Abstract

【発明の属する技術分野】本発明は、進行癌患者の化学
療法における抗癌剤の動脈内注入、栄養補給のための静
脈注入、鎮痛のための髄硬膜等への鎮痛剤の投与、糖尿
病患者へのインシュリン投与等の如く、患者の体内の特
定箇所に長期間継続して、定期的に薬液を注入するため
の、皮下埋め込み可能な薬液注入システムに関するもの
である。

【従来の技術】切除不能な悪性腫瘍に対しては、抗癌剤
投与による化学療法が行われているが、全身投与法では
副作用を生じるため、選択的投与法としてカテーテル先
端を腫瘍局所へ通じる動脈上流に留置し、端末を抗癌剤
注入口として体外に露出させたままにするか、或は抗癌
剤投与の都度カテーテルを再挿入していた。(動注療
法)また、高カロリー輸液の持続注入の場合にも、カテ
ーテル先端を中心静脈内に留置し、端末を輸液注入口と
して体外に露出させていた。(IVH) このためカテーテルを挿入した皮膚創部からの感染の防
止は懸案課題であるが、一方皮膚創部を常に清潔に保つ
ことは実生活上は極めて困難であり、かつ患者の自由な
行動や入浴等にも制約があり、社会復帰に大きな妨げと
なっていた。このような問題の解決手段として、体内に
長期間留置した状態で使用可能な皮下埋込型のカテーテ
ル組立体が開発されている。このカテーテル組立体とは
薬液を貯留するための内部空間と、この内部空間に連通
する円柱形の弾性体を圧縮装着した薬液注入口、および
コネクターが付設された薬液流出路を有するポート本体
と、薬液注入用のルーメンが形成されたカテーテルと
を、前記流出路にルーメンが連通するように前記コネク
ターにカテーテルを接続し、更にカテーテルの固定及び
キンク防止のための保護ロックをポート本体に嵌合させ
たものである。本皮下埋込型カテーテル組立体は、カテ
ーテルを目的の血管内等まで挿入し、薬液注入ポートを
皮下組織に固定した状態で留置される。そして、薬液を
注入する際は、皮膚の上から触診により薬液注入ポート
の薬液注入口を確認し、薬液注入口の弾性体を注射針に
て穿刺することにより薬液をポート本体内に送り込み、
カテーテルを介して目的の血管内等へ薬液を注入する。
しかし、これらの従来品の薬液注入ポートは特開平2−
29269号公報や特開平8−215319号公報に記
載されているように小型円筒形の保護ロックを具備し、
実在する薬液注入ポートにおける保護ロックは外径が接
続カテーテルの2〜3倍程度で全長が10mm程度と小型
なものであり、留置スペースを小さくする関係上一般的
なサイズであるが、術中における組立操作は血液等によ
り滑ることもあり非常に扱い難く、特に交換等の目的で
接続後に再度保護ロックを取り外す場合に顕著であっ
た。

図1、2の如く本発明の薬液注入ポートはポート本体
(1)と保護ロック(11)からなり、ポート本体
(1)は薬液を貯留するための内部空間(4)を有し、
この上部に弾性体(3)を装着した薬液注入口(2)を
有し、更に一半径方向に薬液流出路(5)が設けてあり
その先にカテーテル(21)を接続するためのコネクタ
ー(6)が付設されている。なお、薬液流出路(5)は
必要により薬液を流出させるが、逆流を防ぐための一方
弁を設けることも好ましい実施例の一つである。ポート
本体(1)の材質としては、生体適合性の優れているこ
とが必要で、例えば、ポリウレタン樹脂、ポリエーテル
スルフォン樹脂、シリコーンゴム等の樹脂や、ステンレ
ス、チタニウム等の金属、或いはこれらの組み合わせで
も何ら構わない。また、弾性体(3)の材質としては、
自己封止性を有する弾性状物質であり、例えばシリコー
ン、イソプレーン、ラテックス等のゴムが考えられるが
軟質の塩化ビニル樹脂やポリウレタン樹脂でも針刺し耐
久性および生体適合性に優れれば使用可能である。更に
コネクター(6)の材質としては、強度的にステンレス
やチタニウム等の金属が望ましいが限定されるものでは
ない。ポート本体(1)は以上の構成部材をインサート
成形や接着および溶着等により組み立てられる。次に保
護ロック(11)について説明すると、中央にカテーテ
ル(21)を挿入するための嵌合穴(13)があり、形
状は前記ポート本体(1)との嵌合時にはポート本体

1)とを結ぶ滑らかな曲線形状であることが好ましい。
また保護ロック(11)のポート本体(1)との嵌合面
には双方の位置合わせ(回転防止)のための突起(1
4)又は凹み(15)がそれぞれ嵌合するように設けら
れていることが望ましい。保護ロック(11)の材質と
しては前記ポート(1)と同様であるが、後端部のポー
ト本体(1)との嵌合部は硬質材料が望ましく、先端部
のカテーテルと接触する部分はカテーテルの動きに追随
しキンク等を防ぐ目的で弾性部(12)が設けられてい
ることが望ましい。次に本発明による薬液注入ポートの
使用方法について説明する。組立方法は図1に示すよう
に、予め保護ロック(11)に通したカテーテル(2
1)をポート本体(1)のコネクター(6)に挿入接続
し、更に保護ロック(11)をカテーテル(21)上を
スライドさせポート本体(1)に嵌合固定させる。ま
た、取り外す場合は図3に示すように保護ロック(1
