Abstract

The invention relates to medicine, in particular to gastroenterology and can be used for the treatment of gastroduodenal ulcers.Summary consists in the use of water-soluble oenotannin, obtained by oxidation of oenotannin with hydrogen peroxide, for the treatment of gastric and duodenal ulcer.The result of the invention is to obtain a more effective drug for the treatment of gastroduodenal ulcers with the use of substances of natural origin with high biological activity.

Invenţia se referă la medicină, în special la gastroenterologie şi poate fi utilizată pentru tratamentul ulcerelor gastroduodenale. The invention relates to medicine, in particular to gastroenterology and can be used for the treatment of gastroduodenal ulcers.

În tratamentul ulcerelor gastroduodenale se utilizează frecvent substanţe astringente, regeneratoare şi antiinflamatoare din diverse grupe, o largă răspândire şi o eficacitate înaltă având în special substanţele tanante [1]. In the treatment of gastroduodenal ulcers, astringent, regenerating and anti-inflammatory substances from various groups are frequently used, with tanning substances being particularly widespread and highly effective [1].

Dezavantajul este că majoritatea plantelor cu conţinut ridicat de taninuri cresc în pădurile tropicale, astfel materia primă este inaccesibilă, costisitoare şi procedeele de obţinere sunt scumpe. Datorită obţinerii enotaninului hidrosolubil din seminţe de struguri prin oxidarea enotaninului, putem beneficia de un remediu cu proprietăţi astringente, antiinflamatoare şi regeneratoare ridicate. Avantajele invenţiei constau în utilizarea unui remediu de origine vegetală cu activitate biologică înaltă, obţinut din materie primă autohtonă - deşeuri ale industriei vinicole, în calitate de substanţă cu activitate antiulceroasă, la un preţ accesibil. Enotaninul hidrosolubil şi-a dovedit eficacitatea în studiile efectuate pe plăgi şi arsuri, ca fiind un preparat regenerator efectiv şi cu o activitate antibacteriană înaltă. Water-soluble enotanin has proven its effectiveness in studies conducted on wounds and burns, as an effective regenerative preparation and with high antibacterial activity. Experimentele au fost efectuate pe 20 şobolani albi cu masa corporală cuprinsă între 180...220 g. Modelarea ulcerelor a fost efectuată conform metodei (Derelanko M.G.), prin administrarea unei singure doze intragastrale de indometacină (20 mg/kg). Studiul a fost efectuat pe 2 loturi, la care s-a administrat ca tratament enotaninul hidrosolubil şi soluţie fiziologică (lotul martor). Durata tratamentului a constituit 5 zile depline începând cu a doua zi după administrarea indometacinei. The experiments were performed on 20 white rats with a body mass between 180...220 g. Ulcer modeling was performed according to the method (Derelanko M.G.), by administering a single intragastric dose of indomethacin (20 mg/kg). Stabilirea numărului şi a suprafeţei ulcerelor s-a efectuat după sacrificarea animalelor prin decapitare, cu izolarea ulterioară a stomacului, fiind preventiv anesteziaţi cu eter. Pentru a stabili starea iniţială, s-au sacrificat câte doi şobolani din fiecare lot peste 24 ore, iar pentru a stabili eficacitatea tratamentului timp de 5 zile - la a 6-a zi. Macroscopic s-a determinat numărul de ulcere, suprafaţa şi procentul şobolanilor cu ulcer. Activitatea terapeutică antiulceroasă s-a apreciat comparativ cu lotul martor prin determinarea indicelui ulceros. Evoluţia cea mai benefică a ulcerului a fost determinată în lotul cu administrarea enotaninului, care a fost statistic veridică comparativ cu lotul martor. Utilizarea enotaninului hidrosolubil reduce suprafaţa şi numărul ulcerelor până la 2,2 ± 0,45 mm2 contribuind la o vindecare mai rapidă şi efectivă. Analiza datelor experimentale obţinute a constatat o scădere cu 50% a suprafeţei ulceroase şi de aproximativ două ori a numărului leziunilor stomacale comparativ cu lotul martor, ceea ce ne demonstrează înalta activitate antiulceroasă a enotaninului hidrosolubil. De asemenea, ca urmare a coagulării proteinelor are loc oprirea hemoragiilor capilare din leziunile ulceroase. Un alt efect este cel antiinflamator care se manifestă în faza acută a inflamaţiei prin scăderea răspunsului inflamator, inhibarea infiltraţiei celulare, ameliorarea stresului oxidativ şi micşorarea producerii mediatorilor inflamaţiei. Studiul efectuat în laboratorul şi catedra universităţii stabileşte şi confirmă că enotaninul hidrosolubil contribuie la stimularea regenerării leziunilor mucoasei gastrice exercitând o acţiune antiulceroasă semnificativă şi cu o înaltă siguranţă de utilizare. Astfel el poate fi recomandat în schemele de tratament la pacienţii cu ulcer gastroduodenal şi alte afecţiuni gastroenterologice ulcerative, precum şi în gastrite. The study conducted in the university laboratory and department establishes and confirms that water-soluble enotanin contributes to the stimulation of the regeneration of gastric mucosal lesions, exerting a significant antiulcer action and with a high safety of use. Thus, it can be recommended in treatment regimens for patients with gastroduodenal ulcer and other ulcerative gastroenterological diseases, as well as gastritis.
