Abstract

Inventia se refera la medicina, in special la stomatologie si poate fi aplicata pentru implantarea dentara prin acces transalveolar cu elevarea planseului sinusului maxilar. Esenta inventiei consta in aceea ca se efectueaza ortopantomograma, apoi cu ajutorul sublerului electronic de dantura se determina inaltimea osului rezidual, dupa care se selecteaza implantul, care penetreaza sinusul maxilar nu mai mult de 4 mm, se efectueaza examenul radiologic retroalveolar si se determina locul introducerii implantului, cu freza spada prin strapungerea gingiei fixe se foreaza osul fara decolarea lambourilor mucoperiostale, apoi locasul implantului se prepara cu freze de diametru diferit, in functie de densitatea osului si de diametrul implantului selectat forarea se finalizeaza cu o freza avand diametrul cu 0,5…1,5 mm mai mic decat diametrul implantului, se foreaza pana la contactul cu stratul cortical al planseului sinusului maxilar si aparitia senzatiei de vibratie, dupa care cu un osteotom concav cu diametrul la apex egal cu diametrul apexului implantului prin lovituri dozate cu ajutorul ciocanului se fractureaza planseul sinusului fara a condensa lateral osul rezidual, se apreciaza integritatea membranei sinuzale prin procedeul Valsalva, apoi dupa umplerea cavitatii formate cu sange se insurubeaza implantul cu cheia dinamometrica in asa mod ca partea lui superioara sa fie situata intraosos la 1…2 mm sub stratul cortical al apofizei alveolare, dupa care se efectueaza controlul radiologic.The invention refers to medicine, especially dentistry, and can be applied for dental implantation through transalveolar access with elevation of the floor of the maxillary sinus. The essence of the invention is that the orthopantomogram is performed, then the height of the residual bone is determined with the help of the electronic dental subler, after which the implant is selected, which penetrates the maxillary sinus no more than 4 mm, the retroalveolar radiological examination is performed and the place of insertion is determined the implant, with the sword drill by piercing the fixed gum, the bone is drilled without detaching the mucoperiosteal flaps, then the implant site is prepared with drills of different diameters, depending on the density of the bone and the diameter of the selected implant, the drilling is completed with a drill with a diameter of 0.5 ...1.5 mm smaller than the diameter of the implant, it is drilled until it comes into contact with the cortical layer of the floor of the maxillary sinus and the sensation of vibration appears, then with a concave osteotome with a diameter at the apex equal to the diameter of the apex of the implant by dosed blows with the help of a hammer the floor of the sinus is fractured without condensing the residual bone laterally, the integrity of the sinus membrane is assessed through the Valsalva procedure, then after filling the formed cavity with blood, the implant is screwed in with a torque wrench in such a way that its upper part is located intraosseously 1...2 mm below the cortical layer of the alveolar apophysis, after which the radiological control is performed.
