Abstract

Description
본 발명은 위식도 역류, 소화성 출혈증 및 소화불량과 관련된 질병의 치료, 보다 구체적으로는 위식도 역류, 소화성 출혈 및 소화불량과 관련된 질병 치료용 약물 제조에 있어서 테나토프라졸의 용도에 관한 것이다.
테나토프라졸 또는 5-메톡시-2-[[(4-메톡시-3,5-디메틸-2-피리딜)메틸]술피닐]이미다조[4,5-b]피리딘은 유럽 특허 제254588호에 개시되어 있다. 테나토프라졸 또는 5-메톡시-2-[[(4-메톡시-3,5-디메틸-2-피리딜)메틸]술피닐]이미다조[4,5-b]피리딘은 위 궤양 및 십이지장 궤양의 치료에 유용한 양성자 펌프 저해제로 고려되는 약물 군에 속한다. 다른 양성자 펌프 저해제에는 오메프라졸, 라베프라졸, 판토프라졸 및 란소프라졸이 포함되는데, 이들은 모두 구조적 유사성을 나타내며, 피리디닐-메틸-술피닐-벤즈이미다졸 군에 속한다. 이들 화합물은 황 원자 수준에서 비대칭을 나타내는 술폭시드이며, 따라서 두가지 거울상이성질체의 라세미 혼합물 형태로 관찰되는 것이 일반적이다.
이 계열 중 처음으로 공지된 유도체는 유럽 특허 제005.129호에 개시되어 있고, 위산 분비 억제 특성을 가지며, 인간 치료에서 항-궤양제로 널리 사용되는 오메프라졸이다.
또한, 오메프라졸은 위식도 역류 장애의 치료에 대해서도 예상되어 왔으나, 이러한 적응증에서의 오메프라졸의 활성은 전적으로 만족스럽지는 않다. 실제로, 오메프라졸의 작용 기간은 다른 양성자 펌프 저해제의 경우에서와 같이 야간 역류를 효과적으로 치료하기에 불충분하다는 연구 결과가 밝혀졌다.
위식도 역류는 주로, 식도 하부에서의 비정상적으로 빈번한 일시적 이완 및 괄약근 긴장 감소를 특징으로 하는 운동 장애로 인한 것으로 생각된다. 이러한 이상증의 영향으로 위 내용물이 식도로 역류하게 된다. 또한, 위식도 역류로 고통받는 환자에서 역류하는 산은 정상 대상체에서보다 일반적으로 약 50% 더 느리게 제거되며, 산 공격에 대한 식도벽의 내성은 두드러지게 감소한다. 따라서, 위에 의한 산 분비는 위식도 역류로 고통받는 환자의 식도 점막 병소의 발병 및 지속에서 중요한 역할을 한다.
다양한 연구에 의해 위식도 역류로 고통받는 환자 증상의 중증도는 식도 점막이 산에 노출되는 기간에 비례한다는 사실이 밝혀졌다 [Howden CW, Burget DW, Hunt RH "Appropriate acid suppression for optimal healing of duodenal ulcer and gastrooesophageal reflux disease", Scand. J. Gastoenterol, Suppl (1994) 201:79-82]. 따라서, 무증상 대상체는 노출비(하루 동안 산에 대한 노출 기간의 비율)이 약 1% 이지만, 병소의 중증도에 따라 때때로 위식도 역류에 의해 영향받는 대상체는 노출비가 2%에 가깝고, 매일 증상이 나타나는 대상체는 노출비가 3%이며, 내시경적 병소를 나타내는 대상체는 노출비가 6% 내지 12%이다. 이러한 연구는 정상 pH 값이 일반적으로 pH 5 내지 7인 식도에서 pH 4 미만, 즉 비정상적 으로 낮은 pH의 산에 노출된 환자에서 수행된다.
상기 연구에 의해 산에 더 오래 노출될수록 증상 및 식도 점막 손상이 심해진다는 사실이 입증되었다.
또한, 적절한 약물 치료에 의한 산 억제가 병소 회복 속도와 상관관계가 있으며 중요한 파라미터는 산 억제 기간 및 크기라는 연구 결과가 밝혀졌다. 이러한 이유로, 위식도 역류로 고통받는 환자의 상기 증상을 완화시키기 위해 흔히 제산제, 히스타민 수용체 길항제 또는 양성자 펌프 저해제를 처방한다. 그러나, 사용되는 대부분의 약물은 증상을 단지 부분적으로만 완화시키거나 또는 작용 기간이 너무 짧아 반복적인 약물 섭취를 필요로 하기 때문에, 이들 약물이 전적으로 만족스러운 것은 아니다.
유사하게, 소화불량의 치료에서, 양성자 펌프 저해제가 소정의 완화를 제공할 수 있지만 치료 효과는 거의 없었다는 연구 결과가 밝혀졌다.
기능성 소화불량은 상복부에서 음식물과 관련된 일련의 증상 변화, 및 다양한 정도의 관련 동통 또는 불쾌감, 조기 포만감 또는 느린 소화, 오심, 구토 등으로 이루어진다. 기능성 소화불량의 병태생리학은 여전히 불완전하게 이해된 채로 남아있다.
어떤 환자, 특히 가성-궤양(pseudo-ulcer) 유형으로 고통받거나 또는 위식도 역류 증상과 유사한 증상이 있는 환자에서 양성자 펌프 저해제의 치료 범위에 있는 약물, 예를 들어 오메프라졸 또는 란소프라졸을 투여하여 증상을 완화시킬 수 있는 것으로 밝혀졌다. 이러한 연구에서, 치료 이점은 산에 대한 식도의 노출이 증가된 대상체에서 주로 관찰된다. 그러나, 양성자 펌프 저해제의 제거 반감기는 비교적 짧아 위산 억제에서 문제가 되며, 따라서 상기 저해제를 처방하여 기능성 소화불량을 효과적으로 완화시킬 수 없다.
향상된 특성을 나타내기에 알맞은 양성자 펌프 저해제를 함유하는 제제를 제공하는 여러 기술들이 개발되어 왔다. 예를 들어, 국제 특허 공개 제WO 02.072070호는 오메프라졸 또는 에소메프라졸 마그네슘을 고함량 함유하며 장용 코팅으로 코팅하여 산성 위액과의 접촉으로부터 보호할 수 있는, 분무-동결 기술에 의해 얻어지는 미립자를 개시한다. 국제 특허 공개 제WO 99.59544호는 장용 코팅층으로 코팅된 조성물을 포함하는 미세 과립을 함유하는 경구 붕해성 정제를 기술한다. 상기 조성물은 산에 불안정한 활성 물질, 예를들어 란소프라졸을 포함한다. 상기 특허 문헌들에 기술된 미립자 및 미세 과립 함유 정제는 위산 관련 질병의 일반적인 치료에 유용한 것으로 예상되었지만, 임상 결과는 제공되지 않았다. 문헌 [Adis R&D Profile (2002:3(4) 276-277)]은 테나토프라졸의 일부 특성에 관한 것이며 테나토프라졸이 일본에서 2002년 4월에 역류성 식도염에 대해 등록되었다고 언급하고 있지만, 테나토프라졸의 사용가능성이 그당시까지 입증되지 않았기 때문에 상기 등록은 결코 인정될 수 없으며, 따라서 그러한 정보는 그릇된 것이다.
이러한 이유로, 위식도 역류 및 소화불량으로 고통받는 환자의 증상을 효과적으로 치료하고 완화할 수 있는 약물이 현재 요구되고 있다.
예상치 못하게도, 본 출원인에 의해 수행된 연구 및 실험 작업에 의해 테나토프라졸을 사용하여 위식도 역류 및 소화불량과 관련된 질병을 효과적으로 치료할 수 있지만 오메프라졸 및 유사한 구조를 갖는 다른 양성자 펌프를 사용하면 상기 적응증에서 어떤 만족스러운 치료 효능을 얻을 수 없다는 사실이 밝혀졌다.
따라서, 본 발명의 목적은 위식도 역류의 비정형 식도 증상, 소화성 출혈 및 소화불량의 치료에서의 테나토프라졸의 용도 및 위식도 역류의 비정형 식도 증상, 소화성 출혈 및 소화불량의 치료용 약물 제조에서의 테나토프라졸의 용도에 관한 것이다.
테나토프라졸은 오메프라졸 및 유사한 구조를 갖는 다른 술폭시드와 유사하게 비대칭 구조를 포함하며, 따라서 라세미 혼합물 또는 거울상이성질체 형태로 제공될 수 있다.
예를 들어, 오메프라졸 또는 에소메프라졸과 같은 다른 양성자 펌프 저해제와 달리, 테나토프라졸은 약 7배 더 긴 반감기에서 유래하는 뚜렷하게 더 긴 작용 기간을 갖는다. 따라서, 수집된 의학 자료에 따르면 테나토프라졸이 양성자 펌프 저해제의 동일한 치료 범위에 속하는 다른 약물들에서 나타나는 것보다 우수한 소정의 증상 완화 및 병소 치유를 보장하며, 따라서 위식도 역류의 비정형 식도 증상, 소화성 출혈 및 소화불량의 치료에 효과적으로 사용될 수 있는 것으로 밝혀졌다.
본 발명은 위식도 역류의 비정형 증상, 보다 구체적으로는 표준 양성자 펌프 저해제, 예를 들어 오메프라졸을 사용한 치료에 대해 현재 난치성으로 남아있는 야간 비정형 증상을 더 큰 정도로 완화시키기 위해 테나토프라졸의 사용을 고려한다. 유사하게, 본 발명은 약물 섭취 용량이 치료 효과 기간에 따라 달라지는 위식도 역류의 비정형 증상의 비정기 치료에서 현저한 이점을 제공한다.
본 발명의 다른 이점은 테나토프라졸이 식도 하부 또는 위식도 연결부의 수준에서 장관-유형 점막(원주형)의 존재에 의해 정의되는 바레트(Barrett's) 식도증 또는 내부 단식도증(endobrachyoesophagus)에 대해 효과적으로 작용할 수도 있다는 점이다. 이 증상은 소화성 식도염의 합병증이며, 여러 경우에서 선암종으로 악화될 수 있다.
바레트 식도증으로 고통받는 환자는 일반적으로 평균적인 위식도 역류보다 더 심한 고통을 경험하며, 역류의 산성도는 세포 분화 및 증식에 대해 해로운 결과를 나타내 소화불량의 발병에 일조할 수 있다. 따라서, 위식도 역류와 관련된 증상 및 바레트 식도증과 관련된 조직학적 병소를 나타내는 환자에서는 산 분비를 감소시키는 능력이 중요하다.
바레트 식도증의 경우에 치료는 위식도 역류성 산을 최대로 억제해야 하는데, 테나토프라졸의 투여는 실제로 이러한 억제를 가능케 하며, 보다 구체적으로는 야간 흉통 발생을 예방하는데, 이는 현재 이용가능한 약물, 즉 표준 양성자 펌프 저해제에 의해서는 달성되지 않는다.
하기 나타낸 바와 같이, 테나토프라졸은 본 출원인에 의해 수행된 실험 동안 입증된 바로는 놀랍게도 더 긴 제거 반감기 및 상당한 정도의 조직 노출 때문에 다른 양성자 펌프 저해제와 구별될 수 있다.
백인종(Caucasian) 개인들(군 당 개체수(n)는 8명임)에 대한 임상 제I 상 연구에서 단일 투여량 및 7일 동안의 일일 투여량을 경구 투여하는 경우에 약물동태 학적 파라미터에 대한 상이한 투여량의 테나토프라졸의 영향을 입증할 수 있었다.
시험된 투여량은 테나토프라졸 10 mg, 20 mg, 40 mg 및 80 mg이었다.
얻어진 결과를 하기 표 1에 나타냈다.

상기 표 1에 나타낸 결과는 평균 제거 반감기가 단일 투여량의 투여 후에는 5 내지 6 시간이었고 7일 동안의 투여 후에는 5 내지 9.5 시간이었으며 투여량에 따라 달라진다는 것을 입증한다. 또한, 테나토프라졸은 경구 경로를 통해 느린 대사 속도 및(또는) 높은 생체이용성의 증거를 제공하는 높은 AUC 값(그래프하 면적)을 나타냈다. 또한, 단일 또는 반복 투여의 어떤 투여 조건에서도 Cmax, AUC0 -t 및 AUC0-inf 값은 선형 양상으로 증가하였다. AUC0 - inf 값은 외삽에 의해 계산하였다.
두가지 양성자 펌프 저해제인 란소프라졸과 오메프라졸의 AUC 값의 비교는 톨맨(Tolman) 등의 문헌 [J. Clin. Gastroenterol., 24(2), 65-70, 1997]에 의해 수행하였지만, 이 비교 결과에 의해 하나의 저해제가 다른 저해제보다 우수하다는 것을 판단할 수는 없었다. 물론, 여러 기준, 즉 펌프 재생에 필요한 시간 및 양성자 펌프 억제에 필요한 최소 농도를 초과하는 기간을 고려해야 한다. 펌프 재생 시간에 대해, 펌프는 일반적으로 약 30 시간 내지 48 시간의 반감기를 가지며, 따라서 72 시간 내지 96 시간마다 완전히 교체되는 것으로 관찰된다.
상기 기술한 약물동태학 특성으로 인해, 테나토프라졸은 전술한 구체적인 두가지 기준을 충족시키는 충분히 오랜 시간 동안 억제 농도를 유지함으로써 양성자 펌프 재생 현상을 방해할 수 있다.
따라서, 테나토프라졸의 긴 반감기에 장시간 노출(AUC에 의해 결정됨)을 연계하면 테나토프라졸이 활성 부위에 더 오랜 시간 존재하게 되고, 따라서 긴 시간 동안 약동학적 효과를 얻는다. 실험에 의해, 테나토프라졸이 다른 양성자 펌프 저해제에서 나타나는 것보다 현저하게 더 높은 혈장 반감기/펌프 재생 시간 비율을 가지며, 따라서 테나토프라졸을 사용함으로써 현재 이용가능한 치료에 의해서는 충분한 효능이 얻어지지 않는 질병, 특히 위식도 역류의 비정형 식도 증상, 소화불량 및 바레트 식도증을 치료할 수 있는 것으로 밝혀졌다.
보다 구체적으로, 본 발명에 따라 테나토프라졸을 사용하여 위식도 역류의 비정형 증상, 예를 들어 천식 및 천식 유형의 호흡곤란 발작, 인두염, 발성장애, 슈도-앙기나(pseudo-angina), 발작성 기침 및 야간 기침을 치료할 수 있다. 또한, 테나토프라졸은 가성-궤양성 소화불량을 치료하는데 있어서 특히 효과적이다. 그리고, 상기 나타낸 바와 같이, 테나토프라졸을 사용하여 바레트 식도증을 성공적으로 치료할 수도 있다.
위식도 역류의 비정형 식도 증상, 특히 궤양으로 인한 소화성 출혈, 및 소화불량의 치료에서, 테나토프라졸을 선택된 투여 방식에 맞게 개질된 일반적인 형태로, 예를 들어 경구 또는 비경구 경로를 통해 투여할 수 있지만, 경구 또는 정맥내 경로를 통해 투여하는 것이 바람직하다. 예를 들어, 활성 물질로 테나토프라졸을 함유하는 정제 또는 캡슐 제형, 또는 제약상 허용가능한 표준 기재와 함께 테나토프라졸 염을 함유하는 음용가능한 용액 또는 에멀젼 또는 비경구용 용액을 사용할 수 있다. 테나토프라졸 염은 나트륨염, 칼륨염, 마그네슘염 또는 칼슘염으로부터 선택될 수 있다.
예로서, 제약상 허용가능한 지지체 및 부형제와 배합된 테나토프라졸 20 mg을 함유하는 정제를 위한 적절한 제형을 하기에 나타낸다:

투여량은 의사에 의해 환자 상태와 장애 중증도의 함수로서 결정된다. 테나토프라졸의 투여량은 일반적으로 10 mg/일 내지 120 mg/일, 바람직하게는 20 mg/일 내지 40 mg/일이며, 이러한 투여량은 예를 들어 초기 또는 유지 요법의 경우에 4주 내지 12주일 수 있는 기간 동안 상기 활성 물질을 각각 20 mg 내지 40 mg 함유하는 정제를 하루에 1개 또는 2개 섭취하는 것에 상응한다. 어린이들에 맞게 개질된 소아용 제형, 예를 들어 음용가능한 용액의 경우, 단위 투여량은 적을 수 있으며, 예를 들어 2 mg 또는 5 mg이다. 심각한 장애의 경우, 의약을 먼저 정맥내 경로를 통해 투여한 다음, 경구 경로를 통해 투여하는 것이 효과적일 수 있다. 또한, 본 발명은 매주 20 mg 또는 40 mg을 함유하는 단일 정제를 간단히 투여하여 효과적이면서 연속적으로 치료할 수 있는 이점을 갖는다.
하기에 제시된 몇몇 임상적 예는 위식도 역류 또는 소화불량으로 고통받는 환자에게 테나토프라졸을 경구 투여하여 달성된 치료 효과를 나타낸다.
기호 + 내지 +++는 중간 정도 내지 고도로 바람직한 범위의 증상 진전 및 안정성을 나타낸다.
치료는 테나토프라졸 20 mg을 함유하는 정제를 매일 투여하는 것으로 이루어진다. 표는 14개 경우 중 12개에서 치료가 완전히 관용적이었으며 증상에서 관찰된 진전이 전반적으로 매우 바람직하였음을 나타낸다.
GERD: 위식도 역류 질병
상기 표의 결과는 위식도 역류와의 관련성이 가장 분명한 경우에서 증상 진전이 특히 바람직하였음을 나타낸다.
상기 결과에 의해 소화불량의 치료에서 본 발명에 따라 투여되는 테나토프라졸의 효능이 확인된다.
두가지 개방적(open-labeled) 연구를 수행하여 위식도 역류 질병에 대한 테나토프라졸의 효능 및 안전성을 평가하였다. 미란성 및(또는) 궤양성 유형의 역류 식도염(내시경으로 진단함)으로 고통받는 20세를 넘는 22명의 환자들(제1 연구) 및 24명의 환자들(제2 연구)에게 테나토프라졸 10 mg을 갖는 장용-코팅 과립을 함유하는 정제를 투여하였다.
상기 정제를 8주의 치료 기간 동안 하루에 한번씩 조식 후에 경구 투여하였으며, 어떤 경우에는 치료 기간이 12주까지 지속되었다. 제1 투여 후 4주 및 8주의 시점에서 내시경으로 검사하여 치유를 관리하거나 또는 연구 중지시 질병 단계를 세이베리(Savary) 및 밀러(Miller) 분류에 따라 평가하고, 치유가 확인되면 치료를 중단하였다. 침식증이 소실된 것으로 확인되었을 때 증상이 치유된 것으로 평가하였다.
내시경적 개선 평가를 다음과 같은 6가지 등급에 따라 평가하였다: "치유됨," "현저히 감소됨," "중간 정도 감소됨," "약간 감소됨," "변화 없음" 및 "악화됨."
주관적 증상 및 객관적 증상의 개선 평가를 연구 시작시의 관찰결과와 비교하여 다음과 같은 6가지 등급에 따라 평가하였다: "현저히 개선됨," "중간 정도 개선됨," "약간 개선됨," "변화 없음," "악화됨" 및 "연구 시작시로부터 증상 없음."
치유는 4주 후에 관찰되었으며, 이 단계에서 20개의 경우(제1 연구) 및 23개의 경우(제2 연구)에서 테나토프라졸 투여를 중단하였다. 치료 8주 후에는 1명의 환자만이 치유되지 않았다.
상기 결과를 하기 표 5 및 6에 기록하였다.
상기 결과는, 위식도 역류 질병의 치료에서 본 발명에 따른 테나토프라졸을 사용하면 4주 치료에 의해 치유가 달성되기 때문에 통상의 양성자 펌프 저해제를 사용하는 8주 치료에 의해 얻어진 만족스럽지 않은 결과에 비해 매우 효과적임을 입증한다.
Claims (10)
- 테나토프라졸을 포함하는, 야간 위식도 역류증의 치료용 제약 조성물.
- 제1항에 있어서, 경구 경로를 통해 투여되는 제약 조성물.
- 제1항에 있어서, 비경구 경로를 통해 투여되는 제약 조성물.
- 제1항 내지 제3항 중 어느 한 항에 있어서, 테나토프라졸이 10 mg/일 내지 120 mg/일의 비율로 투여되는 제약 조성물.
- 제4항에 있어서, 테나토프라졸 20 mg 내지 40 mg을 하나 이상의 제약상 허용가능한 부형제 및 기재와 함께 함유하는 단위 투여 형태로 제공되는 제약 조성물.
