「骨髄腫前駆細胞」は、骨髄腫形質細胞に分化する直前の段階の前駆細胞であり、CD38が強発現しているが、成熟形質細胞に特異的なマーカーであるCD138の発現がないことによって特徴付けられる。よって、骨髄腫前駆細胞は、「CD38++CD138−細胞」又は「CD19−CD38++CD138−細胞」又はと表記される場合もある。
一実施形態において、ヒトCD98hcのN型糖鎖付着部位を含み、N型糖鎖修飾の阻害によって露出するエピトープを認識する抗体が提供される。CD98(「4F2」とも称される)は、細胞膜上に発現し、アミノ酸トランスポーターとして機能することが知られる、約120kDaのヘテロ2量体のタンパク質である。CD98hcは、CD98を構成する約80kDaのII型膜貫通型タンパク質であり、「4F2hc」又は「SLC3A2」と称される場合もある。ヒトCD98hcには幾つかのアイソフォーム(b、c、e、f等)が存在するが、細胞外ドメインのアミノ酸配列は共通している。代表的なヒトCD98hcであるアイソフォームfのアミノ酸配列を配列番号21として配列表に示す。ヒトCD98hcは、配列番号21のアミノ酸配列における第264番目、第280番目、第323番目、及び第405番目にN型糖鎖が結合する構造を有する。本書では、ヒトCD98hcのアイソフォームfのアミノ酸配列を代表例として参照するが、対応する配列/アミノ酸残基が他のアイソフォームにも存在し、同等に機能することを当業者は理解する。
ADCC活性は、Brunner K.T.らの方法(Brunner, K.T., et al., Immunology, 1968. 14:181-96)に従って測定することができる。例えば、骨髄腫細胞を10%FCS添加のRPMI1640培地にて培養し、細胞数が0.5×104〜1.0×104個となるように調製する。これに適量のNa2 51CrO4を加え、37℃で1時間反応させ、細胞を51Crでラベル化し、洗浄したものを標的細胞とする。エフェクター細胞としては、SCID マウスの骨髄細胞を10%のFBS、10ng/mlのマウスGM-CSF、及び40IU/mlのヒトIL2を添加したRPMI1640中で6日間培養したもの等を使用することができる。96ウェルプレートに被検抗体又はコントロールとなるそのアイソタイプ抗体を終濃度0.05〜10μg/mLとなるように添加し、さらに標的細胞(1.0×104個)及びエフェクター細胞(5×105個)を添加する。37℃で4時間反応させ、遠心分離後、上清に放出された51Crをγ−カウンターにて測定する。ADCC活性は、以下の式に基づいて求めることが出来る。
CDC性についても、Brunner K.T.らの方法(Brunner, K.T., et al., Immunology, 1968. 14:181-96)に従って測定することができる。例えば、標的細胞となる骨髄腫細胞を10%FCS添加のRPMI1640培地にて培養し、細胞数が0.5×104〜1.0×104個となるように調製する。これに適量のNa2 51CrO4を加え、37℃で1時間反応させ、細胞を51Crでラベル化し、洗浄したものを標的細胞とする。ウシ胎児血清を添加したRPMI1640培地に懸濁した被検抗体又はコントロールとなるアイソタイプ抗体を終濃度0.5〜50μg/mLとなるように96ウェルプレートに加え、次いで前記標的細胞と補体とを加えて1.5時間反応させる。反応液を遠心分離し、上清に放出された51Crをγ−カウンターにて測定する。CDC活性は、以下の式に基づいて求めることが出来る。
(1)CD98hc上のN型糖鎖の存在によって影響を受ける領域内にエピトープを有する抗体を産生するハイブリドーマを準備する。
(2)別途、上記他の抗原で免疫付与した動物から得られるB細胞などの抗体産生細胞を用いてハイブリドーマ作製する。
(3)これらの2種類のハイブリドーマ同士を細胞融合させてハイブリドーマ(二重特性抗体の場合は、「クワドローマ」ともいう)を作製する。
(4)(3)で作製したられたハイブリドーマの中から目的の多重特異性抗体を産生するハイブリドーマをスクリーニングする。
(a)CD98hc上のN型糖鎖の存在によって影響を受ける領域内にエピトープを有するF(Ab’)2の構造の抗体を作製する。
(b)他の抗原に特異的に結合するF(ab’)2の構造の抗体を別途作製する。
(c)(a)および(b)で得られた2種類のF(ab’)2の構造の抗体をDTTなどの還元剤を用いて処理し、更にどちらかの処理物をエルマン試薬で処理する。
(d)(c)で得られた処理後のF(ab’)2構造の抗体を混合して反応させる。
上記セクション2.に記載する抗体(以下、本書において「抗体A」と称する場合もある)をコードするポリヌクレオチドが提供される。「ポリヌクレオチド」とは、ヌクレオチド(例えば、リボヌクレオチド、又はデオキシリボヌクレオチド)が直鎖状に重合した高分子化合物である。ポリヌクレオチドは、一本鎖であっても二本鎖であってもよい。
上述する抗体Aをコードするポリヌクレオチドを含む宿主細胞が提供される。宿主細胞がポリヌクレオチドを含む態様は特に限定されない。例えば、宿主細胞は、ベクターの形態でポリヌクレオチドを有してもよく、宿主細胞内のゲノムDNAにインテグレイトされた形態でポリヌクレオチドを有してもよい。
ヒトCD98hcのN型糖鎖付着部位を含み、N型糖鎖修飾の阻害によって露出するエピトープを認識するキメラ抗原受容体(CAR)、ヒトCD98hc上のN型糖鎖の存在によって影響を受ける領域内にそのエピトープが存在するキメラ抗原受容体、ヒトCD98hcに結合する糖鎖を除去することによって、ヒトCD98hcに対する親和性が上昇する抗ヒトCD98hcキメラ抗原受容体、ヒトCD98hcの395番目〜397番目のアミノ酸残基をマウス由来CD98hcアミノ酸残基に置換した変異型CD98hcに対する親和性が、マウス由来CD98hcに対する親和性と同等である抗ヒトCD98hcキメラ抗原受容体、及びヒトCD98hcの400番目及び401番目のアミノ酸残基をマウス由来CD98hcアミノ酸残基に置換した変異型CD98hcに対する親和性が、マウス由来CD98hcに対する親和性と同等である抗ヒトCD98hcキメラ抗原受容体等が提供される。キメラ抗原受容体とは、一般的に、人工のT細胞受容体(TCR)様のタンパク質であり、T細胞の細胞膜上にて発現する(細胞外ドメインに相当する)抗原認識部位を所望の抗原認識部位に置換し、且つT細胞そのものが有する細胞障害活性などの機能を、より効果的に発揮できるように構築されたタンパク質として知られる。キメラ抗原受容体は、一般的に、抗体の軽鎖可変領域と重鎖可変領域とを直列に結合させた単鎖抗体(scFv)をN末端側に有し、T細胞受容体(TCR)ζ鎖をC末端側に持つ構造を有する。キメラ抗原受容体を発現するT細胞等はscFv領域で抗原を認識した後、その認識シグナルをζ鎖を通じてT細胞等の内部に伝達する。キメラ抗原受容体は、T細胞の活性化を増強するために、scFvとζ鎖の間に共刺激分子(例えば、CD28及び/又は4-1BB)を有することが好ましい。

上記抗体A、CAR−T細胞、又はCAR−NK細胞等を含む医薬組成物、並びに、上記抗体A、CRT−T細胞、又はCAR−NK細胞等を利用した疾患の治療又は予防方法が提供される。
腫瘍の治療用または予防用の医薬組成物の有効成分のスクリーニング方法が提供される。この方法は、化合物ライブラリー(候補物質群)からCD98hc上のN型糖鎖の存在によって影響を受ける領域に特異的に結合する物質を選別することを含む。尚、腫瘍の種類は、上記医薬組成物について説明した通りである。
腫瘍に罹患しているか否かを診断する方法が提供される。この方法は、被験者から採取したサンプルに上述の抗体を接触させることを含む。尚、腫瘍の種類は、上記医薬組成物について説明した通りである。
上記抗体を含む種々の目的に応じたキットが提供される。例えば、キットは、腫瘍の治療又は予防用キット、或いは癌に罹患しているか診断するためのキットであり得る。尚、腫瘍の種類は、上記医薬組成物について説明した通りである。
以下の試験において、細胞の選別のために用いたフローサイトメトリーは次の要領で行った。インフォームドコンセントを得た骨髄腫患者の腸骨から採取した骨髄単核球をACK液(150mM NH4Cl, 10mM KHCO3)に浮遊し、3分間、4℃で静置することにより、赤血球を除去した。2%胎児ウシ血清を添加したPBS(Phosphate-buffered saline)で洗浄したのち、非特異的な抗体の結合を防ぐため、10%ヒトAB型血清を含んだPBS中で、20分間、4℃でブロッキングを行った。その後、蛍光色素でラベルされた各抗体(下記参照)を加え、30分間、4℃で染色を行い、PBSで洗浄したのち、1μg/ml propidium iodide (PI)を含んだPBS中に浮遊し、フローサイトメトリー解析に供した。解析およびセルソーティングはFACS Aria セルソーター(Becton Dickinson Immunocytometry System社製)を用いて行った。細胞の染色には、次のモノクローナル抗体を適宜選択して使用した。

多発性骨髄腫に対する抗体治療においては、骨髄腫細胞には結合するが正常血液細胞には結合しない抗体を用いることが望ましい。そこで、以下の方法により多発性骨髄腫の治療の有用な抗体を同定した。
R8H283抗体が結合する抗原タンパクの同定は発現クローニング法によって行った。まず、R8H283抗体が結合することがわかっているRPMI8226細胞から、superscript choice system for cDNA synthesis (Invitrogen社)を用いてcDNAライブラリーを作製し、BstXIアダプター(Invitrogen社)を用いて、pMXsレトロウィルスベクター(東京大学医科学研究所 北村 俊雄先生より分与)に挿入した。このようにして作製されたcDNA libraryをplat-E細胞(北村 俊雄先生より分与)に導入することにより得たレトロウィルスをBaF3細胞に感染させることによりRPMI8226由来のcDNAライブラリーを発現するBaF3細胞を得た。次に、これらの細胞をR8H283抗体で染色し、陽性細胞をFACS-sortingで濃縮する操作を繰り返した。(図3)3回目のsorting後にはほとんどの細胞がR8H283に結合する細胞となった。そこで、それらの細胞が持つレトロウィルスのインサートを、PCRにて増幅した後、シークエンシングすることにより同定した結果、CD98hcが抗原タンパクであることが明らかになった。
次の手順でCrispa-Cas9システムを用いてCD98hc欠損U266骨髄腫細胞株を作製し、R8H283抗体の抗原タンパク質がCD98hcであることを確認した。まず、PX330(adgene社)ベクターにCD98hc特異的な標的配列の二本鎖DNA配列を挿入してベクターを作製した。それを薬剤選択のためのベクターであるlinear hygromycin-resistance genen expression vector (Clontech社)とともに、NucleofectorII (Lonza社)を用いてU266細胞に導入した。その後、Hygromycin添加培地中で増えてきたクローンについてCD98hcの発現をMEM-108抗体(抗CD98抗体、Biolegend社)を用いて染色してFACSにて解析することにより、CD98hc欠損細胞(U266CD98hcKO)を同定した。次にそのCD98hc欠損細胞をR8H283抗体を用いて染色し、FACSにて解析したところ、野生型のU266細胞にはR8H283抗体が結合するのに対して、CD98hc欠損株ではR8H283抗体の結合が完全に消失していた(図4)。これはR8H283はCD98hcタンパクのみに結合することを示すため、CD98hcタンパクがR8H283抗体の抗原であることが確認された。
市販されている抗CD98抗体(MEM-108, Biolegend社)及びR8H283抗体を用いて、健常人の末梢血および骨髄細胞における様々な細胞分画への結合を測定した。健常人由来の末梢血細胞からHES40をもちいて赤血球を除いた後、Fc receptor blocking reagent (Miltenyi社)を加えて非特異的な抗体の結合をblockingし、その後、R8H283抗体、MEM-108抗体、又はマウスIgG2aアイソタイプコントロールを加えて4℃で30分間インキュベートした。洗浄した後、二次抗体としてPE-conjugated 抗mouse IgG-PE を加えてさらに30分4℃でインキュベートした。洗浄の後、最後に、Cy7-APC-conjugated anti-CD19 antibody (Biolegend社製)、FITC-conjugated anti-CD14 antibody (Biolegend社製)、及びCy7-PE-conjugated anti-CD3 antibody (Biolegend社製)を用いて染色した。それらをフローサイトメトリーを用いて解析することにより各分画におけるR8H283抗体及びMEM-108抗体の結合を測定した(図5A)。また、末梢血1μlをEDTA-PBS100μlに加えたものを、同様にR8H283抗体あるいはMEM-108抗体を用いて染色し、最後にPacific blue-conjugated anti-CD235 antibody (BD paharmingen社)を用いて染色することにより、CD235陽性の赤血球への各抗体の有無も検討した(図5A)。
R8H283抗体の健常人末梢血に対する結合を、他の多発性骨髄腫の治療に有効であることが報告されている抗体それらの細胞に対する結合と比較した。一次抗体に抗CS1抗体(Biolegend社)、抗CD38抗体(Biolegend社)、及び抗BST2抗体(Biolgend社)を用いた以外は上記と同様の方法で染色し、FACS解析を行った(図9)。その結果、R8H283抗体は正常なT細胞には全く結合しないのに対して、抗CS1抗体は一部の正常なT細胞への結合が確認された。抗CD38抗体は、R8H283抗体と比較して全ての正常末梢血細胞に有意に結合することが確認された。抗BST2抗体は、R8H283抗体と比較して、正常なT細胞及び単球に有意に結合することが確認された。これらの結果はR8H283がより骨髄腫細胞を含むがん細胞に対する高い特異性を有する抗体であることを示している。
骨髄腫細胞株RPMI8226および骨髄性白血病細胞株U937に対するR8H283抗体およびMEM-108抗体の結合をFACSを用いて解析した。染色の方法は上記試験6の末梢血等の場合と同じである。その結果、MEM-108抗体はほぼ全ての細胞株に結合する一方、R8H283抗体は同じCD98hcタンパクを抗原とするにもかかわらず、結合する細胞株としない細胞株が存在することが確認された。たとえば、骨髄腫細胞株であるRPMI8226にはどちらの抗体もよく結合するが、骨髄性白血病細胞株であるU937にはMEM-108抗体はよく結合するが、R8H283抗体はほとんど結合しない(図10A)。様々な濃度のR8H283あるいはMEM-108抗体を用いて、RPMI8226細胞あるいはU937細胞を染色しFACSにて各抗体の結合を測定したところ、RPMI8226ではMEM-108抗体、及びR8H283抗体共に濃度依存的に抗体の結合量を表す蛍光強度が上昇するのに対して、U937細胞ではR8H283抗体の抗体濃度を50μg/mlまであげても、R8H283の結合はほとんど見られなかった。(図10B)。
R8H283が認識しているエピトープを同定するために、オーバーラッピングPCR法を用いて、各種のヒト/マウスキメラCD98hcタンパク発現ベクターを以下のように作製した。使用したプライマーの配列は以下の通りである。

R8H283モノクローナル抗体サブクラスの確認をIsotyping kit (Roche社)を用いて行い、IgG2aサブクラスであることを確認した。さらに、ハイブリドーマR8H283が産生する抗体分子の可変領域の塩基配列及びアミノ酸配列を決定した。配列決定の方法は、Smarter RACE cDNA amplification kit (Clontech)社を用いて行った。すなわち、R8H283抗体を産生するハイブリドーマ由来mRNAから作製したcDNAを鋳型にして、PCR反応によりH鎖、κ鎖可変部領域のcDNA断片を増幅し、その塩基配列を解読した。解読したH鎖及びL鎖(κ鎖)の可変領域及び相補性決定領域(CDR1〜3)のアミノ酸配列及び塩基配列を下記の表4及び5に示す。
R8H283抗体産生ハイブリドーマ培養上清からprotein G(GE healthcare社)を用いて精製したR8H283モノクローナル抗体についてin vitroでの細胞傷害活性の有無を調べた。試験は、クロミウム遊離法を用いて補体依存性細胞傷害(CDC)活性の有無を調べることにより行った。Baby rabbit complement (Cedarene)を補体として用いた。骨髄腫細胞としては、R8H283抗体が結合することがわかっているU266、OPM2、NCI-H929、及びRPMI8226を用いた。各骨髄腫細胞株を51Cr で2時間ラベルし、3回洗浄した。標識された細胞(1 x 104cells)を、96-well U-bottomed plates (1 x 104 cells) において、R8H283モノクローナル抗体又はアイソタイプコントロール及び25%のbaby rabbit complementを添加したRPMI1640培地+ウシ胎児血清160μL中で培養した。U266を用いた試験では、終濃度0.1mg/ml又は1mg/mlで各抗体を用いた。OPM2、NCI-H929、及びRPMI8226を用いた試験では終濃度10μg/mlで各抗体を用いた。37℃、5% CO2の条件にて90分間培養した後、上清に放出された51Crをカウントした。特異的細胞傷害活性を以下のように計算した。
上記11.において細胞傷害活性を有することが確認されたR8H283モノクローナル抗体を用いて、in vivoでの多発性骨髄腫に対する治療効果を調べた。SCIDマウスの皮下に骨髄腫細胞株OPM2細胞(1×107 個)を移植した。腫瘍体積が30mm3を超えた時点で、マウスを抗CD98hc抗体投与群とコントロールIgG投与群に分け、10 mg/kg のR8H283モノクローナル抗体又はコントロールIgGを週2回投与した。腫瘍体積の計測は週2回行い、体積は以下の近似値で表現した:長径×短径×高さ/2。移植した骨髄腫細胞株OPM2が形成する腫瘤の体積が30mm3を超えた時点をday0とし、その日からの腫瘍体積の変化を図17Aに示す。また、day21におけるコントロール(IgG投与)マウスおよびR8H283抗体投与マウスでの腫瘍の大きさを図17Bに示す。矢印は腫瘍の幅を示す。図17A, Bに示されるように、コントロールIgGを投与した場合は、骨髄腫細胞が指数関数的に増殖しているのに対し、細胞傷害活性を有するR8H283モノクローナル抗体(R8H283)を投与した場合は、骨髄腫細胞の増殖がほぼ完全に抑制された。次に、投与量を1mg/kgあるいは0.1mg/kgに減量して、全く同じ実験を行ったところ、1mg/kgの投与でも効果は十分であることが示された。(図17C)
より生理的環境下での骨髄腫細胞に対する効果を見るため、経静脈的に移入して骨髄内に生着した骨髄腫細胞に対する抗体投与の効果を検討した。2.4Gyの放射線照射を行ったNOD/SCIDγc-/-(NOG)マウスの静脈中に、luciferase発現ベクターを導入した骨髄腫細胞株U266細胞 5X106個を移植したのち、7日および10日後にR8H283抗体又はコントロールIgGを腹腔内投与した。その後、抗体の効果を検討するため、Day14にマウス骨髄におけるヒト骨髄腫細胞のキメリズムを解析した。コントロールIgG抗体投与群ではいずれも、骨髄中に明らかなCD138+U266細胞の生着が見られたのに対して、R8H283抗体投与群では、10mg/kgおよび1mg/kg投与群では骨髄腫細胞は消失していた。また、驚いたことに0.01mg/kgという非常な低投与量でも骨髄中のU266細胞数が有意に減少していた(図18A、B)。さらに、5mg/kgを投与した実験ではマウスをday14では解析せずにday40にIVISを用いて腫瘍をイメージングしたところ、コントロールIgG投与群では全身に腫瘍の広がりが観察されたが、R8H283投与群では腫瘍は検出されず治癒していると考えられた(図18C)。これらの実験結果から、R8H283モノクローナル抗体(R8H283)及びこれと同一のエピトープを認識する抗体はCD98hcを発現する骨髄腫細胞に対し、強力な細胞傷害活性を有することが明らかとなった。
骨髄腫細胞は多くの場合、骨髄内でのみ増殖する腫瘍であり、また骨髄微小環境から強い生存支持を受けていると考えられる。さらに生理的環境下での骨髄腫細胞に対する効果を見るため、骨髄内に直接投与することにより骨髄内に大量に生着させた骨髄腫細胞に対する抗体投与の効果を検討した。2.4Gyの放射線照射を行ったB6-Rag2-/-γc-/-sirpa(BRGS)マウスの脛骨骨髄中に、luciferase発現ベクターを導入した骨髄腫細胞株U266細胞 4X105個を移植したのち、7日後にIVISによりluciferaseを発現するU266細胞が生着していることを確認した。その後、day7,10, 14, 17にR8H283抗体、コントロールIgGを0.5mg/kg、又はBortezomibを1mg/kg腹腔内に投与した。抗体又はBortezomibの効果を見るためday21にIVISにてluciferaseの発現をイメージングしたところ、コントロールIgG群およびbortezomib投与群ではluciferaseを発現するU266細胞が検出されたが、R8H283抗体投与群では腫瘍は検出されなかった(図19)。これらの実験結果から、R8H283モノクローナル抗体(R8H283)及びこれと同一のエピトープを認識する抗体はCD98hcを発現する骨髄腫細胞に対し、それが骨髄内に存在する場合でも強力な細胞傷害活性を有することが明らかとなった。
R8H283抗体のエピトープについて、上記9.にて検討した結果を更に詳細に検討した。図20に示す6種類のヒト/マウスキメラCD98hcタンパク質の発現用ベクターを作製し、それぞれをリポフェクション法によりCHO細胞に導入し、48時間後にR8H283抗体の結合の有無をFACSにて解析した。

- 抗ヒトCD98hc抗体であって、ヒトCD98hcに結合する糖鎖を除去することによって、ヒトCD98hcに対する親和性が上昇する抗体。
- ヒトCD98hcの395番目〜397番目のアミノ酸残基をマウス由来CD98hcアミノ酸残基に置換した変異型CD98hcに対する親和性が、マウス由来CD98hcに対する親和性と同等である、請求項1に記載の抗体。
- ヒトCD98hcの400番目及び401番目のアミノ酸残基をマウス由来CD98hcアミノ酸残基に置換した変異型CD98hcに対する親和性が、マウス由来CD98hcに対する親和性と同等である、請求項1又は2に記載の抗体。
