Abstract
IT-139, 나트륨 트랜스-[테트라클로로비스(1H-인다졸)루테네이트(III)]는, 정맥내 투여되는 소분자 화합물이다. 임상전 항종양 및 작용 기전 연구에서, IT-139는 표준 항암제(예를 들면, 백금, 빈카 알카로이드, 탁산, 안트라사이클린)에 저항성인 것을 포함하여 광범위한 종양 타입에 대항하는 활성을 나타내었다. 이러한 활성은 GRP78 경로를 표적화하는 IT-139의 신규한 작용 기전에서 일어나는 것으로 고려된다. 발명의 분야
본 발명은 일반적으로 트랜스-[테트라클로로비스(1H-인다졸)루테네이트 (III)] 및 이의 암 치료에서의 용도에 관한 것이다.
발명의 배경
최근 몇년 동안 암 치료에서 많은 발전이 있었다. 그러나, 전이성 질환 대부분의 경우는, 종양 세포가 항암제에 의해 야기된 손상을 극복하고 생존하는 기전을 발달시키기 때문에 치료는 치유법이 아니다. 이들 종양 세포의 생존/저항 기전의 표적화 및 극복은 활발한 연구 주제인 항암 표적화의 영역이다. 따라서, 암, 및, 특히, 저항(resistance)을 치료하기 위해 치료제의 개발해야 할 충족되지 않은 필요성이 여전히 있다.
발명의 요지
본 발명의 화합물, 및 이의 조성물이, 암을 치료하기 위해 유용하고, 특히, 종양 세포의 생존 및 저항 기전을 표적화하기 위해 유용하다는 것이 밝혀졌다.

실시예 1. 모든 사람 일차 및 전이성 세포주를 ATCC에 기재된 바와 같이 유지하였다. 세포를 배지 단독(대조군), IT-139(200 μM) 단독, 탑시가르긴 단독(300nM, 세포 스트레스를 유발하기 위해) 또는 IT-139 플러스 탑시가르긴으로 처리하기 전에 6 웰 플레이트에 시딩하였다. 세포 용해물을 16시간에 수집하고, 웨스턴 블롯에 의해 항-GRP78 항체에 대해 분석하고, GAPDH로 정규화하였다. 도 1에 나타낸 결과는 IT-139가 스트레스 비유발 세포에 미치는 효과가 거의 없거나 없지만(도 1A), 스트레스 유발된 세포에서 IT-139로의 치료(IT-139 플러스 탑시가르긴)는 스트레스 유발된 세포에 존재하는 GRP78의 양을 감소시켰음을 입증한다(도 1B).

실시예 3. CT26 세포(1 백만)를 면역적격 마우스에 피하 이식하고, 감지할 수 있는 종양이 확립될 때까지 성장되게 하였다(3일). 그룹당 4마리 마우스의 그룹을 주 2회 1) 식염수, 2) IT-139 단독(KP1339, 30 mg/kg), 3) RPM1-14(PD-1 항체, 5 mg/kg), 또는 4) RPM1-14 및 IT-139(각각 5 mg/kg 및 30 mg/kg) 중 어느 하나의 총 4회 용량으로 처리하였다. IT-139를 정맥내 투여하고, RPM1-14를 복강내 투여하였다. 투여를 같은 날에 수행하였다. 종양 용적을 18일 내내 측정하였다. PD-1 항체로의 치료는 식염수 대조군으로부터의 변화를 나타내지 않았다. IT-139는 항종양 활성을 입증하였지만, PD-1 항체 및 IT-139의 병용제는 PD-1 항체 단독 또는 IT-139 단독 중 어느 하나와 비교하여 증가된 항종양 활성을 입증하였다. 도 3에 나타낸 결과는 IT-139가 PD-1 항체의 항종양 효능을 증가시킴을 입증한다.
실시예 4. HCT116 세포주를 ATCC 가이드라인에 따라 치료 1일 전에 시당하였다. IT-139(200 μM)로 치료 후 24시간에, 처리된 세포 및 비처리된 세포를 전자 현미경 평가를 위해 고정하였다. 도 4에 나타난 바와 같이, IT-139로 처리된 HCT116 세포(도 4B)는 유의한 액포화를 나타내고, ER 팽창 및 세포내 소기관의 붕괴는 비처리된 세포(도 4A)와 비교하는 경우 ER 스트레스를 제시한다.
실시예 5. 염색질 면역침전(ChIP) 검정을 수행하여 IT-139 처리된 스트레스 유발된 세포 및 스트레스 비유발 세포에서 Pol II 및 GRP78 프로모터 영역에 대한 폴리머라제 II 결합을 조사하였다. HCT116 세포를 80% 컴플루언스(confluence)까지 성장시키고, 이어서, 1.5 μg/mL 투니카마이신 및 200 μM IT-139 또는 DMSO로 16시간 동안 처리하였다. 염색질을 포름알데히드를 사용하여 가교-결합시켰다. 세포를 트립신으로 수거하고, 단리된 핵을 초음파처리하여 200 내지 1000 bp의 분획을 수득하였다. 동량의 염색질을 항-Pol II 항체로 밤새 인큐베이팅하고, 이어서, Staph A 세포로 풀다운(pull down)하였다. 가교결합을 역전시키고, DNA를 GeneElute PCR 클린업 키트(제조원: Sigma)를 사용하여 정제하였다. 정제된 DNA 및 투입 샘플을 Grp78 및 Pol II의 프로모터 영역을 증폭시키는 프라이머로 30 주기의 PCR에 수행하였다. 생성물을 4% 아가로스 겔 상에서 작동시키고, 에티듐 브로마이드 염색으로 가시화하였다. IT-139는 비-스트레스 유발된 세포에서 Pol II 프로모터에 대한 폴리머라제 II 결합에 최소 효과를 미치지만, Tu-스트레스 유발된 세포에서 40%까지 이러한 결합을 감소시켰다. 두드러지게, IT-139는 비-스트레스 유발된 세포 및 Tu-스트레스 유발된 세포 둘 다에서 Grp78 프로모터에 대한 폴리머라제 II 결합을 제로(zero)까지 강하시켰다. 이들 결과를 도 5A 내지 C에 나타낸다.
실시예 6. 면역조직화학적 분석을 수행하여 30mg/kg(q4d)에서 IT-139로 처리된 HT-29 이종이식 종양에서 GRP78의 발현을 식염수 처리된 종양과 비교하여 분석하였다. GRP78의 강한 면역염색이 IT-139 처리된 종양(도 6B)에서 매우 약한 염색과 대조적으로 식염수 처리된 종양(도 6A)에서 관찰되었다. 이들 결과는 IT-139가 생체내 GRP78 발현을 저해함을 나타내었다.
실시예 7. 정상 사람 배아 신장 293T 세포는 비-스트레스 유발되거나(정상 배지로 처리) 또는 스트레스 유발되었다(300 nM EndRet 스트레스 & GRP78 유도자 탑시가르긴으로 처리됨). 세포 배양물을 약물 없음(대조군) 또는 200 μM IT-139로 처리하였다. GRP78 수준을 웨스턴 블롯으로 평가하고; β-악틴은 부하 대조군(loading control)이다. 결과를 도 7에 나타낸다.
실시예 8. 전립선 암 LnCaP-FGC 세포를 비처리하거나(DMSO 대조군 단독) 300 nM 탑시가르긴(Tg)으로 처리하였다. 세포를 IT-139의 지시된 농도로 공-인큐베이팅하고, 노던 블롯 분석을 세포 용해물에서 수행하였다. β-악틴은 부하 대조군이다. 도면은 노던 블롯을 나타낸다. 밴드를 정량화하고, 막대 그래프는 β-악틴 부하 대조군에 대하여 정규화된 GRP78 mRNA 수준의 상대적 수준을 나타낸다. GRP78의 IT-139 억제는 IT-139로 처리된 종양 세포의 노던 블롯 분석으로 나타낸 바와 같이 투약 의존 방식에서 전사 수준에서 존재한다. 결과를 도 8에 나타낸다.
실시예 9. 모든 사람 일차 및 전이성 세포주를 ATCC에 기재된 바와 같이 유지하였다. 모든 세포주(HCT116, HT-29, LNCaP, A549 및 A375)를 IT-139 단독으로 치료하기 24시간 전에 6 웰 플레이트에 시딩하였다. 모든 세포주를 다음 사항을 예외로 하여 30uM, 50uM, 100uM 및 200uM로 처리하였다: 30uM 및 200uM으로의 치료를 HCT116 및 HT29 세포주 각각에 대해 생략하였다. 약물로 24시간 인큐베이션 후, 세포를 트립신화하고, 따뜻한 PBS에서 재-현탁시키고, JC-1 염료로 30분 동안 37℃에서 암실에서 염색하였다. 이어서, 세포를 세척하고, 따뜻한 PBS에 재-현탁시켰다. HCT116 및 HT29 세포주 둘 다는 IT-139의 더 낮은 농도에서, 각각 50 μM 및 30 μM에서 증가된 미토콘드리아 전위 손실을 나타내는 결과는 보여준다. 그러나, 전립선(LNCaP), 폐(A549) 및 흑색종(A375) 세포는 높은 농도의 IT-139에서 미토콘드리아 탈분극의 증가를 나타내었다. 데이터를 도 9에 나타낸다.
실시예 10. 전립선 암 세포주, MiaPaca2는, ATCC에 의해 기술된 바와 같이 유지되었다. IT-139의 농도(50, 100 및 200 μM)를 증가시키면서 24시간 동안 처리한 정상 말초 혈액 단핵 세포(PBMCs)로 공-배양된 MiaPaca2 세포는 세포 생존력에 어떠한 영향도 나타내지 않고, 이를 도 10에 나타낸다. MiaPaca2 세포는 비처리되거나 IT-139로 예비-처리되고(100 μM), 다양한 투약으로 IL-2 활성화된 PBMC로 공-배양되었다. PBMC를 6000 IU의 IL-2를 사용하여 24시간 동안 활성화시켰다. IT-139는 활성화된 PBMC와 공-배양된 MiaPaca2 세포에서 증가된 세포사멸을 나타내고, 이를 도 11에 나타낸다.
실시예 11. IT-139 및 겜시타빈(GEM) 병용 치료는 ASPC 마우스 모델에서 중앙값 및 전체 생존 둘 다를 연장한다. 마찬가지로, 시험관내 ASPC-1 세포에서 IT-139와 병용된 겜시타빈의 48시간에 병용 투약은, IT-139가 먼저 투약된 경우 생존에 유의한 변화를 야기한다. 시험관내 ASPC20 세포를 48시간 동안 DMSO(대조군)으로, 150 μM IT-139, 5 μM 겜시타빈, 150 μM IT-139 및 5 μM 겜시타빈 동시, 24시간 동안 5 μM 겜시타빈 이어서, 150 μM IT-139, 또는 150 μM IT-139 이어서, 24시간 5 μM 겜시타빈으로 처리하였다. 세포를 수거하고 트리판 블루로 계수하여 사멸 세포 대 생존 세포의 수를 계산하였다. ASPC20 세포에서, IT-139와 병용된 스트레스를 유발하기 위한 탑시가르긴(Tg) 투약 순서는, GRP78 발현의 수준에 30시간에 영향을 미친다. 결과를 도 12에 나타낸다. 추가로, 증가된 세포사멸이 IT-139가 겜시타빈 전에 투약되는 경우 관찰되므로, 투약 순서가 또한 세포사멸의 양에 영향을 미친다. 결과를 도 13에 나타낸다. PANC-1 세포를 시험관내 48시간 동안 DMSO(대조군), 150 μM IT-139, 5 μM 겜시타빈, 150 μM IT-139 및 5 μM 겜시타빈 동시, 24시간 동안 5 μM 겜시타빈 이어서, 150 μM IT-139, 및 150 μM IT-139 이어서, 24시간 5 μM 겜시타빈, DMSO(대조군), 150 μM IT-139, 5 μM 겜시타빈, 150 μM IT-139 및 5 μM 겜시타빈 동시, 24시간 동안 5 μM 겜시타빈 이어서, 150 μM IT-139, 및 150 μM IT-139 이어서, 24시간 5 μM 겜시타빈으로 처리하였다. 결과를 도 14에 나타낸다. PANC-1 세포에서, 48시간에서 세포독성 효과는 겜시타빈 및 IT-139의 투약 순서에 의존하는 차이는 없음을 입증한다.
실시예 12. A20 림프종 마우스 모델에서, 104 Balb/c 마우스를 림프종 A20 세포로 마우스의 우측 옆구리 상에 피하 접종하였다. 마우스는 종양이 80 내지 120 ㎣의 평균 용적에 도달하면, 10마리 마우스의 8 그룹 내로 무작위배정하였다. 마우스를 행동 및 생존에 대해 매일 그리고 체중 및 종양 성장에 대해 주 2회 모니터링하였다. 종양을 마트리겔(50:50, v:v, ref: 356237, BD Biosciences, France) 함유 200 μL RPMI 1640 배지에서 5x106의 A20 세포의 피하 주사로 백네 마리(104) Balb/C 마우스의 우측 옆구리 내로 유도하였다. 체크포인트 저해제는 항-PD-L1(클론 10F.9G2; ref: Bioxcell 이소타입 래트 IgG2b), 및 항-CTLA4 항체(클론 9H10; ref: BE0131, bioxcell; 이소타입 햄스터 IgG1)였고, 둘 다 10 mg/kg i.p로 3일마다 투약되었다. IT-139를 30 mg/kg 정맥내로 4일마다 투약하였다. 용량 스케줄을 하기 표 1에 나타낸다. 실험 과정 동안 평균 종양 용적을 도 15에 나타낸다. 각 그룹으로부터 5 종양을 FACS 분석을 위해 수집하였다(40 종양 샘플). 하나의 패널은 Treg 및 T 이펙터 세포(CD45, CD3, CD4, CD8 및 FoxP3)에 대해 수행되고, 하나의 패널은 MDSC(CD45, CD3, CD11b, Gr-1, Ly-6g, Ly-6C, Arg1, NOS2)에 대해 수행되고, 하나의 패널은 종양-관련 마크로파지(CD54, CD11b, Gr-1, CD68, CD80, 및 CD206)에 대해 수행되었다. PD-L1이 IT-139 투여 24시간 전에 투약되는 경우, 유의한 항종양 효능 및 종양의 이펙터 T-세포 침윤의 10% 증가가 있었고, 이를 도 16 및 도 17에 나타내었다. 이러한 효과는 다른 어떤 그룹에서도 나타나지 않았다. 이러한 병용 그룹에서 종양 성장의 감소와 함께, 항-PD-L1 항체를 사용한 면역조절 효과의 증거가 존재한다.
본 발명자들은 다수의 실시형태를 기술하지만, 본 발명의 기본적인 예시는 본 발명의 화합물 및 방법을 이용하는 다른 실시형태를 제공하기 위해 변경될 수 있다는 것이 명백하다. 따라서, 본 발명의 범위가 예의 방식으로 나타낸 특정 실시형태보다는 첨부된 청구범위에 의해 한정되는 것이 명백할 것이다.
Claims (22)
- 췌장암의 치료를 필요로 하는 환자에서 췌장암을 치료하기 위한 방법에 사용하기 위한 약제학적 조성물로서, 상기 조성물이 나트륨 트랜스-[테트라클로로비스(1H-인다졸)루테네이트(III)] (IT-139)를 포함하고, 상기 조성물이 겜시타빈과 조합하여 투여되고, 여기서 상기 IT-139가, 상기 겜시타빈을 상기 환자에게 투여하기 전에, 상기 환자에게 투여되고, 상기 치료가 상기 겜시타빈 단독의 투여와 비교하여 글루코오스 조절 단백질 78 (GRP78)의 양의 감소를 초래하는, 약제학적 조성물.
