발명의 분야
본 발명은 연질 보형 이식물(soft prosthetic implant) 및 더욱 구체적으로는 그러한 이식물, 예를 들어, 유방 이식물의 텍스쳐 외표면(textured exterior surface)에 관한 것이다.

발명의 개요
본 발명은 사람에게 이식하기에 적합한 보형물, 예를 들어, 사람 유방의 재건 또는 확대에 유용한 유방 이식물을 제공한다. 보형물은 일반적으로 이식가능한 부재(implantable member), 예를 들어, 액체 또는 젤로 충전되어 있거나 충전될 수 있는 엘라스토머 쉘을 포함한다. 이식가능한 부재는 향상된 또는 제어된 조직 내부성장 또는 부착을 제공하도록 구성되거나, 위치하거나 또는 구조화된, 그 위에 정의된 하나 이상의 고정 영역(fixation region)을 포함하는 외표면을 갖는다.

상세한 설명
본 발명은 외부 표면에 고정 표면을 갖는, 염수- 또는 젤-충전된 연질 이식물 쉘, 바람직하게는 실리콘 엘라스토머 쉘을 제공한다. 그러한 연질 이식물에 대한 주요 응용은 여성 유방의 재건 또는 확대이다. 다른 보형물 응용은 다른 부위들 중에서도, 엉덩이, 고환, 또는 종아리를 위한 이식물이다.
"고정 표면" 또는 "고정 영역"이라는 용어는, 본 명세서에서 사용될 때, 일반적으로 신체/이식물 경계면에서 조직 내부성장 또는 부착을 촉진하도록 위치, 구조화, 또는 적응되는 이식물의 외표면의 영역 또는 부분을 말한다. 예를 들어, 고정 영역은 고정 영역과 동일한 정도로 조직 내부성장 또는 부착을 촉진하지는 않는 이식물의 인접 표면과 구별되는, 예를 들어, 더욱 두드러진 텍스쳐, 거칠음, 또는 광택을 가질 수 있다. 예를 들어, 이식물 외부의 다른 영역 또는 표면은 고정 영역에 비하여 상대적으로 매끈하거나 덜 텍스쳐화될 수 있다.

이식가능한 보형물을 위한 유연성 이식물 쉘을 형성하기 위한 한 가지 공정은 적합하게 형상화된 맨드렐(mandrel)을 실리콘 엘라스토머 분산물 중에 딥핑하는 것을 포함한다. 다수의 이러한 분산물이 본 기술 분야에서 사용된다. 기본적으로 분산물은 실리콘 엘라스토머 및 용매를 함유한다. 실리콘 엘라스토머는 전형적으로 폴리다이메틸실록산, 폴리다이페닐-실록산 또는 이들 두 가지의 일부 조합이다. 전형적인 용매는 자일렌 또는 1,1,1-트라이클로로에탄을 포함한다. 다양한 제조사가 분산물의 유형 및 양, 분산물의 점도 및 분산물의 고형물 함량을 달리한다. 그럼에도 불구하고, 본 발명은 매우 다양한 실리콘 고무 분산물을 사용하여 구성가능할 것으로 예상된다.

전체 실리콘 엘라스토머 쉘 구조체를 승온의 오븐에서 가황처리하거나 경화한다. 오븐의 온도를 바람직하게는 약 200 °F 내지 약 350 °F로, 바람직하게는 약 20분 내지 약 1시간의 경화시간, 40분의 경화시간 동안 유지한다. 오븐에서 꺼내어, 맨드렐/쉘 조립체를 고체 입자를 위한 용매에 넣고, 고체 입자를 용해시킨다. 용매는 실리콘 엘라스토머의 구조 또는 완결성에는 영향을 주지 않는다. 고체 입자가 용해되면, 조립체를 용매로부터 꺼내고 용매를 증발시킨다. 그 다음, 쉘을 맨드렐로부터 벗겨낸다. 이때, 쉘을 고체 입자를 위한 용매에 넣고 가볍게 흔들어서 모든 고체 입자가 확실하게 완전히 용해되도록 하는 것이 바람직하다. 쉘을 용매로부터 꺼내고, 용매를 증발시킨다.
고체 입자를 용해시키면 염이 있었던 쉘의 표면에 개방된, 상호연결된, 공극이 남는다.
상기한 단계에 따라 쉘을 가공한 후에, 완성된 유방 이식물 보형물을 제조하는 데 필요한 단계는 본 기술 분야에 알려진 것과 유사할 수 있다. 예를 들어, 딥 몰딩 공정에 의해 남겨진 개구는 보통 실리콘 고무로 된 미경화 시트로 패치된다. 그 다음, 보형물이 실리콘 젤로 충전되는 경우, 이러한 젤을 부가하고 경화시키고, 충전된 보형물을 포장하고, 포장된 보형물을 멸균한다. 보형물이 염수 용액으로 부풀려지는 경우, 1방향 밸브(one-way valve)를 조립 및 설치하고, 필요하다면 보형물을 후경화하고, 그 다음 보형물을 세정하고 포장하고 멸균한다. 염수 용액에 의해 둘러싸이도록 쉘 안쪽에 젤-충전된 주머니가 위치하는 복합 유방 이식물 보형물이 또한 제조될 수 있다.
전술한 딥핑 공정에 더하여, 보형 이식물을 위한 유연성 쉘은 몰딩 공정을 사용하여 형성될 수 있다. 예를 들어, 본 명세서에 전체가 참고로 포함된 쉬슬러(Schuessler)의 미국 특허 제6,602,452 호에 기재된 바와 같은 회전 몰딩 공정이 사용될 수 있다. 쉘을 몰딩한 후에 외표면 상에 텍스쳐를 형성하기 위한 방법이 딥핑 기술을 사용하여 수행될 수 있으나, 다른 방법은 몰드의 안쪽을 거칠게 만드는 것이다. 예를 들어, 상기한 바와 같은 거친 영역을 제외하고는 일반적으로 매끄러운 내표면을 갖는 몰드는 이산 고정 표면을 갖는 이식물 쉘을 생성할 것이다. 상대적으로 적은 제조 단계로 전체 이식물 쉘이 형성될 수 있기 때문에 회전 몰딩 공정이 유리하다.
본 발명을 소정 정도로 구체적으로 기재 및 예시하였으나, 본 설명은 단지 예시의 방식으로 이루어진 것이며, 부분들의 조합 및 배열에 따른 수많은 변형이 하기에 청구된 바와 같은 본 발명의 범주를 벗어남이 없이 당업자에게 고려될 수 있다.
Claims (20)
- 사람에 이식하기 위한 보형물로서,
전면 및 후면에 의해 정의되는 외표면을 포함하는 이식가능한 부재를 포함하며;
전면 및 후면 중 하나는 상기 전면 및 후면 중 하나의 나머지의 텍스쳐, 거칠음 또는 광택 각각과 상이한 텍스쳐, 거칠음 또는 광택에 의해 정의되는 적어도 하나의 일반적으로 긴 고정 영역을 포함하고;
근육 그룹에 인접하여 이식될 때, 고정 영역이 일반적으로 상기 근육 그룹과 일직선으로 배열될 수 있는 보형물. - 제1항에 있어서, 이식가능한 부재는 엘라스토머 쉘을 포함하는 보형물.
- 제1항에 있어서, 고정 영역은 제1 텍스쳐를 갖고 상기 나머지는 제1 텍스쳐와 상이한 제2 텍스쳐를 갖는 보형물.
- 제1항에 있어서, 사람의 유방에 이식하기 위해 구성되며, 보형물이 그렇게 사람의 유방에 이식될 때 고정 영역은 흉근 그룹과 일반적으로 일직선이 되도록 위치 및 구성되는 보형물.
- 제1항에 있어서, 사람의 유방에 위치시키기 위해 구성되며, 보형물이 그렇게 사람의 유방에 이식될 때 고정 영역은 대흉근 그룹 및 소흉근 그룹 중 적어도 하나와 일반적으로 일직선이 되도록 위치되는 보형물.
- 제1항에 있어서, 적어도 하나의 고정 영역은 복수의 고정 영역을 포함하는 보형물.
- 제1항에 있어서, 고정 영역은 방사상 형태의 복수의 고정 영역을 포함하는 보형물.
- 제1항에 있어서, 고정 영역은 상기 전면 및 후면 중 하나를 실질적으로 전체적으로 가로질러 연장하는 보형물.
- 제1항에 있어서, 고정 영역은 약 2 mm 내지 약 15 mm의 실질적으로 일정한 폭을 갖는 보형물.
- 외표면의 나머지와 상이한 텍스쳐와 상이한 텍스쳐에 의해 정의되는 적어도 하나의 일반적으로 긴 고정 영역을 포함하는 외표면을 포함하는 이식가능한 부재를 포함하며;
적어도 하나의 일반적으로 긴 고정 영역은 보형물일 사람의 유방에 이식된 때 일반적으로 긴 고정 영역이 대흉근 그룹 및 소흉근 그룹 중 적어도 하나와 일반적으로 일직선으로 배열되도록 구성되는 유방 보형물. - 제10항에 있어서, 외표면은 전면 및 후면에 의해 정의되고, 일반적으로 긴 고정 영역은 전면 및 후면 중 적어도 하나를 가로질러 연장하는 보형물.
- 제10항에 있어서, 외표면은 전면 및 후면에 의해 정의되고 일반적으로 긴 고정 영역은 전면을 가로질러 연장하는 보형물.
- 제10항에 있어서, 적어도 하나의 일반적으로 긴 고정 영역은 실질적으로 방사상 형태를 형성하는 복수의 일반적으로 긴 영역을 포함하는 보형물.
- 제10항에 있어서, 외표면의 나머지는 실질적으로 매끄러운 보형물.
- 제10항에 있어서, 외표면은 전면, 후면 및 전면과 후면 사이의 외연 영역에 의해 정의되고, 고정 영역은 외연 영역을 따라, 그리고 후면 및 전면 중 적어도 하나를 가로질러 연장하는 보형물.
- 사람의 유방을 확대 또는 재건하는 방법으로서,
전면 및 후면에 의해 정의되는 외표면을 포함하는 이식가능한 부재를 제공하는 단계 - 여기서, 전면 및 후면 중 하나는 상기 전면 및 후면 중 하나의 나머지의 텍스쳐, 거칠음 또는 광택 각각과 상이한 텍스쳐, 거칠음 또는 광택에 의해 정의되는 적어도 하나의 일반적으로 긴 고정 영역을 포함함 -; 및
이식가능한 부재의 고정 영역이 대흉근 그룹 및 소흉근 그룹 중 하나와 일반적으로 일직선이 되도록 이식가능한 부재를 사람의 유방에 이식하는 단계를 포함하는 방법. - 제16항에 있어서, 적어도 하나의 고정 영역은 상기 전면 및 후면 중 하나의 상기 나머지에 비하여 향상된 텍스쳐에 의해 정의되는 방법.
- 전면 및 후면을 포함하는 외표면을 갖는 충전가능한 엘라스토머 쉘을 제공하는 단계; 및
외표면의 단지 일부분을 텍스쳐화하여 전면 및 후면 중 적어도 하나를 가로질러 밴드-형상 고정 영역을 형성하는 단계를 포함하는 유방 보형물의 제조 방법. - 제18항에 있어서, 텍스쳐화 단계는 외표면의 나머지를 차폐하는 단계를 포함하는 방법.
- 제18항에 있어서, 밴드-형상 고정 영역을 대흉근 그룹 및 소흉근 그룹 중 하나와 일반적으로 일직선이 되도록 형상화하고 위치시키는 방법.
