometri ved Gn-RH agonisten (Zoladex) med hensyn til resultatene av den hysteroskopiske, endometrium elektrokirurgiske reseksjon. Prepareringen av endometrimet før den hysteroskopiske kirurgi blir anbefalt.
Fra TW. Gynaekol. 6:6,382-390 (1993) kjennes resultater fra infertilitetshandlingen med GnRH analog, ifølge hvilke volumet av leiomyomer foregår før den laparaskopiske myomektomi eller den hysteroskopiske reseksjon.
Anvendelsen av LHRH-antagonister, spesielt av cetrorelix er tidligere ikke kjent i sammenheng med den hysteroskopiske diagnostikk eller ved operasjonsforberedelse.
Formålet med foreliggende oppfinnelse er derfor & frembringe et middel til forbedring av resultatene av hysteroskopi i sammenheng med diagnosen av tilstanden av uterus, spesielt ved endometrium hyperplasi, leiomyomer eller ondartede sykdommer. Oppfinnelsen muliggjør ytterligere en ikke invasiv terapi hhv. en operasjon uten lang forberedelsestid. Den kan således samtidig forbindes med en behandling av de ovenfor nevnte sykdommer.
Formålet oppnås derved ved at LH-RH antagonister, spesielt den f.eks. fra EP 0.299.402 kjente LH-RH antagonist cetrorelix, anvendes i forbedringen av hysteroskopien ved de ovenfomevnte anvendelsestilfeller.
Foreliggende oppfinnelse tilveiebringer således anvendelse av LH-RH antagonister som diagnostisk middel til forbedring av effektiviteten av hysteroskopi.
Videre tilveiebringer foreliggende oppfinnelse anvendelse av LH-RH antagonister, spesielt av cetrorelix, ved hysteroskopi i kombinasjon med etterfølgende endometriosebehandling, fjerning av myomer eller en endometrium hyperplasi.
Gjennom den umiddelbart begynnende virkning av cetrorelix er dennes anvendelse spesielt også vist i sammenheng med diagnosen og terapien f.eks. ved myomblødninger.
Diagnosen kan fordelaktig umiddelbart overgå i terapien med terapeutiske doser.
Behandlingen f.eks. av leiomyomer kan foregå etter følgende skjema:
Eksempel
Ved en utførsel av foreliggende oppfinnelse blir det injisert 0,1 til 2 mg/kg kroppsvekt som enkeltdose eller 0,01 til 0,5 mg/kg kroppsvekt cetrorelix hhv. fordelt over 1 til 14 dager.
Følgen er en reduksjon i sjikttykkelsen av endometriumet og en derved betinget effektivisert hydroskopisk bedømmelsesmulighet og terapi av sykelige tilstander. Gjennom den hurtig begynnende virkning av cetrorelix er denne administrering passende spesielt også ved den diagnostiske forberedelse av slike operasjoner, som ikke tåler noen forsinkelse, som f.eks. mammacarcinom.
I den etterfølgende tabell 1 vises til reduksjon hhv. stagnasjonen i veksten av endometriumet som foregår hos en kvinnelig pasient med initialene 112 KB og blir forklart i det følgende:
Cetrorelix pamoat: foreløpig resultat av de kliniske forsøk:
Behandling av en kvinnelig pasient med uterin fibrose som ble operert. Injeksjon av 60 mg cetrorelix på syklusdag 2 og 28.
Endometriumet ble målt med ultralyd og viste ingen vekst under cetrorelix-behandlingen. Den hysteroskopiske og laparoskopiske myomreseksjon ble gjennomført en gang uten et signifikant blodtap kun 2 måneder etter start av terapi med cetrorelix. Til diagnostisk undersøkelse eller ablasjon av endometriumet er en kort periode fra 7 til 14 dager med cetrorelixbehandling tilstrekkelig på grunn av den straks begynnende reduksjon av sjikttykkelsen av endmetriumet etter injeksjonen av cetrorelix.

Anvendelse av LH-RH antagonister som diagnostisk middel til forbedring av effektiviteten av hysteroskopi.

Anvendelse ifølge krav 1, hvor det som LH-RH antagonist anvendes cetrorelix.

Anvendelse ifølge krav 1 og 2, hvor LH-RH antagonisten, spesielt cetrorelix administreres før hysteroskopien og/eller for operasjonsforberedelse i en engangsdose fra 0,1 til 2 mg/kg kroppsvekt.

Anvendelse ifølge krav 1 og 2, hvor LH-RH antagonisten, spesielt cetrorelix, administreres før hysteroskopien og/eller for operasjonsforberedelse i en flergangsdose fra 0,01 til 0,5 mg/kg kroppsvekt, fortrinnsvis fordelt over 1-14 dager.
