Abstract
항문회음 병변(APL)은 크론병에서 흔히 발생하며, 유도된 조직 파괴 및 재발로 인하여 치료하기가 특히 어렵다. 따라서, 본 발명자들은 새로운 치료법을 시험하고 최적화하도록 하는 직장의 병리학적 염증이 있는 항문주위 누공의 최초 전임상 모델을 개발하였다. 그리고, 본 발명자의 목표는 항문주위 누공성 크론병의 래트(rat) 모델에서 항문주위 누공 치료를 위한 산화철 나노입자 용액을 사용하여 전임상 연구를 수행하는 것이다. 이는 치료된 모든 염증성 누공이 산화철 나노입자 용액으로 영구적으로 채워지거나 닫히는 것을 나타내었다 (1일 내지 7일). 따라서 나노입자의 사용은 항문주위 누공성 크론병의 촉망되는 새로운 치료법으로 여겨진다.

크론병(crohn's disease; CD)은 염증성 창자 질환(inflammatory bowel disease; IBD)의 그룹에 속하고, 이는 주요 염증 부위가 소장, 결장, 항문, 및 회음부이다. 항문회음 병변(anoperineal lesion; APL)은 빈번하고 유도된 조직 파괴 및 이의 재발로 인하여 치료하기가 특히 어렵다. APL은 비-누공성 병변(non-fistulizing lesion)(피부 태그(skin tag), 열창(fissure), 궤양(ulcer), 항문직장 협착(anorectal stricture) 및 항문 암(anal cancer))과 누공성 병변(fistulizing lesion)(단순 또는 복합 회음부 누공(simple or complex perineal fistulas), 직장질 누공(rectovaginal fistulas) +/- 농양(abscessed))으로 분류된다[1]. 누공성 항문회음 병변(FAPL)은 항문 괄약근 파괴, 요실금, 영구 장루 및 항문 암의 장기적인 위험이 있는 가장 심각한 병변이다. FAPL의 관리는 복잡하고 다양하므로, 위장병 전문의, 방사선 전문의 및 외과 의사의 보완적인 전문 지식이 요구된다. 환자의 2/3 이상이 FAPL과 관련된 농양을 갖고 있으므로 치료는 먼저 검사의 수술 단계, 농양 배액 및 누공관을 통한 배액관의 삽입을 포함한다. 두 번째 단계는 질환-관련 염증을 제어하기 위해 TNF 알파 길항제를 사용하는 의학적 치료이다. 누공관을 폐쇄하는 것을 목표로 하는 최종 수술 단계는 농양이 동반되지 않고 의학적으로 제어된 직장염(proctitis)이 있는 환자에서 고려될 수 있다[2]. 누공관을 폐쇄하기 위한 수개의 수출 절차가 이용가능하다: 피브린 글루, 만성 세톤(chronic seton), 직장내 전진 플랩, 플러그, LIFT 및 VAAFT[2,3]. FAPL은 환자, 예를 들면, 제어되지 않는 질환, 전환 장루(diverting stoma)를 지니거나 또는 직장 절제술(proctectomy)을 한 환자의 약 2/3에서 재발하여 수술 절차의 반복을 초래한다. 최근, 중간엽 줄기세포의 국소 주입은 누공 폐쇄와 관련하여 흥미롭고 유망한 결과를 나타내었다[4,5]. 그러나 세포 기반 치료법에는 많은 제한이 존재한다: 줄기 세포의 유형, 조직 기원, 세포의 농도, 줄기세포의 운명, 생산지와 병원 사이의 물류 협동, 치료의 가용성. 따라서, 신규하고 보다 제거가능한 치료가 요구되고 있다.
최근, 산화철 나노입자를 이용한 재생 의약에 대한 유망한 접근법이 제안되었다[7]. 저자는 나노브리징(nanobridging)(수성 나노입자 용액에 의한 접착)을 생체 내(in vivo)에서 사용하여 피부와 간(liver) 내 깊은 상처를 신속하고 강력하게 봉합하고 치유할 수 있음을 입증하고 있다.
본 발명은 청구범위에 의해 한정된다. 특히, 본 발명은 누공성 항문회음 병변의 치료를 위한 나노입자의 용도에 관한 것이다.
본 발명자들은 새로운 치료법을 시험하고 최적화하도록 하는 직장의 병리학적 염증이 있는 항문주위 누공의 최초 전임상 모델을 개발하였다. 이후에, 본 발명자의 목표는 항문주위 누공성 크론병의 래트(rat) 모델에서 항문주위 누공 치료를 위한 산화철 나노입자 용액을 사용하여 전임상 연구를 수행하는 것이었다. 이는 치료된 모든 염증성 누공이 산화철 나노입자 용액으로 영구적으로 채워지거나 닫히는 것을 나타내었다(1일 내지 7일). 따라서 나노입자의 사용은 항문주위 누공성 크론질환의 촉망되는 새로운 치료로 여겨진다.

실시예:
물질 및 방법
연구 설계 및 동물
항문 주위 누공 치료를 위해 USPIO를 시험하는 전임상 연구를 본 발명자의 연구실에서 개발한 항문 주위 누공성 크론병의 래트 모델에서 수행하였다[6]. 이러한 실험 모델은 직장의 병리학적 염증을 동반한 항문주위 누공을 발생시키며, 7일 이상 후에도 지속적인 누공관을 특징으로 한다. 래트에 의한 2개의 누공을 10마리의 래트의 그룹에서 생성시켜 각각의 래트가 이의 자체의 대조군이 되도록 하였다. 염증성 누공 형성의 주요 단계는 도 1 에 요약한다. 동물은 42일 내지 90일령의 수컷 스프라그 다울리(Sprague Dawley) 래트였다. 이는 절차 전날과 당일을 포함하여, 조사된 사료 및 임의로 경구로 수분을 공급한 전통적인 지역에서 사육하고 수용하였다. 2마리 이하의 래트를 동일한 우리에 수용하였다. 각각의 절차 당일과 그 후 7일마다 래트의 체중을 측정하였다. 모든 실험은 유럽 공동체 지침(Eurepean Community guideline)에 따라 수행되었고 기관 동물 관리 및 사용 위원회(Institutional Animal Care and Use Committee)(번호. APAFIS#23031-2019112613522589v5 Paris Nord 윤리 위원회 및 프랑스 연구부)의 승인을 받았다.
모든 절차는 동일한 마취 및 진통제 프로토콜에 따라 수행하였다. 동물을 이소플루란 흡입(유도 동안 이소플루란 3 L/분 + O2 2 L/분, 이어서 이소플루란 1.5 L/분 + O2 2 L/분)으로 마취시켰다. 진통은 20% 리도카인 염산염(1mg/kg)의 국소 주사와 및 부프레노르핀(0.05mg/kg/래트)의 복부 피하 주사에 의해 수행하였다. 수술 후, 동물이 통증-관련 스트레스의 징후를 나타내는 경우, 사용된 진통제는 동일한 투여량의 부프레노르핀이었다.
항문주위 누공성 크론병의 전임상 모델
- 0일째, 직장염 유도: 직장염을 TNBS 용액(2.4.6-트리니트로벤젠술폰산 용액, 피크릴술폰산, SIGMA 실험실)을 함유하는 500μL의 직장 관장으로 유도하였다. TNBS 용액은 85.3μL의 TNBS(즉, 25mg), 250μL의 100% 에탄올 및 164.7μL의 식염수 용액을 함유하였다. 직장 관장은 적어도 1분 동안 유지하였다. 직장 염증 피크는 관장 후 5일에서 7일 사이에 도달하였다.
- 7일째, 누공 형성: 직장 염증의 피크에서, 2개의 누공이 각각의 래트에서 생성되었다. 래트를 누운 자세로 두고 이러한 위치를 기준으로 누공(12시 방향의 음낭, 6시 방향의 꼬리)을 위치시켰다. 수술용 실(Vicryl® 1, ETHICON Laboratory)을 직장(내부 구멍)에 삽입하고 항문 가장자리(외부 구멍)에서 약 1cm 떨어진 회음부로 빠져나와 3시와 9시 방향에 누공을 생성시켰다. 좋은 구경을 수득하기 위해, 누공관을 수술용 실로 날카롭게 하였다. 18G의 무딘 충전 바늘을 누공뿐만 아니라 10μL의 필터 팁에 통과시켰다. 본 발명자는 직경이 대략 2mm인 내부 및 외부 구성을 수득하였다. 수술의 종료시에, 실을 누공관에 유지시키고 100 μL의 TNBS 용액을 각각의 관에 주입하였다. TNBS 용액에는 100μL 주입(instillation)의 경우 17.06μL의 TNBS(즉, 5mg), 50μL의 순수한 에탄올 및 32.94μL의 식염수 용액을 함유하였다. 이러한 농도는 직장 관장액과 동일한 농도이었다.
- 8일 내지 34일까지, 위치내 실의 모니터링: 실을 28일 동안 유지시켰다. 이 기간 동안, 래트를 1 내지 2일마다 시험하여 누공이 잘 견디고 실이 빠지지 않는지를 확인하였다. 실이 손실된 경우, 짧은 마취 동안 당일에 이를 재삽입하였다. 주 당 2회, 동일한 TNBS 용액 100 μL를 각각의 누공관에 주입하였다.
- 35일째, 기준선 MRI: 28일 동안 실을 유지한 후, 2개의 염증성 누공을 수득하고 치료 전 경로를 평가하기 위해 회음부 MRI를 수행하였다.
산화철 나노입자의 수용액을 사용한 치료
USPIO는 산화철 Fe3O4 초소형 초상자성 산화철 나노입자(USPIO-NP)로 구성되었다. USPIO-NP를 마이크로웨이브 비-수성 졸-겔 방법을 사용하여 합성하였다[12]. 산화철 나노입자 표면을 기능성화되도록 함으로써 생리학적 조건 내에서 안정성과 생체-적합성을 수득하였다. 여기서 USPIO-NP 표면을 시트레이트 리간드로 기능화되어 pH 7 및 고 농도(30 중량%)에서 안정한 USPIO 현탁액을 생성하였다. TEM으로 측정된, 자기 코어 크기는 9.0 ± 2.2 nm( 도 2a )이며 51 ± 2 emu/gUSPIO의 포화 자화로 초상자성 거동을 나타내어 강력한 T2 MRI 조영제를 생성하였다[12]. 시트레이트 코팅은 푸리에 변환 적외선(FTIR) 측정(도 2b,c)과 열중량 분석(thermogravimetric analysis)을 통해 정성적 및 정량적으로 평가하여 나노입자당 350개의 시트레이트 분자를 생성시켰다.
기준선 MRI(35일) 후, 수술용 실을 제거하고 치료할 누공관을 무작위로 뽑았다. 300mg/ml에서 USPIO 용액 2μL를 누공관에 직접 주사하였다. 관을 폐쇄하기 위하여, 회음부에 외부 압력을 1분간 유지하였다. 대조군 누공관은 2 μL의 식염수 용액으로 처리하였다.
후속 조치 및 누공 평가 방법
치료 후 후속 조치는 도 3 에 요약되어 있다. 래트는 수술일 후(post-operative day; POD) 1, POD 4 및 POD 7일에 평가되었습니다. 각 평가를 위해, 래트를 전신 마취하에 임상적으로 검사하고 회음부 MRI를 시행하였다.
임상 검사의 주요 사항은 CDAI 및 PDAI 점수에 따라 정의되었다: 거동, 외관, 직장 출혈, 대변 농도, 체중 및 체중 감소, 및 회음부의 팽윤(swelling)/경화(induration)/침윤(infiltration). 누공의 임상적 진단은 외부 구멍 및/또는 내부 구멍의 존재로 정의하였다. 직장염의 진단은 항문-직장 염증(ano-rectal inflammation) 및/또는 혈성 설사(bloody diarrhea)의 존재로 정의하였다.
회음부 MRI를 소형 동물용 7T MRI 시스템(Bio Spec, Bruker BioSpin, Ettlingen, 독일)에서 수행하였다. 이소플루란으로 마취된 동물을 엎드린 자세로 MRI 크래들(MRI cradle) 위치시키고 다리를 먼저 터널에 삽입하였다. 등중심(isocenter)을 누공의 외부 구멍으로 추정되는 위치(항문에서 약 0.5cm)에 위치시켰다. 형태학적 축상 T1 및 T2-강조 영상을 항문으로부터 시작하여 30mm를 덮는, 지방 포화도를 사용하여 획득하였다. 축방향 초-단 반향 시간(axial ultra-short echo time; UTE) 영상을 동일한 영역에서 획득하였다. 마지막으로, 상이한 b 값(0, 150, 400 및 800 s/mm2)과 지방 포화도를 사용하여, 3개의 직교 방향을 따라 확산 강조 영상화(diffusion-weighted imaging; DWI)을 수행하여 누공관을 중심으로 11개의 슬라이스를 획득하였다. 획득 매개변수는 표 1 에 자세히 설명되어 있다. 영상 해석 및 매개변수 측정을 누공 그룹 및 병리학적 결과에 대해 블라인드 처리된 복부 영상의학 전문의(IBD 경력 14년)와 연구 엔지니어(전-임상 MRI 경력 16년)에 의해 수행하였다. 각각의 래트는 4번의 MRI 세션을 겪었다: 제1 세션은 치료의 투여 전 수행하였다. 다른 세션은 치료 후 1일, 4일, 7일째에 수행하였다. 제1 세션으로부터의 T2-강조 영상을 사용하여 전체 직장염 점수(global proctitis score; GPS)를 계산하였다. 요약하면, T2-강조 영상에서 고강도 신호로 보이는 염증 위치 및 정도를 방사선 전문의가 평가하였다. 수술용 실 제거 후 잔여 누공의 지속성은 이의 가시성(방사선 전문의가 본 관의 퍼센트), 이의 최대 직경(mm), 및 제1 MRI 세션 시간에 평가된 동일한 매개변수와 비교된 외부 및 내부 구멍의 존재를 기준으로, 각각의 치료 후 MRI 세션 동안 평가하였다. 이러한 평가는 우선적으로 T2-강조 영상을 사용하여 수행하였며 필요한 경우 UTE 영상(즉, T2-강조 영상의 흐린 영역, 투여된 치료에 존재하는 철 나노입자로 인한 감수성 인공물)을 사용하여 완료하였다. 누공관과 관련된 조직 변화는 T2-강조 영상(근육 신호 강도로 표준화된, 임의 단위[a.u.])에서의 신호 강도 및 겉보기 확산 계수(ADC, 10-3mm2/s)를 측정하여 평가하였다. 관심 영역을 3개의 연속 슬라이스, T2-강조 영상, 또는 b0 DW 영상의 관 주위에 직접 그렸다. DWI의 경우, 추적 영상을 각각의 b 값에 대해 계산한 다음, ADP 맵(map)을 신호 강도를 단일지수로 표현하는 픽셀별 방식(pixel-by-pixel manner)을 사용하여 계산하였다.
임상 검사 후 및 전신 마취 하의 MRI 후 POD 7에서, 흉골 절제술 동안 심장 적출을 통해 모든 래트를 안락사시켰다. 누공관과 항문직장 접합부의 단일블록 절제(monobloc resection)을 얻기 위하여, 회음 접근법을 통해 복회음 절제를 수행하였다. 해부는 누공관을 운반하는 회음부의 원주 절편(section)을 사용하여 아래에서 위로 이루어졌다. 직장을 골반기저근에서 항문으로부터 약 3cm 정도 절개하였다. 누공을 분리하기 위해 수술 부분을 반으로 절단하였다. 조직 샘플을 포름알데히드(공식 용액, 중성 완충액, 10%, Sigma laboratory) 속에 24시간 동안 고정시키고 4℃에서 보관하였다. 이후에, 표본을 방향을 정확하게 맞추고 파라핀에 포함시켰다. 수 μm 두께의 단면을 누공관을 따라 마이크로톰(microtome)으로 절단하였다. 각각의 샘플에 대해, 1개의 절편을 헤마테인 에오신으로 염색하고, 5개의 절편을 면역조직화학적 분석을 위해 제조하였다. 누공관의 진단은 다음의 조직학적 기준: 직장 점막의 내부 구멍에서 회음부 피부의 외부 구멍까지 내강이 있는 관, 직장염의 병리학적 징후(B 및 T 림프구, 호중구, 대식구, 육아종), 누공관의 성질은 섬유증(fibrosis), 상피화(epithelialization)의 존재를 기준으로 하였고, 급성 또는 만성 염증(acute or chronic inflammation)의 유무에 따라 정의하였다.
결과
1차 결과는 임상 검사, MRI로 평가하고 병리학적 실험으로 확인한 ASION에 의한 외부 또는 내부 개구부를 포함한 누공관의 충전 또는 폐쇄이었다. 2차 결과는 누공관 염증의 진화, 제어된 누공 분석을 통한 모델 재평가 및 ASION의 안전성이었다.
통계적 분석
정량적 데이타는 평균 ± 표준 편차(범위)로 나타낸다. 정성적 데이타는 빈도 및 퍼센트로 보고한다.
결과
전임상 연구 기간은 42일이었다. 본 발명자는 20마리의 래트를 연구하고 각 래트에 대해 2개의 누공을 만들었다. 모든 래트는 TNBS 관장 후 7일째에 묽은 변과 회음부 종양을 겪었다. 실이 28일 동안 유지되는 동안 실이 존재하는지 확인하기 위해 모든 래트를 평균 8회(최소 6회 내지 최대 11회) 마취하고, 각각의 누공관에 100 μL의 TNBS 용액을 주입하기 위해 주당 2회 마취하였다. 이 기간 동안 새로운 실을 평균 6회(최소 0회 내지 최대 11회) 재삽입하였다. 단 1 마리의 래트 만이 28일 동안 동일한 실을 유지하였다. 이 단계 동안 12마리의 래트에서 농양(abscess)이 발달하였고 약 1주 내에 치유되었다. 여전히 이 단계 동안, 3마리의 래트가 2개의 실 중 1개를 찢어서 누공절개술(fistulotomy)을 유발하였다. 누공관이 1개만 있는 래트 3마리는 전체 분석에서 제외하였다. 이 중 2마리는 USPIO로 처리하였고 1마리는 식염수로 처리하였다. USPIO 정지의 장기적인 운명을 평가하기 위해 이를 POD 14까지 생존시켰다. 주요 분석을 구성하는 17마리의 래트에 대한 치료 전 누공의 특성은 표 2 에 요약한다. 모든 래트는 임상적 및 방사선학적 평가에서, 치료 전 2개의 염증성 누공을 나타내었다. 15마리의 래트는 1 등급의 만성 직장염이 있었고, 2마리의 래트는 치료 전에 직장염이 없었다. 평균 MRI로 계산된 직경은 치료된 누공의 경우 2.2mm(± 0.6[1.6 - 3.3])이고 대조군 누공의 경우 1.9mm(± 0.3[1.5 - 2.3])이었다. 표 5 에 자세히 설명된 바와 같이, 제외된 3마리의 래트에 대해서도 유사한 결과가 관찰되었다.
누공 제어
표 3 및 표 4 에 제시된 바와 같이, 대조군 누공은 시간이 지남에 따라 자연 치유 과정이 낮고 열악하였다. POD 7에서, 단지 4개의 관이 내부 구멍이 닫혀 있었다. 12개의 누공이 여전히 이의 관의 50% 이상에서 가시적이었다. 7개의 대조군 누공 중 총 6개가 여전히 가시적이거나 또는 POD 7에서 내부 구멍이 열려 있었다. 단 1개의 대조군 누공관 만이 POD 7에서 주로 폐쇄되었다. 그러나 모든 대조군 누공에는 POD 7까지 말초 관 염증이 있었다. 이러한 결과는 항문주위 누공성 크론병 모델의 개발을 위해 이전에 발표된 것과 전체적으로 유사하다[6].
산화철 나노입자 수용액으로 치료한 누공
표 3 에 제시된 바와 같이, 치료된 누공에 대한 임상 검사는 모든 외부 및 내부 구멍이 POD 7에서 USPIO에 의해 폐쇄되었음을 나타내었다. MRI 결과는 표 4 에 나타나 있다. MRI 평가는 모든 치료된 누공관이 USPIO로 POD 7까지 영구적으로 채워지거나 폐쇄되었음을 나타내었다. 모든 외부 및 내부 구멍은 POD 7까지 폐쇄되었다. 처리된 관 직경은 산화철 나노입자의 존재로 유발된 블루밍 효과(blooming effect)로 인해 MRI에 의해 과대평가되었다. 관찰된 직경은 시간이 지남에 따라 감소하는 것으로 보이며 이는 나노입자의 물질대사를 시사한다. 말초 관 염증은 나노입자의 인공물 때문에 측정할 수 없었다. 표 5 에 나타낸 바와 같이 USPIO를 사용한 유사한 결과가 POD 14까지 관찰되었다.
안전성
전임상 연구 전체에서, 래트는 죽지 않았으며 모든 래트는 체중 감소 없이 양호한 전반적인 상태를 유지하였다. USPIO 투여는 매우 단순하고 어떠한 침습적 절차도 요구하지 않았다. ASION으로 치료받은 래트 1마리는 POD 1에서 농양이 발생했으나 1주 내에 자연 치유되었다. 다른 부작용은 보고되지 않았다. 간은 일반적으로 산화철 나노입자를 제거하는 주요 기관이기 때문에[13,14], 본 발명자는 간 기능(트랜스아미나제, 알칼리성 포스파타제 및 빌리루빈)을 평가하고 POD 7에서 철 시험(iron test)을 실시하였다. 모든 생물학적 결과는 정상 범위 내이었다. 동일한 결과가 단일 관에서 ASION으로 치료된 래트 2마리의 POD 14에서도 관찰되었다.
결론
본 연구는 말초 염증이 있는 항문 주위 누공의 본 발명자의 전임상 모델이 치료를 시험하여 최적화하기 위해 재현가능하였음을 나타내었다. 치료된 모든 염증성 누공은 USPIO로 영구적으로 채워지거나 닫혔으나(1일부터 7일까지), 대조군에서는 14% 만이 1일째에 폐쇄되었다. 산화철 나노입자 용액은 항문주위 누공성 크론병의 유망한 새로운 치료법인 것으로 여겨진다.

- 제1항에 있어서,
환자는 염증성 창자 질환(inflammatory bowel disease)을 앓고 있는, 방법. - 제2항에 있어서,
환자는 크론병(Crohn's disease)을 앓고 있는, 방법. - 제1항에 있어서,
나노입자는 약 5, 10, 15 또는 20 nm의 크기를 가지는, 방법. - 제1항에 있어서,
나노입자는, 산화철(FeO, Fe2O3, Fe3O4), 산화세륨(CeO), 알루미나(Al2O3), 산화지르코늄(ZrO2), 산화티탄(TiO2), 티타네이트(BaTiO3, Ba0.5Sr0.5TiO3, SrTiO3), 산화인듐(In2O3), 산화주석(SnO2), 산화안티몬(Sb2O3), 산화마그네슘(MgO), 산화칼슘(CaO), 산화망간(Mn3O4, MnO2), 산화몰리브덴(MoO3), 실리카(SiO2), 산화 아연(ZnO), 산화 이트륨(Y2O3), 옥시염화비스무트, 산화 구리(CuO, Cu2O)와 같은 금속 산화물로 제조되는, 방법. - 제5항에 있어서,
