Abstract
所から離れて移動したり、その内部に裂傷を引き起こし
たりすることがあった。 【解決手段】 本発明は、第1および第2の端部と第1
および第2の端部間に配置された壁表面と、この壁表面
は複数の交差する細長い部材によって形成されること
と、カテーテルと関連して拡大可能で膨張可能な部分を
有しこの部分に拡大可能な管形状人工装具を取り付け保
持するための手段を含むカテーテルとを備えたものであ
り、これにより、カテーテルの拡大可能で膨張可能な部
分の膨張時に、この人工装具が身体通路と接触して半径
方向で外方向に形成される。丸みのついた線材フレーム
ループの形状をした関節接合は類似したステント本体同
士をつなぐものである。ステントにおける丸みのついた
開いたコネクタと短くしたセルはより大きな可撓性を考
慮したものである。
Description
管路内に使用するための拡大可能な管腔内ステントに関
するものであり、特に、病気により狭窄ないし閉塞した
血管を修復するためにとりわけ役立つ、改良型の可撓性
があって拡大可能な管腔内血管ステントと、拡大可能な
管腔内ステントを移植するための装置に関するものであ
る。
(stenting))は実験により証明され、従来の
血管手術に対して可能性のある代替手段を示している。
管腔内血管内部移植は、管状人工装具移植片を血管中に
経皮挿入することや血管組織内部の所望の位置にカテー
テルを介してこの移植片を送出することを必然的に含む
ものである。従来の血管手術を越えたこの方法の利点
は、外科手術上、欠陥のある血管を露出したり、切り目
を入れたり、除去したり、置換したり、バイパスしたり
する必要性をなくすことができることを含む。

と、拡大可能な感熱材料から製造され、らせん状に巻か
れた渦巻きばねと、ジグザグパターンのステンレススチ
ール線材から形成され、拡大するステンレススチールス
テントとを含むものである。一般に、前述の構造物は、
共通してある重大な不都合点を有する。この構造物をつ
ぶれた状態にある特定の定められた体内通路内の所望位
置に送出せねばならない限りにおいて、この体内通路を
通過させるための、各構造物の最終的な拡大した配置構
造におよぶ効果的な制御方法はない。例えば、特別な渦
巻きばねタイプの移植片即ち「ステント」の拡大性は、
渦巻きばね構造を製造するために利用される特別な材料
のばね定数と弾性係数によって決定される。これらの因
子はジグザグパターンのステンレススチール線材から形
成されているつぶれたステントの拡大量を予め決定する
ことになる。加熱時に拡大する感熱材料から形成される
管腔内移植片即ち人工装具の場合には、その拡大量は管
腔内移植片の製造に使われる特殊合金の熱拡大特性によ
って同様に予め決定される。
プの管腔内「ステント」が動脈ないし静脈内部のような
体内通路内部の所望の位置で一旦拡大すれば、当該ステ
ントの拡大規模を変更できない。もしも所望の体内通路
の直径が誤って計算されたならば、小さめのステントは
体内通路の内表面と接触して固着するには不十分にしか
拡大しないかもしれない。それゆえ、体内通路の所望の
位置から離れて移動していくおそれがある。
は、カテーテルを取り付けた血管形成バルーンの使用に
よる弾性のある血管の狭窄症ないし閉塞症の経皮バルー
ン拡張法がある。この手法においては、当該血管の壁の
構成部分を切り裂き破裂させて拡大した管腔を得るため
に、血管形成バルーンは狭窄血管ないし体内通路の内部
で膨張される。動脈アテローム性硬化病変に対しては、
相対的に圧搾できない局面は不変のままであるが、身体
通路のより弾性のある中央および外膜層はその局面の周
りでいっぱいに広がってしまう。この過程により、身体
通路壁層の分裂、即ち分解と断裂を生み、これにより内
膜即ち動脈や身体通路の内面は裂け目発生を被ってしま
う。この分裂が管腔を通じて血液の流れを減少させた
り、管腔を閉塞するおそれのある下部組織の「フラップ
(flap)」を形成する。一般に、身体通路内部に広
がる管内圧力は分裂層即ちフラップを適所に押さえ込む
ことができる。バルーン拡張手法により創り出された内
膜フラップが拡張した内膜に対して適所に維持されない
ならば、その内膜フラップは管腔中へ折り畳まれて管腔
の流れをせき止めたり、切り離されて身体通路に入り込
むおそれがある。この内膜フラップが身体通路の流れを
せき止める場合には、当面の外科手術にはこの問題を矯
正する必要がある。

が、内膜フラップが血管ないし体内通路を遮断するとい
けないので待機中の外科医がいつも必要となる。さら
に、内膜フラップが血管から離れて管腔を遮断する可能
性のために、バルーン拡張手法は、心臓に通ずるたとえ
ば左方主要冠状動脈などの一定の重大な身体通路には行
うことはできない。もしもバルーン拡張手法によって形
成された内膜フラップが突然下がってきて、たとえば左
方主要冠状動脈などの重大な身体通路の流れをせき止め
たならば、患者はどんあ外科手術手法であろうともそれ
が済むまえに死んでしまうであろう。
と関連したさらなる不都合点は、狭窄症病変の弾性反動
が原因で多くは失敗してしまうことである。これは、そ
の病変中の高度な線維コラーゲン性の内容物のせいで大
抵発生し、時には拡張されべき領域のある機械的特性の
せいで発生する。このように、身体通路は初めにバルー
ン拡張手法によってうまく広げることができるけれど
も、次に、先に拡張された身体通路の管腔の大きさを減
少させる身体通路壁の反動が原因で初期の再狭窄症が起
こることがある。例えば、拡大する力が腎性動脈それ自
身に対してよりもむしろ大動脈壁に対して加わるので、
腎性動脈のその入り口における狭窄症はバルーン拡張法
には制御しがたいことが知られている。
テル(fistulas)においてみられるものなどの
新内膜線維症によって生ずる血管狭窄は拡大することが
困難であることが証明されたが、これは高い拡大圧力と
より大きなバルーン直径が必要だからである。同様な困
難さは、移植動脈吻合狭窄症および動脈内膜切除後(p
ostendarterectomy)の回帰性狭窄症
の血管形成においても認められる。タカヤス動脈炎およ
び神経線維腫症動脈狭窄症の経皮血管形成では、これら
の病変の線維上の性質のせいと思われる貧弱な初期反応
と再発を示すおそれがある。
動物における身体管腔の開通性を維持するための管腔内
ステント移植に関するものであり、特に、血管中に使用
するための管腔内ステント移植に関するものである。本
発明は一般に形状が円柱状でバルーン拡大可能ステント
の改良からなるものであり、円筒状の本体の縦軸に沿っ
てこれに柔軟性を与えるように接合された複数の金属構
成要素を備え、これにより当該ステントは曲がった身体
管腔に整合できるものである。このようなステントの一
つは、独立したループが互いに移動できる巻線構造の線
材ループで作られた金属構成要素を有する。この構造を
したステントは体内管腔中に拡大される場合、その巻き
具合は身体管腔の曲線にそったものとすることができ
る。本文中に参考文献で取り入れ、パルマツ(Palm
az)に付与された米国特許4,733,665号と同
4,776,337号に開示されたステントは、ステン
ト同士を接合して使用する関節接合の特色をよく示すも
のである。

ってつながれた独立ステント切片でできた金属構成要素
を備え、このステント切片が曲がった身体管腔に適用で
きることによって改良されたものである。このようなス
テントはそのデザインに適した手段によって身体管腔中
に配置することができる。例えば、ヴィクタタイプのス
テントの場合には、バルーンカテーテルの拡大可能な構
成要素に対して適合させバルーンによって拡大しこのス
テントを身体管腔に押し付けて接触させることができ
る。あるいは、例えば、パルマツとパルパツ−シャッツ
ステント(Palmaz and Palmaz−Sc
hatz)(ジョンソンアンドジョンソンインターベン
ショナルシステム)の場合には、身体管腔を通して送出
されるようにステントをしっかり把持しそれからステン
トを解放しこれが身体管腔と接触して拡大できるカテー
テルにこれを取り付けることができる。この配置展開
は、ステントが経皮的に差し込まれ、経管腔的に(tr
ansluminally)移送され、カテーテルによ
って所望の位置に配置することにより達成される。
や狭窄症の局面の一部が血管内の血液の流れを遮断ない
し閉塞する場所で見いだされる。血管の拡張では通常血
管の閉塞を矯正することが受け持たれ、即ち、バルーン
カテーテルはPTCA手法にて利用され血管の閉塞箇所
を拡大するものである。しかしながら、この閉塞の拡張
では、拡張された血管の再閉鎖のおそれがあるフラッ
プ、裂溝、分裂を形成し、あるいは血管壁に穿孔さえも
作ることがある。金属ステントの移植によりこのような
フラップと分裂に対する支持物を付与することができ、
それによって血管の再閉鎖を防止したり、矯正外科手術
が行われるまで穿孔した血管壁に対してつぎはぎ修理を
与えることができる。その上、このような良好な縦方向
の可撓性を有した金属ステントは曲面と凹凸を有する血
管に対して容易に整合することができる。しかしなが
ら、このようなステントは、金属構成要素間の間隔の取
り方のせいで、穿孔した血管を効果的につぎ当てをする
能力を制限している。同様に、このような金属ステント
はたとえば抗狭窄症剤、抗凝固剤などの薬剤を運搬し送
出する能力もまた制限を受けることになる。
ステントの開発に先だって、以下のような拡大可能な管
腔内血管ステントと、身体通路の管腔を拡大するための
方法および装置は存在しておらず、それは身体管腔にお
ける狭窄症の再発を防止し、たとえばある患者の心臓の
左方主要冠状動脈などの重大な身体管腔に対して柔軟に
利用でき、身体通路壁の反動を防止し、管腔内移植片を
多様な大きさに拡張することができ、これにより移植片
が所望の場所から離れて移動してしまうのを防止すると
ともに拡張した移植片によって身体通路が裂傷するのを
防止することができるものである。それ故に、当該技術
は、以下のような拡大可能な管腔内血管移植片と、身体
通路の管腔を拡大するための方法および装置を追求し、
それは身体通路における狭窄症の再発を防止し、たとえ
ば心臓の左方主要冠状動脈などの重大な身体通路に役立
てることができるものと思われ、身体通路の反動を防止
し、身体通路の内部で多様な大きさに拡張し所望の場所
から離れて移植片が移動するのを防止するとともに拡張
された移植片による身体通路の裂傷を防止することがで
きる。
血管ステントを介して上述の利点は達成された。本発明
は、第1および第2の端部を有した管形状部材と第1お
よび第2の端部の間に配置された壁表面とを含み、この
壁表面は複数の交差した細長い部材によって形成され、
少なくとも管形状部材の第1および第2の端部の中間に
あって少なくとも細長い部材のいくつかは互いに交差し
ており、管形状部材は管腔を有する身体通路中へ管腔内
送出するのを許す第1の直径を有しており、管形状部材
は拡大した第2の直径を有しており、管形状部材の内部
から半径方向、外方向に拡大する力を加えた時に、この
第2の直径は可変であり管形状部材に加えられる力量し
だいであることと、これにより管形状部材は拡大され身
体通路の管腔を広げることができる。

る。本発明は、第1および第2の端部を有した拡大可能
な管形状人工装具と、第1および第2の端部の間に配置
された壁表面とを含み、壁表面は複数の交差した細長い
部材によって形成され、カテーテルと組み合わせて用い
ることができ、これと関連した拡大可能で膨張可能な部
分を有し、この拡大可能で膨張可能な部分に拡大可能な

により、カテーテルの拡大可能で膨張可能な部分が膨張
する際に、この人工装具は身体通路と接触して半径方
向、外方向に形成される。丸みのある線材フレームルー
プないしステントの縦軸に略垂直である半分離れた正弦
曲線状のコネクタ形式の関節接合により同様なステント
本体同士をつなげるものである。同様に、ステントの丸
みのあるコネクタと短くしたセルはより大きな可撓性を
考慮したものである。
身体通路の管腔を拡大する方法、および身体通路を管腔
内的に補強する装置は、以前に提案された従来技術の管
腔内ステントと比較すると、これを移植する方法および
バルーン拡張技術は以下の利点を有しており、狭窄症の
再発を防止し、たとえば心臓の左方主要冠状動脈の重大
な身体通路におけるステントの移植を可能にすると思わ
れ、身体通路の反動を防止するとともに移植片の拡大を
可能にし、これにより身体通路内部の状態に依存すると
ともにステントの関節接合の位置に依存して移植片を多
様な大きさにするものである。
拡大可能な人工装具であるステントが例示されている。
本願の方法、装置および構造は、血管即ち身体通路の部
分的に閉塞した区域を広げるための拡大可能な管腔内血
管移植に関連して利用できるだけでなく、多くの他のタ
イプの身体通路に対して拡大可能な人工装具として多く
の他の目的に対してもまた利用できる限りにおいて、
「ステント」と「人工装具」は本願記載ではある程度ま
で交換可能に使用されることが理解されよう。例えば、
拡大可能な人工装具は、以下のような目的に対してもま
た使用できる。 (1)経管腔内的(transluminal)再疎通
によって開かれた遮断動脈内部への支持移植片の配置。 (2)手術ができない癌によって閉塞された縦隔および
他の静脈を通してカテーテル通路を続ける類似の使用。 (3)門脈圧亢進症を患っている患者の門脈静脈と肝静
脈の間に創り出された肝臓内連絡カテーテルの増強。 (4)食道、腸、尿管、尿道の狭い所の支持移植片の配
置。 (5)再開および以前に閉塞された胆管の支持移植片の
増強。 したがって、「人工装具」という用語の使用は、各種タ
イプの身体通路内部への前述の使用を網羅し、「管腔内
移植」という用語の使用は、身体通路の管腔を拡大する
ための使用を網羅する。さらに、この点については、用
語「身体通路」は、人間の血管組織内部のいかなる静
脈、動脈、血管と同様に、たとえば前に記載されたもの
などの人体内部の管路を網羅している。
腔内血管移植片(即ち人工装具)70は、一般に、第1
および第2端部72,73を有する管形状部材71と、
第1および第2端部72,73間に配置された壁表面7
4とから成ることが示されている。好ましくは、この壁
表面74は、複数の交差した細長い部材75,76によ
って形成され、これらは、例えば交差点77に示される
ように、管形状部材71の第1および第2端部72,7
3間に介在して、少なくともいくつかの細長い部材7
5,76は互いに交差している。以下丹念に述べると、
管形状部材71は第1の直径dを有しており、これによ
り当業者において既に公知の管腔を有する身体通路70
中への管形状部材71の管内送出を可能にする。管形状
部材71の内部からの半径方向、外方向に広がっていく
力を加えた場合には、以下丹念に述べると、管形状部材
71は拡大した第2の直径d’を有し、この第2の直径
d’は大きさが可変であり、管形状部材71に加えられ
る力量にしたがうものである。
長い部材75,76は、血管移植片即ち人工装具70が
接触する可能性のある人体および体液と共存できる適当
な材料ならいかなるものでもよい。細長い部材75,7
6はまた、管形状部材71が図1に示される配置構造か
ら拡大構造まで広げられることができ、さらに管形状部
材71はその拡大構造が拡大直径d’で保持するのに必
要な強度および弾性特性を備えた材料からできていなけ
ればならない。管形状部材71の製作に適した材料は、
銀、タンタル、ステンレススチール、金、チタン、ある
いは先に述べた必要な特性を備えた適当な塑性材料なら
いかなるものをも含むものである。好ましくは、細長い
部材75,76は316Lステンレススチールから製作
される。好ましくは、この細長い部材75,76は、円
筒状の断面形状を有する小径のステンレススチール線材
である。もちろん、細長い部材75,76はそれぞれ、
三角形、四角形、長方形、六角形等の他の断面形状を有
することができることは理解されよう。さらに、複数の
部材75,76は、たとえば交差点77などにおいて互
いに交差する場合には、互いにしっかりと固定されるの
が好ましい。細長い部材75,76は、いかなる従来方
法においても互いにしっかりと固定されることができ、
たとえば溶接、はんだづけ、適当なエポキシ樹脂を用い
たようなのりづけなどによっても固定できる。しかしな
がら、交差点77は連続した部片であることが好まし
い。細長い部材75,76を互いにしっかりと固定する
ことにより、管状部材71は半径方向のつぶれに対して
比較的高抵抗が与えられ、管形状部材71はその拡大径
d’を保持する能力を備えている。好ましくは、管形状
部材71は、十字模様の管状パターンで編まれた連続し
たステンレス線材でできていて、ワイヤメッシュ管とし
て一般に表現することができるものを形成する。
作する時に、初めに、図1に示される直径dをもつ配置
構造に製作することができる。あるいはまた、初めの直
径dよりも大きな直径で製作することができ、この製作
後に管形状部材71は図1に示される直径dをもつよう
に慎重につぶすことができる。管形状部材即ちワイヤメ
ッシュ管71をつぶしている最中に、隣接した細長い部
材75,76の重なりを避けされることを保証するよう
に注意が払われなければならない。
では曲線状のコネクタ77aは、戦略的に人工装具70
の外周辺りに配置される。これらのコネクタ77aは
「長方形」77,75,76,77の長さをを短くし、
これによりステント70の可撓性を高めている。これが
生ずるのは、ある開いた「長方形」77,75,76,
77aにみられる2つのセル長(2c)と比較すると、
スロットの長さに沿って1つのセル長(c)と同じくら
いに閉じた「長方形」77,75,76,77は小さく
できるからである。開いた「長方形」と閉じた「長方
形」との比率は、所望の可撓性しだいであり、1:1よ
りも大きい程度に低く、5:1程度に高くすることがで
きる。最も望ましくは、1つには約1.5から約3.0
までの可撓性比率を使用することができることが分かっ
ている。即ち、「開いた」セル長は、「閉じた」セル長
の1.5倍から3倍の間のどれかである。スロット
(c)それら自身は1.5cm以下で役立つことが分か
っている。その上、この新規な「長方形」は開いた側面
を有しており、いっそう大きな可撓性を創り出してい
る。ステント全長は、冠状動脈ステントに対する約3c
mから大動脈に対する約18cmまでに渡り、0.00

と開口部182を形成するのを助ける複数の曲線状のコ

は、一般にワイヤメッシュ管としても表現できることが

方法による身体通路180のカテーテル挿入法によって
身体通路180内部に挿入される。
移植片即ち人工装具170は身体通路180内部の所望
の位置に送出されが、しかるに一方、管腔内移植片17
0を介して身体通路180の管腔181を拡大すること
が望まれたり、人工装具170を移植することが望まれ
る。X線蛍光透視法および/または他の従来の技術はカ
テーテル183と移植片即ち人工装具170は身体通路
内部の所望の位置に送出されることを保証するために役
立てることができる。移植片即ち人工装具170は、そ
れからカテーテル183の拡大可能で膨張可能な部分1
84を広げることによって拡大され、これにより、移植
片即ち人工装具170は、半径方向、外方向に身体通路
160に押し付けられて接触される。その点について
は、カテーテル183の拡大可能で膨張可能部分は従来
の血管形成バルーン188であってもよい。人工装具即
ち移植片170の所望の拡張が達成された後には、血管
形成バルーン188は身体通路180から通常の方法で
除去してもよい。尚、必要に応じてカテーテル183は
身体通路180から通常の方法で除去してもよい。尚、
必要に応じて移植片即ち人工装具170を有するカテー
テル183は初期に通常のテフロン(商標名)シース1
89のケースに入れられ、このシース189は人工装具
即ち移植片170の拡大よりも前にこれから引き抜かれ
るようにしてもよい。
ヤメッシュ管即ち管形状部材171は、先に述べたよう
に身体通路180中に挿入することができるように、図
1と関連して述べたような、つぶれた所定の第1の直径
dを有していることが気づかれよう。先に述べたような
目的のために身体通路180内部に人工装具170を移
植することが望ましい場合には、このワイヤメッシュ管
即ち人工装具170は第2の直径d’まで拡大され、拡
大された第2の直径d’は可変であり身体通路180の
内径によって決定される。したがって、血管形成バルー
ン188が膨らむときに、この拡大した人工装具170
は身体通路180内部の所望の位置から移動できなかっ
たり、人工装具170の拡大により身体通路170の破
裂を引き起こすことはない。

ン188による管腔内血管移植片170の拡大は、狭窄
領域は制御した拡張が与えられると同時に血管移植片1
70の拡張も制御されたものとなり、これにより、血管

な管腔内血管移植片170の拡張された直径d’の大き
さをつぶし減少させることを防止し、これは可変であり
身体通路180の所望の拡張された内径によって決定さ
れる。このようにして、拡大可能な管腔内移植片170
は血管形成バルーン188のしぼみ時には、身体通路1
80内部に所望の位置から離れて移動したりはせず、管
腔内移植片170の拡大により身体通路180の破裂を
引き起こすことはない。

のそのままの内皮つぎはきは、実験的研究によって示さ
れるように、急激な多中心の内皮化パターンをもたらす
ことになると思われる。

トによれば、身体通路における狭窄症の再発を防止し、
身体通路の反動を防止しその内部で多様な大きさに拡大
することができるので、身体通路の所望の場所から離れ
てステントが移動していくのを防止するとともに拡張し
たステントによる身体通路の裂傷を防止することができ
る効果がある。

Claims (5)
- 【請求項1】 複数のスロットが円筒形の部材の外周を
取り巻いて間隔を置いて配置され、前記円筒形の部材を
取り巻いたメッシュ状の配置構造を形成し、前記スロッ
トは長さが可変であることを特徴とする全体にスロット
が付いた管からなる管状のステント。 - 【請求項2】 縦軸を有するとともに円筒形の部材を取
り巻いて周囲に配置された複数の支柱を含む、全体に管
状のステントであって、前記支柱のうち隣接するものす
べてよりも少ない数を除いた所定の数の支柱のみが前記
円筒形の部材の縦軸に略垂直な角度で配置されている接
合部材によりつながれその中にスロットを形成し、前記
支柱により形成された一定のスロットは閉じていないこ
とを特徴とする全体に管状のステント。 - 【請求項3】 複数の周辺に配置された部材によってつ
ながれた複数の細長い支柱を有する円筒形のステントで
あって、前記周辺に配置された部材は全体に前記支柱付
近に長方形の形状をした配置構造を形成し、前記長方形
の形状をした配置構造のすべてよりも少ない数を除いた
ある所定の数の前記配置構造は、前記長方形の形状をし
た配置構造のうちの残余がその中に開口部を有すること
を特徴とする円筒形のステント。 - 【請求項4】 複数の開いたセルと複数の閉じたセルと
を含んでいるステントであって、前記開いたセルは接合
部材によって第1の端部でつながっているとともに第2
の端部ではつながっていない隣接した細長いコネクタに
よって形成され、閉じたセルは第2の細長い接合部材に
隣接した細長い接合部材によって形成され、前記第1お
よび第2の細長い接合部材は第1および第2の接合部材
によってつながれていることを特徴とするステント。 - 【請求項5】 接合部材によってつながれた1対のステ
ントであって、前記接合部材は渦巻き線を形成してお
り、この渦巻き線は半径方向と円周方向のいずれの方向
にも拡張できることを特徴とするステント。
