Abstract

요소들이 원추형 연결체를 통해 서로 연결된 내장식 인공 보철기(EP-B 474 015)는 공지되어 있다. 원추형 연결체는 한 요소 상의 원추형 보어와, 다른 요소 상의 원추형 플러그를 포함하며, 원추형 플러그는 원추형 보어와 결합하여 원추형 보어에 의해 수용된다. 충분히 작은 원추각이 선택된 때, 원추형 보어와 원추형 플러그는 조립 시에 충분한 힘으로 함께 결합되는 동안에 다른 요소 내에 한 요소를 고정적으로 끼운다. 손상의 기회로부터 보호하기 위해, 이러한 원추형 연결체에는 한 요소의 나사 보어 내에 놓이며, 해제 방향으로 가로지르는 다른 요소 내의 대응 리세스 내로 팁(tip)으로 전방 나사조임될 수 있는 고정 스크류가 제공된다. 재수술의 경우에, 고정 스크류는 접근이 용이하지 않다. 따라서, 대체될 인공 보철기의 제거가 어려워진다.
따라서, 본 발명은 재수술의 경우에 이러한 종류의 고정 볼트의 해제를 용이하게 하는 목적에 기초를 둔다.
본 발명에 따른 해결책은 청구항 1에 따른 방법에 있다. 따라서, 연결체는 고정 볼트가 파단 또는 전단 파단됨으로써 분리된다. 이렇게 하기 위해, 인공 보철기의 정상 사용 동안에 예상되지 않는 크기로 되는 하중이 인가된다. 더 용이하게 파단 또는 전단 파단시키기 위해, 고정 볼트는 취성 또는 연성 또는 소정 파단점을 갖도록 설계될 수 있다.
예컨대, 취성 고정 볼트의 경우에, 적절한 도구가 2개의 요소들 중 하나의 요소 상에 해제 방향으로 충격을 가하기 위해 재수술 동안에 사용되며, 상기 충격에 의해 고정 플러그가 파괴되고 연결 보어 내의 연결 플러그도 동시에 대체로 해제되며, 이후에 요소들은 서로로부터 용이하게 분리된다.
스크류 볼트는 대체로 고정 볼트로서 사용되지만, 이것은 절대적인 필수 요건이 아니다.
고정 볼트의 파단부가 2개의 요소 사이에 끼이지 않고 분리를 방해하지 않도록, 파단부를 수용하기에 충분한 크기로 된 리세스가 근접하게 제공되어야 한다. 이러한 목적을 위해, 볼트의 일부 길이가 고정 상태로 결합하는 리세스가 편의상 사용된다.
본 발명은 이로운 예시적인 실시예를 나타내는 도면을 참조하여 이하에서 더 상세히 설명된다.
도1은 내장식 무릎 관절 인공 보철기의 대퇴부 요소와 결합된 골수강 플러그 사이의 원추형 연결체를 도시한다.
도2는 도1에 따른 구조를 통해 다른 실시예의 부분 단면을 도시한다.
도1에 따른 실시예에서, 내장식 무릎 관절 인공 보철기(knee-joint endoprosthesis)의 대퇴부 요소(1)는 하부 부분(2)이 부분적으로 절결되어 도시된골수강(medullaty cavity) 축에 연결된다. 골수강 축의 원추형 보어(4) 내에 끼워진 원추형 플러그(3)는 대퇴부 요소(1)에 연결된다. 원추형 플러그 내부에는, 결합되는 고정 리세스로서 역할하는 보어(7) 내로 팁이 전방 나사조임될 수 있는 고정 스크류(6)를 포함하는 나사 보어(5)가 있다. 보어(5, 7)의 축(8, 9)은, 고정 스크류(6)의 팁이 해제 방향으로 보어(7) 측에 있는 보어(7)의 모서리(A)와 협동하도록 소정의 양(13)만큼 오프셋된다. 스크류(6)가 죄어질 때, 원추형 연결체(3, 4)를 강화하는 힘이 생성된다. 보어(7)의 다른 모서리(B)는 고정 스크류가 도달하지 않는 원래 규격의 원주 상에 있다.
조립되기 전에, 고정 스크류(6)는 원추형 플러그(3)가 보어(4) 내로 끼워지도록 완전히 나사 보어(5) 내에 놓인다. 후에, 고정 스크류는 고정 위치로 나사조임될 수 있도록 하부(2) 내의 보어(12)를 통해 접근이 용이하다. 요구된다면, 로킹 스크류(도시 안됨)는 이후에 고정 스크류(6)를 고정 위치에 고정시키기 위해 끼워질 수 있다.
고정 스크류(6)는 취성 재료이기는 하지만 인공 보철기의 정상 사용 동안에 고정 기능을 보장하기에 충분히 강한 재료로 구성된다. 재수술의 경우에는, 적절한 도구가 해제 방향으로 대퇴부 요소(1) 상에 충격을 가하기 위해 사용된다. 따라서, 고정 스크류의 팁은 파단되어 보어(7)로 떨어진다. 요소들은 이후에 서로로부터 용이하게 분리될 수 있다.
다르게는, 고정 스크류(6)가 고정 기능에 대한 강도는 충분하지만, 예컨대 요소(1, 2)들을 분리하는 힘이 스프레더(spreader) 도구를 사용하여 가해질 경우에전단 파단되는 연성 재료로 구성된다.
구조 상의 구성(도시 안됨)이 도1의 구성에 비유될 수 있는 도2의 다른 실시예에서, 고정 기능에 있어서 보어(7)와 협동하는 고정 스크류(6)의 팁은 소정 파단점(11)을 통해 스크류의 주요부로부터 분리된다. 이것은 스크류 팁이 파단 또는 전단 파단된 후에 형성된 파단부가 보어(7)로 수용되기에 너무 크지 않도록 한다. 파단부가 보어(7)의 수용 능력을 초과하는 크기가 될 경우에, 고정 기능이 변하지 않는 모서리(A)에만 의존하므로 보어(7)는 모서리(B) 측을 향해 더 넓게 용이하게 설계될 수 있다.

    상기 제1 요소는 그의 볼트 보어 내에 고정 볼트를 갖고, 상기 제2 요소는 고정 볼트의 길이의 일부를 수용하기 위해 형성된 리세스를 갖고, 상기 고정 볼트는 고정 볼트를 파단 또는 전단시키는 하중이 내장식 인공 보철기의 일반적인 사용 중에 예상되지 않는 크기이도록 소정의 파단점을 갖는 것을 특징으로 하는 내장식 인공 보철기.
  7. 제6항에 있어서, 상기 리세스는 상기 고정 볼트의 분리된 부분을 수용하기에 충분한 크기인 것을 특징으로 하는 내장식 인공 보철기.
