Abstract
인간 환자에서의 종양의 치료 방법이 개시된다. 상기 방법은 (a) 티보자닙 1.5 mg/일의 용량과 (b) 템시롤리무스 25 mg/주의 용량을 환자에게 공동-투여하는 것을 포함한다. 본 발명의 일부 실시양태에서, 티보자닙은 3주 동안 1일당 1회 용량으로 투여된 후, 1주 동안 투여되지 않는 반복되는 스케쥴에 따라 투여된다. 특히, 개시된 방법은 신장 세포 암종의 치료에 적합하다.

<본 발명의 배경>
티보자닙 (또한, AV-951 및 KRN951로도 알려짐)은 VEGF 수용체 1, 2 및 3의 효능있고 선택적인 소분자 억제제이다. 티보자닙은 모든 3개 수용체에 대해 피코몰 억제 활성을 나타내고, 전임상적 모델에서 항종양 활성을 나타낸다 (문헌 [Nakamura et al., 2006, Cancer Res. 66:9134-9142]). 티보자닙은 272명 환자의 2상 임상 시험 (문헌 [Bhargava et al., 2009, ASCO Annual Meeting Proceedings, Vol 27, No. 15s, Abstract No. 5032)에서 양성 중간 결과를 수득하였다. 1상 및 2상 임상 시험에서 티보자닙 치료와 연관된 가장 흔한 부작용은 고혈압 및 발성장애이다.

I. 정의

II. 본 발명의 방법
티보자닙과 템시롤리무스의 동시 투여는 개별 제형, 즉 티보자닙 제형과 템시롤리무스 제형의 동시 투여에 의해 가능하다. 본 발명에 관하여, 개별 제형의 투여 시기가 티보자닙과 템시롤리무스의 약리상 활성이 적시에 겹쳐지는 것이고, 이에 따라 환자에서 합쳐진 항암효과를 발휘한다면, 개별 제형을 "동시에" 투여한다. 본 발명의 취지상, 동시 투여 (즉, 조합 요법)는, 2개의 약물이 동시에 투여되는 것이나, 동일한 투여 경로에 의해 투여되는 것이나, 또는 환자의 신체에서 동일한 시간에 존재하는 것을 요구하지 않는다. 티보자닙과 템시롤리무스의 약리상 활성의 일시적인 겹침은 (2개의 약물 각각에 관하여) 투여량, 투여의 빈도 및 시기, 반감기 및 약동학을 비롯한 인자에 따라 달라질 것이다.
인간 신체에서의 티보자닙의 반감기는 3.8-4.7일의 범위에 있다. 티보자닙은 장기적인 투여 후, 그의 반감기를 기초하여 예상될 정도로 혈청에 축적된다. 혈청 수준은 약 2-3주 후에 정상 상태에 도달한다.
인간 신체에서의 템시롤리무스의 반감기는 대략 17시간이다. 템시롤리무스는 그의 반감기를 기초하여 예상되는 것보다 많이 축적되는 것으로 보이지는 않는다. 그러나, 템시롤리무스의 1차 대사물, 즉 시롤리무스 (라파마이신)는 mTOR 활성을 가지며, 이는 모화합물 반감기의 약 3배인 반감기를 갖는다. 따라서, 템시롤리무스는 템시롤리무스가 제거된 후, 생리학적인 효과를 발휘할 것으로 예상된다.

본 발명에 따라 티보자닙과 템시롤리무스가 동시에 투여되는 경우, 템시롤리무스의 투여량은 25 mg/주이다. 본 발명의 일부 실시양태에서, 시롤리무스 (라파마이신)의 약리상 동등한 용량이 템시롤리무스를 대체한다.
본 발명에 따라 티보자닙과 템시롤리무스가 동시에 투여되는 경우, 약물 조합물은 단일 제형으로 또는 개별 제형으로서 투여될 수 있다. 본 발명의 일부 실시양태에서, 티보자닙은 경구 정제 또는 캡슐로써 투여되고, 템시롤리무스는 정맥내 (IV) 주입으로써 투여된다. 본 발명의 바람직한 실시양태에서, 티보자닙은 1.5 mg의 캡슐로써 3주 동안 1 캡슐/일 투여된 후, 1주 휴약하는 반복되는 스케쥴에 따라 투여되는 한편, 25 mg의 템시롤리무스는 1회/주의 IV 주입으로써 투여된다.
본 발명의 방법은 고형 종양, 예컨대 신장 세포 암종, 결장직장 종양 및 신경내분비 종양의 다양한 유형을 치료하는데 적합하다. 특히, 본 발명은 신장 세포 암종을 치료하는데 적합하다.

실시예
>
본 발명은 하기 실시예에 의해 추가로 설명된다. 하기 실시예는 단지 예시목적을 위해 제공되며, 이는 어떠한 방식으로도 본 발명의 범위 또는 내용을 제한하는 것으로 해석되어서는 아니된다.
티보자닙과 템시롤리무스의 조합물을 1상 임상 시험에서 시험하였다. 사전에 VEGF-표적화 요법을 받지 않았거나 사전에 1회의 VEGF-표적화 요법에 반응을 보이는데에 실패한, 진행된 신장 세포 암종 (투명 세포 성분과 함께)을 가진 인간 환자가 등록하기 위한 지원자였다. 각 환자는 경구로 티보자닙 (3주 투약, 1주 휴약)과 정맥내로 템시롤리무스 (매주 1회)를 투여받았다. 이전의 시험에서, 4주 동안 투여된 후 2주 동안 중단된 경우 (4주 투약, 2주 휴약)이거나, 또는 3주 동안 투여된 후 1주 동안 중단된 경우 (3주 투약, 1주 휴약)에, 티보자닙의 안정성은 1.5 mg/일의 용량에서 확립되었다. 템시롤리무스의 승인된 용량은 매주 1회 25 mg이었다. 하기 용량 수준을 본 1상 연구에서 평가하였다.
16명의 환자 (16명의 남성 및 0명의 여성, 중위 연령 : 62세 (43-70세 범위), 카르노프스키(Karnofsky) 수행 상태 (KPS) : 100% (11 pts), 90% (4 pts) 또는 80% (2 pt))가 본 연구에 등록하였다. 12명의 환자 (75%)가 사전 VEGF 표적화 요법을 받았다. 치료의 중위 지속기간은 16.8주 (0.7-51.4주 범위)였다. 예비 데이터를 기초하여, 모든 용량 수준에 걸쳐 환자의 20% 이상에서 관찰된 치료-관련 이상 반응은 구내염/점막염 (38%), 혈소판 감소증 (31%), 설사 (31%), 피로/무력증 (19%), 고혈압 (19%), 발진 (10%), 구역 (19%) 및 구토 (19%)였다. 어떠한 용량-제한 독성 및 등급 4 독성도 발견되지 않았다. 본 1상 임상 시험에서, 조합물의 MTD는 티보자닙 1.5 mg/일과 템시롤리무스 25 mg/주이었다. 이용가능한 데이터는 두 약물 사이의 임의의 유의한 약동학적 상호작용의 결여를 나타내었다. 2명의 환자는 확인된 부분적 반응을 가졌다. 5명의 환자는 적어도 20주 동안 안정적 질병을 가졌다.

(2) 티보자닙은, 1.5 mg/일의 티보자닙과 25 mg/주의 템시롤리무스의 작용제 둘다의 충분한 용량에서, 템시롤리무스와 조합될 수 있는 제1 VEGF 티로신 키나제 억제제이다.
(3) 티보자닙과 템시롤리무스의 조합물은, 진행된 신장암을 위해 상업적으로 입수가능한 약물을 사용한 사전 요법에 실패한 환자를 비롯하여, 상기 질병을 갖는 환자에서 임상 활성을 고무시키는 것으로 나타난다.

- 제1항에 있어서, 종양이 신장 세포 암종, 결장직장 종양 및 신경내분비 종양으로 이루어진 군으로부터 선택되는 것인 방법.
