Abstract
선모를 이용하여 역류성 식도염 예방 또는 치료 효능을 갖는 약학 조성물이 개시된다. 본 발명은 선모(Curculigo orchioides) 추출물을 유효성분으로 함유하는 역류성 식도염 예방 또는 치료용 약학 조성물 및 역류성 식도염 예방 또는 개선용 기능성 식품을 제공한다.
Description
본 발명은 역류성 식도염 예방 또는 치료용 약학 조성물에 관한 것으로, 보다 상세하게는 선모 추출물을 함유하는 역류성 식도염 예방 또는 치료용 약학 조성물에 관한 것이다.
본 발명은 2012년 교육과학기술부에서 지원하는 기초의과학자 육성(MRC) 사업의 일환으로 수행된 연구로부터 도출된 것이다.

수선화(Amaryllidaceae)과에 속하는 선모(Curculigo orchioides Gaertner)의 뿌리줄기(curculiginis rhizome)는 아시아 국가, 특히, 중국, 인도, 한국 등에서 주로 사용되는 약초이다. 중국 한의학에서 선모의 뿌리줄기는 치칠, 천식, 황달, 설사, 산통 및 임질의 치료에 사용되어져 왔으며(비특허문헌 1), 면역자극제로서 보고된 바 있다(비특허문헌 2).
선모의 뿌리줄기에는 커큘리고사포닌(curculigosaponins) A~M(비특허문헌 3, 4), 커큘리고사이드(curculigoside) A~C(비특허문헌 5), 오르시놀 글리코사이드(orcinol glycoside)(비특허문헌 6), 2,6-디멘톡실벤조산(2,6-dimenthoxyl benzoic acid), 커큘리진(curculigines)(비특허문헌 7), 오르키오사이드(orchiosides) A, B(비특허문헌 8), 탄닌, 수지, 알칼로이드(비특허문헌 9) 등 다양한 종류의 화합물이 함유된 것으로 보고되고 있다.
커큘리고사이드 A는 선모에 존재하는 주요 생활성 화합물 중 하나로서, 1차배양 과립막세포(granulosa cell)에서 에스트라디올의 분비를 자극하고, 배양된 인간 피부 섬유아세포(human skin fibroblasts)에서 (MMP)-1 효소(matrix metalloproteinase)에 대하여 잠재적 억제 활성을 나타내며(비특허문헌 10, 11), 과산화수소에 의해 유발되는 인간 탯줄 정맥 내피세포(human umbilical vein endothelial cell)의 손상을 약화시키고(비특허문헌 12), 조골전구세포(MC3T3-E1 Cells)에서 혈관내피세포 증식인자(vascular endothelial growth factor; VEGF)를 상향조절시킨다(비특허문헌 13). 본 발명자들은 금은화(lonicerae flos)의 항산화 활성을 통한 급성 역류성 식도염에 대한 억제 효과에 대해 보고한 바 있다(비특허문헌 14).
위식도역류병(gastroesophageal reflux disease; GERD)은 전세계 약 40% 인구에 영향을 주는 가장 일반적인 병 중 하나이다(비특허문헌 15, 16). 역류성 식도염은 다인성 질환으로서, 일시적 하부식도괄약근(lower esophageal sphincter; LES) 수축, 점막 저항 및 다른 인자들을 포함하고, 종종 하부식도괄약근 압력에도 관련이 있다.
활성 산소종(reactive oxygen species; ROS), 종양괴사인자(tumor necrosis factor; (TNF)-α)나 인터루킨(IL-1, IL-6)과 같은 염증유발 사이토카인(proinflammatory cytokines), 프로스타글란딘 E2(prostaglandins E2; PGE2)나 iNOS(inducible nitric oxide synthase)의 유도로부터 생성되는 일산화탄소(nitric oxide; NO)와 같은 염증유발 매개물질(proinflammatory mediators)은 역류성 식도염의 병리생리학에서 중요한 역할을 할 수 있는 것으로 보고되고 있다(비특허문헌 17, 18). 급속 또는 지연된 위 배출(gastric emptying)과 관련된 비정상적인 위 운동 또한 위궤양으로의 발전에 영향을 준다. Yasunaga 등은 조직 및 혈정에서의 IL-1β의 생성량 증가가 거대 주름 위염(enlarged fold gastritis)에서의 소와세포 과증식(foveolar hyperplasia)에 영향을 줄 수 있다고 보고한 바 있다(비특허문헌 19).
위산(gasric acid)은 점막하 염증(submucosal inflammation)을 유발할 수 있고, TNF-α나 IL-1β와 같은 사이토카인의 발현을 증가시키고, 후두상피(laryngeal epithelium)의 농밀화를 촉진시키는 것으로 추측되었다. 역류성 식도염은 산소-유래 자유라디칼에 의해 매개되며(비특허문헌 20, 21), 다양한 자유라디칼 스캐빈저(scavengers)의 투여로 식도 점막 손상을 방지할 수 있다는 것이 알려졌다(비특허문헌 22).
커큘리고사이드는 강력한 항산화제 능력과 항골다공증 활성을 보이며(비특허문헌 23), HMGB-1(high mobility group box protein-1) 방출 및 TNF-α로 인한 핵인자-B(nuclear factor-B) 활성을 소멸시켜 신경보호 역할을 하고, 중뇌동맥폐색(middle cerebral artery occlusion; MCAO)으로 유도되는 뇌 빈혈/재관류(cerebral ischemia/reperfusion) 손상을 개선하는 것으로 보고되었다.
이와 같이, 선모의 항산화 작용이나 항염증 작용 등에 대해서는 보고되고 있으나, 역류성 식도염에 대한 효과에 대해서는 알려진 바 없다.

따라서, 본 발명은 선모를 이용하여 역류성 식도염 예방 또는 치료 효능을 갖는 약학 조성물을 제공하고자 한다.
또한, 선모를 이용하여 역류성 식도염 예방 또는 개선 효능을 갖는 기능성 식품을 제공하고자 한다.
전술한 기술적 과제를 해결하기 위하여 본 발명은, 선모(Curculigo orchioides) 추출물을 유효성분으로 함유하는 역류성 식도염 예방 또는 치료용 약학 조성물을 제공한다.
상기 또 다른 과제 해결을 위하여 본 발명은, 선모(Curculigo orchioides) 추출물을 유효성분으로 함유하는 역류성 식도염 예방 또는 개선용 기능성 식품을 제공한다.
본 발명에 따르면, 선모(Curculigo orchioides) 추출물을 유효성분으로 함유하여 역류성 식도염 예방 또는 치료에 탁월한 효능을 발휘하는 약학적 조성물 및 역류성 식도염 예방 또는 개선에 효능을 발휘하는 기능성 식품을 제공할 수 있다.

이하에서는 본 발명의 바람직한 실시예를 첨부한 도면을 참고하여 상세하게 설명한다. 본 발명을 설명함에 있어서 관련된 공지 기술에 대한 구체적인 설명이 본 발명의 요지를 흐리게 할 수 있다고 판단되는 경우 그 상세한 설명을 생략하기로 한다.
본 발명자들은 종래 남자의 신양을 보하는 약재로 쓰이고, 항노화 작용, 항산화 작용, 항고온 작용, 면역기능, 항진 작용, 항염증 작용 등이 있는 것으로 알려진 선모 한약재에 대하여, 선모 추출물 분리 및 이의 약학적 조성물, 기능성 식품 등으로의 활용에 관해 예의 연구를 거듭하였다. 사람의 위식도역류병에 대한 병리생리학적 조사에 관해서는 한계가 있으므로, 동물 모델을 이용한 실험이 필수적인데, 유문부 및 전위부의 결찰술을 시행하여 역류성 식도염을 유발한 생쥐에 대해 선모 추출물을 먹인 쥐에서 놀랍게도 위산 분비 감소, 펩신 분비 감소 및 식도 점막의 방어 효과, 항산화 효과 등이 있어 선모 추출물이 역류성 식도염 예방 및 치료에 탁월한 효과가 있음을 발견하고 본 발명에 이르게 되었다.
따라서, 본 발명은 선모(Curculigo orchioides) 추출물을 유효성분으로 함유하는 역류성 식도염 예방 또는 치료용 약학 조성물 및 선모(Curculigo orchioides) 추출물을 유효성분으로 함유하는 역류성 식도염 예방 또는 개선용 기능성 식품을 개시한다.
실시예
선모의 건조된 뿌리 500g을 두껍게 슬라이스하고 물 2ℓ를 첨가하여 열탕 추출기에서 추출하여 얻은 수용성의 선모 추출물을 회전증발기(Buchi, Flawil, Switzerland)를 이용하여 여과 및 증발시킨 후 동결 건조기(Eyela FDU-540, Tokyo, Japan)를 이용하여 분말가루를 얻고 이를 -20℃에서 보관하였다. 수율은 약 14%(w/w)이었고, 수용성 추출 분말은 증류수에 용해(2㎖/㎏)시켜 경구 투여에 사용되도록 하였다. 선모의 건조된 뿌리는 옴니허브社(한국)로부터 구입한 것을 대구한의대학교 한의과대학 본초약리학교실에서 관능검사상 약전에 합격한 것을 사용하였다.
실험예
상기 실시예에 따라 제조된 선모 추출물의 역류성 식도염 예방 또는 치료 효능 여부를 확인하기 위해 동물실험을 시행하였다. 실험은 미국 가이드라인에 기술된 바와 같이, 국제적으로 승인된 실험동물 사용 및 보호 규정을 준수하여 시행하였다.
체중 180~200g인 6주령의 스프라그-돌리 웅성 랫드를 구입(Central Lab. Animal Inc., Seoul, Korea)하여 사용하였다. 실험동물은 25±1℃, 명암주기(light/dark cycle) 12h/12h의 표준 실험실 조건하에서 사육시키고, 표준식이(standard rodent chow and tap water)로 유지시켜 사용하였다. 필요에 따라, 실험 18시간 전부터 절식시켰으나 물은 제공하였다. 모든 실험동물은 식분(coprophagy)을 방지하기 위해 높은 메쉬 바텀 케이지(raised mesh-bottom cages)에 보관하였으며, 각 그룹 당 6~7수의 랫드를 사용하였다.
모두 5그룹의 랫드에 대해 실험 전 18시간 동안 절식(물은 제공)시키고, 졸레틸(zoletil)(Virbac S.A., France) 0.75㎖/㎏을 주입하여 마취시켰다. 이후, 중심선 개복을 수행하여 위를 노출시키고 유문부(pylorus)와, 전위부(fore stomach) 및 몸통부(corpus) 사이의 연결 접합부(transitional junction)를 노출시켜 2-0 실크실(silk thread)로 묶되, Omura 등(Omura, N., H. Kashiwagi, G. Chen, Y. Suzuki, F. Yano and T. Aoki. Establishment of surgically induced chronic acid reflux esophagitis in rats. Scand . J. Gastroenterol . 34: 948-953, 1999.)이 처음 제시한 방법과 달리 유문륜(pyloric ring) 없이 수행하였다.
미주신경은 손상되지 않도록 하였고, 정상군을 제외한 역류성 식도염이 유발된 총 36수의 랫드 중에서 6수씩 5그룹으로 나누었다. 즉, 정상군(intact control group)(이하, 'INT'라고도 칭함) 및 대조군(이하, 'RE'라고도 칭함)은 상기와 같은 수술 이후에 추가 처리를 수행하지 않았다. 그러나, 양성 대조군(이하, 'PC'라고도 칭함)은 수술 1시간 전에 항산화제(α-tocopherol)를 처리하였고, 실험군은 상기 실시예에 따라 제조된 선모 추출물 600㎖/㎏(이하, 'ECR-A'라고도 칭함), 300㎖/㎏(이하, 'ECR-B'라고도 칭함) 및 150㎖/㎏(이하, 'ECR-C'라고도 칭함)을 복개술 1시간 전에 처리하였다.
상기 정상군, 대조군 및 실험군에 대한 분석방법은 다음과 같다.
[급성 역류성 식도염의 식도 점막 손상 평가]
랫드의 사후 즉시 위식도로부터 인두(pharynx)에 이르는 종방향으로 절개하여 식도를 적출하고, 마이크로피펫을 이용하여 PBS(phosphate-buffered saline)로 내부 점액을 세척하였다. 절개된 식도는 종이 위에 전개하였으며 이를 광학 디지털 카메라를 이용하여 촬영하였고, i-solution lite 프로그램(Innerview Co., Korea)을 이용하여 분석 진단하였다.
[위액 분비 분석]
랫드의 사후 즉시 위를 적출하고, 마이크로피펫을 이용하여 PBS(phosphate-buffered saline) 1㎖로 세척하고 위 내용물을 수집하였고, 수집된 위 내용물을 3,000rpm에서 5분간 원심분리하여 부피를 측정하였다. 또한, pH 페이퍼(MFRS Toshniwal, instrument manufacturing limited, Ajmer, India)를 이용하여 수집된 위액의 pH를 측정하였다. 위액 부피는 각 군의 랫드로부터 얻어진 것으로부터 측정되었으며, 산도를 결정하기 위해 부분표본이 사용되었고, 샘플들에 대해 0.1N 수산화나트륨을 이용하여 pH 7.0의 종말점으로 적정하였다. 각 샘플에서 뮤신(mucin)의 양은 ELISA(enzyme-linked immunosorbent assay)를 이용하여 측정하였고, ELISA 과정은 이전 문헌(Shimizu, T., S. Shimizu, R. Hattori, E.C. Gabazza and Y. Majima. In vivo and in vitro effects of marcrolide antibiotics on mucus secretion in airway epithelial cells. Am. J. Respir . Crit . Care Med . 168: 581-587, 2003.; Ishbashi, Y., F. Kobayashi, A. Idesawa, A. Taniguchi and S Matsuzawa. Effects of carbocisteine on altered activities of glycosidase and glycosyltransferase and expression of MUC5AC in SO2-exposed rats. Eur . J. Pharmacol . 487: 7-15, 2004.)에 보고된 방법에 기초하여 적절히 수정하여 수행되었다.
위액을 PBS로 1:10 비율로 희석하고 각 샘플의 100㎕씩에 대해 96-웰 플레이트(well plate)에서 2시간 동안 상온 조건으로 배양하였다. 이후, PBS로 웰을 3회 세척하고 상온에서 1시간 동안 2% BSA(bovine serum albumin)로 블록(block)하였다. 웰을 다시 PBS로 3회 세척하고 100㎕의 IgG 17Q2(마우스의 모노클로날 항체(monoclonal anti-total mucin antibody)로서, 0.05% Tween 20(1:1000)을 함유한 PBS로 희석하여 각 웰에 분주함)와 함께 배양하였다. 1시간 후, PBS로 웰을 3회 세척하고 100㎕의 겨자무과산화효소(horseradish peroxidase)-고우트 안티마우스(goat antimouse) IgG 콘쥬케이트(1:3000)를 각 웰에 분주하였다. 1시간 후, PBS로 웰을 3회 세척하고, TMB(3,3',5,5'-tetramethylbenzidin) 과산화수(peroxide solution)로 전개하여 정색반응을 실시하고 1N 황산으로 정지시켰다. 흡광도는 450㎚ 파장에서 측정되었다. 뮤신 분비에 대한 효과는 다음과 같이 확인하였다. 처리 시간 동안 분비된 뮤신의 양을 전처리 시간 동안 분비된 뮤신의 양으로 나누고, 그 비율을 분비지수(secretory index)로 나타내었다. 각 군의 분비지수의 평균을 비교하고 통계를 이용하여 그 차이를 평가하였다.
[혈장에서의 사이토카인 레벨에 대한 선모 추출물의 효과]
혈장에서의 사이토카인 레벨에 대한 선모 추출물의 효과를 확인하고자, 실험 종료 직후 복부정맥으로부터 정맥혈 샘플을 취하여 EDTA-함유 바이알에 넣고 플라즈마 IL-1β, IL-6 및 TNF-α 측정을 위해 사용하였다. 수집된 정맥혈은 3,500rpm에서 10분간 15℃로 유지하여 원심분리시켰다. 혈장은 마이크로피펫으로 수집하여 ELISA 분석까지 -80℃에서 보관하였다.
염증유발 사이토카인 IL-1β, TNF-α 및 IL-6의 혈청 레벨은 Multi-Analyte ELISArray® Kit(Milipore, Rockford, IL, USA)를 이용하여 제조사의 지시에 따라 평가되었다. 정색반응의 강도는 Luminex luminometer(Awareness Technology Inc., Pal City, FL, USA)를 이용하여 490㎚ 파장에서 측정되었다.
[RT-PCR(Reverse Transcriptasepolymerase Chain Reaction)에 따른 TNF-α 및 COX2 mRNA 전사 발현 레벨에 대한 선모 추출물의 효과]
TRIZOL Reagent(Invitrogen Life Technologies, Karlsruhe, Germany)를 이용하여 식도 샘플로부터 RNA를 추출하고, RETROscript kit(MACROgene, Seoul, Korea)를 이용하여 제조사의 지시에 따라 총 5㎍의 RNA로부터 cDNA를 생성하였다. COX2, TNF-α 및 β-actin에 대한 유전자는 이전에 기술된 문헌(Konturek, P.C., T. Brzozowski, J. Kania, S.J. Konturek, S. Kwiecien, R. Pajdo and E.G. Hahn. Pioglitazone, a specific ligand of peroxisome proliferator-activated receptor gamma, accelerates gastric ulcer healing in rat. Eur . J. Pharmacol . 472: 213-220, 2003.; Wei, R., S.J. Listwak and E.M. Sternberg. Lewis hypothalamic cells constitutively and upon stimulation express higher levels of mRNA for pro-inflammatory cytokines and related molecules: comparison with inflammatory resistant Fischer rat hypothalamic cells. J. Neuroimmunol . 135: 10-28, 2003.)에 따라 특이적 프라이머 세트로 증폭시켰다.
cDNA 샘플을 94℃에서 5분간 어닐링하고 다음의 순환 조건으로 35회 반복 증폭하였다: 94℃에서 1분; 어닐링 온도 TNF-α 70℃, β-actin 55℃에서 1분; 72℃에서 1분 후 72℃에서 10분간 최종 연장 및 Bioer XP Cycler를 이용하여 증폭을 수행. 증폭된 PCT 산물에 대해 100-bp의 래더(ladder)(MACRO gene, Seoul, Korea)를 이용하여 1.0% 아가로스 겔 전기영동을 수행하고, Biovis gel documentation 소프트웨어를 이용하여 강도를 측정하고, PCR 산물/β-actin 비의 상대적 강도로 표현하였다. 랫드의 β-actin, COX2 및 TNF-α에 대한 프라이머는 Biometra(MACRO gene, Seoul, Korea)에서 구입하였다.
TNF-α 프라이머의 시퀀스는 다음과 같다: up-stream, 5'-TAC TGAACTTCG GGG TGA TTG GTC C-3'; downstream, 5'-CAG CCT TGTCCC TTG AAG AGA ACC-3'. PCR 산물의 길이는 296bp로 예상되었고, RNA의 안정성을 확인하기 위해 동일한 샘플에 대해 대조 랫드 β-actin의 증폭을 수행하였다.
COX2 프라이머의 시퀀스는 다음과 같다: up-stream, 5'-ACA ACA TTC CCT TCC TTC-3'; downstream, 5'-CCT TAT TTC CTT TCA CAC C-3'. PCR 산물의 길이는 201bp로 예상되었다. COX2에 대한 프라이머 어닐링을 56℃에서 수행하였고, RNA의 안정성을 확인하기 위해 동일한 샘플에 대해 대조 랫드 β-actin의 증폭을 수행하였다.
β-actin 프라이머의 시퀀스는 다음과 같다: up-stream, 5'-TTG TAA CCA ACTGGG ACG ATA TGG-3'; downstream, 5'-GAT CTT GAT CTTCAT GGT GCT AGG-3'. 프라이머 어닐링을 54℃에서 수행하였고, PCR 산물의 길이는 764bp로 예상되었다. 0.5㎍/㎖ 브롬화 에티듐(ethidium bromide)을 함유한 2% 아가로스 겔 전기영동으로 PCR 산물을 분리하고, UV 광을 통해 보이도록 하였다. 예상되는 PCR 산물의 위치는 표준 마커로서 O'Gene Ruler 50bp DNA ladder(Fermentas, Life Sciences, San Francisco, USA)를 사용하여 확인하였다.
[식도 점막의 육안 및 현미경적 손상 측정]
랫드를 절식(물은 제공)시키고 6시간 후에 사망시키고 전체 식도를 적출하여 점막 손상을 육안으로 확인하였다. 식도를 절개하고 0.9% 염화나트륨으로 헹군 후 해부현미경으로 손상된 특정 부위를 촬영하였다. 식도 점막의 근층은 벗겨내고 RT-PCR을 포함하는 생화학 분석을 위해 -80℃에서 저장하였다.
현미경적 평가를 위해, 절개된 식도를 중간 분체로 분할하였다. 분체를 파라핀에 넣고 2㎛ 크기로 절단한 후 현미경 평가를 위해 헤마토실린(hematoxylin) 및 에오신(eosin)(H&E)으로 염색하였다. 광학현미경으로 염색된 조각을 관찰하고 i-solution lite 프로그램으로 측정 진단하였다. 편평상피(squamous epithelim)의 육안적 손상율을 하기 수학식 1에 따라 측정하였다.
[통계처리]
모든 결과는 (평균±표준평균오차)로 나타내었다. 분산분석 및 two-way ANOVA test를 이용하여 통계분석을 수행하였고, 필요할 경우 Tukey post - hoc test를 함께 수행하였다. 모든 수치는 각 군의 (평균±표준편차)로 나타내었다. student's t-test로 검증하여 p<0.05, p<0.01 및 p<0.001인 경우 통계적인 유의성이 있는 것으로 판정하였다.
점막 손상에 대한 영향
도 1 및 도 2는 급성 역류성 식도염에 있어 식도 점막 손상에 대한 선모 추출물의 영향을 설명하는 비교 사진 및 식도 점막 손상율 측정 결과를 나타낸 그래프이다.
도 1 및 도 2를 참조하면, 정상군(INT)에서는 충혈(hyperemia)이나 장액혈액성 삼출(serosanginous exudates)과 같은 점막 손상이 나타나지 않았다(도 1의 A 및 B 참조). 그러나, 역류성 식도염이 유발된 대조군(RE)의 식도에서는 장액혈액성 삼출과 함께 심각한 종주 궤양(longitidinal ulcerations)이 관찰되었다(도 1의 C 및 D 참조). 대조군의 식도의 병 부위는 모의 수술 그룹과 비교하여 육안으로도 증가된 것을 알 수 있다. 그러나, 30㎎/㎏의 α-토코페롤이 처리된 양성 대조군(PC)은 대조군(RE)에 비해 손상이 덜한 것으로 나타났고(도 1의 E 및 F 참조), 선모 추출물(ECR)이 매개된 그룹인 실험군, 즉, 600㎎/㎏의 ECR이 처리된 ECR-A(도 1의 G 및 H 참조), 300㎎/㎏의 ECR이 처리된 ECR-B(도 1의 I 및 J 참조) 및 150㎎/㎏의 ECR이 처리된 ECR-C(도 1의 K 및 L 참조)에서는 모두 부식이 산포되어 나타나거나 식도를 따라 산포된 희끗한 삼출을 동반한 약한 출혈점만이 보이는 것으로 나타났다.

도 4 내지 도 6은 급성 역류성 식도염이 유발된 랫드에 있어 위액에서의 pH, 산도 및 뮤신 방출에 대한 선모 추출물의 영향을 나타낸 그래프이다.
각 식도에 나타나는 급성 역류성 식도염의 심각도 및 발병 정도를 6시간 후에 조사하고자, 실험을 위한 제한시간을 6시간으로 선택하였다.
먼저, 도 4를 참조하면, α-토코페롤(PC) 및 선모 추출물(ECR-A 내지 ECR-C) 투여에 따라 위산의 pH에 유의적인 효과를 보이고(p<0.05), ECR-B 및 ECR-C의 경우 대조군(RE)에 비해 유의적으로 감소한 것으로 나타났다. ECR-A 내지 ECR-C의 경우 상기 위 내용물의 유의적 감소와 함께 pH에 대한 효과로부터 위산에 대해 자극을 덜 받도록 유도된 것을 알 수 있다.
도 5를 참조하면, α-토코페롤(PC) 및 선모 추출물(ECR-A) 투여에 따라 위액의 산도가 유의적으로 감소한 것으로 나타났다(p<0.05).
도 6을 참조하면, α-토코페롤이 처리된 그룹(PC)에서는 위액 내 뮤신의 농도(846.2±55㎍/㎖)가 대조군(RE)(552.2±42.3㎍/㎖)에 비해 유의적으로 높아진 것으로 나타났다(p<0.05). 또한, 선모 추출물이 600㎎/㎏ 및 300㎎/㎏ 처리된 그룹(ECR-A 및 ECR-B)의 위액 내 뮤신 농도(831.9±51.3㎍/㎖ 및 745.6±58.4㎍/㎖)는 각각 대조군(RE)(552.2±42.3㎍/㎖)에 비해 높은 것으로 관찰되었다. 이로부터, 선모 추출물이 위액 분비에 영향을 주지 않으면서도 위액으로의 뮤신 방출에 효과적임을 알 수 있다.

외과적으로 유발되는 급성 역류성 식도염에서 (TNF)-α, (IL)-1β 및 (IL)-6과 같은 염증유발 사이토카인의 효과를 파악하기 위해, 선모 추출물(ECR)이 사이토카인 생성에 어떠한 영향을 미치는지 조사하였다.
급성 식도 점막염은 위액 역류의 주요한 결과로서, 식도 역류가 유발된 랫드에서 COX2 mRNA 및 단백질 발현의 급격한 증가를 초래하는 것으로 보고되고 있다. 따라서, 급성 역류성 식도염의 진행을 막기 위한 선택적 COX2 억제제의 효능을 평가하기 위한 임상실험이 진척되고 있다. 또한, 이러한 효과는 절제된 동물에서 염증 유발 사이토카인 IL-1β 및 TNF-α의 혈장 레벨의 상승을 동반한다(Katsunobu, O., F. Takashi, N. Itasu, M. Tomoharu, K. Shinichi, F. Sachio, O. Tetsuo and K. Miwa. A COX-2 inhibitor prevents the esophageal inflammation-metaplasia-adenocarcinoma sequence in rats. Gut 51: 316-322, 2002.)와 일치하며, 상기 연구에서는 TNF-α, IL-1β, IL-6와 같은 염증 유발 사이토카인이 추가적인 사이토카인을 생성할 수 있는 타깃 세포를 구성 및 활성화시킴으로써 급성 식도 역류와 식도 점막의 점진적 손상 및 충혈에 관련된 염증에 중요한 역할을 한다고 제시하고 있다. 혈청 내 IL-1의 생물학적 성질은 열병, 염증 및 혈류역학적 쇼크 유발을 포함하여 TNF-α의 성질과 유사하다(Akira, S., T. 도 7은 급성 역류성 식도염이 유발된 랫드의 혈장에서의 TNF-α, IL-1β 및 IL-6에 대한 선모 추출물의 영향을 나타낸 그래프이다.
도 7을 참조하면, 먼저, TNF-α는 정상군(INT)에서 14.9±0.7pg/㎖인 것으로 나타났으나, 대조군(RE)에서는 TNF-α 생성량이 2배 정도(40±1.5pg/㎖)로 나타났다. 반면, α-토코페롤로 처리된 양성 대조군(PC)(36.1±2.0pg/㎖)과 선모 추출물 600㎎/㎏이 처리된 실험군(ECR-A)(37.2±1.5pg/㎖)에서는 유의적으로 억제된 것으로 나타났다(p<0.05).
IL-1β는 대조군(RE)에서 53.8±4.6pg/㎖인 것으로 나타났으나, α-토코페롤로 처리된 양성 대조군(PC)(37.1±3pg/㎖), 선모 추출물 600㎎/㎏이 처리된 실험군(ECR-A)(37.9±5.0pg/㎖), 선모 추출물 300㎎/㎏이 처리된 실험군(ECR-B)(42.0±5.7pg/㎖) 및 선모 추출물 150㎎/㎏이 처리된 실험군(ECR-C)(45.4±4.9pg/㎖)에서는 대조군(RE)에 비해 유의적으로 감소된 것으로 나타났다(p<0.01).
IL-6 레벨은 정상군(INT)에서 0.38±0.21pg/㎖인 것으로 나타났으나, 대조군(RE)에서는 증가(13.7±0.5pg/㎖)된 것으로 나타났다. 반면, α-토코페롤로 처리된 양성 대조군(PC)(12.6±1.4pg/㎖)과 선모 추출물 600㎎/㎏이 처리된 실험군(ECR-A)(12.4±1.4pg/㎖)에서는 대조군(RE)에 비해 유의적으로 감소된 것으로 나타났다(p<0.05).
이러한 결과는 선모 추출물이 TNF-α, IL-1β, IL-6와 같은 사이토카인의 생성을 방해함으로써 염증, 궤양 및 식도 점막으로의 면역세포 침윤을 강하게 억제한다는 것을 나타낸다. 이는 후술하는 조직학적 분석 결과(도 9 참조)와 함께 상기 도 2에 나타낸 점막 손상율 분석 결과와도 일치한다.

도 8은 급성 역류성 식도염이 유발된 랫드의 식도 점막에서의 COX2 및 TNF-α mRNA 전사에 대한 선모 추출물의 영향을 나타낸 전기영동 사진이다.
도 8을 참조하면, 정상군(INT)의 식도 점막에서는 COX2 mRNA 및 TNF-α mRNA 발현이 약한 시그널을 보이는 반면, 대조군(RE)에서는 식도에서의 염증 반응으로 유의적으로 시그널이 증가한 것으로 나타났다. 그러나, α-토코페롤로 처리된 양성 대조군(PC)과 선모 추출물이 처리된 실험군(ECR-A 내지 ECR-C)에서는 COX2 mRNA 및 TNF-α mRNA 발현이 처리 농도에 비례하나, 대조군(RE)에 비해 유의적으로 억제된 것으로 나타났다.

Gut 55: 450-456, 2005.)은 COX2의 mRNA 및 단백질 발현은 식도염이 유발되지 않은 랫드에 비해 급성 역류성 식도염이 유발된 랫드에서 철저하게 증가하는 것을 제시하였다. 상기 도 1 및 도 2에서와 같이, α-토코페롤이나 선모 추출물을 경구 투여 처리한 경우 역류성 식도염에 있어 식도 점막에 유리한 효과를 나타내며, 또한, COX2 mRNA 발현량의 감소를 통해 호중구 및 다른 면역세포의 염증 작용에 대항하여 식도 점막을 보호할 수 있게 된다.
식도 점막 및 위벽 손상에 대한 조직학적 분석
도 9는 급성 역류성 식도염이 유발된 랫드의 식도 점막 및 위벽 손상에 대한 선모 추출물의 영향을 나타낸 현미경 사진이다.
도 9를 참조하면, H&E로 염색 후, 정상군(INT)의 점막에서는 현미경 수준에서 어떠한 변화도 관찰되지 않았다(도 9의 A 참조). 보통의 식도는 편평상피 세포(squamous cells)에서 얇은 상피층(epithelial layer)을 보이며, 약간의 염증 세포만이 점막하층에서 발견된다. 반대로, 역류성 식도염 유발 6시간 후, 대조군(RE)에서는 식도 중·하부에서 큰 덩어리의 종주 궤양으로 발전된 것이 관찰되었고, 상피층의 점막 손상 및 충혈과 점막 및 점막하에서 부종(edema)이 관찰되었다. 또한, 심한 손상을 동반한 현저한 점막 농밀화, 부종 및 심한 염증 세포의 침윤이 관찰되었다(도 9의 B 참조). 반면, α-토코페롤이 처리된 양성 대조군(PC)(도 9의 C 참조), 600㎎/㎏의 ECR이 처리된 ECR-A(도 9의 D 참조), 300㎎/㎏의 ECR이 처리된 ECR-B(도 9의 E 참조) 및 150㎎/㎏의 ECR이 처리된 ECR-C(도 9의 F 참조)에서는 식도 손상, 부종 및 호중성 침윤(neutrophil infiltration), 및 위 출혈이 여전히 발생하였으나, 대조군(RE)에 비해 덜 심한 것으로 나타났다.
하기 표 1에서는 식도 및 위에 대한 조직형태계측 결과로서 선모 추출물의 영향을 비교하여 나타내고 있다.
표 1을 참조하면, 선모 추출물이 처리된 그룹(ECR600, ECR300 및 ECR150)은 대조군(RE)에 비해 점막 및 점막하에서의 손상 및 부종이 유의적으로 감소한 것을 알 수 있다.
이상과 같이, 다양한 농도의 선모 추출물을 동물에 경구 투여 처리한 결과, 위 내용물 양이나 위액의 pH에 영향을 주지 않고, 뮤신 생성을 증가시키는 것을 확인하였고, 대조군(RE)에 비해 선모 추출물이 처리된 그룹(ECR-A 내지 ECR-C)에서의 추가적인 뮤신 분비가 식도 점막의 손상을 보호해 줄 수 있는 것을 알 수 있다. 더욱이, 선모 추출물은 항산화 및 항염증 활성을 보유하여 TNF-α, IL-1β, IL-6와 같은 사이토카인 레벨을 감소시키고, COX2와 같은 일산화탄소 합성 및 생장 인자의 양을 감소시킴으로써, 식도 및 위벽에서의 손상, 궤양 및 점액성 출혈(myxoid hemorrhage)을 유의적으로 억제할 수 있음을 제시하였다. 이러한 선모 추출물의 역류성 식도염에 대한 효능 메카니즘은 다음과 같은 세 가지 활성에 기인한 것으로 사료된다.
첫째, 선모 추출물에 의한 조직 손상의 직접적인 감소이다. 선모 추출물은 위 내 뮤신 방출을 증가시키고, 이로 인해 위산으로부터 식도 조직을 보호하는 것이다. 염증성 면역세포는 대조군(RE)에서보다 덜 침윤되는데, 이것이 TNF-α, IL-1β 및 IL-6의 감소를 가져오게 된다.
둘째, 식도 손상 감소로 인해 염증 유발 사이토카인으로부터의 산물이 감소되어 NF-kB가 불활성화되는 것이다. 선모 추출물에 의한 NF-kB 억제 효과는 식도 조직에서 COX2 mRNA 억제를 유도하게 된다.
셋째, 무세포계(cell-free system)에서 과산화물 라디칼을 처리(scavenge)할 수 있는 강력한 항산화제 활성이다. 선모 추출물은 p53, caspases-3 및 다른 많은 키나아제의 활성을 유도함으로써 산화스트레스(oxidative stress)로부터 세포 손상을 감소시킨다. 흥미로운 것은 커큘리고사이드의 첨가로 p53 mRNA의 상향조절이 약화되고, p53의 상향조절은 역시 NF-kB 활성과 관련이 있다는 것이다.
이하에서는 본 발명에 따른 선모 추출물을 함유하는 조성물로서 약학적 조성물 및 기능성 식품으로의 적용 제제예를 설명하나, 이에 한정되는 것은 아니다.

하기 표 3에서는 상술한 본 발명에 따른 선모 추출물을 유효성분으로 함유하는 정제 조성을 예시하고 있다. 정제는 하기 성분들을 혼합한 후 통상의 정제 제조방법에 따라 타정하여 제조될 수 있다.

하기 표 5에서는 상술한 본 발명에 따른 선모 추출물을 유효성분으로 함유하는 주사제 조성을 예시하고 있다. 주사제는 통상의 주사제 제조방법에 따라 1앰플당(2㎖) 하기 성분 함량으로 제조될 수 있다.

하기 표 6에서는 상술한 본 발명에 따른 선모 추출물을 유효성분으로 함유하는 액제 조성을 예시하고 있다. 액제는 통상의 액제 제조방법에 따라 정제수에 각각의 하기 성분들을 가하여 용해시키고 레몬향을 적량 가한 후 하기 성분들을 혼합한 다음 정제수를 가하여 전체 100㎖로 조절한 후 갈색병에 충진 및 멸균시켜 제조될 수 있다.

6: 건강 식품 제조
하기 표 7에서는 상술한 본 발명에 따른 선모 추출물을 유효성분으로 함유하는 기능성 건강 식품 조성을 나타내고 있다. 하기 조성에서 비타민 및 미네랄 혼합물의 조성비는 비교적 건강식품에 적합한 성분을 바람직한 제제예로 혼합 조성한 것을 나타내고 있으나, 그 배합비를 임의로 변형 실시하여도 무방하며, 통상의 건강식품 제조방법에 따라 각 성분을 혼합한 다음, 과립을 제조하고, 통상의 방법에 따라 건강식품 조성물 제조에 사용할 수 있다.

7: 건강 음료 제조
하기 표 8에서는 상술한 본 발명에 따른 선모 추출물을 유효성분으로 함유하는 기능성 건강 음료 조성을 나타내고 있다. 건강 음료 제조는 통상의 건강음료 제조방법에 따라 예컨대, 하기 성분을 혼합한 다음, 약 1시간 동안 85℃에서 교반 가열한 후, 만들어진 용액을 여과하여 멸균된 2ℓ 용기에 취득하여 밀봉 멸균한 후 냉장 보관하여 사용할 수 있다. 하기 조성비는 비교적 기호음료에 적합한 성분을 바람직한 실시예로 혼합 조성하였으나, 수요계층, 수요국가, 사용용도 등 지역적, 민족적 기호도에 따라서 그 배합비를 임의로 변형 실시하여도 무방하다.
이상으로 본 발명의 바람직한 실시예를 도면을 참고하여 상세하게 설명하였다. 본 발명의 설명은 예시를 위한 것이며, 본 발명이 속하는 기술분야에서 통상의 지식을 가진 자는 본 발명의 기술적 사상이나 필수적인 특징을 변경하지 않고서 다른 구체적인 형태로 쉽게 변형이 가능하다는 것을 이해할 수 있을 것이다.
따라서, 본 발명의 범위는 상기 상세한 설명보다는 후술하는 특허청구범위에 의하여 나타내어지며, 특허청구범위의 의미, 범위 및 그 균등 개념으로부터 도출되는 모든 변경 또는 변형된 형태가 본 발명의 범위에 포함되는 것으로 해석되어야 한다.
