Description

The invention relates to medicine, in particular to dentistry, and can be used for hemostatic treatment after dental extractions in patients with various coagulopathies.

This, being sometimes severe, endangers the patient's life, even after minor surgical interventions. At present, the problem of hemostasis after tooth extraction remains as relevant, although multiple hemostatic preparations and methods are known.

The method of hemostasis after tooth extraction by applying supra- or intraalveolar compressive packing is known [1].

Ca rezultat, alveola devine incomplet protejată şi suferă formarea unui cheag stabil (hemoizolator perfect).[0004] The disadvantage of the known method is that the supraalveolar tamponade traditionally serves as an absorbable sponge that absorbs part of the blood clot. As a result, the alveolus becomes incompletely protected and undergoes the formation of a stable clot (perfect hemoisolator).

[0005] Este cunoscută metoda de hemostază după extracţia dentară prin aplicarea suturilor pe gingie [2].[0005] The method of hemostasis after tooth extraction by applying sutures to the gingiva is known [2].

[0006] Aplicarea suturilor uneori este inefectivă şi provoacă o traumă suplimentară a ţesuturilor, ceea ce duce la agravarea microcirculaţiei locale.[0006] The application of sutures is sometimes ineffective and causes additional tissue trauma, which leads to worsening of local microcirculation.

[0007] Este cunoscută metoda de hemostază după extracţia dentară la pacienţii cu diverse coagulopatii, care constă în aplicarea topică a cristalelor de permanganat de potasiu [3].[0007] The method of hemostasis after dental extraction in patients with various coagulopathies is known, which consists in the topical application of potassium permanganate crystals [3].

The disadvantage of the known method is that the use of potassium permanganate crystals must be considered non-biological, since even if it momentarily stops bleeding, through the consecutive necrotic lesions, which are caused by these crystals, it favors the occurrence of infections and even the recurrence of hemorrhage.

[0009] Este cunoscută metoda de hemostază după extracţia dentară la pacienţii cu diverse coagulopatii, care constă în administrarea vitaminei K [4].[0009] The method of hemostasis after dental extraction in patients with various coagulopathies is known, which consists in the administration of vitamin K [4].

[0010] Dezavantajul metodei menţionate constă în aceea că efectul după administrarea vitaminei K este îndelungat, peste 24…48 de ore, fiind totodată necesar de a administra topic un hemostatic.[0010] The disadvantage of the mentioned method is that the effect after administration of vitamin K is long, over 24…48 hours, and it is also necessary to administer a hemostatic topically.

The problem solved by the invention consists in developing an effective method of physiological hemostasis after dental extractions in patients with various coagulopathies, which would quickly stop bleeding and reduce the risk of posthemorrhagic complications with alteration of the general condition, which would lead to the formation of a stable clot with the reduction of repeated bleeding and at the same time would reduce the duration of hospitalization.

[0012] Esenţa invenţiei constă în aceea că după extracţia dentară plaga se irigă cu o soluţie antiseptică, apoi în alveolă se injectează 125…250 UI de trombină dizolvată în 1…2 ml de soluţie fiziologică.[0012] The essence of the invention is that after tooth extraction the wound is irrigated with an antiseptic solution, then 125…250 IU of thrombin dissolved in 1…2 ml of physiological solution is injected into the alveolus.

The result of the invention is that after administration of the mentioned solution, the bleeding is stopped quickly, in 5...7 min, does not require the administration of intramuscular or intravenous hemostatic preparations, the general condition is stabilized and with the prevention of early and late posthemorrhagic complications, and at the same time the hospitalization period is reduced.

Efectul hemostatic are loc timp de 5…7 minute.[0014] Thrombin solution, injected endoalveolarly, initiates the effect of thrombogenesis, using fibrinogen of the underlying tissues as the second component. Thrombin directly converts fibrinogen into fibrin, simultaneously activating factor XIII. The latter facilitates the interlacing and stabilization of fibrin filaments, which corresponds to the phase of transformation of soluble fibrin into insoluble. In addition, thrombin stimulates platelet aggregation, which, in turn, leads to the release of platelet factors, activators of the coagulation system. The hemostatic effect occurs for 5…7 minutes.

Therefore, the blood clotting process is ensured locally without the application of sutures, thus avoiding their disadvantages (additional tissue trauma, increased risk of recurrence of hemorrhage upon their removal).

[0016] Avantaje:[0016] Advantages:

[0017] - Este o metodă efectivă de oprire a hemoragiilor postextracţionale dentare fără aplicarea suturilor.[0017] - It is an effective method of stopping post-extraction bleeding without applying sutures.

[0018] - Este o metodă minimal invazivă, ce permite formarea trombului sangvin, favorizând vindecarea plăgii postextracţionale precoce.[0018] - It is a minimally invasive method, which allows the formation of a blood thrombus, favoring early post-extraction wound healing.

[0019] - Tehnica de efectuare este simplă.[0019] - The technique is simple.

[0020] - Poate fi efectuată de orice medic în policlinică sau în condiţii de staţionar.[0020] - It can be performed by any doctor in a polyclinic or inpatient setting.

[0021] - Nu necesită aparatură şi instrumentar deosebit.[0021] - It does not require special equipment and instruments.

[0022] - Nu este costisitoare.[0022] - It is not expensive.

[0023] - Durata scurtă a procedurii.[0023] - Short duration of the procedure.

[0024] - Acţionează local şi nu expune pacientul la complicaţii precoce şi tardive după administrare.[0024] - It acts locally and does not expose the patient to early and late complications after administration.

[0025] Metoda se efectuează în modul următor. The method is carried out in the following manner.

The patient is admitted and examined for dental extraction or after extractions performed until addressing. The place of the alveolus from where the tooth was extracted is examined, where the presence of an unstable clot with active bleeding at the time of examination can be determined. To ensure local hemostasis, lyophilized human thrombin, manufactured from human blood plasma, was used. The dry thrombin powder, released in vials in a dose of 125 IU or 250 IU, is dissolved immediately before use in 1…2 ml of sterile physiological solution. The solution obtained is injected after performing an irrigation with antiseptic solution with an insulin syringe into the alveolus, slowly and without pressure, through the space between the edge of the unretracted blood clot and the edge of the alveolus, or if hemostasis is performed immediately after extraction, the aforementioned solution is introduced directly into the alveolus.

[0027] Metoda a fost studiată pe un lot de 26 de pacienţi cu hemoragii postextracţionale dentare de diversă etiologie internaţi în secţia chirurgie OMF a CNŞPMU, Chişinău, în perioada aprilie 2008-aprilie 2009.[0027] The method was studied on a group of 26 patients with post-extraction dental bleeding of various etiologies admitted to the OMF surgery department of the National Center for Primary and Secondary Medical Education, Chisinau, between April 2008 and April 2009.

The method of ensuring hemostasis by endoalveolar injection of thrombin solution (125 IU or 250 IU in 1 or 2 ml of physiological solution) is effective, and through its advantages it avoids the shortcomings of traditional methods of stopping hemorrhage.

The proposed method takes into account the mechanisms of physiological hemostasis. The clinical effect of the method consists in ensuring local hemostasis with minimal trauma. The economic effect is highlighted by the lack of need for special equipment and instruments during the intervention; short duration of the procedure; does not expose the patient to complications.

[0030] Exemplul 1Example 1

[0031] Pacienta N., 54 de ani, a fost internată pe data 24.12.2008, la CNŞPMU s-a adresat de sine stătător cu următoarele acuze: prezenţa hemoragiei din cavitatea bucală după extracţia a doi dinţi, slăbiciune generală, ameţeli, lipsa poftei de mâncare.[0031] Patient N., 54 years old, was admitted on 24.12.2008, she addressed the CNŞPMU independently with the following complaints: presence of bleeding from the oral cavity after the extraction of two teeth, general weakness, dizziness, lack of appetite.

From the anamnesis - teeth 36 and 38 were extracted 30 hours before the address. After the extraction, supraalveolar compressive tamponade was applied and the patient went home. Although a compressive dressing was applied, the bleeding continued. The patient had to seek medical help. In February 2005, she underwent heart surgery (mitral valve replacement), after which she receives Trombostop (2.5 mg/day). The effect of the oral anticoagulant is monitored monthly by determining the INR values, maintaining them within the limits of 2.0...3.0.

Regional lymph nodes are not palpable. The mouth opens freely. The endooral examination reveals the presence of saliva mixed with blood, a blood clot has formed in the alveoli of teeth 36, 38 without a tendency to adhere to the edge of the alveolus. The marginal gingiva at the level of the extracted teeth is slightly damaged. Palpation of both sides of the alveolar processes of teeth 36 and 38 is painful. The orthopantomographic examination revealed root remains, no granulomas were detected. Determination of hemodynamic indices: BP = 130/90 mm Hg, pulse = 80 b/min. The patient was consulted by the internist. The results of the emergency laboratory tests: Hb = 147 g/l; Er = 4.3x1012/l; Ht = 0.41; L = 8.3x109/l. Duke bleeding time = 4 min, Lee-White clotting time = 11 min.

Medicaţie anticoagulantă (Trombostop).[0034] Based on the clinical and paraclinical examination, the diagnosis was established: Post-extraction hemorrhage from the alveolus of teeth 36 and 38. Condition after mitral valve prosthesis (2005). Anticoagulant medication (Trombostop).

Medicaţia anticoagulantă (Trombostop) nu a fost anulată.[0035] The patient was urgently admitted to the OMF surgery department with a general condition of moderate severity. Anticoagulant medication (Trombostop) was not canceled.

In dynamics, signs of recurrence of hemorrhage were not determined.

[0037] Cazul clinic descris denotă ineficacitatea aplicării tamponamentului compresiv supraalveolar cu scop hemostatic. Hemoragia postextracţională dentară a contribuit la alterarea stării generale şi scăderea capacităţii de muncă.[0037] The described clinical case demonstrates the ineffectiveness of applying supraalveolar compression packing for hemostatic purposes. Post-extraction dental hemorrhage contributed to the deterioration of the general condition and decreased work capacity.

[0038] Exemplul 2Example 2

Din 2004 suferă de hepatită virală cronică B.[0040] From the anamnesis - teeth 16 and 35 were extracted 3 days before the address. After the extraction, sutures and supraalveolar compressive tamponade were applied, and the patient was discharged home. Although local hemostatic measures were applied, the bleeding continued. The patient repeatedly contacted the doctor who performed the dental extraction, who resorted to the local application of potassium permanganate crystals. These hemostatic methods were ineffective, and the patient had to go to the emergency hospital for medical assistance. Since 2004, she has been suffering from chronic viral hepatitis B.

The marginal gingiva at the level of the extracted teeth was slightly damaged. During the orthopantomographic examination, no root remains were detected. Determination of hemodynamic indices: BP = 120/80 mm Hg, pulse = 74 b/min. Results of emergency laboratory tests: Hb = 120 g/l; Er = 3.7x1012/l; Ht = 0.35; L = 4.1x109/l. Duke bleeding time = 5 min, Lee-White clotting time = 9 min.

[0042] Analiza generală a sângelui (13.02.09): Hb = 119 g/l; Er = 3,8x1012/l; Ht = 0,33; tromb = 54x109/l; Lc. = 4,0x109/l; N. = 6%; S. = 79%; Lf. = 9%; Mn. = 6%; VSH = 11 mm/h.[0042] General blood analysis (13.02.09): Hb = 119 g/l; Er = 3.8x1012/l; Ht = 0.33; thrombus = 54x109/l; Lc. = 4.0x109/l; N. = 6%; S. = 79%; Lf. = 9%; Mn. = 6%; ESR = 11 mm/h.

Hepatită virală cronică B. Trombocitopenie.[0043] Based on the clinical and paraclinical examination, the diagnosis was established: Post-extraction hemorrhage from the alveolus of teeth 16 and 35. Chronic viral hepatitis B. Thrombocytopenia.

In dynamics, signs of hemorrhage recurrence were not determined.

Aceste procedee de asigurare a hemostazei locale au fost inefective şi nocive, constituind o traumă suplimentară a ţesuturilor.[0045] Summarizing this clinical case, we must emphasize the ineffectiveness of applying sutures, supraalveolar compressive tamponade, potassium permanganate crystals for hemostatic purposes. These procedures for ensuring local hemostasis were ineffective and harmful, constituting an additional trauma to the tissues.
