Titel: Implantaatmateriaal uit keramisch, materiaal . De uitvinding heeft "betrekking op een implantaatmateriaal uit keramisch materiaal h.v. aluminiumoxyde of gesinterd tricalciumfosfaat-materiaal ter vervanging van hard weefsel, voor tandheelkundige en medische toepassingen.
5 Een dergelijk implantaatmateriaal is bekend uit het Amerikaanse octrooischrift 4.222.128. Hierin wordt een samengesteld implantaatmateriaal beschreven, dat een. gesinterd apatietmateriaal, liefst hydroxylapatiet ,. en een thermoplastische of een thermohardende hars omvat. Deze hars bevindt zich in de poriën of kunstmatig aangebrachte 10 perforaties van het apatietmateriaal. Dankzij de opname van een dergelijke hars kan een hoge slagsterkte worden gerealiseerd. De druksterkte en de buigsterkte worden daardoor echter niet verbeterd.
Voor sommige soorten implantaten is de buig- of treksterkte van weinig belang, zoals voor kunstmatige tandwortels_(zie Amerikaans-15 octrooischrift 4.222.128, kolom 4, regels 39-43). Voor andere soorten implantaten, zoals botverankeringen, kunstmatige tanden, belaste botten, en gewrichten is deze. sterkte daarentegen van groot belang. Gesinterde tricalciumfosfaatmaterialen zoals gesinterd, hydroxylapatiet waarvan de bereiding bijv. in het Britse octrooischrift 1.522.182 20 wordt beschreven, blijken voor dergelijke toepassingen in hun buig- en treksterkte te kort te schieten.
Door de implantaten, van metaal of kunststof te maken kan dit bezwaar weliswaar worden opgeheven, maar daarvoor in de plaats treden dan vele andere bezwaren, waarvan de slechte verenigbaarheid mèt het 25 levende materiaal, tot uiting komende in slechte hechting en afstotings-verschijnselen, het belangrijkste bezwaar is. Tricalciumfosfaatmaterialen zoals gesinterd hydroxylapatiet hebben dit bezwaar niet: ze zijn niet giftig, niet carcinogeen en zijn goed verenigbaar met levend, menselijk of dierlijk materiaal.
30 Uit implantatieproeven is gebleken, dat onder wisselende buig- belastingen, een combinatie van trek- en buigspanningen, hydroxyl-apatriéf: na zekere tijd bezwijkt aan vermoeiingsbreuk.

Metingen aan proefstukjes uit hydroxylapatiet hebben uitgewezen dat de treksterkte een faktor 5 lager ligt dan de druksterkte. Het is daarom duidelijk dat in de praktijk hydroxylapatiet minder weerstand biedt tegen trek- dan tegen drukkrachten-.- Be meeste materialen, waar-5 onder hydroxylapatiet, blijken, indien men <"j bij een belastingschom-meling de spanning laat wisselen tussen bepaalde grenswaarden, te bezwijken bij een uiterste spanning die verder onder de statische breeksterkte en zelfs onder de vloeigrens ligt. Men spreekt dan van ver-moeiingsbreuk. Indien het materiaal wisselend belast wordt met -een 10 spanning die ligt onder de zogenaamde wisselspanning, treedt breuk zelfs na een oneindig aantal wisselingen, niet meer op. De trekwissel-spanning van hydroxylapatiet ligt aanzienlijk lager dan de drukwissel-spanning. Oefent men nu van buiten af een konstante drukkracht uit op het onder wisselspanning belaste hydroxylapatiet zal de spanning wis-15' selen rond. deze konstante drukkracht. Kiest men. de konstante drukkracht groter of gelijk aan de trekwisselsterkte dan treedt breuk door trekkrachten niet meer op.
Het is duidelijk dat de konstante drukkracht kleiner gekozen dient te worden dan de helft van de som van de trekwis selspannig en de 20/ drukwisselspanning en bij voorkeur 50 tot 75 procent van deze waarde, omdat anders de kans op drukwisselbreuk te groot zou kunnen worden.
De uitvinding wordt derhalve gekenmerkt, doordat het keramisch materiaal door middel van een compressie-orgaan onder compressie wordt gehouden.
25 De gewenste compressie kan op diverse manieren tot stand worden gebracht. Volgens een voorkeursuitvoeringsvorm omvat het compressie-orgaan een centraal in het implantaatmateriaal gelegen staaf. Deze. staaf kan op verschillende manieren voor de benodigde compressie zorgen.
30 Volgens een voorkeursuitvoeringsvorm is de staaf aan beide uit einden voorzien van een schroefdraad, waarop moeren zijn geschroefd tot over een zodanige afstand, dat de door de moeren op het keramisch materiaal uitgeoefende compressiekracht de gewenste waarde heeft.
Liefst bevinden zich tussen de moeren en het keramisch materiaal 35 ringetjes van bijv. platina of kunststof, die de uitgeoefende compres- 8101674

- 3 - siekracht oerdelen over het oppervlak van het implantaatmateriaal, Volgens een andere voorkeursuitvoeringsvorm is de staaf aan heide uiteinden voorzien van soldeer, dat als gevolg van een smelt-verhittingshehandeling en daaropvolgende stol-afkoelingsbehandeling 5 door de krimp van de staaf tegen het keramisch materiaal is getrokken.
Het compressie-orgaan kan op verschillende vijzen en uit verschillende materialen zijn uitgevoerd. In de meeste gevallen zal een metalen staaf uit een legering met gewenste sterkte- en corrosie-eigenschappen worden geprefereerd. Gedacht kan worden aan elastische 10 Hi-Cr legeringen met hoge vloeigrens en treksterkte.
Het compressie-orgaan kan tevens dienst doen als hot ver anker ings - orgaan.
De uitvinding zal aan de hand van de tekening, waarin een voor-keursuitvoeringsvorm van een implantaatmateriaal volgens de uitvinding 15 in doorsnede is weergegeven, nader worden toegelicht..
Weergegeven is een implantaatmateriaal 1 uit gesinterd trical-ciumfosfaatmateriaal 2, hij voorkeur hydroxylapatiet dat een zekere hoeveelheid whitloekiet kan bevatten. Centraal door het implantaatmateriaal strekt- zich een staaf 3 van hijv. li-Cr staal uit, die aan 20 heide uiteinden voorzien is van een schroefdraad Op deze uiteinden zijn moeren 5 geschroefd, die via tussengelegen ringen 6 van bijvoorbeeld platinafoelie tegen het tricalciumfosfaatmateriaal 2 drukken.
Voorbeeld
Een concreet voorbeeld van een implantaatmateriaal volgens de 25 tekening betreft een gesinterd hydroxylapatiet cilindert je van 12 mm lengte en h mm diameter met een centraal gat van 1,4 mm. De moeren hebben een diameter van U mm en een lengte van bijv. 2 mm. Het platinafoelie heeft een dikte van bijv. 0,03 mm. Be moeren zijn aangedraaid totdat het hydroxylapatiet onder een constante drukkracht van ongeveer 30 8 kg/mm^ was gebracht.
Bij meting van de buigsterkte' in een 3-punts buigproef bleek in • * het implantaat breuk :op te treden bij een centrale belasting van - 30 kg bij niet aangedraaide moertjes en - ^0 kg bij met een kwartslag (0,30 mm spoed) aangedraaide moertjes.
35 Een andere uitvoeringsvorm zou kunnen bestaan uit een gesinterd 8101674 -fc - hydroxylapatiet cylindertje met daarin een centrale uitsparing in de vorm van een centraal gat van 1 Λ mm diameter en twee schijfvormige uitsparingen met een diameter die 0,5 mm kleiner is dan de buiten-diameter van het cylindertje mm), Door in de uitsparing een legering 5 met een grotere lineaire thermische uitzettingscoëfficiënt dan het hydroxylapatiet te gieten in het voorverwarmde cylindertje, zal het hydroxylapatiet tussen de beide schoteltjes bij afkoelen tot kamertemperatuur onder druk komen te staan. Hierdoor zal de buigsterkte toenemen net zoals in het voorgaande voorbeeld.

Claims (4)
1. Implantaatmateriaal uit keramisch materiaal zoals b.v. aluminium-oxyde of gesinterd tricalciumfosfaatmateriaal ter vervanging van hard weefsel, voor tandheelkundige en medische toepassing®!, met het kenmerk, dat het keramisch materiaal door middel -van een compressie-orgaan onder 5 compressie wordt gehouden.
2. Implantaatmateriaal volgens conclusie 1, met het kenmerk, dat het compressie-orgaan een centraal in het implantaatmateriaal gelegen staaf omvat.
3. Impantaatmateriaal volgens conclusie 2, met het kenmerk, dat de 10 staaf aan beide uiteinden voorzien is van een schroefdraad, waarop moeren zijn geschroefd tot over een zodanige afstand, dat de door de moeren op het keramisch materiaal uitgeoefende compressiekracht de gewenste waarde heeft. U. Implantaatmateriaal volgens conclusie 3, met het kenmerk, dat 15 zich. tussen de moeren en het keramisch materiaal ringetjes bevinden die de uitgeoefende compressiekracht verdelen.
5. Implantaatmateriaal volgens conclusie 2, met het kenmerk,dat de staaf aan beide uiteinden voorzien is van soldeer, dat als gevolg van een smelt-verhittingsbehandeling en daaropvolgende stol-afkoelingsbe-20 handeling door de krimp van de staaf tegen het keramisch materiaal is getrokken.
