高リポプロテイン血症、高トリグリセリド血症、高脂血症又は高コレステロール
血症の治療又は予防のための、又は抗血液凝固処置のための医薬組成物の製造の
ための3,4−ジフェニル・クロマンの使用
本発明の分野
本発明は、高リポプロテイン血症、高トリグリセリド血症、高脂血症又は高コ
レステロール血症を患う患者の治療及びその予防のための、一般式(I)の化合
物の使用に関する。本発明は、さらに、アテローム硬化症を含む動脈硬化症を患
う患者の治療及びその予防のための一般式(I)の化合物の使用、そしてさらに
、例えば、冠状血栓症又は術後、すなわち、外科手術後の抗血液凝固処置又は予
防の必要な患者の治療のための一般式(I)の化合物の使用に関する。本発明は
、上記化合物を含む医薬組成物並びに上記化合物及びそれらの医薬組成物の使用
方法をも包含する。
本発明の背景
虚血性心臓疾患(Ischemic heart disease(IHD))、閉経後女性における関連
心臓血管疾患は、1次的には、アテローム硬化症により引き起こされる(Havel a
nd Raport,N Eng J Med 1995;332:1491-1498)。アテローム硬化症の他の頻繁
にみられるものは、脳血管疾患、及び間欠性跛行(intermittent claudication)
である。アテローム硬化症の発達のための重要な危険因子は、アテローム形成性
脂質特性、すなわち、高脂血症が、増加した LDL−コレステロール及び比較的減
少された HDL−コレステロールを伴うことである(
Samsioe G.Int J Fertil,1993;38,suppl.1:19-23)。閉経後女性における
エストロゲン治療が、冠状動脈の狭窄を減少させ、それにより非処置集団に比較
して生存率が増加することが示されている。冠状系に対する上記効果における重
要な因子は、血清脂質における減少及び LDL−コレステロールと HDL−コレステ
ロールの間の関係の正常化である(Samsioe G.Int J Fertil 1994;39 suppl.1
:43-49)。
本発明の1の目的は、高リポプロテイン血症、高トリグリセリド血症、高脂血
症又は高コレステロール血症の治療又は予防において有効に使用されることがで
きる化合物を提供することである。
男性についてのデータは、 LDL−コレステロールにおける35%の減少が、心臓
血管疾患、すなわち、冠状アテローム硬化症において50%の減少を達成するため
に必要であることを示唆している。但し、 LDL−コレステロール減少が、心臓保
護の全原因である場合に限る。しかしながら、エストロゲンは、5〜10%程 LDL
−コレステロールを減少させる。それ故、この脂質効果は、エストロゲン置換治
療後の冠状心疾患事件における減少のほんの25−40%の原因であると信じられて
いる。可能性のあるメカニズムは、血流を改善し、そして血漿脂質に対する効果
に依存しないアテローム形成のメカニズムを示す血管壁に対する直接的な効果で
あることができるであろう。
本発明の他の目的は、アテローム硬化症を含む動脈硬化症の治療又は予防にお
いて有効に使用されることができる化合物を提供することである。
血液凝固及び血栓症は、アテローム硬化症及びその合併症、例えば、血管閉塞
及び塞栓の病因に関係する重要なメカニズムである。さらに、血塊形成は、閉塞
した動脈の手術的介在後の血管再狭窄の
発達のために重要である。再狭窄は6ケ月後の患者の約35%において生じる。最
近の治療法、例えば、ヘパリン、低分子量ヘパリン及びアスピリン又はステント
は、再狭窄事件を減少させることに失敗している。内皮損傷後の血栓症への傾向
を抑制することができる新たな治療的可能性が、それ故に、必要とされる。
本発明のさらに他の目的は、例えば、冠状血栓後又は術後の、抗血液凝固処置
又は予防の必要な患者の治療又は予防において有効に使用されることができる化
合物を提供することである。
最近の研究(PEPI試験、JAMA 273:199,1995のための執筆者ら)は、単独で
又は酢酸メドロキシ・プロゲステロン又は微粉化プロゲステロンと共に摂取され
た経口エストロゲンが、リポプロテイン及びフィブリノーゲン特性に対する改良
された効果を通じて心臓血管疾患の発達の危険に対する有益な効果と関係するこ
とを確認した。しかしながら、エストロゲンは、長い処置の後に子宮内膜及びた
ぶん胸組織領域内での悪性腫瘍の頻度を増加させることによりそれらの組織に対
する悪影響をもつことも知られている。
これ故、再生組織における有意な効果を導入せずに、高リポプロテイン血症、
高トリグリセリド血症、高脂血症又は高コレステロール血症、又はアテローム硬
化症を含む動脈硬化に対する、又は抗血液凝固剤としての、有益な効果をもつ新
規化合物の必要性が在る。
セントクロマンは、抗エストロゲン活性をもつことが知られている非ステロイ
ド化合物である。インドにおいては、経口避妊薬として使用されている(例えば
、Salman et al.,米国特許明細書第4,447,622号;Singh et al.,Acta Endoc rinal
(Copenh)126 (1992),444-450;Grubb,Curr Opin Obstet Gynecol
3(1991),491-495;Sankaran et al.,Contraception 9(1974),279-289;
インド特許明細書第129187号を参照のこと。)。セントクロ
マンは、進行した胸癌の治療のための抗癌剤としても研究されてきた(Misra et
al.,Int J Cancer 43(1989),781-783)。最近、ラセミ体としてのセント
クロマンがその血清濃度の有意な減少により示される、コレステロール低下医薬
としての効能をもつことが発見された(S.D.Bain et al.,J Min Bon Res
9(1994), S 394)。
米国特許第 5,280,040号は、3,4−ジアリールクロマン及びそれらの医薬と
して許容される塩を使用して骨損失を減少させるための方法及び医薬製剤につい
て記載している。
本発明の簡単な説明
驚ろくべきことに、請求項1に記載する一般式(I)の化合物が、高リポプロ
テイン血症、高トリグリセリド血症、高脂血症又は高コレステロール血症の治療
において使用されることができることが発見され、そしてさらに、驚ろくべきこ
とに、請求項2及び3に記載するような一般式(I)の化合物が、アテローム硬
化症を含む動脈硬化の治療又は予防において使用されることができ、そして請求
項4に記載するような一般式(I)の化合物が、例えば、冠状血栓後又は術後の
、抗血液凝固処置又は予防の必要な患者の治療又は予防において使用されること
ができることが発見された。
本発明の詳細な説明
本発明は、一部、代表的な3,4−ジアリールクロマン、セントクロマン(3
,4−トランス−2,2−ジメチル−3−フェニル−4−〔p−(ベータ−ピロ
リジノエトキシ)フェニル〕−7−メトキシクロマン)が、とりわけ、コレステ
ロール含有食餌により飼養されたウサギにおいて、高リポプロテイン血症、高ト
リグリセリド
血症、高脂血症又は高コレステロール血症化合物として有効であるという発見に
基づく。これらの動物モデルは、一般に、高リポプロテイン血症、高トリグリセ
リド血症、高脂血症又は高コレステロール血症の一般に認められたモデルである
。従って、これらのデータは、3,4−ジアリールクロマンが、霊長類、例えば
、ヒトを含む哺乳類において、治療的及び予防的高リポプロテイン血症、高トリ

示している。
さらに、本発明は、一部、代表的な、3,4−ジアリールクロマン、セントク
ロマン(3,4−トランス−2,2−ジメチル−3−フェニル−4−〔p−(ベ
ータ−ピロリジノエトキシ)フェニル〕−7−メトキシクロマン)が、とりわけ
、コレステロール含有食餌により飼養されたウサギにおいて、アテローム硬化症
を含む動脈硬化の直接的血管効果に対しても有効であるという発見に基づく。こ
れらの動物モデルは、アテローム硬化症を含む動脈硬化の一般に認められたモデ
ルである。従って、これらのデータは、3,4−ジアリールクロマンが、霊長類
、例えば、ヒトを含む哺乳類において、アテローム硬化症を含む動脈硬化に対す
る治療剤として有用であることを示している。
本発明はさらに、一部、代表的な3,4−ジアリールクロマン、セントクロマ
ン(3,4−トランス−2,2−ジメチル−3−フェニル−4−〔4−(2−(
ピロリジン−1−イル)エトキシ)フェニル〕−7−メトキシクロマン)が、と
りわけラットに投与されるとき、予防的又は治療的抗血液凝固活性をもつという
発見に基づいている。これらの動物モデルは、一般に、例えば、冠状血栓症後又
は術後に抗血液凝固の治療又は予防の必要な患者の一般に認められたモデルであ
る。従って、これらのデータは、式(I)の3,4−

本発明は、Lipid Research Clinics Program J.A.M.A.251 (1984),3
51-364及び J.A.M.A.251 (1984),365-374中に定義されたような疾患の又は当
業者が治療又は予防の対象として考えるものの治療又は予防に関する。
本発明においては、式(I)の化合物は、患者における、請求項1〜4に述べ
るような症状のために使用される。式(I)中、R1,R4とR5は、個々に、
水素、ハロゲン、トリフルオロメチル、低級アルキル、低級アルコキシ又は(第
3アミノ)(低級アルコキシ)であり;そしてR2とR3は、個々に、水素又は
低級アルキルである。本明細書中に使用するとき、用語“低級アルキル”は、1
〜6の炭素原子を含む直鎖及び分枝鎖のアルキル基、例えば、メチル、エチル、
n−プロピル、イソプロピル、n−ブチル、tert−ブチル、n−アミル、sec−
アミル、n−ヘキシル、2−エチルブチル、2,3−ジメチルブチル等を含む。
用語“低級アルコキシ”は、1〜6の炭素原子を含む直鎖及び分枝鎖のアルコキ
シ基、例えば、メトキシ、エトキシ、n−プロポキシ、イソプロポキシ、n−ブ
トキシ、tert−ブトキシ、n−アミルオキシ、sec−アミルオキシ、n−ヘキシ
ルオキシ、2−エチルブトキシ、2,3−ジメチルブトキシその他を含む。“ハ
ロゲン”は、クロロ、フルオロ、ブロモ及びヨードを含む。第3アミノ基は、N
,N−ジアルキルアミン、例えば、N,N−ジメチルアミノ、N,N−ジエチル
アミノ、N,N−ジピロピルアミノ及びN,N−ジブチルアミノ又はポリメチレ
ンイミン、例えば、ピペリジン、ピロリジン、N−メチルピペラジン又はモルフ
ォリンであることができる。本明細書中、用語“(第3
アミノ)(低級アルコキシ)”は、第3アミノ基により置換された低級アルコキシ
基である。好ましい化合物は、式中、R1が低級アルコキシであり;R2とR3が
低級アルキル、特にメチルであり;R4が水素であり;そしてR5がトリメチレン
イミン型の(第3アミノ)(低級アルコキシ)であるものを含む。特に好ましい態様
においては、R1は、その7位にあり、そして低級アルコキシ、特にメトキシで
あり;R2とR3の各々がメチルであり、R4が水素であり、そしてR5が4位にあ
り、そして(第3アミノ)(低級アルコキシ)基、例えば、2−(ピロリジン−
1−イル)エトキシである。本発明には、上述の式(I)の化合物の医薬として
許容される塩の全てが含まれる。
トランス配置の式(I)の化合物を使用することも好ましい。これらの化合物
は、ラセミ混合物として使用されることもでき、又は単離された立体異性体、例
えば、d−又はl−エナンチオマーが使用されてもよい。トランス−l−エナン
チオマーがより好ましい。
本発明における使用のために特に好ましい化合物は、請求項4に記載するよう
な式(IV)に表わされるセントクロマンである。
1のエナンチマーだけが示されるけれども、式(IV)は、その3−と4−フェ
ニル基のトランス配置を表すために本明細書中で使用され、そしてそのd−とl
−エナンチオマー、並びにラセミ混合物も含まれると理解されるべきである。
3,4−ジアリールクロマンは、知られた方法、例えば、Carney et al.に付
与された米国特許の明細書第 3,340,276号、Bolgerに付与された米国特許の明細
書第 3,822,287号、及びRay et al.,J Med Chem 19(1976),276-279(こ
れらの内容を、引用により本明細書中に取り込む。)中に開示されたものに従っ
て、調製される。有機金属塩基により触媒された転位によるシス異性体からトラ
ンス異性体への変換は、米国特許出願第 3,822,287号中に開示されている。光学
活性なd−とl−エナンチシマーは、所望のエナンチオマーを作り出すためにア
ルカリ加水分解に供される光学活性な酸塩を形成することにより(引用により本
明細書中に取り込む)米国特許第 4,447,622号中にSalman et al.により開示さ
れている。R2がR3と相違し、そしてR4がR5と相違する場合、一般式(I)は
、8つの光学異性体をカバーする。
本発明においては、3,4−ジアリールクロマンは、医薬として許容される塩
、特に、有機酸及び無機酸の塩を含む酸付加塩の形態で調製されることができる
。このような塩の例は、有機酸、例えば、ギ酸、酢酸、プロピオン酸、フマル酸
、グリコール酸、乳酸、ピルビン酸、シュウ酸、コハク酸、リンゴ酸、酒石酸、
クエン酸、安息香酸、サリチル酸等の塩を含む。好適な無機酸付加塩は、塩化水
素、臭化水素、硫酸及びリン酸その他の塩を含む。
酸付加塩は、化合物合成の直接生成物として得られることができる。あるいは
、その遊離の塩基は、適切な酸を含む好適な溶媒中で溶解され、そしてその塩は
、その溶媒を蒸発させることにより単離され又は他の方法でその塩と溶媒を分離
することができる。
3,4−ジアリールクロマン及びそれらの塩は、例えば、高リポプロテイン血
症、高トリグリセリド血症、高脂血症又は高コレステロール血症又はアテローム
硬化症を含む動脈硬化症を患い、又は例えば、冠状血栓症後又は術後の抗血液凝
固治療又は予防の必要な患者の治療又は予防において、ヒト及び獣医薬において
、有用である。本発明における使用のために、3,4−ジアリールクロマン及び
それらの医薬として許容される塩は、常法により非経口、経口、経鼻、経直腸、
皮下又は皮膚内又は経皮投与のための医薬を提供するために、医薬として許容さ
れる担体と配合される。配合品は、さら
に、1以上の希釈剤、増量剤、乳化剤、保存剤、バッファー、賦形剤、等を含む
ことができ、そして液体、粉末、エマルジョン、座剤、リポソーム、経皮パッチ
、徐放性剤、皮膚インプラント、錠剤、等の如き形態で提供されることができる
。当業者は、適切なやり方で、そして許容された手順、例えば、Remington's Ph armaceutical Sciences

990 中に開示されたものに従って、上記化合物を配合することができる。
経口投与が好ましい。従って、活性な化合物は、経口投与のために好適な形態
、例えば、錠剤又はカプセルの形態で調製される。典型的には、本化合物の医薬
として許容される塩は、担体と組み合され、そして錠剤に成型される。この点で
好適な担体は、デンプン、糖類、リン酸2カルシウム、ステアリン酸カルシウム
、ステアリン酸マグネシウム、その他を含む。このような組成物は、さらに、1
以上の補助的物質、例えば、水和剤、乳化剤、保存剤、安定剤、着色剤、等を含
むことができる。
式(I)の化合物を含む医薬組成物は、1日又は1週間当り1回以上投与され
ることができる。このような医薬組成物の有効量は、高リポプロテイン血症、高
トリグリセリド血症、高脂血症又は高コレステロール血症剤として臨床的に有意
な効果を又はアテローム硬化症を含む動脈硬化に対して臨床的に有意な効果を提
供し、又は例えば、冠状血栓症後又は術後に抗血液凝固治療又は予防の必要な患
者に臨床的に有意な効果を提供する量である。このような量は、一部、治療され
るべきその患者の特定の症状、年齢、体重、及び一般健康状態及び当業者に明白
な他の要因に依存するであろう。
式(I)の化合物を含む医薬組成物は、1日又は1週間当り1回以上単位投与
形態において投与されることができる。あるいは、そ
れらは、皮膚移植のために好適な徐放性配合物として提供されることができる。
インプラントは、数年までであることができる所望の時間期間にわたり活性化合
物の放出を提供するように配合される。徐放性配合品は、例えば、Sanders et a
l.,J Pharm Sci 73(1964),1294-1297,1984;米国特許明細書第 4,489,
056号;及び米国特許明細書第 4,210,644号(これらを、引用により本明細書中
に取り込む。)により開示されている。
以下の実施例は限定のためではなく説明のために提供される。
好ましい式(I)の化合物の例は、ラセミ混合物としての、そしてl−セント
クロマン及びd−セントクロマンとしての、セントクロマンである。さらに、3
,4−トランス−2,2−ジメチル−3−フェニル−4−〔4−(2−(ピロリ
ジン−1−イル)エトキシ)フェニル〕−7−ヒドロキシクロマンが、好ましい
化合物である。より好ましい化合物は、トランス−l−セントクロマン(l−3
,4−トランス−2,2−ジメチル−3−フェニル−4−〔4−(2−(ピロリ
ジン−1−イル)エトキシ)フェニル〕−7−メトキシクロマン)である。
医薬として許容される酸付加塩の例は、非毒性の酸、無機酸、例えば、塩酸、
硫酸、及びリン酸、又は有機酸、例えば、ギ酸、酢酸、プロピオン酸、コハク酸
、フマル酸、グルコン酸、乳酸、クエン酸、アスコルビン酸、安息香酸、エンボ
ン酸(embonic acid)、メタンスルホン酸、及びマロン酸との塩である。
本発明を、さらに以下の実施例により説明する。しかしながら、保護範囲を限
定するものと解釈されるべきではない。以上の説明及び以下の実施例中に開示す
る特徴は、別個に又はそれらのいずれかの組合せにおいて、本発明の多様な形態
において本発明を具現化するためのものであることができる。実施例1
血漿コレステロールに対するl−セントクロマンの効果を、ラットとウサギの
両方において調べた。
第1の研究において、シャム手術又は卵巣切除(ovariectomy(OVX))を、56雌Sp
rague-Dawleyラットについて行い、そしてその動物を、以下の処置に割り当てた
(1群当り8ラット):1)シャム/媒体;2)OVX/媒体;3)OVX/エチニル
・エストラジオール 0.2mg/kg;4)OVX/l−セントクロマン1mg/kg;5)O
VX/l−セントクロマン5mg/kg;6)OVX/l−セントクロマン10mg/kg;7
)OVX/l−セントクロマン25mg/kg。上記投与量を、経口胃管栄養により5週
間にわたり1週間当り3回投与した。本実験の終わりに、コレステロール・レベ
ルの測定のために血清を採取した(米国特許第 5,407,955号)。
l−セントクロマンは、全ての投与量において血清コレステロールに対する顕
著な効果をもち、そしてシャム手術動物のレベルよりも低くコレステロール・レ
ベルを減少させた(表1)。
値は、平均±SDである。*は、エチニル・エストラジオールにより処置された
OVXラットに比較して血清コレステロールの有意な減少を示す;p<0.002。
血清コレステロールに対する明確な投与量−応答効果を示すために、l−セン
トクロマンを用いてさらなる研究を行った。
再び、シャム手術又は卵巣切除を56匹の雌ラットについて行い、そしてこれら
のラットを、以下の処置に割り当てた;1)シャム/媒体;2)OVX/媒体;3
)OVX/エチニル・エストラジオール 0.2mg/kg;4)OVX/l−セントクロマン
0.05mg/kg;5)OVX/l−セントクロマン0.1mg/kg;6)OVX/l−セントク
ロマン 0.5mg/kg;7)OVX/l−セントクロマン1mg/kg。上記投与量を、経
口胃管栄養により5週間にわたり1週間当り3回投与した。本実験の終わりにお
いて、血清を、コレステロール・レベルの測定のために採取した(米国特許第 5
,407,955号)。本実験においては、血清コレステロールは、投与量に依存して減
少された(表2)。
値は、平均±SDである。*は、シャム/媒体処置動物に比較して血清コレステ
ロールの有意な減少を示す;p<0.003。
血漿脂質に対するl−セントクロマンの効果を、コレステロール飼養ウサギに
おいても調べた。実験を開始する1週間前に、30匹の性的に成熟した New Zeala
nd Whiteウサギを、ペントバルビタール麻酔の間に卵巣切除した。回復後、皮下
処置を、3群において開始した:媒体、17−l−エストラジオール又はl−セン
トクロマン 7.5mg/kg。これらの投与量を、4週間にわたり1週間当り3回投与
した。この実験の終わりにおいて、血清コレステロールの分析のための血漿サン
プルを採取した。表3中に示すように、l−セントクロマンは、媒体処置動物及
びエストラゲン処置動物におけるレベルと比較して、血清コレステロールを有意
に減少させた。

大動脈壁内への脂質の蓄積に対するl−セントクロマンの効果、すなわち、ア
テローム形成過程における中心的メカニズムを、New
Zealand Whiteウサギにおいて調べた。実験を開始する1週間前に、45匹の性的
に成熟した New Zealand White雌ウサギを、ペントバルビタール麻酔の間に卵巣
切除した。さらに45匹の齢及び体重のマッチした雄ウサギの1群を、本実験に含
めた。回復後、経口処置を開始した。雄と雌を、3群(1群当り15動物)にお
いて処置した:1)偽薬処置動物;2)17−β−エストラジオール2.5mg/kg/
日;3)l−セントクロマン、2.5mg/kg/日。処置14日後、コレステロールを
、その食餌に添加した。2日毎に、血液サンプルを、各ウサギから採取し、そし
て全血清コレステロールを測定した(Holm P et al.Atherosclerosis 115:191
,1995)。この計測に基づき、食餌中のコレステロール濃度を、次の計測まで以
下の日数に基づき調節した。この方法で、血清コレステロールを、たとえこれら
の処置のいくつかが血漿コレステロール・レベルに対する効果をもつことが知ら
れていても、その処置期間を通して一定に保った。次に、上記血管壁に対する直
接的処置効果を、単離することができた。処置3ケ月後、動物を殺し、各動物か
らの大動脈を単離し、そしてその大動脈内膜内に蓄積したコレステロールを測定
した(Holm P et al Atherosclerosis,115:191,1995)。
これらの結果を表4に示す。雄と雌ウサギの両方におけるエストロゲンとl−
セントクロマンの両方が、上記脈管内脈コレステロール含量に対する有意な減少
効果をもつことが明確に分かる。上記血清コレステロールは、本実験を通じてこ
れが一定に保たれるので、これに依存するものではない。l−セントクロマンの
抗−アテローム硬化効果は、それ故、部分的に、直接的血管壁効果を原因とする
ことができる。

インビトロにおいては、ウサギの血小板に富む血漿中のl−セントクロマンが
、血小板活性化因子(platelet activating factor(PAF))により誘導された血
液凝集を阻害した。この効果についてのEC50は、3〜10μg/mlの間にあった。
