Description
Fremgangsmåte til fremstilling av et inhaleringsanestetikum.
Denne oppfinnlse angår en fremgangsmåte til fremstilling av nye anestetika,
spesielt et inhalasjonsanestetikum som består av en konstant kokende blanding av dietyleter og trifluormetylbromklormetan, en forbindelse som har formelen
CF2CHClBr.
Før den foreliggende oppfinnelse frem-kom har man stadig søkt etter en uan-tennbar form for dietyleter til alminnelig bruk som anestetikum, for blant alle de forbindelser som er blitt prøvet som al-minnelige anestetika er det bare dietyleter som oppfyller de egenskaper som et anestetikum bør ha, nemlig et middel som fram-kaller bevisstløshet, muskelavslappelse, analgesi og post-operativ amnesi. Etyleter har imidlertid den ulempe at den er meget lett antennelig, hvorfor det medfører fare å anvende den i operasjonsrommet. Det er hittil blitt foreslått forskjellige fortyn-ningsmidler for å nedsette dietyleterens antennelighet. Disse har imidlertid ikke vist seg å være egnet, da de var farmako-logisk inaktive og derved nedsatte effekten av anestetikumet, og/eller fordi de hadde høyere kokepunkt enn dietyleter, hvorfor de ble skilt fra denne under anvendelse ved fraksjonert fordampning.
Et formål med oppfinnelsen er å skaffe et nytt anestetikumpreparat som er uan-tennelig og som er meget effektivt anvendt som anestetikum.
Et annet formål med oppfinnelsen er å skaffe et konstant kokende preparat som kan måles nøyaktig ut som anestetikum.
Disse formål oppnås ved fremstilling av
preparatet i henhold til oppfinnelsen som
består i at dietyleter og trifluormetylbromklormetan blandes i et forhold på minst 40 til 60 mol-pst. og den ved 52° C og atmosfæretrykk konstant-kokende blanding ut-vinnes.
Det ble uventet funnet at dietyleter danner en konstant-kokende blanding med trifluormetylbromklormetan og at en slik blanding er et effektivt anestetikum av konstant sammensetning og ikke er an-tennbart. Dette er overraskende i betrakt-ning av at dietyleter og trifluormetylbromklormetan har forskjelligartet kjemisk struktur og derfor ikke skulle ventes å skulle danne en konstant-kokende blanding. Hvis den ikke gjorde det ville blandingen være verdiløs som anestetikum, for ved anvendelse ville den laverekokende komponent (f. eks. dietyleter) bli brukt først, hvoretter en blanding av stadig mindre eter ville bli fordampet og til slutt ville det bare forbrukes rent trifluormetylbromklormetan slik at det ville være umulig for anvenderen å bestemme på forhånd den mengde og art av anestetikum som patienten får, hvorved man ikke utnytter egenskapene ved begge komponenter. Det er videre blitt funnet at den konstant-kokende blanding inneholder ca. 40 mol-pst. dietyleter og 60 mol-pst. trifluormetylbromklormetan, slik at effekten av begge anestetika utnyttes. Til forskjell kan det bemerkes at andre etere, som f. eks. etyl-vinyleter, divinyleter og diisopropyleter ikke danner konstant-kokende blandinger med trifluormetylbromklormetan, slik at dietyl-eteren kan ansees å være enestående blant eterne i denne henseende. Hvis den konstant-kokende blanding som fås hadde in-neholdt f. eks. 95 mol-pst. etyleter, ville det ikke ha vært noen grunn til å anvende blandingen, da blandingen ville ha omtrent de samme fysiske, kjemiske og farma-kologiske egenskaper som ren. dietyleter, deriblant også den lette antennbarhet og de farer dette medfører.
Trifluormetylbromklormetan er et kjent stoff som har anestetiske egenskaper. Det har imidlertid ikke alle de egenskaper som kreves hos et helt tilfredsstillende anestetikum, idet det nemlig frembringer brady-cardia- og nedsettelse av åndingen, og ikke bevirker fullstendig muskelavslappelse eller analgesi (se. Bryce-Smith et al., British Medical Journal, 2, 969—972 (1956), men ved å kombinere denne forbindelse med dietyleter slik at det dannes en konstant-kokende blanding i henhold til oppfinnelsen bibeholdes de fulle virkninger og fordeler ved dietyleter samtidig som man unngår den brannfare som den sistnevnte alene betegner. Da trifluormetylbromklormetan selv er et anestetikum blir dietyleterens anestetiske virkning ikke for-tynnet i uheldig grad og den.totale anestetiske virkning av blandingen er ikke ene og alene virkningen av dietyleterkompo-nenten alene. Ennvidere blir de uheldige sidevirkninger av trifluormetylbromklormetan i høy grad moderert ved anvendelse i bare den mengde som kreves for å oppnå at blandingen ikke er antennbar.
Den konstantkokende blanding i henhold til oppfinnelsen kan fremstilles ved fremgangsmåten i henhold til oppfinnelsen, som i hovedsaken består i at man des-tillerer en blanding av dietyleter som har kp ved atmosfæretrykk på 34° C (i ca. 40 mol-pst. eller større proporsjon) og trifluormetylbromklormetan med kp ved atmosfæretrykk på 49° C (i ca. 60 mol-pst. eller mindre proporsjon) og utvinner den derved erholdte konstant-kokende blanding som har et kp ved atmosfæretrykk på 52° C.
Det følgende eksempel illustrerer frem-gangsmåtene i henhold til oppfinnelsen.
Eksempel.
I et destillasjonskar anbringes 75 ml vannfri dietyleter og 75 ml trifluormetylbromklormetan (CF2CHBrCl). Destilla-sjonsbeholderen forbindes deretter med en 1 m lang destillåsjonskolonne som er pak-ket med glasspiral og har et destillasjons-hode. Kolonnen bringes først i likevekt under betingelser for total tilbakeløps-kjøling; destillasjonshodets temperatur er 34° C og beholdertemperaturen er 45° C. Deretter innstiller man på en overdestille-ring av 10 ml i løpet av 35 min. Hoved-mengden av destillatet er en konstant-kokende blanding av dietyleter og trifluormetylbromklormetan, kokepunkt ca. 52° C. En undersøkelse av denne konstant-kokende blanding viser at den inneholder ca. 40 mol-pst. dietyleter og ca. 60 mol-pst. trifluormetylbromklormetan.
For anvendelse som anestetikum blir blandingen i henhold til oppfinnelsen anvendt enten ved den velkjente åpne dryppe-metode eller ved anvendelse av en hvilken som helst av de mange typer av inhaierings-utstyr som nå er i alminnelig bruk; f.eks. Epstein og Macintosh inhaleringsapparat
(1956) som er innstilt på å levere kjente
konsentrasjoner av anestetikumet. Den først, nevnte metode er enkel og grei og har visse fordeler, mens den sist nevnte har den fordel å levere et kjent og reguler-bart forhold mellom det azeotropiske anestetikum og surstoffkilden. Surstoffkilden kan enten levere en surstoffkvelstoffblan-ding eller en surstoff-heliumblanding eller, mere vanlig, luft. Med fordel er anestetikumet tilstede i surstoffkilden i en kon-sentrasjon av minst ca. 2 pst. (vol./vol.).

- Fremgangsmåte til fremstilling av et inhaleringsanestetikum, karakterisert ved at man blander dietyleter og trifluormetylbromklormetan i et forhold på minst 40 til 60 mol-pst. og utvinner den ved 52° C og atmosfæretrykk konstant-kokende blanding.
