Abstract
본 발명은 체외막형산화치료 동물모델 및 이의 제조방법에 관한 것으로, 동물모델의 면역기능을 보다 안정하게 유지할 수 있으면서도, 동물을 장시간 안정적으로 유지할 수 있으므로, 체외막형산화치료의 다양한 반응, 변화 및 추가 치료 접목 방법에 대한 연구를 수행하는데 유용하게 활용할 수 있는 동물모델의 제공이 가능하다.
Description
본 발명은 체외막형산화치료 동물모델 및 이의 제조방법에 관한 것이다.
체외 순환(extracorporeal circulation: ECC) 장치는 현대 의학의 필수 구성 요소이며, 치료 과정에서 수십년 동안 광범위하게 사용되고 있다. 그 중 하나인 체외막형산화치료(Extracorporeal Membrane Oxygenation: ECMO)는 기존 치료에 반응하지 않는 심각한 심장이나 폐에 심장 및/또는 폐 기능을 보조하는 장치를 사용하는 치료이다. ECMO에 의해 지원되는 순환 및 호흡으로 인하여, 인공 호흡기 유도 폐 손상(ventilator induced lung injury: VILI) 또는 높은 용량의 강심제 및 혈관수축제 사용에 따른 합병증을 예방할 수 있으며, 손상된 장기의 치료, 회복 또는 이식이 가능하다.
ECMO로 인한 전신성 염증 반응 증후군(systemic inflammatory response syndrome: SIRS) 및 혈액-인공 표면 상호 작용 등의 부작용을 연구하기 위한 수많은 실험적 연구가 보고되고 있으나, 주요 메커니즘 및 억제 방법은 아직 완전히 이해되지 않고 있다. 합병증을 완전히 통제할 수 없기 때문에, 임상에서의 탁월한 개선 효과와는 상관없이, 여전히 이와 같은 부작용이 발생할 우려가 존재한다.
따라서, ECMO의 병인학 및 병리생태학적 기전을 더 명확하게 이해하기 위하여, 여러 동물모델이 개발되었다. ECMO 관련 합병증을 연구하기 위해 사용되는 개, 양, 돼지 및 토끼와 같은 ECMO 대형 동물모델의 경우, 수술 비용, 인력 소요 및 오랜 회복 시간과 같은 문제점이 존재한다. 반면, ECMO 소형 동물모델은 ECMO의 병태 생리학적 기전을 밝히기 위하여 더욱 편리하게 사용할 수 있으며, 큰 표본 크기로 인한 저렴한 비용 및 단일 실험자에 의한 모든 연구 과제의 수행이 가능하다는 등의 장점이 있다. 그러나, ECMO 소형 동물모델은 원활한 혈액 도출이 되지 않아 소동물이 치료 시작 전 사망하게 되는 문제점이 존재하여, 완전히 완성이 되지 않은 상황이다.
전술한 문제점을 해결하기 위하여, 본 발명자는 알부민을 포함하는 충진액(priming solution) 및 다중 구멍이 형성된 배액 도관을 사용하여 체외막형산화치료 동물모델을 확립하였다.
구체적으로, 본 발명의 하나의 목적은 알부민(albumin) 및 생리 식염수를 포함하는 체외막형산화치료(Extracorporeal Membrane Oxygenation) 동물모델의 충진액(priming solution)용 조성물을 제공하는 것이다.
본 발명의 다른 목적은 체외막형산화치료 동물모델의 제조방법을 제공하는 것이다.
본 발명의 또 다른 목적은 체외막형산화치료 동물모델을 제공하는 것이다.
본 발명의 일 양상은 알부민(albumin) 및 생리 식염수를 포함하는 체외막형산화치료(Extracorporeal Membrane Oxygenation: ECMO) 동물모델의 충진액(priming solution)용 조성물을 제공한다.
ECMO에 의하여 부정맥, 기흉, 폐출혈, 헤파린 사용으로 인한 뇌출혈 및 뇌경색, 인공 호흡기 유도 폐 손상 및/또는 높은 용량의 근육수축제 및 혈관수축제 사용에 따른 합병증 등의 부작용이 발생할 수 있다. 다만, ECMO는 말기 심부전 또는 호흡부전 환자들의 보조를 위하여 수행되며, 이를 대체할 수단이 마땅치 않다는 점에서, 이러한 부작용의 위험성이 있음에도 광범위하게 사용되고 있는 실정이다.
따라서, ECMO의 부작용을 최소화할 수 있는 방법의 개발의 필요성이 대두되고 있으나, ECMO의 기전에 대해서는 아직 명확하게 밝혀지지 않았고, 체외막형산화치료 상태를 유사하게 반영한 동물모델 역시 현재 개발되지 못하였다는 점에서, ECMO의 기전을 밝히기 위한 연구에 걸림돌이 되고 있다.
종래 ECMO의 충진(priming)에 주로 사용된 결정질 및 콜로이드로 구성된 충진액과는 달리, 본 발명의 충진액용 조성물을 사용할 경우, ECMO 동물모델을 2시간 이상 안정하게 유지할 수 있으므로, 본 발명의 충진액용 조성물은 ECMO의 기전 연구에 유용하게 활용될 수 있다.
본 발명에서 사용되는 용어, "체외막형산화치료(ECMO)"란 심폐기능을 보조할 목적으로 수술장 밖에서 심폐바이폐스 기법을 사용하는 것을 의미한다.
본 발명에서 사용되는 용어, "충진액"이란 ECMO 회로의 프라이밍에 사용되는 용액을 말한다. "ECMO 회로의 충진"이란 기포가 없는 전해액으로 ECMO 회로를 채우는 것을 말한다.
본 발명의 일 구체예에 따르면, 상기 알부민 및 상기 생리 식염수는 1:1의 부피비로 포함되고, 상기 알부민의 농도는 20%(w/v)인 것일 수 있다.
체외막형산화치료 동물모델에 본 발명의 충진액용 조성물을 적용함에 있어서, 충진액의 부피는 15㎖가 초과되지 않도록 함으로써, 체외 순환으로 인한 혈액 희석을 최소화할 수 있다.
본 발명의 다른 양상은 체외막형산화치료 회로와 연결된 배액 도관(drainage cannula)을 래트의 우측 외경정맥(external jugular vein)에 삽입하고, 상기 회로에 연결된 관류 도관(perfusion cannula)을 상기 래트의 우측 경동맥(carotid artery) 또는 좌측 경정맥에 삽입하는 단계를 포함하고, 상기 체외막형성산화치료 회로의 충진액은 알부민(albumin) 및 생리 식염수를 포함하는 충진액용 조성물인 것인 체외막형산화치료 동물모델의 제조방법을 제공한다.
본 발명에서 사용되는 용어, "정맥-정맥형(VV mode)"은 ECMO 순환에 있어서, 동물모델의 혈액을 정맥에서 빼내어 정맥으로 돌려주는 방식을 말한다. "정맥-동맥형(VA mode)"은 동물모델의 혈액을 정맥에서 빼내어 동맥으로 돌여주는 방식을 말한다.
본 발명의 동물모델은 정맥-정맥형 또는 정맥-동맥형으로 ECMO 순환이 이루어질 수 있으며, 정맥-동맥형 대비 정맥-정맥형에서 동물의 면역력이 더욱 안정적으로 유지될 수 있다.
본 발명의 동물모델은 ECMO의 충진액을 알부민 및 생리 식염수를 포함한 용액, 특히, 20%(w/v) 농도의 알부민과 생리 식염수를 1:1의 부피비로 혼합한 용액을 사용함으로써, 동물을 장시간, 예를 들어, 2시간 이상 안정하게 유지할 수 있다.
본 발명의 ECMO 동물모델은 ECMO 순환에 따른 동물모델의 생리적 변화를 연구하기 위하여 사용될 수 있다. 본 발명의 동물모델은 소형 동물, 예를 들어 래트를 사용하기 때문에, 경제적으로 연구에 사용할 수 있으면서도, ECMO 순환을 정확하게 반영할 수 있기 때문에, ECMO의 기전 연구에 유용하게 활용할 수 있다.
본 발명의 일 구체예에 따르면, 상기 배액 도관은 측면에 구멍이 형성된 것일 수 있다.

본 발명의 일 구체예에 따르면, 상기 배액 도관은 신생아 먹이 튜브(neonatal feeding tube)일 수 있다.
종래에 정맥 배액을 위하여 사용된 다중-구멍 16 게이지 도관은 도관의 끝부분이 날카롭기 때문에 동물모델의 심장이 손상될 우려가 있었다. 반면, 본 발명에서는 배액 도관의 측면에 4개의 구멍을 형성한 신생아 먹이 튜브를 사용할 수 있으며, 이와 같은 도관을 사용함으로써, 블라인드 에이펙스(blind apex)로 인한 동물모델의 심장 손상 확률을 낮추고, 혈액 순환 동안 우수한 배액이 가능하도록 할 수 있다.
본 발명의 또 다른 양상은 체외막형산화치료 회로와 연결된 배액 도관(drainage cannula)을 래트의 우측 외경정맥(external jugular vein)에 삽입하고, 상기 회로에 연결된 관류 도관(perfusion cannula)을 상기 래트의 우측 경동맥(carotid artery) 또는 좌측 경정맥에 삽입하는 단계를 포함하고, 상기 체외막형성산화치료 회로의 충진액은 알부민(albumin) 및 생리 식염수를 포함하는 충진액용 조성물인 것인 체외막형산화치료 동물모델의 제조방법으로 제조된 체외막형성산화치료 동물모델을 제공한다.
본 발명의 ECMO 모델은 정맥-정맥형 또는 정맥-정동형에서, 알부민이 첨가된 충진액이 사용되기 때문에, 동물모델의 면역기능을 안정적으로 유지할 수 있고, 장시간 동안 동물모델의 상태를 안정적으로 유지할 수 있으므로, ECMO 병리학의 연구에 유용하게 활용할 수 있다.
체외막형산화치료 동물모델 및 이의 제조방법에 따르면, 동물모델의 면역기능을 보다 안정하게 유지할 수 있으면서도, 동물을 장시간 안정적으로 유지할 수 있으므로, 체외막형산화치료의 다양한 반응, 변화 및 추가 치료 접목 방법에 대한 연구를 수행하는데 유용하게 활용할 수 있는 동물모델의 제공이 가능하다.

이하 본 발명을 하나 이상의 실시예를 통하여 보다 상세하게 설명한다. 그러나, 이들 실시예는 본 발명을 예시적으로 설명하기 위한 것으로 본 발명의 범위가 이들 실시예에 한정되는 것은 아니다.
실시예 1. ECMO 동물모델 제작 및 분석 방법
1-1. 동물 사육
한국 동물 학회 및 실험동물 자원관리 및 사용지침에 규정된 '실험동물 관리원칙'에 의거하여 모든 래트(rat)를 다루었다. 본 연구는 전남 대학교 동물 실험 연구원(CNU IACUC-H-2018-36)의 승인을 받았으며, 500 내지 600g인 수컷 SD(Sprague-dawley) 래트(SAMTAKO BIO KOREA. CO. LTD. Company) 20마리를 사용하였다. 12h/12h 명암 사이클의 통제된 온도(20℃)에서, 음식 및 물을 표준 상태에서 섭취할 수 있도록 래트를 사육하였다.
1-2. 마취, 수술 준비 및 ECMO 회로
모든 수술 절차는 멸균 상태에서 수행하였다. 모든 래트는 케타민(ketamine)(80㎎/㎏)과 자일라진(xylazine)(40㎎/㎏)의 근육 내 주사 (intramuscular injection)에 의하여 마취되었다. 이소플루란(isoflurane)을 사용하여 수술 과정 중 래트의 마취를 유지하였다. 래트를 외과 테이블에서 앙와위로 고정시키고 온도 조절을 위해 가열 패드를 위치시켰다. 래트에 16 게이지 정맥 도관을 삽관하고, 설치류 인공호흡기(rodent respirator)(Harvard Apparatus Inc., Holliston, Massachusetts)를 사용하여 산소 90% 및 이소플루란 2.5%를 유지하면서 기계적으로 순환시켰다. 1회 호흡량은 10㎖/㎏ 및 호흡수는 60회 호흡/분으로 설정하였다. 우측 대퇴 동맥에 24 게이지 도관을 삽관하여 전신 동맥 혈압을 모니터하였고, 실험 종료시 동맥혈 샘플을 채취하였다(도 1). 정맥-정맥형(Veno-venous: VV)-ECMO 그룹(도 2A)에서는 배액 도관(drainage cannula)을 우측 외경정맥(external jugular vein)에 삽입하고, 관류 도관(perfusion cannula)을 좌측 외경정맥에 삽입하였다. 정맥-동맥형(Veno-Arterial: VA)-ECMO 그룹(도 2B)에서는 배액 도관을 우측 외경정맥에 삽입하고, 관류 도관을 우측 경동맥(carotid artery)에 삽입하였다. 배액 도관의 경우 개조된 신생아 신생아 먹이 튜브(neonatal feeding tube)(5FR 10cm)를 사용하였다. 우심방과 상대정맥의 교차점에 도관 끝을 위치시키기 위해 여러 차례의 부검을 진행한 후 신생아 먹이 튜브을 선택하였다. 또한, 보다 효율적인 배액을 위하여 도관 측면에 4개의 구멍을 뚫었다. 이와 같이 개조한 측면에 다중 구멍이 형성된 신생아 먹이 튜브(도 3A)를 우측 외경정맥을 통해 우심방으로 삽입하였다(도 3B). 도관을 삽입한 후, 헤파린 나트륨(heparin sodium)(500IU/㎏)을 투여하였다. 관류 도관의 경우, 정맥-정맥형-ECMO 그룹에서는 좌측 외경정맥을 통하여 24 게이지 혈관 도관(angiocatheter)을 삽입하였고, 정맥-동맥형-ECMO 그룹에서는 우측 경동맥을 통하여 24 게이지 혈관 도관을 삽입하였다.
1-3. ECMO 회로의 설치
ECMO 회로는 막성 산소 공급기(membranous oxygenator)(Micro-1 래트 산소공급(oxygenation), 교환 표면(exchange surface) 0.05m2, 프라이밍 부피 3.5㎖, 폴리프로필렌 타입, Dongguan Kewei Medical Instrument Co. LTD. China), PVC 튜브 라인(내부 지름: 2.5mm), 정맥 저혈조(10㎖ 주사기) 및 롤러 펌프(roller pump)(Watson-Marlow pumps, Falmouth, Cornwall, TR11 4RU, UK)로 구성하였다. 정맥 도관(venous cannula), 정맥 튜빙 라인(venous tubing line) 및 정맥 저혈조(venous reservoir)는 3 방향 멈춤 꼭지(three way stopcock) 커넥터로 서로 연결하였다. 5㎖ 20% 알부민(albumin) 용액을 포함한 정맥 저혈조는 우심실의 정맥 도관의 위치가 바뀔 때 발생할 수 있는 펌프 흐름을 보조하기 위해 필요한 경우 추가 유체의 역할을 한다. ECMO 회로를 14㎖ 충진액으로 채웠다. ECMO 순환은 200rpm(50㎖/분)의 유속으로 120분 동안 유지하였다. 순환 중에는 저체온증을 예방하기 위해 실험용 필드 주변에 가열 램프와 패드를 적용하였다.
1-4. 알부민 기반(albumin-based) 충진액 또는 생리 식염수 기반(saline-based) 충진액을 사용한 ECMO 수행
실시예 1-1에서 준비한 래트를 생리 식염수 기반 충진액의 생리 식염수 그룹(n=5, 14㎖ 생리 식염수)과 알부민 기반 충진액의 알부민 그룹(n=5, 7㎖ 20% 알부민 용액(알부민 함유량: 0.2g/㎖) + 7㎖ 생리 식염수)의 두 그룹으로 나누었다. 실시예 1-3에서 준비한 ECMO를 실시예 1-2의 정맥-정맥형 방법으로 모든 래트에 적용하였다. 심박수, 혈압 및 산소 포화도를 지속적으로 모니터링하면서, 혈액 순환 동안 6회 기록하였다(3, 15, 30, 60, 90 및 120분). ECMO 순환 전(ECMO 전, 실험 하루 전, 2.5㎖) 및 ECMO 순환 종료 직후(ECMO 후 2.5㎖)에 혈액 샘플(각각 2,5㎖)을 수집하였다. 완전 혈구 측정(complete blood count: CBC) 및 동맥혈 가스 분석(arterial blood gas analysis: ABGA)을 수행하여 수집한 혈액 샘플을 평가하였다. 또한, IL-1β, IL-6, IL-10 및 TNF-α의 사이토카인 수준도 측정하였다. 시료 채취 후 충진액을 보충하여 손실된 혈액을 보충하였다. ECMO 후 혈액 샘플링 후, 혈압(BP) 조절하에 5㎖/분의 유속으로 3 내지 5㎖ 충진액을 투여하였다. 모든 수술 절차를 마친 후 약 30분 동안 기계 환기를 유지하고 래트에서 조심스럽게 탈관하였다.
1-5. 유세포 분석
래트를 정맥-정맥형-ECMO 그룹(n=10)과 정맥-동맥형-ECMO 그룹(n=10)의 두 그룹으로 나누었다. 두 그룹 모두 7㎖ 20% 알부민과 7㎖ 생리 식염수로 구성된 충진액을 사용하였다. 기타 모니터링, 혈액 샘플링 및 수술 전 치료 방법은 실시예 1-4의 방법과 동일하게 수행하였으며, CD3, CD4, CD8, CD43, CD45, CD45R, CD48, CD161 및 His48의 항체로 유세포 분석을 추가로 수행하였다.
1-6. 통계분석
통계적 유의성은 독립표본 t-검정(Independent t-test)과 반복 측정 ANOVA를 사용하여 결정되었다. P value < 0.5는 모든 분석(MedCalc®Version 18.5, Belgium)에서 유의한 것으로 간주되었다. 모든 데이터는 평균±95% 신뢰 간격으로 나타내었다.
실시예 2. 알부민 기반 충진액의 우수성 확인
ECMO 동물모델 제작시 알부민 기반 충진액의 우수성 확인하기 위하여, 실시예 1-4를 수행하여, ECMO 후, 래트의 혈액 샘플 및 사이토카인 수준을 평가하였다.
그 결과, ECMO 시간의 중앙값은 생리 식염수 그룹(40분) 대비 알부민 그룹(120)에서 유의하게 길었다(p value < 0.01). 또한, 알부민 그룹에서는 120분 후에 3마리의 래트가 생존하였다. 반면, 생리 식염수 그룹에서 살아남은 래트는 없었다. ECMO 후 젖산 농도의 중앙값의 경우, 생리 식염수 그룹(13.6) 및 알부민 그룹(8.2)간에서 유의한 차이가 나타남을 확인하였다(p value < 0.03).

상기 결과를 통하여, 본 발명의 알부민이 포함된 충진액을 사용함으로써, 2시간 이상 안정한 ECMO 동물모델을 제작할 수 있음을 확인하였다.
실시예 3. 정맥-정맥형-ECMO 동물모델의 우수성 확인
정맥-정맥형-ECMO 동물모델의 우수성을 확인하기 위하여, 실시예 1-4를 수행하여 래트의 혈액 샘플 및 사이토카인 수준을 분석하였고, 실시예 1-5를 수행하여 선천선 면역 세포를 분석하였다.
그 결과, 순환 형태에 상관없이 모든 래트는 ECMO의 탈관 후 생존하였음을 확인하였다(도 4). 다만, 사전 샘플링 후 죽은 래트(1마리) 및 AA-ECMO 순환을 받아 큰 심방중격결손(atrial septal defect)이 나타낸 래트(1마리)는 제외하였다.
두 그룹에서 수축기 혈압(systolic BP: SBP), 이완기 혈압(diastolic BP: DBP) 및 심박수(heart rate: HR)의 혈류역학 측정치(hemodynamic parameters)를 측정한 결과는 하기 표 2 및 도 5와 같다. SBP(도 5A) 및 DBP(도 5B)는 정맥-정맥형-ECMO 그룹에 비해 정맥-동맥형-ECMO 그룹에서 훨씬 높았고 안정적이었다(p=0.017, p=0.02). SBP 및 DBP는 모든 그룹에서 15분 대비 90분 및 120분에서 크게 낮아졌다. HR(도 5C)은 정맥-정맥형-ECMO 그룹에 비해 정맥-동맥형-ECMO 그룹에서 훨씬 높았다(p=0.005).

정맥-동맥형-ECMO 그룹에 비해 정맥-정맥형-ECMO 그룹에서 혈청의 젖산(serum lactate) 수준이 2배 이상 증가하였다(도 6). 그러나 ECMO 전후의 혈액 가스(O2, CO2)는 두 그룹간에 유의한 차이가 없음을 확인하였다(표 3).

한편, 단핵구(monocyte)(도 8), B 세포, 도움 T 세포, 독성 T 세포 및 NK 세포 등 선천성 면역 세포는 ECMO 후 유의하게 감소하였다(p < 0.05). 정맥-동맥형-ECMO 그룹에서는 T 세포 수가 통계적으로 감소했지만(p <0.05), 정맥-정맥형-ECMO 그룹에서는 감소하지 않았다(표 5).

한편, IL-1β의 수준은 ECMO 전과 비교하여 ECMO 후 상태에서 유의하게 감소하였다(p<0.05). 또한, IL-6, IL-10 및 TNF-α의 수준은 ECMO 후 상태에서 유의하게 증가하였다(p<0.05)(표 6).

상기 결과를 통하여, 충진액에 알부민을 첨가하면 안정한 ECMO 동물모델을 제작할 수 있으며, 특히, 정맥-정맥형-ECMO로 제작하는 것이 동물모델의 면역기능을 보다 안정하게 유지할 수 있으므로, 본원발명의 EMCO 모델은 혈역학 및 면역 학적 연구에 유용하게 활용될 수 있다.
이제까지 본 발명에 대하여 그 실시예들을 중심으로 살펴보았다. 본 발명이 속하는 기술 분야에서 통상의 지식을 가진 자는 본 발명이 본 발명의 본질적인 특성에서 벗어나지 않는 범위에서 변형된 형태로 구현될 수 있음을 이해할 수 있을 것이다. 그러므로 개시된 실시예들은 한정적인 관점이 아니라 설명적인 관점에서 고려되어야 한다. 본 발명의 범위는 전술한 설명이 아니라 청구범위에 나타나 있으며, 그와 동등한 범위 내에 있는 모든 차이점은 본 발명에 포함된 것으로 해석되어야 할 것이다.

- 체외막형성산화치료 회로와 연결된 배액 도관(drainage cannula)을 래트의 우측 외경정맥(external jugular vein)에 삽입하고, 상기 회로에 연결된 관류 도관(perfusion cannula)을 상기 래트의 좌측 경정맥에 삽입하는 단계
를 포함하고,
상기 체외막형성산화치료 회로의 충진액은 알부민(albumin) 및 생리 식염수를 포함하는 체외막형산화치료(Extracorporeal Membrane Oxygenation) 동물모델의 충진액(priming solution)용 조성물로서
상기 알부민 및 상기 생리 식염수는 1:1의 부피비로 포함되고,
상기 알부민의 농도는 20%(w/v)인 것인 체외막형산화치료 동물모델의 충진액용 조성물인 것인 체외막형산화치료 동물모델의 제조방법.
- 제 3 항에 있어서, 상기 배액 도관은 측면에 구멍이 형성된 것인 체외막형산화치료 동물모델의 제조방법.
- 제 4 항에 있어서, 상기 구멍의 개수는 4개인 것인 체외막형성산화치료 동물모델의 제조방법.
- 제 3 항에 있어서, 상기 배액 도관은 신생아 먹이 튜브(neonatal feeding tube)인 것인 체외막형산화치료 동물모델의 제조방법.
- 제 3 항의 제조방법으로 제조된 체외막형성산화치료 동물모델.
