Abstract
본 발명은 아세토시클로프로필 탁소테레 또는 그의 유도체, 및 1종 이상의 알킬화제, 항-대사물질, 방추체 저해제, 에피도필로톡신, 항생제, 효소, 토포이소머라제 억제제, 백금 배위 착물, 생물학적 반응 개질제 또는 성장 인자 억제제로 구성된 제약 조성물을 기재한다.

Description
본 발명은 신생물 질환의 치료에 치료학적으로 유용한, 아세틸시클로프로필 도세탁셀 및 상표명 헤르셉틴(Herceptin)으로 알려진 트라스투주맙의 조합물에 관한 것이다.
아세토시클로프로필 탁소테레의 구조는 다음과 같다:
일반적으로, 사용되는 투여량은 처치를 받는 대상체 특유의 인자에 따라 달라지며, 1 내지 100 mg/kg으로 복강내 투여되거나, 1 내지 100 mg/kg으로 정맥내 투여된다.
ErbB2에 대한 모노클로날 항체는 트라스투주맙이라고도 알려져 있으며, EP 153114에는 이것이 ATCC HB 8488 (260F9), ATCC HB 8490 (113F1), ATCC HB 8486 (266B2), ATCC HB (8484C11), ATCC HB 8697 (33F8), ATCC HB 8485 (317G5), ATCC HB 8696 (520C9) ATCC HB 8662 (260F9-1C9)를 포함하는 하이브리도마 리스트에서 선택된 참조 항체도 결합하는 인간 유방암 항원에 결합하는 뮤린(murine) 항체로 기재되어 있다. 헤르셉틴은 배양 배지로 항체를 분비하는 유전자 조작된 차이니스 햄스터 난소 (Chinese Hamster Ovary, CHO) 세포주를 대규모로 성장시켜 생산된다. 상기 항체는 표준 크로마토그래피 및 여과 방법을 이용하여 CHO 배양 배지에서 정제된다. 헤르셉틴은 전세계 거의 모든 곳에 등록되어 있는 제품이다.
현재, 항암 치료에 치료학적으로 유용하고 탁산 유도체와 동일하거나 상이한 메카니즘을 갖는 1종 이상의 물질과의 조합물로 투여되는 경우에 아세틸시클로프로필 도세탁셀의 효능이 상당히 개선될 수 있다는 것이 밝혀져 있고, 본 발명에서는 상기 1종 이상의 물질이 트라스투주맙 또는 헤르셉틴으로 제한된다.
추가로, 생성물의 활성이 사용되는 투여량에 따라 달라지기 때문에, 조혈계 성장 인자, 예컨대 G-CSF 또는 GM-CSF 또는 특정 인터루킨을 아세틸시클로프로필 도세탁셀과 조합하거나, 또는 이것들과 다른 치료 활성 물질과의 조합물을 사용함으로써, 더 낮은 투여량을 사용하고 활성을 증가시키면서 독성 현상은 감소시키거나 발병을 지연시키는 것이 가능하다.
본 발명에 따른 조합물의 개선된 효능은 치료 상승효과를 결정하여 입증될 수 있다. 조합물은, 각각의 성분들이 단독으로 그의 최대 허용되는 투여량으로 사용될 때 나타내는 것보다 치료학적으로 더 우수한 경우에 치료 상승효과를 나타내는 것이 증명된다.
조합물의 효능을 입증하기 위해서는, 본 연구에서 조합물의 최대 허용되는 투여량을 별개의 성분들 각각의 최대 허용되는 투여량과 비교하는 것이 필요할 수 있다. 이러한 효능은 예를 들어 하기 식으로 결정되는 log10 세포 사멸로 정량화될 수 있다:

(1979)]). log10 세포 사멸이 0.7 이상인 경우에는 해당 생성물이 활성인 것으로 간주된다. log10 세포 사멸이 2.8 초과인 경우에는 해당 생성물이 매우 활성인 것으로 간주된다.
고유의 최대 허용되는 투여량으로 사용되고, 각각의 성분이 일반적으로 그의 최대 허용되는 투여량을 초과하지 않는 투여량으로 존재하는 조합물은, 이 조합물의 log10 세포 사멸이 가장 우수한 성분의 단독 투여시 얻어지는 log10 세포 사멸 값보다 더 높은 경우에 치료 상승효과를 나타내는 것이 증명된다. 더욱이, 각각의 화합물이 단독으로 사용될 때와 비교하여 log10 세포 사멸에서의 차이가 1 초과인 경우에는 이것이 더욱 높은 상승효과를 나타내는 것으로 간주된다.
충실성 종양에 대한 조합물의 효능은 하기 방식을 통해 실험적으로 결정할 수 있다:
일반적으로 마우스를 사용하는 실험 대상 동물의 양쪽 옆구리에 30 mg 내지 60 mg의 종양 단편을 제0일에 피하 이식한다. 종양을 보유한 상기 동물들을, 각종 처치군과 대조군으로 적용하기 전에 섞어 둔다. 진행성 종양을 치료하는 경우, 종양이 원하는 크기로 자라도록 하고, 종양이 불충분하게 자란 동물은 제거한다. 선별된 동물들을 무작위로 나누어서 처치를 받게 하거나 대조군으로 사용한다. 종양에 대한 특이적 효과로부터 독성 효과를 구별할 수 있도록 하기 위해서, 종양을 보유하지 않는 동물에게도 종양-보유 동물과 동일한 처치를 시행할 수 있다. 화학요법은 일반적으로 종양 유형에 따라 이식 후 제3일 내지 제22일에 시작하며, 동물들을 매일 관찰한다. 상이한 동물군을 최대 체중 감소에 도달할 때까지 매주 3회 또는 4회 칭량한 후, 상기 군들을 실험 종료시까지 매주 1회 이상 칭량한다.
종양이 대략 2 g에 도달할 때까지, 또는 종양이 대략 2 g에 도달하기 전에 동물이 사망하는 경우에는 그때까지 종양을 매주 2회 또는 3회 측정한다. 동물이 죽거나 동물을 죽인 후에 부검한다.
기록된 여러가지 파라미터에 따라 항-종양 활성을 결정한다.
백혈병에 대해 조합물을 연구하기 위해서, 동물들에게 특정 수의 세포를 이식하고, 항-종양 활성은 대조군에 비교된 처치된 마우스의 생존 시간의 증가로 결정한다. P388 백혈병의 경우, 생존 시간 증가율이 27% 초과인 경우에는 해당 생성물이 활성인 것으로 간주되고, 75% 초과인 경우에는 매우 활성인 것으로 간주된다.
아세틸프로필 도세탁셀과 헤르셉틴의 조합물을 최적의 투여량으로 사용하여 얻은 결과는 하기 표에 예로서 제시한다.
아세토시클로프로필 탁소테레 및 헤르셉틴의 조합물을 피하 이식가능한 종양을 보유한 마우스에서 평가하였다. 일반적으로, 각각의 약물 조합물 평가에 사용된 종양 모델은 각각의 작용제가 단일요법으로 사용될 때의 그에 대한 반응성을 기초로 하여 선택하였다. 간헐적 i.v. 스케쥴을 이용하여, 각각의 단일 작용제 및 각각의 조합물에 대하여 전체 투여량 반응 실험을 수행하였다. 본 경우에서는 선택된 종양이 악성 흉수(胸水)에서 유래된 인간 유방 암종 UISO-BCA-1이었다. 원발성 종양은 잘 차별화된, 유방의 침윤성 도관 암종이었다. 유방완전절제술(masectomy) 후에 방사선요법을 실시하였다. 좌측 쇄골위 종괴를 처치한 후에 3년 동안 화학요법을 실시하였다 (시톡산, 메토트렉세이트, 5-Fu 및 타목시펜을 사용하여 10 코스). 요법 후 제1개월에 질환 진행 동안 종양 세포의 샘플을 채취하였다 [Mehta et al, 1992]. 상기 종양 모델은 2×107개 세포 이식물을 사용하여 누드 마우스에서 수립되었고, 종양 단편으로서 연속 피하 계대배양하여 유지시켰다. 유동 세포 계측법을 이용할 때, UISO-BCA-1 종양은 HER-2는 발현하지만 PgP (mdr)는 발현하지 않았다.
이종이식편은 도세탁셀에 내성이 있었고, 아세틸시클로프로필 도세탁셀 및 5-플루오로우라실에 감응성이 있었으며, 독소루비신, 시클로포스파미드 및 빈크리스틴에 대한 감응성은 불량하였다.
상기 연구는 3가지 부류의 투여량-반응으로 스케쥴이 짜여졌다:
- 아세틸시클로프로필 도세탁셀 단독 (52.1 내지 12.4 mg/kg/정맥내 주사, 제17일, 제21일, 제25일)
- 헤르셉틴 단독 (40 내지 2.5 mg/kg/피하 주사, 제17일, 제21일, 제25일, 제29일)
- 상기 두가지 작용제 모두를 동시 주사하는 조합.
하기 종료점(end point)이 이용되었다:
● 체중 감소가 > 20%로 유도되거나, 또는 약물로 인한 사망이 > 10%로 유도된 투여량을 독성으로 간주하였다.
● 최고의 비-독성 투여량 (Highest NonToxic Dose, HNTD)은 log 세포 사멸 = (T - C)/[3.32×(종양 배가 시간 (일))]로 결정하였다.
(T는 처치된 마우스가 1 g에 도달하는 시간을 의미하고, C는 대조군 마우스가 상기와 동일 크기에 도달하는 시간 (26.2일)임).
log 세포 사멸 < 0.7인 경우에는 항-종양 활성이 없는 것으로 간주하였고, log 세포 사멸 > 2.8인 경우에는 해당 처치가 고도로 활성인 것으로 간주하였다.
● 반응률: 부분적 퇴행 (Partial Regression, PR)은 > 50% 초기 종양 부하인 퇴행에 상응하고, 완전 퇴행 (Complete Regression, CR)은 촉진(觸診) 한계 미만으로의 퇴행에 상응한다.
● 치료적 상승효과: 조합물은 각 작용제 단독인 경우보다 더 활성 (log 세포 사멸이 1 이상)인 경우에 치료적 상승효과가 있다.
종양 배가 시간 = 5.0일
군 당 중앙(median) 종양 부하 = 125 mg
대조군에서 1 g에 도달하는 시간 = 26.2일
사용된 약어: bwl = 체중 손실, T - C = 종양 성장 지연, CR = 완전 퇴행
상기 조합물은, 가장 우수한 단일 작용제인 아세틸시클로프로필 도세탁셀에서 관찰된 것보다 log 세포 사멸이 1 초과 만큼 더 높은 활성을 갖는 상승작용을 나타냈다. 본 실험에서, 가장 높은 비-독성 투여량의 아세틸시클로프로필 도세탁셀 (주사 1회 당 32.3 mg/kg, 총 투여량 96.9 mg/kg)과 가장 낮은 투여량의 헤르셉틴 (2.5 mg/kg/일, 총 투여량 10 mg/kg)은, 단독 사용된 아세틸시클로프로필 도세탁셀이 가장 높은 비-독성 투여량에서 log 세포 사멸이 1.3인 것에 비해 log 세포 사멸 2.6으로 상승작용을 나타냈다는 것에 주목하는 것이 중요하다. 추가로, 상기 조합물에 의해 완전 퇴행은 6/8로 달성되었고 장기간 종양이 없는 생존 동물은 2/8로 달성되었고, 이는 아세틸시클로프로필 도세탁셀 단독의 경우에는 퇴행이 없었다는 것과 비교된다. 상기 조합물의 7가지 투여량 수준에서는 양호한 항-종양 활성이 관찰되었다.
특히, 상기 조합물은 성분들이 단독 사용될 때의 투여량보다 상당히 더 낮은 투여량으로 사용될 수 있다는 이점을 제공할 수 있다.
상기한 표는 연구의 각 부류, 단일 작용제 및 조합물의 치료 반응 및 가장 높은 비-독성 투여량을 요약한다.
따라서, 본 발명은 또한 본 발명에 따른 조합물을 함유하는 제약 조성물에 관한 것이다.
상기 조합물을 구성하는 성분들은 조합물의 최대 효능을 달성하기 위해서 동시에, 거의 동시에, 별개로 또는 시간 간격을 두고 투여될 수 있다. 각각의 투여는 기간 면에서 신속한 투여 내지 지속적 관류까지 달라질 수 있다.
결과적으로, 본 발명의 목적상, 상기 조합물은 성분들의 물리적 회합에 의해 달성되는 것만으로 제한되는 것이 아니라, 동시에 행해질 수도 있고 시간 간격을 두고 행해질 수도 있는 별개의 투여를 허용하는 것까지도 포함된다.
본 발명에 따른 조성물은 비경구 투여될 수 있는 조성물인 것이 바람직하다. 그러나, 이들 조성물은 국소적인 국부 요법의 경우에 경구, 피하 또는 복강내 투여될 수 있다.
비경구 투여용 조성물은 필요에 따라 사용시에 임의로 제조할 수 있는, 일반적으로 제약상 허용가능한 멸균 용액제 또는 현탁액제이다. 비-수성 용액제 또는 현탁액제의 제조에는, 천연 식물성 오일, 예컨대 올리브유, 호마유 또는 액상 광유 또는 주사가능한 유기 에스테르, 예컨대 에틸 올레에이트가 사용될 수 있다. 멸균 수성 용액제는 물 중 생성물의 용액으로 구성될 수 있다. 수성 용액제는 정맥내 투여에 적합하지만, 이 경우에는 pH를 적절하게 조정하고 예를 들어 충분량의 염화나트륨 또는 글루코스를 사용하여 상기 용액을 등장성으로 만든다. 멸균은 가열로 수행될 수도 있고, 또는 조성물에 해로운 영향을 주지 않는 임의의 다른 수단으로 수행될 수도 있다. 상기 조합물은 또한 리포좀의 형태일 수도 있고, 또는 시클로덱스트린 또는 폴리에틸렌 글리콜과 같은 담체와 회합된 형태일 수도 있다.
경구, 피하 또는 복강내 투여용 조성물은 수성 현탁액제 또는 용액제인 것이 바람직하다.
본 발명에 따른 조합물에서, 그의 성분들은 동시에, 별개로 또는 시간 간격을 두고 투여될 수 있으며, 탁산 유도체의 양이 조합물의 10 중량% 내지 90 중량%라는 점이 특히 유리하고, 상기 양은 회합된 물질의 특성, 나타난 효능 및 치료될 암의 특성에 따라 달라질 수 있다.
본 발명에 따른 조합물은 유방, 난소 또는 폐의 암 치료 뿐만이 아니라 흑색종 및 백혈병의 치료에도 특히 유용하다. 이것들은 탁솔 또는 탁소테레와 같은 통상 사용되는 탁소이드에 내성이 있는 암을 치료하는데 주로 유용하다. 이것들을 탁솔 및/또는 탁소테레에 내성이 있는 유방암 치료에 사용하는 것이 더욱 바람직하다.
