Description

【発明の詳細な説明】 （産業上の利用分野） 本発明は股関節の補綴部材、特に股関節補綴部材の寛骨
臼移植組織片にして、寛骨臼内に錠着するためのねじを
右する寛骨臼移植組織片に関する。
〈従来の技術） 股関節と代替するための補綴部材は周知である。
元来は大腿骨の球状先端だけが置換されていたが、すで
に股関節の何れかの部分、すなわち関節の寛骨臼受口部
分または球形端を取替え得るようになっている。
人工股関節の受口部分を形成する周知の寛骨臼カップ移
植組織片は思考の骨盤の寛骨臼内に装着する金属カップ
シェルと、関節の球状部分を受入れるための球形支承面
となるプラスチック材料の内方ライナーとよりなってい
る。この金属カップシェルは骨盤に対する碇着を容易に
づる雄ねじを有するものとし、またはセメントあるいは
ねじのような伯の手段によって装着し得るようになすこ
とができる。
雄ねじを有する金属カップシェルは広く使用されており
、たとえば米国特許第３，８９４．２９７号および第４
，６６２．８９１号ならびに国際的出版物Ｎ（ＩＷＯ８
７１０５４９０に記載されている。しかしながら金属カ
ップの外側の全表面部分が寛骨臼の表面と確実に接触し
、補綴部材から骨盤に力を最大限に伝達し得るようにす
るために必要なセルフタッピングねじを有する金属カッ
プシェルを完全に挿入することは困難である。移植組織
片は詰まりを発生させる傾向があり、この時は移植組織
片を何れかの方向に回転させる必要があり、これは骨を
破壊させるおそれがある。
（発明が解決しようとする課題） 本発明の目的は股関節の補綴部材にして、これを挿入す
る時に起こり易い移植組織片の詰まりを減少せしめ得る
補綴部材を供することである。
（課題を解決するための手段） 一般的に言って本発明による寛骨臼移植組織片は金属カ
ップシェルにして、該シェルの周囲を延びるセルフタッ
ピング雄ねじを有する金属カップシェルよりなっている
。前記ねじは複数のタッピング溝によって複数のセグメ
ントに分割される。
これらねじセグメントのねじ半径は該セグメントの先導
端から追従端の方に漸次減少ｕしめられるようになつ−
Ｃいる。
次に添付図面によって本発明の詳細な説明する。
（実施例） 金属カップシェル２は適当な金属、たとえばチタンまた
はチタンとアルミニウムおよび（または）バナジウムと
の合金によって形成することができる。カップシェル２
は半球形の空洞６を画定する全体が半球形または切頭円
錐形の壁４を有している。この空洞の中に挿入されたプ
ラスチックライナー（図示せず）は股関節の球形部分に
対する球形支承面を画定する。
金属カップシェル２はその外面に沿ってセルフタッピン
グねじを備え、このねじは交互に配置された長いねじセ
グメント８と短いねじセグメント１０とよりなっている
。長いセグメント８の長さ８はなるべくは１．５八と１
８Ａとの間にあるようにされる。ここにＡは短いセグメ
ント１０の長さである。
各ねじセグメント８または１０はくねじセグメン１−の
外側から金属カップシェル２の縦ＩＦ＊線まで測って）
、該セグメント８または１０の先導端１２から追従端１
４に向って漸次減少するようにされている。これは挿入
を行う時に骨の粘弾性に起因する移植組織片の詰まりを
減少させると考えられるためで、このようにしな【ノれ
ば骨はねじセグメントの切削縁から偏向し、したがって
ねじセグメントの追従部分の上に凝集づるようになる。
前えないようにされる。このようにすればねじセグメン
ト８または１ｏの先＊　’ｊＥ　１２は骨の方に向って
鋭い切削面を有するようになり、したがって骨を偏向せ
しめることなく、王台良く切削することができる。した
がって図においては Ｒａ＞　Ｒｂ＞　Ｒｃ＞　Ｒｄ である。
各ねじセグメント８および１０の幅はなるべくは先導端
１２から追従端１４に向って減少せしめられ、それによ
ってさらにねじセグメント８または１０の追従部分１４
における骨の凝集傾向を減少させるようになっている。
短いねじセグメント８および長いねじセグメント１ｏを
タッピング溝１６によって分離し、このタッピング溝の
全容積がねじセグメントの全容積を越えるようになすこ
とが望ましい。
タッピング溝１６を設ける目的はねじセグメント８また
は１０によって切削された骨の破片に対して空間を形成
することである。したがってタッピング溝１６の全容積
が、少なくともセルフタッピングの行われる時に発生す
る骨の破片の容積に等しいことが重要である。ものこの
ようにしなければタッピング溝１６は密集した骨の破片
で詰まり、移植組織片２のそれ以上の挿入を阻止するよ
うになる。タッピング溝１６の配置が不適当であれば、
移植組織片２自体が詰まるようになる。移植組織片２の
タッピング溝１６の全容積はねじセグメント８および１
ｏの全容積に等しいか、これより大きくすることが望ま
しい。
タッピング溝の形態は、挿入が行われる時にカップシェ
ル２が詰まる傾向に大きな影響をおよぼすものと考えら
れている。タッピング溝が広ければ骨に大きなでこぼこ
を発生せしめ、これが溝に人って移植組織片の詰まりを
起こし、加えられる力で大であれば、骨を破壊させる危
険がある。均等に隔買された多数の狭いタッピング溝を
使用する場合も、ねじセグメントが比較的短いことに起
因して同様な問題が発生する。したがってタッピング溝
１６は、ねじを交互に長いセグメン１へ８と短いセグメ
ント１０とに分割することによって、対をなずように配
置される。長いねじセグメン１−８は少なくとも短いセ
グメント１０より５０％長４゜ くなるように、しかも普通は１８００％を越えないよう
にすることが望ましい。できれば長いねじセグメント８
は短いセグメント１０より５００％長くなるようにされ
る。
上述のねじセグメント８および１０はねじの完全成形部
分に亘って延びる。ねじの切込み部分および切上げ部分
は前記形態に適合させる必要はない。特に半球形カップ
シェル２の場合はねじの切込み端は半球形の極の部分か
ら出発し、かつ前記の形態は、ねじの半径が切込み部分
で増加するから実用的ではない。
前述の構造は特許請求の範囲を離れることなく種々の変
型を行うことができ、以上に図示説明したすべての事項
は例証的なもので、制限的意味を有するものではない。

Claims (8)

    【特許請求の範囲】
  1. （１）股関節補綴部材の寛骨臼移植組織片において、全
    体が半球形または切頭円錐形の金属カツプシエル（２）
    よりなり、該カツプシエル（２）がその周囲を延びるセ
    ルフタツピング雄ねじを有し、このねじが複数のタツピ
    ング溝（１６）によつて先導端（１２）および追従端（
    １４）を有する複数のねじセグメント（８，１０）に分
    割され、これらねじセグメント（８，１０）が各セグメ
    ント（８）または（１０）の先導端（１２）から追従端
    （１４）に向つて漸次減少する半径Ｒａ、Ｒｂ、ＲＣお
    よびＲｄを有していることを特徴とする寛骨臼移植組織
    片。
  2. （２）各ねじセグメント（８，１０）のねじ半径Ｒａ、
    Ｒｂ、ＲｃおよびＲｄが、該セグメント（８，１０）の
    先導端（１２）から追従端（１４）に向つて少なくとも
    ３０％を越えないように減少している請求項１記載の寛
    骨臼移植組織片。
  3. （３）各ねじセグメント（８，１０）のねじ半径Ｒａ、
    Ｒｂ、ＲｃおよびＲｄが該セグメント（８，１０）の先
    導端（１２）から追従端（１４）に向つてほぼ１０％減
    少している請求項２記載の寛骨臼移植組織片。
  4. （４）各ねじセグメント（８，１０）がその先導端（１
    ２）から追従端（１４）に向つて漸次減少する幅を有し
    ている請求項２記載の寛骨臼移植組織片。
  5. （５）ねじタツピング溝（１６）が少なくともねじセグ
    メント（８，１０）の全容積に等しい全容積を有してい
    る請求項２記載の寛骨臼移植組織片。
  6. （６）タツピング溝（１６）が対として配置され、ねじ
    セグメント（８，１０）を交互に長いねじセグメント（
    ８）と、短いねじセグメント（１０）とに分割するよう
    になつている請求項２記載の寛骨臼移植組織片。
  7. （７）長いねじセグメント（８）が短いねじセグメント
    （１０）より５０−１８００％長い請求項６記載の寛骨
    臼移植組織片。
  8. （８）長いねじセグメント（８）が短いセグメント（１
    ０）よりほぼ５００％長い請求項７記載の寛骨臼移植組
    織片。
