Abstract
본 발명은 물질로 만들어진 바디(22), 및 이 바디(22)에 접촉할 수 있게 되는 액체(20)를 갖는 압축성 블래이더(14)를 포함하는 추간판 디스크 인공 보철물(2)에 관한 것이다. 액체 및 물질은 액체가 바디(22)를 적시지 않게 하는 액체 및 물질이다.
Description
추간판 디스크를 대체하도록 한 추간판 디스크 인공 보철물은 예를 들면 EP-0,277,282 문헌에 공지되어 있다. 이것은 대체할 디스크에 인접한 척추의 척추판을 지탱할 수 있는 2 개의 판과, 이들 판 사이에 개재된 쿠션을 포함한다. 쿠션은 압축성 바디와, 액체를 에워싸는 챔버를 포함한다. 이러한 인공 보철물에 있어서 문제는 압축성 바디가 마모되기 쉬어 결국엔 인공 보철물의 기구적 작용이 변경된다는 것이다. 더욱이, 버다의 판에 대해 마찰로 인해 입자부식이 유발되어 환자의 몸 내에서 옮겨 다닐 수 있다.
본 발명의 목적은 기구적 작용이 장기간에 걸쳐 동일하며 환자의 몸에 입자들이 방출될 위험을 줄인 인공 보철물을 사용할 수 있게 하는 것이다.
이러한 목적의 달성하기 위해서, 본 발명은 물질로 만들어진 바디와, 이 바디에 접촉하게 될 수 있는 액체를 갖는 압축성 쿠션을 포함하는 추간판 디스크 인공 보철물을 제공하는 것으로, 여기서 액체 및 물질은 액체가 바디를 적시지 않게 하는 액체 및 물질이다.
액체가 바디를 적시지 않을 때, 액체를 바디에 접촉되게 하려면 어떤 에너지가 필요하게 된다. 이 에너지는 액체가 바디로부터 다시 분리하게 되었을 때 그전체가 회복된다. 일종의 스프링 효과가 이러한 식으로 하여 생긴다. 이 효과는 액체와 바디와의 접촉면이 클 때 보다 더 두드러지게 된다. 이에 따라 인공 보철물은 상당한 크기의 스트레스를 흡수할 수 있어 압축 하에 변형 혹은 만곡되며, 이 보철물은 저장된 에너지를 회복시킴으로써 역으로 변형할 수 있다. 이러한 인공 보철물은 작은 용적에 상당량의 에너지를 축적할 수 있다. 또한, 바디와 접촉한 액체의 이동으로 어떠한 마모도 발생하지 않는 데, 이에 따라 인공 보철물은 매우 장기간에 걸쳐 동일한 기구적 행동을 나타낼 수 있다. 더구나, 고형 부분들 간에 마찰이 없으므로 실제적으로 몸에 입자가 방출되는 어떠한 위험도 제거된다.
스프링 효과는 매우 한정된 용적의 바디이어도, 다공성 바디이므로 매우 클 것이다.
바디가 매우 긴 망을 따라 서로 통하는 공동을 갖는다면, 입구에서 출구로 액체가 이동하는 경로는 특히 길다. 그 결과는 단지 바디 내에서의 순환에 기인하여 일부 에너지를 소진한다는 것이다. 결국, 전체 구성은 부분적인 댐퍼를 구성할 수 있다. 따라서, 스프링 효과와 댐핑 효과의 결합이란 인공 보철물에 있어서 인공 보철물의 주 축 상에서의 인공 보철물의 길이 변화의 함수로서 압축 스트레스를 나타내는 곡선은, 건강한 자연상태의 추간판 디스크처럼, 히스테리시스 형태를 갖는다는 것을 의미한다. 이에 따라 인공 보철물은 실제 디스크의 기구적 행동에 매우 가깝게 된다.
인공 보철물은 쿠션의 어느 일측으로 확장하는 2 개의 판을 포함하고, 바디는 판 중 기껏해야 하나의 판에 고정된다.
본 발명의 다른 특징 및 잇점은 비제한적인 예로서 주어진 바람직한 실시예에 대한 다음 설명으로부터 보다 명백해질 것이다. 첨부한 도면에서,
도 1은 본 발명의 바람직한 실시예에 따른 인공 보철물을 축 방향으로 관통한 단면도이다.
도 2는 인공 보철물에 인가된 압축 힘의 변화를, 인공 보철물의 축을 따른 높이 변화의 함수로서 나타낸 곡선이다.
도 3은 도 1의 인공 보철물의 사시도이다.
도 1 및 도 3을 참조하여, 인공 보철물(2)은 대체로 원형 형상의 평면, 바람직하게는 포스테리어 하일럼(posterior hilum)을 갖는 빈(bean) 형상의 2개의 판(4)을 포함한다. 2 개의 판은 평탄하며, 판에 수직한 인공 보철물의 주 축(9)을 중심으로 하여 서로 평행하게 확장하여 있다.
각각의 판(4)은 판의 평면에 수직한 판(4)의 외측면(8)으로부터 돌출한 2 개의 돌출부(6)를 구비한다. 각각의 돌출부(6)는 판의 중심 방향으로 돌출부를 관통하는 오리피스(10)와, 판(4)에서 먼쪽의 돌출부(6)의 면 위에 구형 형상의 오목부(12)를 구비한다. 오리피스(10)는 골 나사(11)를 수용할 수 있는데, 이 골 나사의 헤드는 그 하측면이 돌출부(6) 상의 암(female) 오목부(12)와 상호작용하는 수(male) 구형형상으로 되어 있어 나사의 방향을 관련되는 돌출부에 대해 자유롭게 취할 수 있다. 도 1에서, 각각의 판의 2개의 돌출부(6)의 단면을 동일한 면으로 하였으나, 실제로는 도 3에 도시한 바와 같이 그렇지 않다.
컬럼에 디스크 인공 보철물을 단기간 고정함에 있어서는, 대체할 디스크에인접한 척추의 바디에 나사(11)를 고정시킬 수 있다.
그러나, 이에 더하여, 인접 척추와 접촉하는 판(4)의 표면을 하이드로시아파타이트 혹은 뼈 성장을 자극하는 공지의 어떤 다른 물질로 피복하는 장기간 고정을 제공하는 것이 가능할 것이다. 피복 전에, 뼈 조직과 보다 양호하게 확실하게 접촉되게 하기 위해서 상기 표면(8)을, 뼈 조직에의 고정점에 대하여, 얼마간의 다공성 표면 상태를 얻도록 처리할 수 있다.
인공 보철물은 판 사이에 개재된 쿠션(14)을 구비한다. 이 쿠션은 벨로우즈(16)를 포함한다. 이것은 축(9)에 관하여 회전대칭인 형상을 갖는다. 이의 벽 프로파일은 축(9)을 가로지르는 단면의 표면적을 상당량 변화시킴이 없이 축방향(9)으로 벨로우즈(16)의 길이를 변화시킬 수 있게 하는 주름을 포함한다. 이 경우에, 판(4)과 같이, 벨로우즈는 어느 정도의 축방향의 강도를 갖게 하고 압축 스프링을 형성하도록 티타늄 혹은 티타늄 합금으로 만들어진다. 이것은 또한 축(9)에 수직한 방향으로 변형될 수 있고, 혹은 축(9) 또는 이에 수직한 임의의 축에 관하여 비틀림을 받을 수 있다.
벨로우즈(16)는 이의 2 개의 축방향 양단에, 예를 들면 도시되지 않았으나, 플레이트의 내면으로부터 돌출하는 판(4)의 각각의 에지에 본딩되는 에지를 갖는다. 상기 본딩은 기밀하게 하여 벨로우즈(16) 및 두 개의 판(4)이 기밀한 가변 용적의 챔버를 형성하도록 한다.
벨로우즈(16)는 예를 들면, 도시되지 않았으나, 10 개의 회선(covolution), 즉 8 개의 외측 크레스트와 판(4)에 부착된 2 개의 크레스트를 갖고 있다. 여기서는 외직경이 약 30mm이고 내직경은 약 17mm이다. 인공 보철물에 부하가 없을 때, 높이는 10mm이다. 벨로우즈의 벽은 각각이 두께가 0.1mm에 이르고 두께의 합이 벽의 두께를 이루는 하나, 두 개 혹은 3 개의 시트를 사용하여 만들어 질 수 있다. 여기서 벨로우즈는 약 1.6 N/mm의 본연의 강도를 갖는다.
벨로우즈(16)와 두 개의 판(4)으로 크기가 정해지는 챔버는 액체(20)로 채워진다. 쿠션(14)은 여기서는 두 개의 판 중 한 판, 예를 들면 도 1에서 하측 판(4)의 평탄한 내면(18)에 고정되는 바디(22)를 포함한다. 이 목적을 위해서, 판의 융기부(24)는 바디 안으로 확장하여 있다. 바디의 형상은, 예를 들면, 축(9)에 관하여 원통형이다. 바디는, 예를 들면, 축(9)에 수직하여 충분히 폭이 좁게 하여 인공 보철물의 변형시 벨로우즈와 절대로 접촉하지 못하도록 한다. 마찬가지로, 바디(22)의 자유 단부, 여기서는 상측단부와 대향하는 판이 서로 접촉하지 못하도록 이들 사이엔 충분한 자유공간이 형성될 것이다. 바디(22)는 이와 접촉할 수 있는 액체로 둘러싸인다.
액체(20) 및 바디(22)의 물질은 생체에 적합한 것이 바람직할 것이다. 액체가 바디를 적시지 않게 하는, 즉, 이 목적을 위해 스트레스가 없을 때 바디와 접촉하지 않은 상태에 있게 하는 액체 및 바디 물질이 선택된다. 액체는 예를 들면 물이 될 것이며 바디 물질은 이를테면 티타늄 혹은 티타늄 합금 등의 생체에 적합한 물질이 될 것이다. 이 물질은 바람직하게는 액체와 접촉하는 큰 면적의 표면을 제공하기 위해서 다공성인 것이 바람직할 것이다. 더욱이, 구멍을 구성하는 바디의 공동은 스프링 효과 외에 댐핑 효과를 주기 위해서 꽤 긴 망으로 서로 통하게 만들어 질 것이다. 도 1에서, 바디(22)의 구멍 혹은 공동(26)은 명료하게 하기 위해서 도식적으로 나타내었다. 말할 나위 없이 이들 구멍은 도 1의 스케일로는 눈으로 볼 수 없는 것이고 도시된 것보다는 무한히 많다. 따라서, 축(9)의 높이로 한 판에서 다른 판까지 이르는 높이를 높이 h라 할 때, 축(9)을 따른 인공 보철물의 압축 F 변화를 높이 Δh의 네가티브 변화의 함수로서 도시한 도 2의 곡선은 히스테리시스 형태를 가질 것이다. 이에 따라, 힘 F가 증가할 때 이 힘 F 변화를 나타내는 곡선 Ca는 전적으로, 힘 F의 감소를 나타낸 곡선 Cd 위에 있다. 이 형태는 스프링 효과와 댐퍼 효과의 결합에 기인한다.
액체 챔버 내에 가스 포켓을 설치할 수도 있을 것이다.
본 발명에 따른 인공 보철물은 등뼈의 허리 영역에 특히 맞게 한 것이다.
본 발명의 범위에서 벗어남이 없이 많은 수정이 행해질 수 있다. 바디(22)는 판에 전혀 고정되지 않을 수도 있다. 이에 따라 판 각각에 관하여 임의로 이동이 자유롭게 될 것이다. 그러면 인공 보철물은 이의 어떠한 위치나 스트레스를 받는 어떠한 상태에서도, 바디(22)는 기껏해야 판 중 하나의 판에 접촉하게 되도록 구성될 수 있을 것이다.
바디(22)는 타원형상을 가질 수 있을 것이다.
Claims (4)
- 물질로 만들어진 바디(22), 및 이 바디(22)에 접촉할 수 있게 되는 액체(20)를 갖는 압축성 쿠션(14)을 포함하는 추간판 디스크 인공 보철물(2)에 있어서, 상기 액체 및 상기 물질은 상기 액체가 상기 바디(22)를 적시지 않게 하는 액체 및 물질인 것을 특징으로 하는 추간판 디스크 인공 보철물.
- 제1항에 있어서, 상기 물질은 다공성인 것을 특징으로 하는 추간판 디스크 인공 보철물.
- 제1항 및 제2항에 있어서, 상기 인공 보철물은 상기 쿠션(14)의 어느 일 측으로 확장하는 2 개의 판을 포함하고, 상기 바디(22)는 상기 판 중 기껏해야 하나의 판에 고정된 것을 특징으로 하는 추간판 디스크 인공 보철물.
- 제1항 내지 제3항 중 어느 한 항에 있어서, 상기 인공 보철물의 주 축(9)을 따른 상기 인공 보철물의 크기(h)의 변화(Δh)의 함수로서 상기 축을 따른 인공 보철물의 압축(F)의 곡선(C)은 히스테리시스 형태를 갖는 것을 특징으로 하는 추간판 디스크 인공 보철물.
