발명의 기술분야
본 명세서에 개시된 주제는 일반적으로 국소 치료 및 자외선 B ("UVB") 광선요법(phototherapy)의 조합을 백반증 병변을 갖는 환자에게 투여하는 단계를 포함하는 백반증 병변의 치료 방법에 관한 것이다. 본 명세서에 개시된 바와 같이, 본 발명의 발명자들은 협대역(narrowband) UVB 광선요법과 조합된 국소적으로 적용된 JAK 저해제 SHR0302가 얼굴 및 목의 백반증 병변을 포함하여 백반증 병변을 극적으로 재색소침착한다는 놀라운 발견을 하였다.
백반증은 세계 인구의 0.5% 내지 2%에 영향을 미치는 일반적인 자가면역 질환이다. 일반적으로 더 어두운 피부 유형을 가진 환자에서 더 두드러지지만, 백반증은 모든 인종과 남녀 모두에게 영향을 미친다. 백반증은 종종 시간 경과에 따라 크기가 증가하며, 더 많아지고, 더 큰 부위로 합쳐지는 경향이 있는 후천적인 작고 비대칭적인 병변으로 나타난다. 백반증은 신체의 어느 부위에서나 발생할 수 있지만, 일반적으로 얼굴이 영향을 받는다.
백반증은 건선 및 아토피성 피부염에서 볼 수 있는 것과 유사하게 환자의 삶의 질에 크게 영향을 미치는 상당한 질병 부담과 관련이 있다 (Linthorst 2009). 백반증은 얼굴 및 목 부위가 눈에 잘 띄며 위장(camouflage)하기 어려울 수 있기 때문에, 얼굴 및 목에 발생할 때 특히 귀찮고 보기 흉할 수 있다 (Elbuluk 2017). 수많은 연구에서 삶의 질 및 정신 건강이 얼굴 및 기타 눈에 보이는 부위에 백반증이 있는 환자에게 더 부정적인 영향을 미친다는 사실이 입증되었다 (Elbuluk 2017, Amer 2016, Eleftheraidou 2012). 삶의 질에 대한 얼굴/머리/목의 부정적인 영향은 질환의 정도와 무관한 것으로 보고되었다 (Bhandarkar 2012). 백반증의 영향은 전문적인 전망 및 관계에 영향을 미치면서, 다른 사람들에 의한 부정적인 치료로 확대될 수 있다. 치료와 관련하여, 환자 및 제공자의 설문조사 자료는 특히 얼굴 및 손의 재색소침착이 특히 중요하다고 확인하였다 (Eleftheriadou 2012).
현재 백반증의 재색소침착에 대해 FDA-승인된 국소 치료제는 없다. 환자의 삶의 질에 상당한 영향을 미치는 이 일반적인 질환을 더 잘 치료하기 위하여 백반증에 대한 신규한 화합물, 치료 전략, 및 병용 요법에 대한 미충족 욕구(unmet need)가 있다. 특히 얼굴 백반증은 환자에게 지대한 영향을 미치기 때문에 더 나은 치료법이 필요하다.
본 발명은 국소 치료 및 자외선 B ("UVB") 광선요법의 조합을 백반증 병변을 갖는 환자에게 투여하는 단계를 포함하는 백반증 병변의 치료 방법에 관한 것이다. 본 명세서에 개시된 바와 같이, 본 발명의 발명자들은 협대역 UVB 광선요법과 조합된 국소적으로 적용된 JAK 저해제 SHR0302가 얼굴 및 목의 백반증 병변을 포함하여 백반증 병변을 극적으로 재색소침착한다는 놀라운 발견을 하였다. 본 발명의 일 실시예에 따르면, 놀랍게도 협대역 UVB 광선요법과 조합된 국소적으로 적용된 JAK 저해제 SHR0302가 얼굴, 목, 팔뚝, 팔꿈치, 및 손의 백반증 병변을 포함하나 이에 한정되지 않는 백반증 병변을 극적으로 재색소침착한다는 사실을 발견하였다. 바람직한 실시예에서, 협대역 UVB 광선요법과 조합된 국소적으로 적용된 JAK 저해제 SHR0302는 얼굴 및 목의 백반증 병변을 재색소침착한다.
본 발명의 특정 실시예에서, 백반증의 치료를 필요로 하는 피험자에서 백반증의 치료 방법을 제공한다. 방법은 치료적 유효량의 SHR0302 또는 이의 염을 피험자에게 국소 투여하는 단계를 포함한다. 방법은 자외선 B 광선요법을 피험자에게 투여하는 단계를 더 포함한다.

특정 실시예에서, 백반증을 갖는 피험자는 UVB 광선요법을 투여받는다. 바람직한 실시예에서, UVB 광선요법은 협대역 UVB 광선요법이다. 자외선 B 광선요법은 1주에 1 내지 3회 피험자에게 투여될 수 있다.
본 발명의 방법은 얼굴, 목, 팔뚝, 및/또는 손의 백반증 병변을 갖는 환자의 치료를 포함하나 이에 한정되지 않는, 백반증 병변의 치료를 더 포함할 수 있다. 방법은 치료적 유효량의 SHR0302 또는 이의 염을 피험자에게 국소 투여하는 단계를 포함한다. 방법은 자외선 B 광선요법을 피험자에게 투여하는 단계를 더 포함한다. 본 명세서에 기재된 방법은 피험자의 백반증 병변을 재색소침착하는 단계를 포함하여, 백반증의 증상을 완화시킬 수 있다. 본 명세서에 기재된 방법은 피험자의 백반증 부위 및 중증도 지수 (vitiligo area and severity index, VASI)를 더 감소시킬 수 있다. 실시예 1

36-리터 스테인리스 스틸 스톡 포트에서, 5257.89 그램의 프로필렌 글리콜 (PG), 5253.18 그램의 폴리에틸렌 글리콜 200 (PEG 200), 279.57 그램의 카보머 공중합체 (PemulenTM TR-1) 및 528.17 그램의 카보머 동종중합체(Homopolymer) (Carbopol 974P)를 합하고 혼합하였다. 20-리터 스테인리스 스틸 스톡 포트 측면 용기에서, 8884.65 그램의 물을 17.48 그램의 에데테이트 이나트륨(Edetate Disodium), 이수화물 USP과 합하고 균일하게 혼합하였다. 이 20-리터 스테인리스 스틸 스톡 포트의 전체 내용물을 PG, PEG 200 및 2개의 카보머의 36-리터 스테인리스 스틸 스톡 포트에 일정하게 혼합하면서 첨가하였다. 그 다음, PG, PEG 200, 카보머 공중합체, 카보머 동종중합체, 물 및 에데테이트 이나트륨-이수화물을 앵커 혼합 장치가 장착된 50-리터 재킷형 주 반응 용기로 옮긴다.

실시예 4
실시예 1의 0.3% SHR0302 국소 크림 (ARQ-252 크림 0.3%)의 안전성 및 효능에 대한 개념 증명, 병렬군, 이중 맹검, 비히클-대조 연구는 비-분절형(non-segmental) 얼굴 백반증이 있는 피험자를 위해 설계되었다. 비-분절형 얼굴 백반증이 있는 피험자는 ARQ-252 0.3% 크림 BID 또는 비히클 크림 BID를 얼굴, 목, 손, 팔뚝, 및 팔꿈치의 모든 백반증 환부(affected areas)에 적용하였다. 제품을 모든 환부, 탈색 부위에 적용하였으며, 해부학적으로 가능한 경우, 치료는 경계에서 사방으로 약 3cm 확대되었다. 또한, 이러한 동일한 피험자들은 표준화된 광선요법 프로토콜에 따라 최대 3X/주까지 얼굴 전체와 목, 손, 팔뚝, 및 팔꿈치의 환부에 Daavlin Series 1 NBUVB Home Phototherapy Unit을 사용하였다. 한 환자는 Daavlin Series 1 NBUVB Home Phototherapy Unit을 이용하여 광선요법과 함께 1일 2회 ARQ-252 크림 0.3%를 적용하였다. 한 환자는 Daavlin Series 1 NBUVB Home Phototherapy Unit을 이용한 병용 광선요법에서 1일 2회 비히클 크림 (SHR0302 활성 성분이 물로 대체된 것을 제외하고 ARQ-252 크림과 동일한 조성)을 적용하였다.
2명의 환자는 얼굴 백반증 부위 및 중증도 지수 (F-VASI), 얼굴 체표면적 (F-BSA), 얼굴/목 BSA (F/N-BSA), BSA (손, 팔뚝, 팔꿈치), 얼굴/목 VASI (F/N-VASI), 팔뚝 및 팔꿈치의 VASI (FOREARM-VASI) 및 손만의 VASI (HAND-VASI)를 포함한 척도로 평가되었다. 부위 "얼굴"은 이마에서 원래 헤어라인까지, 뺨에서 턱선까지 수직으로 턱선까지 및 측면으로 입가에서 이주(tragus)까지의 부위를 포함하는 것으로 정의된다. 부위 "얼굴"은 입술, 두피, 귀, 또는 목의 표면적을 포함하지 않으나 눈꺼풀과 코를 포함할 것이다. VASI는 기준선에서 전체 백반증 패치 부위 및 시간 경과에 따라 이러한 패치 내의 황반 재색소침착 정도의 복합 추정치를 기반으로 하는 정량적 임상 도구이다. 치료 효능의 척도로서 F-VASI 기기의 신뢰성 및 타당성이 확인되었다 (Rosmarin, 2020). 백반증으로 침범된 BSA는 총 BSA의 백분율로 평가될 것이며, 가이드로서, 손바닥 + 5 자릿수(digits)를 이용하여 가장 가까운 0.01%로 결정될 것이고, 깍지 낀 손가락 및 옆으로 끼운 엄지손가락 (손자국)은 1% BSA로, 엄지손가락은 0.1% BSA로 결정될 것이다. 각 백반증 침범 부위에 대한 탈색 정도를 결정하고 하기의 백분율 중 가장 가까운 값으로 추정하였다: 0, 10%, 25%, 50%, 75%, 90%, 또는 100%. 100% 탈색에서는, 색소침착이 존재하지 않는다; 90%에서는, 색소침착의 얼룩이 존재한다; 75%에서는, 탈색 부위가 색소침착된 부위를 초과한다; 50%에서는, 탈색 부위 및 색소침착된 부위가 동일하다; 25%에서는, 색소침착된 부위가 탈색 부위를 초과한다; 10%에서는, 탈색의 얼룩만 존재한다. 그 다음, VASI는 백반증 침범에 대해 평가된 값에 얼굴의 각 부위에 대해 침범된 피부의 백분율을 곱하고 모든 부위의 값을 함께 합산하여 도출되었다. 백반증 침범의 백분율은 전체 연구 과정 동안 동일한 연구자에 의해 손 단위(hand units)로 추정되었다. 표 4에서 알 수 있는 바와 같이, JAK 저해제 SHR0302를 협대역 UVB 광선요법과 함께 국소 적용할 때, 백반증 병변은 협대역 UVB 광선요법과 조합된 비히클 크림 (SHR0302 없음)의 국소 치료에 비해 극적으로 재색소침착된다. 이 개선된 요법은 얼굴 및 목의 백반증 병변을 치료하는데 가장 효과적이었다.
