척추 관절성형술을 위한 인공장치{PROSTHETIC DEVICE FOR SPINAL ARTHROPLASTY}
본 발명은 인접하는 척추골에 의해 형성되는 척추간 공간(intervertebral space) 내로 삽입되는 인공장치 및 삽입방법에 관한 것이다.
디스크 관절성형술은 손상, 퇴화 또는 질환이 있는 척추 조인트(spinal joints)를 치료하는 하나의 방법이다. 몇몇 디스크 관절성형술은 척추 조인트 관절화 (articulation) 또는 움직임이 가능하도록 운동-보존적 척추 디스크(motion-preserving spinal disc)로 조인트의 손상 디스크를 대체하는 것을 포함한다. 삽입 디스크(inserted disc)는 환자에게 조인트 관절(joint articulation)을 제공할 수 있지만, 척추 디스크의 삽입은 침습적이면서 집중적인 시술일 수 있다. 예를 들면, 종래의 척추 디스크는 상당히 크기 때문에 정형적으로 전방 시술을 통하여 장착될 수 있다. 전방 시술은 때로는 대동맥 및 하대정맥과 같은 혈관들을 변위 시킬 필요가 있기 때문에 상당히 주의 깊게 수행되어야 한다. 또한, 수술 부위 주변에 생길 수 있는 흉터조직으로 인하여, 요구되는 이차 치료는 더욱 어려울 수 있 고 환자에게 추가의 통증을 일으킬 수 있다.
의사에게 수술적 선택을 제공하는, 종래의 디스크 보다 더 작은 구조를 가지는 척추간 공간 내로 삽입되는 인공장치가 요구된다. 본 명세서에서 개시되는 인공장치는 선행 기술의 하나 또는 그 이상의 문제를 해결할 수 있다.

[발명의 상세한 설명]

도 1은 척주(10)의 일부분을 도시하는 측면도이며, 본래의 척추간 디스크들 D1, D2, D3에 의해 분리되는 1군의 인접한 상부 및 하부 척추골 V1, V2, V3, V4를 도시한 것이다. 4개의 척추골은 단지 일례로써 도시된 것임을 의도한다. 다른 예로는 하나의 천골 및 하나의 척추골을 들 수 있다.
또 다른 예로써, 도 2를 참조하여 두 개의 척추골을 설명할 수 있다. 두 개의 척추골은 하나의 하부 척추골(VL) 및 상부 척추골(VU)을 포함하는 하나의 척추 운동 분절(spinal motion segment, 12)을 형성한다. 어떤 류의 디스크 관절성형술은, 두 척추골(VL, VU) 사이에 위치되었던 본래의 디스크 일부 또는 전부를 추간판절제술(discectomy) 또는 이와 유사한 외과적 수술에 의해 제거한다. 질환 또는 퇴행성 디스크를 제거하면 상부 척추골(VU) 및 하부 척추골(VL) 사이에 척추간 공간(S)이 생성된다. 도 2는 요추 조인트(lumbar vertebral joint)로서의 척추 분절(12)을 도시한 것이지만, 본 발명의 장치, 시스템 및 방법은, 경추 및 흉추 영역을 포함하는 척주의 모든 영역에 적용될 수 있다는 것을 이해하여야 할 것이다. 또한, 예를 들면 후관절(facet joints)과 같은 척추의 다른 영역에서도 본 발명의 장치, 시스템 및 방법이 사용될 수 있다. 또한, 본 발명에서 척추간 디스크 공간(intervertebral disc space)이라는 용어의 사용은 후관절을 포함한다.
종래의 인공장치는 고정(fixed) 구조를 가진다. 따라서, 종래의 장치를 척추간 공간(S)에 삽입하는 경우, 인공장치가 삽입될 수 있는 정도의 공간뿐 아니라, 그것이 조작될 수 있는 정도의 여유 공간을 요구한다. 따라서, 종래의 인공 디스크를 삽입하기 위해서는 상당히 큰 개구부와 침범적 시술이 필요하다. 그러나, 본 명세서에 개시되는 관절장치는, 이식 전에는 인공장치의 구조가 완전히 고정되지 않기 때문에 더 작은 개구부가 필요하고, 덜 침범적 시술이 가능하다. 대신, 관절장치는 작고 수축된 상태에서 척추간 공간(S)에 도입될 수 있는 색(sack) 또는 백(bag)과 같은 하나 이상의 팽창부재를 포함한다. 제자리에 놓이면, 백은 그 자리에서 팽창되어 관절장치를 형성할 수 있다. 인공장치가 척추간 공간(S)에 배치될 때까지는 구조가 고정되지 않으므로, 인공장치는 종래의 디스크들보다 더 작은 프로필(profile)을 가질 수 있다. 또한 고정되지 않으므로, 인공장치의 프로필은 삽입되는 동안 필요하다면 용이하게 조작될 수 있다. 더 작고 연성의 프로필로 인하여 의사는 덜 침범적 장착 기술을 사용하여 인공장치를 장착할 수 있다. 예를 들면, 디스크를 전방에서 장착하지 않고, 의사는 척추에 대하여 후방, 전사위방 또는 측방 접근과 같은 다른 방향으로 디스크를 장착할 수 있다. 이것은 의사로 하여금 장착에 있어서 더 많은 선택을 할 수 있도록 하는 것이고, 의사는 덜 침범적이고 덜 고통스러운 시술을 수행할 수 있다.

도 3은 척추간 디스크 공간(S)에 있는 인공장치(100)의 일구현예를 도시한 도면이다. 도 3에서, 상부 척추골(VU) 및 하부 척추골(VL)은 과장되게 떨어지도록 도시되어 인공장치(100)의 구성을 더욱 상세하게 보여준다. 인공장치(100)는 최상부(top portion, 102) 및 최하부(bottom portion, 104)를 포함한다. 최상부(102)는 최상관절부재(106) 및 최상팽창부재(108)를 포함한다. 유사하게 최하부(104)는 최하관절부재(110) 및 최하팽창부재(112)를 포함한다.
도 4는 팽창부재들(108, 112)에서 분리된 관절부재들(106, 110)의 부분 단면도를 도시한다. 최상관절부재(106) 및 최하관절부재(110)는 상호 협력하도록 형성되어 인공장치(100)에 관절을 제공하고, 하부 척추골(VL)에 대하여 상부 척추골(VU)의 교합을 가능하도록 한다. 도 3 및 4는 상부 척추골(VU) 및 하부 척추골(VL) 및 서로 이격된 인공장치(100)를 도시한다. 도 3 및 4에 도시되지는 않았으나, 최상관절부재(106)는 관절 조인트를 형성하기 위하여 최하관절부재(110)와 상호 협력하도록 형성될 수 있다. 하나의 구현예에서, 최상관절부재(106)는 소켓(socket)을 형성하는 구형 오목 리세스부를 포함하고 최하관절부재(110)는 볼(ball)을 형성하는 볼록면을 포함하며, 함께 볼 및 소켓 조인트를 형성한다.

최상팽창부재(108) 및 최하팽창부재(112)는 충진 재료를 채울 수 있는 생체적합성 색(sack)으로 구성될 수 있다. 하나의 구현예에서, 최상팽창부재(108) 및 최하팽창부재(112)는 본 명세서에 참조로서 완전히 편입되는, 미국특허 제6,827,743호에 기재된 직물재료(woven material)로 구성될 수 있다. 팽창되지 않은 상태에서, 팽창부재(108, 112)는 유연하고 연성이 있어 변형 및 조작이 가능하다. 따라서, 말리거나 포개져서 상대적으로 적은 부피를 차지한다. 이로써 전방, 복막경유/ 복막뒤(trans/retro-peritoneal), 후방, 횡측 경유, 천추 경유, 전측방, 추간공 경유, 기타 기술과 같은 임의의 수술 기술을 사용하여 인공장치(100)는 도 2의 공간(S)에 도입될 수 있다. 하나의 구현예에서, 팽창부재(108, 112)는 삽입 과정에서 고정된 구조가 아니므로, 후방 접근으로부터 척추 공간(S)으로 통로(access)를 제공하는 작은 캐뉼러(cannula)를 통하여 말리거나 포개지고 맞추어질 수 있다. 따라서, 인공장치(100)는 통상의 고정-구조의 인공장치를 사용할 때보다 덜 침습적인 수술방법을 통해 장착될 수 있다.
삽입 전에, 의사는 척추간 공간의 적절한 높이를 결정하기 위하여 시도할 수 있다. 일단 결정되면, 소정의 높이를 얻도록 형성된 인공장치(100)는 척추간 공간에 배치되도록 선택될 수 있다. 따라서, 몇몇 예에서, 다수의 다른 크기의 팽창부재가 제공될 수 있고 적당한 치수에 맞도록 선택될 수 있다. 몇몇 예에서, 팽창부재(108, 112)를 팽창시켜 척추를 벌어지도록 (distract) 할 수 있다. 또한, 이러한 몇몇 예에서, 팽창부재는 척추가 원하는 정도로 벌어질 때까지만 충진될 수 있다.

최상부(102) 및 최하부(104)를 포함하는 인공장치(100)가 인접하는 척추골 사이에 배치되면, 최상팽창부재(108) 및 최하팽창부재(112)는 팽창되어 관절부재(106, 110)에 지지를 제공할 수 있다. 최상팽창부재(108) 및 최하팽창부재(112)는 소정의 형상 및 높이까지 팽창되도록 설계될 수 있다. 예를 들면, 팽창부재(108, 112)는 비-탄성일 수 있고, 설계된 형상이 얻어진 후 형상은 유지된다. 또한, 팽창부재는 소정의 높이까지만 팽창되도록 설계될 수 있다. 다른 예에서, 팽창부재는 탄성이고, 백(bag)이 팽창되는 동안 신장될 수 있다.
하나의 양상에서, 생체적합성 시멘트와 같은 충진 재료가 주입되어 팽창부재(108, 112)를 사전-설정된 크기 및 구조로 팽창시킬 수 있다. 다른 예에서, 팽창부재(108)는 적어도 일부분이 어느 정도의 강도(rigidity)를 제공할 수 있는 과립성 재료로 충진될 수 있다. 과립성 재료는 예를 들면 비드(bead), 그래뉼(granule), 골 페이스트(bone paste), 골 분말(bone powder), 및 그 밖의 다른 것들을 포함할 수 있다.
도 6a, 6b 및 7은 팽창상태에 있는 인공장치(100)의 일구현예를 도시한다. 팽창될 때, 팽창부재(108, 112)는 상부 척추골(VU) 및 하부 척추골(VL) 사이에서 교합이 가능한 위치로 각각의 관절부재(106, 110)를 고정시킨다. 도 3 및 도 6a은, 척추 사이를 과장되게 도시하여 최상부(102) 및 최하부(104)를 명백하게 나타낸다. 그러나, 도 6b는 관절운동을 제공하기 위하여 체결되는 관절부재(106, 110)와 함께, 보다 일반적인 간격의 척추골을 나타낸다. 도시된 구현예에서, 최상관절부재(106) 및 최하관절부재(110)는 볼 및 소켓 조인트를 형성한다. 그러나, 또한 관절부재는 볼 및 트러프(trough) 조인트, 피(pea) 및 소서(saucer), 또는 다른 관절 조인트를 형성할 수도 있다. 볼 및 소켓, 볼 및 트러프, 및 피 및 소서 조인트들은 모두 환자에게 더 큰 자유도를 제공하는 어떠한 축에 대하여 교합을 허용한다. 다른 구현예에서 (미도시), 최상관절부재(106) 및 최하관절부재(110)는 단일 축에 대하여만 교합을 허용하는 힌지를 형성하는 핀 조인트에 의해 고정된다. 다른 류의 조인트 역시 척추골에 관절을 제공할 수 있다.
팽창부재(108, 112)는 초기에는 적어도 일부분이 유동적이며, 경화되거 패킹되어 실질적으로 강하고 경직된 부재를 형성하는 충진 재료에 의해 팽창될 수 있다. 몇몇 구현예에서, 충진 재료는 관절부재(106, 110)에 지지력을 제공하며 팽창부재(108, 112)를 특정 배향 및 구조로 고정시킨다.
도 8-10은 팽창부재(108, 112)를 충진하는 예시적인 시스템을 도시한다. 도 8은 수축 상태의 팽창부재(108, 112)를 도시한다. 따라서, 인공장치(100)가 상부 척추골(VU) 및 하부 척추골(VL) 사이에 배치된다고 하더라도, 상부 및 하부 척추골은 도시된 바와 같이 서로 눌릴 수 있다. 따라서, 팽창부재(108, 112)가 팽창되지 않은 상태에서는, 디스크 공간(S) 뿐 아니라 인공장치(100)는 h1으로 표시되는 높이를 가질 수 있다. 아래에서, 도 9를 참조하면, 높이 h1은 팽창부재(108, 112)가 팽창 상태에 있을 때의 인공장치(100)와 척추간 디스크 공간의 높이 h2와 비교될 것이다.
도 8에 도시된 예에서, 충진 재료를 주입하여 팽창부재(108, 112)를 팽창시키기 위하여, 바늘(132)을 구비한 실린저(130)가 사용될 수 있다. 충진 재료는 주입되는 동안에는 부분적으로 액상일 수 있고 경화되거나 굳어져 관절부재(106, 110)에 대한 기반을 형성하도록 구성될 수 있다.
도 9에서, 충진 재료의 단순한 주입을 위하여 물질 주입기(substance injector, 134)가 팽창부재와 함께 사용될 수 있다. 도 9에서, 팽창부재(112)에만 연결되어 있는 물질주입기(134)가 도시되고 설명되지만, 팽창부재(108)를 채우기 위하여 유사한 주입기가 연결되어 사용될 수도 있다. 물질 주입기(134)는 미리 형성될 수 있고, 팽창부재(112)에 부착되고 연장될 수 있다. 따라서, 팽창부재(108, 112)가 척추간 공간(S) 내부에 배치될 때, 물질 주입기(134)는, 그것을 통하여 팽창부재들(108, 112) 중 하나 또는 모두에 충진 재료를 주입하고자 하는 의사로 하여금 단순한 경로를 제공할 수 있다.
하나의 구현예에서, 물질 주입기(134)는 팽창부재(112) 자체와 일체로 형성되고, 다른 구현예에서, 물질 주입기(134)는 팽창부재(112)에 부착된 재료로 형성될 수 있다. 또 다른 예에서, 물질 주입기(134)는 팽창부재(112)가 척추간 공간(S)에 위치된 후 팽창부재(112) 내부로 삽입될 수 있다. 팽창부재(112)가 채워지면, 물질 주입기(134)는 제거되거나 장치(100)의 관절 또는 척주를 방해하지 않는 방식으로 처리될 수 있다. 물질 주입기(134)가 팽창부재(112)로부터 제거될 수 있는 경우를 포함하는 구현예에서, 물질 주입기(134)는 팽창부재(112)로부터 끊어져서 제거될 수 있다. 달리, 물질 주입기(134)는 묶여서 끊어지거나, 절단되거나 제거될 수 있다. 하나의 구현예에서, 물질 주입기(134)는 팽창부재(112)와 연결되고 팽창부재(112)와 수술위치에서 유지된다. 팽창부재(108)는 팽창부재(112)와 유사하게 형성될 수 있다.

Claims (20)

  1. 인접하는 척추골에 의해 형성되는 척추간 공간(intervertebral space) 내로 삽입되는 인공장치로서, 상기 인공장치가
    제1 관절면(articulating surface) 및 제1 부착면(attachment surface)을 구비한 제1 관절부재(articulation member);
    상기 제1 관절면과 협력하여 관절 운동(articulating motion)을 할 수 있도록 형성된 제2 관절면 및 제2 부착면을 구비한 제2 관절부재;
    최상관절부재(top articulation member)와 최하관절부재(bottom articulation member)를 서로에 대해 고정하기 위한 연결수단(connecting means); 및
    상기 제1 관절부재에 연결되며, 상기 척추간 공간 내에서 상기 제1 관절부재를 지지하고 배치하도록 형성된 팽창부재(expandable member)를 포함하는 인공장치.

    상기 제1 관절면과 협력하여 관절 운동을 할 수 있도록 형성된 제2 관절면 및 제2 부착면을 구비한 제2 관절부재;
    상기 제1 관절부재에 연결되며, 상기 척추간 공간 내에서 상기 제1 관절부재를 지지하고 배치하도록 형성된 팽창부재; 및
    상기 제2 관절부재에 대하여 상기 제1 관절부재를 선택적으로 고정시키도록 형성된 연결수단을 포함하는 인접하는 척추골에 의해 형성되는 척추간 공간 내로 삽입되는 인공장치를 포함하는 척추 통증 경감 시스템.
