Abstract

본 발명은 골절, 종양, 감염 등으로 파괴된 추체를 제거하고 추체의 결손 부위를 복원하기 위하여 그 부위에 골 이식편 혹은 수산화아파타이트와 함께 삽입하여 추체간 유합을 촉진시키고 회복시킨 추체간의 간격을 확고히 유지하며 정상적인 척추 만곡의 복원과 복원의 유지를 위한 추체간 유합 고정장치로서 추체까지 제거하는 경우에 사용되는 길이조절형 및 길이조립형 척추체 대체 유합 고정장치에 관한 것으로
측면이나 전방에서 접근하여 문제가 발생한 추간판 및 추체를 제거하고 추체 사이에 삽입, 고정하고 확보하고자 하는 높이를 쉽게 조절하기 위해 추체의 형상과 원형의 골유합 통로를 가지는 실패형태나 블록형태로 만들어 각 크기에 따른 조립에 의한 길이확보를 통하여 추체간 유합을 촉진할 수 있도록 추체간 고정장치의 형태를 다양화하고 모듈화함으로써 삽입방향이 다양해지고 수술방법이 간단해지며 폭과 높이 조절이 편리해져서 광범위한 추체의 결손을 보완하고 보완된 추체간 유합에 간편하게 적용할 수 있도록 하였다.

본 발명은 척추 추체간 유합 고정장치에 관한 것으로 척추에 있어 척추체의 골절이나 종양, 감염 등으로 인하여 파괴된 추체를 제거하고 추체의 결손부위를 대체하여 결손부위의 높이를 복원함과 동시에 복원된 높이를 확고히 유지한 채로 골유합을 촉진시키는 추체간 유합 고정장치로서 추체까지 제거하는 경우에 사용하는 길이조절형 및 길이조립형 척추체 대체 유합 고정장치에 관한 것이다.
추체간의 유합 성공여부는 크게 골편에 따른 요인과 생체역학적인 요인으로 나눌 수 있으며, 골편에 따른 요인으로는 유합시 사용되는 골편의 종류(자가골 또는 동종골), 골편의 강도 및 골편이 추체와 접촉하는 면적 등이 있으며 생체역학적인 요인으로는 유합이 이루어지는 동안 분절의 안정성이 매우 중요하다. 과거에는 추체의 골결손을 회복하기 위해서는 장골에서 커다란 크기의 단단한 지주골을 얻어야하였기 때문에 골 채취부위의 통증과 복원된 결손부위의 높이가 유지되지 못하는 단점을 피할 수 없었다. 따라서 추체간 유합 고정장치는 유합이 이루어지는 동안 장골에서 커다란 크기의 단단한 지주골을 얻지 않고도 부스러기 형태의 뼈를 채워 체중부하를 견디며 결손된 추체를 복원하여 추체간의 간격을 그대로 유지시키는데 중요한 의미를 갖는다.
길이조절형 척추체 대체 유합 고정장치에는 크게 두 가지 종류로 나눌 수 있는데 하나는 역나사를 이용한 형태의 것이 있고 또 다른 하나는 높이에 맞게 잘라서 사용하는 것 두 가지가 주를 이루고 있다. 이때, 어떤 것이나 고정장치 내부에 환자의 추체간골 유합을 가속화하고 안정화하기 위해 환자 자신의 장골에서 채취한 골편이나 타인의 골편, 또는 골편 대체물을 넣어 사용한다.
역나사형태의 고정장치는 추체골단면과의 접촉면적이 다른 형태의 고정장치에 비해 넓은 장점을 가지고 있으며 최소 형태의 고정장치가 들어갈 부위만 있으면 역나사를 돌려 추체간의 원하는 높이까지 쉽게 얻을 수 있다는 장점이 있으나, 원하는 높이까지 계속 나사를 돌려야 하는 시술방법으로 인해 시술시간이 길어지는 단점과 나사산의 과도한 사용으로 인한 마모의 문제가 발생하여 추체간 높이 확보에 문제가 발생할 수도 있다는 단점이 있다.
길이를 잘라서 사용하는 형태의 고정장치는 높이에 맞춰 잘라낸 후 뼈를 채워 결손된 부위에 삽입하는데 삽입시 결손된 부위의 높이를 회복하기 위해서는 결손부위보다 큰 고정장치가 필요하므로 삽입이 어렵고 잘라낸 부위가 뾰족하고 단면적이 적기 때문에 추체 골단면과의 접촉 면적이 적어 추체 상하의 단단한 골단면을 파고 들어갈 가능성이 존재하기 때문에 추체간 높이를 확고히 유지하지 못하고 추체간 간격이 가라앉는 단점도 지적되고 있다.
이들 고정장치는 모두 분절의 안정성을 높여주고 결손된 추체의 복원이라는 생체역학적인 목적은 달성하였으나 시술상의 간편함과 안정적인 형태 유지 등의 면에서는 아직 보완해야 할 부분이 많다.
따라서 본 발명의 목적은 골유합을 유도하기 위한 고정장치의 디자인에 있어 두께를 얼마나 두껍게 해야 뼈로 파고 들어가는 것을 막아 디스크 간격을 유지시킬 수있느냐, 원하는 길이를 쉽게 얻을 수 있느냐, 뼈와의 접촉면적을 얼마나 넓히느냐, 얼마나 쉽게 삽입할 수 있는가 등을 고려한 길이조절형 척추체 대체 유합 고정장치를 제공하는데 있다.
본 발명에서는 이를 위해 원통형의 결합중간지지대가 있고 상·하면에 추체의 형태와 같은 상·하면 지지대를 이용하여 원통형의 결합중간지지대에 암나사 형태로 들어가 있는 길이조절형 고정장치와 블록처럼 다양한 크기의 고정장치 블록 조립을 통하여 길이를 조절할 수 있는 형태의 길이조립형 고정장치를 제공하였다.
또, 고정장치 외부에 슬릿을 만들어 고정장치의 측면에 뚫린 슬릿을 통해 골편을 쉽게 넣을 수 있고, 유합과정을 확인함과 동시에 뼈가 보다 치밀하고 안정적으로 잘 자라 들어가도록 하고, 골편의 이탈을 방지하도록 하였다. 상·하면지지대는 요추만곡의 형태에 맞춰 측면에서 봤을 때 전방을 후방보다 척추체 각각의 각도에 맞게 높이를 다르게 설계하여 필요에 따라 골라 사용하여 요추 전만을 회복시킬 수 있도록 하였으며, 추체간골의 해부학적 단면적과 유사하게 휘어진 만곡형으로 설계, 제작하여 이들을 사용하고자 하는 부위의 추체의 면적에 따라 적합하게 조합하여 시술할 수 있게 하였다. 그리고 인체내 삽입 후 다시 튀어나오는 현상을 방지하기 위하여 돌기를 만들어 넣었으며 하나 이상의 나사구멍은 삽입 또는 제거에 사용되고 높이확보 및 고정장치 자체의 움직임을 방지해주는 역할을 수행한다.

상기의 목적을 달성하기 위하여 본 발명은 모듈로서 각 척추체의 형태에 따라높이를 조절하여 사용하거나 여러 크기의 모듈을 조합해서 사용할 수 있는 척추체 대체 유합 고정장치로써 척추체와 접하는 부분은 만곡형으로 이루어져 있으며, 하나의 결합중간지지대와 2개의 상하면 지지대로 구성되는 길이조절형과 블록의 형태로 일정 사이즈별로 구성되어있는 중간모듈와 상면모듈, 하면모듈을 조립하여 사용하는 길이조립형으로 되어있다. 길이조절형의 척추체 대체 유합 고정장치의 경우, 가운데 연결부분을 기준으로 척추체에 접하여 척추의 하중을 견디는 상·하면 지지대 부분과 몸통부분의 높이를 조절하여줄 수 있는 결합중간지지대의 3부분으로 구성되어 있다. 길이조립형의 척추체 대체 유합 고정장치의 경우, 윗면은 매식된 고정장치가 튀어나오지 않도록 돌기가 형성되어있고 밑면은 다른 모듈과 결합할 수 있도록 쐐기형태로 되어있어 다양한 크기의 조합을 통하여 척추체의 높이를 회복시켜줄 수 있도록 되어있다.
이하, 첨부 도면을 참조하여 본 발명의 바람직한 실시 예를 상세하게 설명하면 다음과 같다.
상기한 길이조절형 척추체 대체 유합 고정장치는 척추체(1)의 상하단과 접하는 윗부분(8)과 아랫부분(9)에 고정돌기(5)를 가공하여 척추체의 비틀림이나 굽힘에 대해 미끌리지 않도록 하였으며, 척추체의 형상에 맞게 만들어 척추체의 골단판과의 접촉면적을 향상시킴으로써 척추체의 하중에 견디도록 하여 골밀도가 약한 골조송증 환자에서도 복원된 추체의 결손을 유지할 수 있도록 하였다. 길이조절형 척추체 대체 유합 고정장치의 경우 결합 중간지지대(4)와 상·하면 지지대(3)를 상·하면 지지대 삽입구멍(7)에 결합하여 척추체의 결손정도에 따라 높이를 조절할 수 있도록 높이조절나사산(10)을 만들어 시술자가 원하는 척추체의 높이를 선택할 수 있도록 하였고, 측면에는 원형이나 장방형의 골유합 통로(11)를 만들어, 뼈가 보다 잘 밀착하여 안정되게 자라고 비틀림에 의해 고정장치의 높이가 가라앉지 않도록 결합나사(12)를 설치하였다. 또한, 인체의 해부학적 구조에 맞는 형태로 제작하기 위하여 측면에서 보았을 때 앞쪽의 높이가 뒤쪽의 높이와 같은 스트레이트 형태와 앞쪽의 높이가 뒤쪽보다 높도록 만들어 각 환자마다 분절의 전만곡이 회복되도록 하였다.
또, 다른 형태의 길이조립형 척추 대체 유합 고정장치는 블록의 형태로 일정 사이즈별로 구성되어있는 중간모듈(14)와 상면모듈(13), 하면모듈(15)을 조립하여 사용하도록 되어있으며 윗면 접촉부위(18)와 밑면 접촉부위(19)는 척추체에 밀착되어 척추체의 하중을 견딜 수 있도록 되어있으며, 요(凹)형태의 결합부위(18)와 철(凸)형태의 결합부위(19)가 완전 밀착되어 결합이 되도록 되어있다. 중간모듈(14), 상면모듈(13), 하면모듈(15)은 중간부위에 원통형으로 뚫려 있어 골편을 채워 넣어 골유합을 촉진시킬 수 있도록 되어있다. 측면에서 보았을 때 앞쪽의 높이가 뒤쪽의 높이와 같은 스트레이트 형태와 앞쪽의 높이가 뒤쪽보다 높도록 만들어 각 환자마다 분절의 전만곡이 회복되도록 하였고, 상하 추간골의 비틀림에 대해 튀어나가지 않도록 돌기를 만들어 안정성을 향상시키고 횡하중이 전달될 때 조립된 형태가 흐트러지지 않도록 결합나사를 사용하였다. 또한, 각 모듈의 옆면에는 골유합을 촉진시킬 수 있도록 슬릿 또는 원형의 구멍을 뚫어 골성장을 촉진하여 척추체 대체 유합 고정장치의 안정도를 높여주었다.
상기한 길이조절형 척추체 대체 유합 고정장치와 길이조립형 척추체 대체 유합 고정장치는 기존의 고정장치들보다 길이 조절 및 조합이 용이하여 삽입이 용이하도록 하였고, 골단판과의 접촉면적을 늘려 골이 약한 고령의 환자에서도 회복된 골 결손부위의 높이를 확고히 유지할 수 있도록 하였으며, 고정장치 내에는 후궁 제거술시 얻어진 뼈를 채워 사용할 수 있어 따로 커다랗고 단단한 지주골 이식편을 얻어야 하는 불편을 없애므로써 골 공여부에서 발생하는 수술 후 통증을 줄이거나 없앨 수 있도록 하였고, 경우에 따라 고정장치 내에 골편 대신 수산화 아파타이트를 삽입하여 뼈 대신에 사용함으로써 골이식편을 따로 채취해야 하는 번거로움과 골 공여부의 합병증을 줄일 수 있도록 하였다. 또한, 역나사형태의 고정장치의 경우 높이확보를 위해 계속적으로 나사를 돌려 높이확보를 해야되는 것을 본 발명에서는 결합중간지지대를 일정각도만 돌려주면 상·하면 지지대가 풀려 원하는 높이까지 쉽게 확보할 수 있어 수술시간 단축 및 시술의 간편함에 도움을 줄 수 있다. 길이조립형의 경우, 각 사이즈별로 조립을 하여 원하는 길이를 미리 만들어 삽입할 수 있어 과거에 원하는 길이로 잘라서 쓰던 고정 장치에 비해 원하는 높이를 쉽게 만들 수 있고 잘라서 버리던 재료를 절약할수 있는 장점이 있다.
또한, 이 고정장치는 벽이 두껍지 않아 같은 부피의 다른 고정장치에 비해 많은 양의 골편을 채울 수 있고 고정장치의 단면적이 그대로 추체골과의 접촉면을 이루며, 고정장치 사이사이에도 골편을 채움으로써 확실한 골유합을 기대할 수 있고,높이 확보를 위해 상·하면 지지대와 중간연결지지대와의 접촉면적을 최대로 넓혀 척추에 가해지는 수직응력 및 횡응력에 대한 저항력을 높였다. 재질로써는 현재까지 알려진 생체용 금속재료로는 생체적합성이 가장 뛰어난 티타늄을 재료로 하여 생체내에서 발생할 수도 있는 염증반응을 최소화하였다.
이상 설명한 것과 같이 본 발명은 척추, 특히 척추 골절이나 척추에의 종양, 감염으로 척추체를 제거해야하는 모든 질환에서 척추체의 결손 부위를 쉽게 복원, 유지하고 유합에 필요한 최적의 환경을 만들어 주어 유합을 촉진시키고 불유합율을 감소시킬 수 있다. 또한 변형이 된 분절의 교정을 도울 뿐만 아니라 교정된 변형을 교정된 상태로 그대로 유지시켜 줌으로써 수술의 결과를 좋게 하고 향후 발생할 수 있는 후유증을 예방할 수 있다.

Claims (3)

  1. 측면으로 접근하여 손상된 추체와 추간판을 하나이상 제거한 후 추체 사이에 삽입이 가능하고 단면적이 추체의 형상으로 되어있으며, 추체의 상하면과 접촉하는 부분에 핀 혹은 톱니모양의 돌기를 가지고 있으며, 고정장치 측면에 고정장치 삽입기구와 연결되는 원형 나사 구멍과 일정 높이를 확보해주기 위한 고정나사구멍을 가지며, 일정한 간격으로 원형 또는 장방형의 골유합통로를 가지는 조절형 척추체 대체 유합 고정장치
  2. 제 1항에 있어서, 상기한 모듈형 척추체 대체 유합 고정장치의 위아랫부분이 척추 추체의 상하면과 접하도록 되어 있으며, 삽입 후 시술자가 원하는 높이까지 결합중간지지대를 돌려서 상하면 지지대를 풀어주고 상하면 지지대를 양쪽으로 늘려 원하는 높이를 만든 후 결합중간지지대를 다시 원상태로 돌려 고정장치의 높이를 확보하는 구조의 척추체 대체 유합 고정장치
  3. 제 1항에 있어서, 상기한 모듈형 척추체 대체 유합 고정장치의 위아랫부분이 척추 추체의 상하면과 접하도록 되어 있으며, 다른 높이의 블록을 조립하여 원하는 높이를 확보하는 구조의 조립형 척추체 대체 유합 고정장치
