Abstract
본 발명은 서열번호 1의 아미노산 서열(CQRPPR)을 갖는 펩타이드를 유효성분으로 포함하는 골관절염 진단용 또는 예후 판단용 시약에 관한 것이다. 본 발명의 펩타이드를 사용하면 분자 영상화 기법에 기초하여 초기 단계의 골관절염을 정확하게 진단할 수 있다. 본 발명의 펩타이드는 분자량이 작기 때문에 혈액으로부터의 빠른 클리어런스(clearance), 효과적인 조직 침투, 낮은 면역원성 및 저-생산비용의 이점을 갖는다. 또한, 본 발명의 시약은 연골파괴가 정상으로 회복될 수 있는 가역적 특성을 갖는 초기 단계의 골관절염을 진단할 수 있으므로 골관절염의 효과적인 치료에 크게 기여할 수 있다.
Description
본 발명은 ApoPep-1 펩타이드 프로브를 유효성분으로 포함하는 조기골관절염 진단용 또는 예후 판단용 시약에 관한 것이다.
골관절염(Osteoarthritis, OA)은 연골에서 흔히 발생하는 퇴행성 관절질환이며, 노년층에서 사회 활동 참여 및 건강과 관련된 삶의 질을 저하시키는 중요한 질환이다(1-3). 골관절염은 관절연골세포가 정상적으로는 분화 중간 단계에서 머물고 있는데 어떤 원인에 의하여 성장판 연골세포처럼 분화가 진행되고 사멸되어 그 특성을 잃어버려 나타나는 현상이다. 관절연골세포 사멸(apoptosis)은 정상 관절연골에 비해 골관절염에서 더 자주 발생한다. 이로 관절연골세포 기능이 감소되어 세포외 기질 타입 Ⅱ 콜라겐 합성이 저하되고 이화성 기질-분해 효소(catabolic matrix-degrading enzymes)가 증가하여 세포외 기질 콜라겐 섬유 분해를 특징으로 한 관절연골의 퇴화가 시작된다(4-8). 이러한 사실을 종합하면 관절연골세포 사멸은 노화와 관련된 연골퇴화의 이해와 해명에 큰 기여를 한다고 사료된다.
최근 말 관절연골의 연구에 의하면 관절연골세포의 세포사멸이 골관절염의 초기 단계와 후기의 연골 파괴에도 연관되어 있다는 것을 보여주고 있으며, 이는 연골세포 사멸이 연골 파괴와 본질적으로 연관되어 있고 골관절염에서의 연골 병변의 발생과 연관되어 있음을 암시한다(11). 관절연골세포의 사멸은 골관절염 초기에 연골의 표면이나 중간부위에서 시작되는데, 이는 관절이 받는 끊임없는 기계적 부하손상으로 나타난 것이다(12-14). 골관절염 병변 발생초기에 일어나는 미세한 병리학적 변화에 대한 진단이 이루어지면 골관절염을 가역적 단계에서 쉽게 치료할 수 있다(15). 골관절염의 조기 진단은 골관절염의 조기 치료를 가능하게 하여 진행을 멈추게 하고 비가역적인 장애를 예방할 수 있기 때문이다(15). 그러나 골관절염의 조기 진단은 골관절염 초기 단계 동안에 임상적 징후나 방사선 검사로는 관찰되지 않기 때문에 매우 어려워 새로운 기술이 필요하다. 현재, 골관절염은 방사선사진술, CT, 초음파검사, MRI와 같은 다양한 영상기법으로 진단하고 있다. 전통적인 방사선기술은 연변부의 골돌기 형성(marginal osteophyte formation), 비대칭적인 관절강 협착(asymmetric joint space narrowing), 연골하골의 경화(subchondral sclerosis, eburnation), 그리고 낭 형성(cyst formation) 등을 관찰할 수 있지만 조기 골관절염 탐지는 불가능하며 후기 단계에서만 진단이 가능하다(16). 최근에, 관절연골세포 사멸이 주목을 받고 있는 바, 골관절염 발생 시 관절연골세포에서 사멸현상이 일어나는 특징을 이용하여 세포사멸을 선택적으로 표적할 수 있는 방법의 개발은 골관절염 연골 퇴화 조기진단에서 획기적인 사건이 될 것으로 암시되고 있다(17).
인간 또는 동물에서 류마티스 관절염의 진행을 추적하기 위한 분자 영상기법이 개발되어 있다(18). 하지만, 생체 내 골관절염 진단을 위한 분자 영상화 기법은 만족할 만한 결과가 없다(19). 비침습성으로 단기간 배출 가능한 저분자 분자 영상화 기법이 골관절염의 조기 진단에 필요하다. 형광이 달려있는 세포사멸을 선택적으로 표적할 수 있는 펩타이드 프로브인 ApoPep-1은 CQRPPR로 구성되어 있고, 종양 및 뇌조직에서 사멸 세포들을 정확하게 생체내 영상화가 가능하다는 것이 보고되었다(20-22). 그러나, ApoPep-1 펩타이드가 암조직 또는 뇌질환 조직 이외에 골관절염 조기 진단에 사용될 수 있다는 점은 아직 보고되어 있지 않다.

따라서 본 발명의 ApoPep-1 펩타이드는 골관절염의 진단 또는 예후 판단용 시약으로 유용하게 사용될 수 있다.
본 발명의 ApoPep-1 펩타이드는 종래에 사멸세포 표적용 펩타이드로서 개발되었으며, 종양, 뇌졸중, 심근경색 및 동맥경화와 같은 질환에서의 분자 영상화 프로브로 사용될 수 있음이 공지되어 있다(대한민국 등록특허 제10-0952841호).
본 명세서에서 용어 "진단"은 골관절염에 대한 한 객체의 감수성(susceptibility)을 판정하는 것, 한 객체가 골관절염을 현재 가지고 있는지 여부를 판정하는 것, 골관절염에 걸린 한 객체의 예후(prognosis)(예컨대, 골관절염 상태의 동정, 골관절염의 단계 결정 또는 치료에 대한 관절염의 반응성 결정)를 판정하는 것, 또는 골관절염 치료 효능에 대한 정보를 제공하기 위하여 객체의 상태를 모니터링 하는 것을 포함한다.
본 명세서에서 용어 "예후"는 골관절염의 진행 가능성, 특히, 질병의 차도, 질병의 재생, 골관절염 재발 측면에서의 예측을 포함한다. 바람직하게는, 본 발명에서의 예후는 골관절염 환자의 질병이 완치될 가능성을 의미한다.
본 명세서에서 용어 "골관절염(osteoarthritis, OA)"은 관절에 염증이 발생한 관절염 질환들 중에서 가장 오래되고 가장 일반적인 질환 중 하나로, 관절 연골(joint's cartilage)의 파괴로 특징화되는 만성 상태를 의미한다. 연골은 뼈의 끝 부위에 완충 기능(cushion)을 하여 관절이 쉽게 움직일 수 있도록 하는 관절의 한 부위이다. 연골의 파괴는 인접한 뼈들 간의 마모를 야기하고, 관절의 이동의 어려움으로 인한 경직(stiffness) 및 고통을 초래한다.
골관절염은 질환의 진행에 따라 다음과 같은 다양한 단계로 나눌 수 있다: (i) 연골이 탄력성을 잃어 상처나 이용에 의해 보다 더 쉽게 상처받는 단계; (ⅱ) 연골의 마모가 밑에 있는 뼈에 변화를 야기시키는 단계로, 뼈가 비후화되고 낭종이 발생할 수 있다. 돌기(spurs) 또는 골증식체(osteophytes)로 불리는 뼈의 증식이 영향 받은 관절에 있는 뼈의 말단에서 일어나고, 가려움과 고통을 야기하는 단계; (ⅲ) 뼈 또는 연골의 조각이 관절강(joint space)에 느슨하게 부유하는 단계; 및 (ⅳ) 연골 파괴(breakdown)로 인해 관절막 또는 활액막에 염증을 일으키는 단계로, 연골에 손상을 입히는 사이토카인 및 효소를 추가적으로 야기시키는 단계이다.
본 발명의 바람직한 구현 예에 따르면, 상기 골관절염의 진단은 초기 단계의 골관절염의 진단이다. 본 발명의 ApoPep-1 펩타이드는 초기 단계의 골관절염을 정확히 진단할 수 있다.
본 명세서에서 "초기 단계의 골관절염"은 골관절염에서의 연골의 파괴가 정상으로 회복될 수 있는 가역적(reversible) 단계에 있는 골관절염을 의미한다. 즉, 치료, 중재 또는 연골 파괴 원인의 제거 등에 의해 연골의 파괴가 중지되고 정상의 연골 상태로 되돌아 갈 수 있는 단계에 있는 골관절염을 의미한다.
본 발명의 바람직한 구현 예에 의하면, 상기 초기 단계의 골관절염은 OARSI (Osteoarthritis Research Society International) 에 따른 등급 분류상 등급 I - 등급 Ⅲ의 진행 단계에 있는 골관절염이다. 상기 OARSI에 따른 등급은 골관절염의 위중도를 조직학적으로 수치화한 것으로서, 이에 대한 내용은 문헌 "Osteoarthritis Cartilage 14:13-29, 2006, Pritzker, K.P. et al., Osteoarthritis cartilage histopathology: grading and staging" 에 설명되어 있으며, 상기 문헌은 본 명세서에 참조로 삽입된다.
본 발명의 펩타이드에는 진단 시약으로서의 유용성을 높이기 위해, 검출 가능한 신호를 발생시키는 물질을 직접적 또는 간접적으로 결합시킬 수 있다. 본 발명의 펩타이드에 결합시킬 수 있는 신호 발생 물질은 방사성 동위원소(예컨대, C14, I125, P32 및 S35), 화학물질(예컨대, 바이오틴), 형광물질[예컨대, 플루오레신, FITC(fluoresein Isothiocyanate), 로다민 6G(rhodamine 6G), 로다민 B(rhodamine B), TAMRA(6-carboxy-tetramethyl-rhodamine), Cy-3, Cy-5, Texas Red, Alexa Fluor, DAPI(4,6-diamidino-2-phenylindole) 및 쿠마린(Coumarin)], 발광(luminescence)물질, 화학발광(chemiluminescence)물질, FRET(fluorescence resonanceenergy transfer) 발생물질, 및 CT, MRI 또는 감마 카메라, SPECT, PET와 같은 분자 영상화가 가능하도록 하는 Fe, Gd, Mn, Zn 및 란탄족 원소 등을 포함하나, 이에 한정되는 것은 아니다. 다양한 결합 및 결합방법은 Ed Harlow and David Lane, Using Antibodies:A Laboratory Manual, Cold Spring Harbor Laboratory Press, 1999에 기재되어 있으며, 이 문헌은 본 명세서에 참조로 삽입된다.
상술한 검출 가능한 신호 발생 물질을 펩타이드에 직접 결합시킬 수도 있지만, 간접적으로 결합시킬 수도 있다. 예를 들어, 펩타이드에 바이오틴을 결합시키고, 상술한 표지가 결합된 스트렙타비딘(또는 아비딘)을 상기 바이오틴에 결합시키면, 펩타이드에 신호 발생 물질을 간접적으로 결합시킬 수 있다.
본 발명의 펩타이드를 이용하여, 시험관내(in vitro) 또는 생체외(ex vivo)의 방식으로 골관절염을 검출하는 경우, 생체에서 분리된 생시료(biosample)을 이용하여 골관절염을 검출하게 된다. 이 경우, 본 발명의 진단 시약은 통상적인 면역분석 프로토콜에 이용될 수 있다. 이러한 면역분석은 종래에 개발된 다양한 정량적 또는 정성적 면역분석 프로토콜에 따라 실시될 수 있다. 상기 면역분석 포맷은 방사능면역분석, 방사능면역침전, 면역침전, ELISA(enzymelinked immunosorbent assay), 캡처-ELISA, 억제 또는 경쟁 분석, 샌드위치 분석, 유세포 분석(flow cytometry), 면역형광염색 및 면역친화성 정제를 포함하지만, 이에 한정되는 것은 아니다. 상기 면역분석 또는 면역염색의 방법은 Enzyme Immunoassay, E. 본 발명의 펩타이드를 이용하여 생체내 (in vivo) 방식으로 골관절염을 검출하는 경우에는, 펩타이드를 생체 내에 직접 주입한 후 골관절염을 검출한다. 이 경우 골관절염의 검출은 방사선 사진술 (radiography) 또는 분자영상화 (molecular imaging) 기법을 이용하여 행할 수 있다. 본 발명에서 이용가능한 분자영상화 기법은 광학영상화 (optical imaging), 컴퓨터단층촬영 (CT, computed tomography), 및 자기공명영상(MRI, magnetic resonance imaging)을 포함하나 이에 한정되지 않는다.
본 발명은 서열번호 1의 아미노산 서열(CQRPPR)을 갖는 펩타이드를 유효성분으로 포함하는 골관절염 진단용 또는 예후 판단용 시약에 관한 것이다. 본 발명의 펩타이드를 사용하면 분자 영상화 기법에 기초하여 초기 단계의 골관절염을 정확하게 진단할 수 있다. 본 발명의 펩타이드는 분자량이 작기 때문에 혈액으로부터의 빠른 클리어런스(clearance), 조직내로의 효과적인 침투, 낮은 면역원성 및 저-생산비용의 이점을 갖는다. 또한, 본 발명의 시약은 연골파괴가 정상으로 회복될 수 있는 가역적 특성을 갖는 초기 단계의 골관절염을 진단할 수 있으므로 골관절염의 효과적인 치료에 크게 기여할 수 있다.
도 1은 골관절염의 진행을 형태학적 및 조직학적으로 분석한 결과이다. 패널 A는 DMM (destabilization of the medial meniscus) 수술 후 발생된 관절염 진행의 각 단계에서 관절염 마우스의 관절의 형태를 보여준다. 패널 B는 DMM 수술후 발생된 관절염 진행의 각 단계에서 정상 및 관절염 마우스의 관절을 Safranin O로 염색한 절편을 보여준다. 패널 C-D는 데이터를 평균±SD으로 표시하였다. 모의수술군과 비교하여 *p<0.05, **p<0.01. (각 군당 n=5).

도 4는 ApoPep-1의 세포사멸성 연골세포에의 결합을 TUNEL분석을 통해 측정한 결과이다. DMM 마우스 및 모의수술 마우스에서 수술 후 2주에서의 골관절염 연골을 ApoPep-1으로 염색하였다. 실험재료 및 방법에 기재된 바와 같이 동결절편에 대해 TUNEL(붉은색) 및 ApoPep-1(노란색)으로 면역형광염색을 수행하였다.
도 5는 골관절염 및 대조군 연골에서 타입 Ⅱ 콜라겐, MMP 13, 및 ApoPep-1 양성 신호의 발현 패턴을 보여준다. DMM 수술후 2주에서 골관절염 연골 및 모의수술 대조군 마우스에서 다양한 마커에 대해 염색을 행하였다. 면역형광염색은 실험방법 및 재료에서 설명된 바와 같이 동결절편에 대해 Col 2(붉은색), MMP13(녹색) 및 ApoPep-1(노란색)으로 행하였다. 눈금막대는 50μm.

이하, 실시예를 통하여 본 발명을 더욱 상세히 설명하고자 한다. 이들 실시예는 오로지 본 발명을 보다 구체적으로 설명하기 위한 것으로, 본 발명의 요지에 따라 본 발명의 범위가 이들 실시예에 의해 제한되지 않는다는 것은 당업계에서 통상의 지식을 가진 자에 있어서 자명할 것이다.
실시예
실험 재료 및 방법
1. 실험동물 및 외과적 수술
C57BL/6N 마우스를 KOATECH 社(경기도, 한국)로부터 구입하였고, 동물 관리 및 실험은 경북대학교의 동물실험윤리위원회(IACUC)에 따라 수행하였다. 동물을 특정 병원균 미감염 상태에서 22-25℃로 명조건 12시간 암조건 12시간을 유지하면서 표준 쥐 식이를 공급하고 물을 무제한으로 공급하였다. 12주령의 수컷 마우스를 다음의 3개의 군으로 나누었다: 모의수술(Sham)-ApoPep-1군, 골관절염(OA)-대조 펩타이드군 및 OA-ApoPep-1군(n=4-8/군). 마우스 골관절염 모델은 기존의 연구(8, 23)에서 설명된 바와 같이 DMM 수술을 시행하여 제작하였다.
2. Safranin O 염색 및 조직학적 분석
에탄올의 농도를 증가시켜 탈수한 탈회조직을 파라핀 포매한 후 3㎛의 두께로 잘라 절편(section)을 만들었다. Safranin O 염색을 위해, 절편에서 파라핀을 제거한 후 다시 수화시키고, Weigert's iron hematoxylin 용액(Sigma Aldrich)에 10분간, fast green 용액(Sigma Aldrich)에 5분간, 및 0.1% Safranin O 용액에 5분간 침지하였다. 경골 고평부(tibia plateau)에서의 골관절염 진행 정도는 연골 손상에 대한 조직학적 등급 스코어 0 내지 4로서 정량화하여 평가하였다.
3. 생체내(in vivo) 및 생체외(ex vivo) 광학 영상화
FlammaTM675-ApoPep-1을 꼬리정맥에 정맥주사(i.v.)를 통해 투여(1 mM 농도 100μl/20g)하고, 일정시간 동안 순환되도록 두었다. 생체내 광학 영상화는 80%N2O/20%O2내에서 이소플루란(JW pharmaceutical, 한국)을 흡입시켜 마취시킨 상태의 마우스를 Optix Explore(ART, Montreal, Canada)을 사용하여 스캐닝하여 실행하였다. FlammaTM675의 여기/방출 파장은 676nm/704nm 이었다. 생체외 광학 영상화는 설명한 방법과 동일한 방법을 사용하여 마우스의 뒷다리에 대해 행하였다. 대조 펩타이드로는 FlammaTM675-NSSSVDK를 사용하였다(18).
4. 면역 형광 염색(Immunofluorescence staining)
마우스의 뒷다리를 0.1M PBS(phosphate-buffered saline, pH 7.4)내에서 4% 파라포름알데히드를 사용하여 12시간 동안 고정시키고, 10% EDTA(ethylene-diamine tetraacetic acid)으로 3주간 동안 탈회하였다. 탈회된 조직을 20% 수크로스/PBS 용액내에 하룻밤 침지하고, OCT compound (Tissue-Tek)로 포매하여 동결시료를 제조하였다. 관절을 관찰할 수 있도록 동결시료를 대퇴골로부터 경골까지의 부위를 10 μm의 두께로 연속적으로 절단하였다. 관절 연골의 동일한 부위에서 비교하기 위해, 각 동물로부터 절단한 절편을 선별한 후 동일한 항체로 염색하였다. 절편을 블로킹 용액(0.1% tween-20, 1% bovine serum albumin, 5% normal donkey serum in PBS)으로 처리하고, PBS로 세척한 후에, 토끼 폴리클로날 타입 Ⅱ 콜라겐 항체(1:200; abcam, MA, USA), 마우스 모노클로날 MMP-13 항체(1:200; abcam, MA, USA)와 같은 1차 항체와 함께 4℃에서 인큐베이션하였다. PBS로 세척한 후에, 절편을 Alexa-594 또는 Alexa-488-컨쥬게이트된 2차 항체와 함께 30분간 인큐베이션하였다. 이어서, 핵 염색을 위해 절편을 DAPI (4′, 6′-diamidino-2-phenylindole, Sigma-Aldrich, St Louis, MO, USA)와 함께 인큐베이션하여 ProLongㄾAntifade 키트(Invitrogen, USA)를 사용하여 마운팅하였다. 염색한 절편의 영상을 Zeiss LSM-510 Meta confocal microscope(Zeiss, Oberkochen, Germany)을 사용하여 찍은 후, 모든 사진을 현미경 조작기를 사용하여 수득하였다.
5. TUNEL 분석
TUNEL 분석은 ApopTagㄾ Red In Situ Apoptosis Detection Kit(Millipore, USA & Canada)를 사용하여 제조사의 지시서에 따라 행하였다. 동결절편을 상온에서 30분간 공기중에서 건조시키고 PBS/3분(min)으로 3회 세척하였다. 절편은 상온에서 1% PFA으로 10분간 전-고정하고, 미리 냉각시킨 에탄올/아세트산(2:1)으로 -20℃에서 5분간 후-고정하였다. 고정한 조직 절편은 상온에서 10초간 평형화 완충액으로 적용하고, TdT 효소로 37℃에서 30분간 반응시키고, 핵염색을 위해 DAPI와 함께 인큐베이션한 후, ProLong ㄾ Antifade 키트로 마운팅하였다. 염색한 절편의 영상은 Zeiss LSM-510 Meta 공초점 현미경을 사용하여 찍고, 모든 사진을 현미경 조작기를 사용하여 수득하였다.
6. 세포배양 및 면역조직화학 분석
ATDC5 연골세포는 5% FBS (fetal bovine serum, Gibco-BRL, USA), 10μg/ml의 인간 트랜스페린(Sigma) 및 3 x 10-8 M의 소디엄 셀레네이트(Sigma) 및 페니실린/스트렙토마이신을 포함하는 DMEM 및 Ham's F-12 (DMEM/F12) 배지(Lonza, USA) 혼합물내에서 유지하였다. 세포를 0.5μM의 스타우로스포린(Staurosporine) (cell signaling, USA)과 함께 일정시간 배양하여 세포사멸을 유도하였다. 세포사멸의 단계는 아넥신(annexin) V 및 flamma 675 컨쥬게이트된 ApoPep-1으로 세포를 염색하여 결정하였다. 면역조직화학 염색을 위해, 세포를 37℃에서 1% BSA으로 30분간 인큐베이션하여 블로킹하고 4℃에서 10μM의 flamma 675-컨쥬게이트 펩타이드와 1시간 동안 결합시켰다. 이어서, 세포를 결합 완충액(10 mM HEPES, pH 7.4, 140 mM NaCl, 및 2.5 mM CaCl2)내에서 Alexa-594-아넥신 V (Invitrogen, Carlsbad, CA, USA)으로 상온에서 15분간 공동 염색시켰다. 고정한 후에, 세포를 핵 염색을 위해 DAPI와 인큐베이션하였고 ProLongㄾ Antifade Kit으로 마운팅하였다.
7. 통계
통계 분석은 Sigma Plot 소프트웨어를 사용하여 행하였다. 데이터는 그룹 평균±SD으로 표시하였다. 실험데이터는 Sigma Plot 소프트웨어를 사용하여 분석한 후 t' 테스트를 행하였다. p<0.05 값은 통계적으로 유의성을 갖는 것으로 하였다.
실험결과
1. DMM 모델에서 ApoPep-1의 광학 영상
DMM 모델에서 조직학적 분석을 통해 골관절염의 진행 정도를 평가하였다(도 1). 먼저 형태학적 및 조직학적 분석을 통해 골관절염 진행 동안에 연골 손상을 확인하였다(도 1의 패널 A 및 C). 모의수술(Sharm) 마우스에서 무릎관절 절편은 연골과 석회화된 뼈 표면 사이의 뚜렷한 경계를 갖는 규칙적인 연골 표면을 나타내었다. 그러나, DMM 마우스 관절에서는 Safranin O 양성 프로테오글리칸이 수술후 2주부터 점차적인 연골손상과 함께 약간 감소하였다(도 1의 패널 B 및 패널 D). OARSI(OA Research Society International) 등급 분류의 조직학적 스코어에 따라, 수술후 DMM 모델을 평가하였다. 등급 I은 2주에 해당하고, 등급 Ⅲ은 4주에 해당하며, 등급 VI는 8주에 해당하는 것으로 나타났다(도 1). 상기 결과로부터 종전의 연구결과와 마찬가지로 본 실험의 DMM 모델은 잘 확립된 것으로 확인되었다.
다음으로, 분자 영상법을 사용하여 생체내(in vivo) 및 생체외(ex vivo)에서 골관절염의 진행 동안에 ApoPep-1 결합을 측정하였다(도 2). 모의수술-ApoPep-1군 및 골관절염(OA)-대조 펩타이드군의 마우스를 비교한 결과, 1주에서는 유의한 차이점이 발견되지 않았다. 그러나, 2주에서 관절염 마우스의 관절로부터 ApoPep-1 형광 신호가 유의하게 증가하였으며, 4주에서 형광 신호의 증가가 최대치를 나타내었다. 도 2의 패널 A 및 C는 생체내 결과이며, 도 2의 패널 B 및 D는 생체외의 결과를 보여준다. 그러나, 8주에서는 연골 손상이 더 진행되었음에도 불구하고, 4주에서의 형광 신호와 비교하여 관절로부터의 ApoPep-1 형광 신호가 감소하였다. 이러한 결과로부터 ApoPep-1 프로브가 DMM 골관절염 모델에서 적어도 2주에서부터 골관절염의 진행 및 변화를 인지할 수 있다는 점을 명확히 알 수 있었다.
형광 표지된 ApoPep-1 프로브를 사용하여 더 정확하게 골관절염의 조기 진단 및 골관절염의 진행을 모니터링할 수 있는 지를 확인하기 위해, DMM 마우스에서 골관절염 진행의 각 단계에서의 형광 영상을 관찰하였다. 먼저, 프로브를 DMM 수술후 2주의 마우스에 정맥투여한 후 30분 내지 24시간 까지 연속적인 광학영상을 얻었다(도 3의 패널 A). 형광신호는 프로브를 주입한 후 30분 시점에서 관절염의 관절로부터 명확히 검출할 수 있었고, 시간이 지남에 따라 감소하였다(도 3의 패널 B). 이러한 결과들은 ApoPep-1 프로브를 주입한 후 30분의 시점이 진단을 위한 최적의 시점이라는 것을 지시한다. 상기의 결과들을 종합하면, ApoPep-1은 골관절염을 초기 단계에서 검출할 수 있는 영상화 프로브로 유용하게 사용될 수 있음을 암시한다.
2. 골관절염에서 ApoPep-1 결합에 대한 조직학적 분석
골관절염 모델에서 조직학적 분석을 통해 ApoPep-1의 결합을 검사하였다. 양성의 ApoPep-1 신호 패턴이 분자 영상 신호의 패턴과 연관성이 있는 지 여부를 확인하기 위해 골관절염의 관절을 인시투 면역형광염색 (in situ immunofluorescent staining)으로 분석하였다. 골관절염의 초기 단계에서, 연골세포의 세포사멸은 연골의 표면과 일부 중앙 부위에서 시작된다. 먼저, ApoPep-1 펩타이드가 세포사멸성 연골세포에 결합하는지 여부를 검사하였다. TUNEL 염색 결과, DMM 모델의 수술후 2주에서 ApoPep-1의 초기 단계 검출과 비교하여 TUNEL 염색과 강한 상관성이 있음을 알 수 있었다(도 4). ApoPep-1 펩타이드의 양성 신호가 대부분 TUNEL에 의해 양성으로 염색되는 연골조직의 세포사멸성 연골세포에 위치하고 있음을 확인하였다. 흥미롭게도, ApoPep-1 신호는 모의수술군 조직과 비교하여 골관절염 조직에서 우세하게 증가하였는데, 이는 양상은 TUNEL 염색에서도 비슷한 패턴을 보여주었었다(도 4). 타입 Ⅱ 콜라겐의 발현은 골관절염 연골 퇴행 부위에서 감소하였으나, 반면 MMP13의 발현은 증가되어 있었다. 이와 동시에, ApoPep-1 신호는 퇴행성 연골부위에서 증가하였다(도 5). 이러한 결과들은 ApoPep-1 펩타이드가 퇴행성 연골에서 세포사멸성 연골세포에 결합한다는 것을 시사하고, ApoPep-1 펩타이드가 골관절염을 조기 진단할 수 있다는 점을 암시한다.
3. 시험관내(
in vitro
)에서 ApoPep-1의 세포사멸성 세포에의 결합
ApoPep-1의 세포사멸성 세포에의 결합을 보다 확실히 규명하기 위해, 스타우로스포린(staurosporine)으로 처리된 연골세포를 검사하였다. 형태학적 분석결과 세포는 스타우로스포린의 처리에 의해 수축되었고 세포사멸이 일어났다. 형광염색 결과 ApoPep-1는 사멸된 세포에 결합하였으나, 살아있는 세포에는 결합하지 않았다(도 6). 사멸된 세포에서 아넥신 V 신호는 ApoPep-1과 공통되는 위치에 있었다. 이러한 결과는 생체내의 결과와 함께 ApoPep-1 프로브에 의한 양성 신호가 연골세포의 세포사멸에 기인한다는 것을 뒷받침한다.
고찰
본 발명자들은 ApoPep-1 프로브가 골관절염의 가역적 단계에서 조기에 진단할 수 있는 도구로 사용될 수 있음을 발견하였다. 형광 ApoPep-1은 DMM 골관절염 모델에서 세포사멸성 세포를 특이적으로 표적할 수 있는 비침습성 분자 프로브이므로, 골관절염의 조기 진단 뿐 아니라 골관절염 치료 예후를 모니터링 하는데 적용이 가능하다. 생체내 및 생체외 모두에서 ApoPep-1 프로브의 광학 영상은 골관절염 그룹에서 증가하였다. OARSI 등급은 골관절염의 위중도를 조직학적으로 수치화한 것이다(24). 등급 I 및 등급 Ⅱ 변화는 연골부종과 글리코스아미노글리칸이 조기 고갈된 콜라겐 섬유의 응축을 포함한다. 표면 부위는 미세한 섬유성 연축과 갈라진 틈이 발견될 수 있으나 아직 견고하게 유지되고 있으며, 중간 및 깊은 부위는 영향을 받지 않는 단계이다. 등급 Ⅲ에서는 수직 틈이 중간 부위까지 뻗어나갔으나 아직 심각한 연골 손실이 발견되지 않는다. 등급 I 내지 등급 Ⅲ는 초기 골관절염에 해당하며, 이 단계에서는 질환이 가역성 가능성을 가지고 있는 것으로 추정된다. 등급 Ⅳ는 갈라진 틈의 증가가 연골침식에 이른 단계에 해당한다. 등급 V 및 등급 VI는 관절연골이 거의 완전히 침식되어 기초 뼈에 영향을 미치는 변화로서 연골이 경화 및 상아질화되는 단계이다(25).
DMM 수술 후 2주에서, DMM 모델은 약간의 글리코스아미노글리칸 고갈을 나타내었으나 병리학적 변화는 거의 나타나지 않았는데, 이는 골관절염 진행에서 등급 I의 초기 단계에 해당하는 것이었다. ApoPep-1 프로브는 DMM 모델에서 골관절염 질환이 가역성을 유보하고 있는 단계인 등급 Ⅱ 단계에서의 세포사멸성 연골세포를 검출할 수 있었다. ApoPep-1 결합은 주입후 30분에서 최대치를 나타내었으며 이후로 감소하여 8 시간에서는 검출되지 않았다. 노화와 관련된 인간의 골관절염은 관절연골에서 연골세포의 세포사멸이 나타난다(27, 28). 실제, TUNEL이 양성인 세포 부위에서 ApoPep-1 프로브 결합이 높게 나타났는데, 이는 ApoPep-1이 세포사멸성 연골세포를 검출할 수 있다는 것을 나타낸다. 골관절염의 초기 단계에서, 관절연골의 표면 부위는 safranin-O 양성 프로테오글리칸 생성의 감소와 같은 뚜렷한 변화 없이 생물학적으로 손상된 세포인 것으로 알려져 있다(8). 예상한 바와 같이, safranin O 염색의 약간의 소실 및 타입 Ⅱ 콜라겐 생성 감소가 DMM 모델의 골관절염 초기 단계의 관절 연골세포에서 나타났다. 세포사멸성 연골세포는 타입 Ⅱ 콜라겐 합성능을 잃는 반면(29, 30), MMP13과 같은 기질 분해 효소가 증가한다[6-8]. ApoPep-1 프로브가 세포사멸성 연골세포에 결합하는 생체내 결과와 동일한 결과가 연골세포주 ATDC5를 사용한 시험관내 실험에서도 반복되어 나타나는 것을 확인하였다. 골관절염의 예방은 연골 퇴화가 가역적이고 정상으로 회복될 수 있는 단계에 있는 골관절염을 확인하는 기술의 개발이 필요하다. 등급 I 내지 III은 초기 관절염에 해당하며 골관절염 질환이 가역적인 가능성을 보유한 단계인 것으로 생각된다. ApoPep-1 프로브는 DMM 수술후 2주 시점에 있는 등급 I 로부터의 골관절염의 진행을 검출할 수 있다. 이러한 실험결과들은 연골퇴화가 가역적인 단계에 있을 때인 초기 단계의 골관절염을 ApoPep-1 프로브를 사용하여 검출할 수 있다는 점을 제시한다. 결론적으로 ApoPep-1 펩타이드가 가역성 단계에 있는 초기 단계의 골관절염을 조기 진단에 사용될 수 있음을 규명하였으며, 이는 ApoPep-1 펩타이드를 신속한 골관절염의 생체진단 및 골관절염 치료 예후 및 예방을 모니터링 하는데 사용할 수 있음을 확인하였다.
이상으로 본 발명의 특정한 부분을 상세히 기술하였는 바, 당업계의 통상의 지식을 가진 자에게 있어서 이러한 구체적인 기술은 단지 바람직한 구현 예일 뿐이며, 이에 본 발명의 범위가 제한되는 것이 아닌 점은 명백하다. 따라서, 본 발명의 실질적인 범위는 첨부된 청구항과 그의 등가물에 의하여 정의된다고 할 것이다.
