【発明の詳細な説明】
【０００１】
【発明の属する技術分野】本発明は、手の骨と指の骨と
の間の中手指節関節並びに指の骨間の指節間関節の切除
後に、装着され得る人工指関節に関する。
【０００２】
【従来の技術】指関節がリウマチに犯されて、患者に激
痛を与えるときは、通常、指関節を切除する。従来、所
謂スワンソン関節(Swanson-Gelenk)が自然の関節の代わ
りをしている。しかし、この関節は本物の関節でなく、
むしろ２つの縦長の錨形状物を有するシリコーン製の代
用物である。この錨形状物は管状骨に装着することがで
き、シリコーン製の基体と結合されている。シリコーン
に可撓性があるので、患者は治療された指を一定程度可
動にすることができる。しかし、スワンソン関節を使用
した際、この「関節」の回りに結合組織カプセルが形成
され、該結合組織カプセルは元の位置にあるスワンソン
関節の可動性及び耐久性に不都合であることが観察され
ている。

【発明が解決しようとする課題】こうした背景に対し
て、本発明は、自然の指関節の自然な生理的な動きを模
倣する点において高い正確度を達成する人工指関節を提
供することを目的とする。更に、出来るだけ高い受入れ
を達成するために、患者にとって出来る限り快適な着用
の条件を作らねばならない。

【課題を解決するための手段】前記課題を解決するた
め、指管状骨に装着可能な第１及び第２の中空の固定用
シャフトを有し、該固定用シャフトの間には玉継手の形
態をなす単軸ヒンジ継手が設けられ、第１の中空シャフ
トは単軸ヒンジ継手のボールケージと結合され、該ボー
ルケージには１本のシャフトを具備する球体が取り付け
られており、シャフトはボールケージ及びそこに設けら
れたスリットを通り、第２の中空シャフトと結合してお
り、ボールケージに形成されたスリットは、シャフトが
単軸ヒンジ継手の伸展位置でボールケージを貫通すると
きの地点から、シャフトが単軸ヒンジ継手の屈曲位置で
ボールケージを貫通するときの地点へと連続的に拡大し
てなる指関節を提供する。他の好都合な構成は従属の請
求項から生じる。
【０００５】従って、まず第１に、指関節は指管状骨又
は手管状骨に装着可能な第１及び第２の中空の固定用シ
ャフトすなわち中空シャフトを有し、その間には単軸ヒ
ンジ継手が設けられていることが提案される。前記固定
用シャフトは、骨小柱が成長するときに通り、かつかく
してシャフトを骨に永続的に固定させるのに用いる目の
荒いメッシュの網目格子状組織、好ましくは金属で形成
されるのは好都合である。
【０００６】第１のシャフトは単軸ヒンジ継手のボール
ケージと結合され、該ボールケージには１本のステムを
具備する継手の球体が枢支されている。従って、このス
テムはボールケージ及びそこに設けられたスリットを通
り、第２の中空シャフトと結合している。ボールケージ
に形成されたスリットは、ステムが単軸ヒンジ継手の伸
展位置でボールケージを貫通するときの地点から、ステ
ムが単軸ヒンジ継手の屈曲位置でボールケージを貫通す
るときの地点へと連続的に広がっている。
【０００７】従って、球継手は伸展位置から屈曲位置へ
の単軸ヒンジ継手の通常の屈曲運動を可能にするのみな
らず、継手の屈曲が増えるに連れて、屈曲平面に対し垂
直方向にある平面で遊びを増す。このことは自然の指関
節の運動経過にほぼ対応している。出来る限り正確に再
現された生理的な運動経過の重要性は、特に、継手を囲
繞する軟質部、例えば結合組織及び腱が、自然の状態と
比較して装着後も実際に変わらない環境内で自らの機能
を果たすことができることにある。これによって、患者
の着用の快適性が高められて、これによって装着の受入
れが促進される。
【０００８】単軸ヒンジ継手と、骨に装着する２つの中
空のシャフトとは、好ましくは、長らく装着技術(Impla
ntattechnik)の分野で知られている錐形の締付結合によ
って結合される。
【０００９】好ましい実施の形態では、ボールケージ自
体がボールである。球体に取着されたシャフトが通過す
る前記スリットは、子午線のー部上で球体の極からほぼ
赤道まステムが球形のボールケージの壁部を貫通する。
ボールケージをボールとして形成することによってコン
パクトな装着物が生じる。該装着物によって、指関節の
装着された状態で腱等をボール表面に沿って問題なく案
内することができる。この結果、指の軟質部及び腱が慎
重に取り扱われ、従って装着物の耐久性が高まる。
【００１０】出来る限り高い耐久性を達成して、例えば
金属磨耗を防止するために、好都合な実施の形態では、
内側の球体が前記ステムと一体的に形成されており、ポ
リエチレン層を具備していることが定められている。球
体全体がポリエチレンで被覆されていることは好まし
い。更に、前記ステムの、球体に形成された突出領域
は、ステムがボールケージの壁部を貫通する領域も同様
にポリエチレンで被覆されていることは好ましい。従っ
て、装着物が金属で製造されているときは、金属同士の
接触が効果的に防止される。
【００１１】注意深くしていても、万一修繕の処置を行
なって、球体を交換することが必要になる場合には、好
都合な実施の形態では、少なくとも球体の寸法の極冠が
具備されており、該極冠は覆いとしてボールケージに螺
着可能であり、該ボールケージの球形の外形を補完して
いる。かくして、非常にコンパクトなユニットは極冠を
球体から捩じって外すことによって分割され、続いて球
体をそれに取着されたステムと共に除去することができ
る。容易な方法で、球体及びステムからなる新部品をボ
ールケージに組み込み、極冠を再度閉じることができ
る。
【００１２】本発明の指関節の作動法を更に高めること
ができる。しかし、このために予め注意すべきことは、
自然の指関節においても、上記の遊びの増大を伴う屈曲
運動のみが必要という訳ではないことである。長手方向
の伸展も行なわれる。かくして、自然の関節が屈曲する
間、自然の腱は指が伸展位置にある場合よりも大きな応
力下に置かれている。長さの変化を補償するために、好
都合な実施の形態では、指関節の球体に取り付けられス
テムが、前記第２の中空シャフトと結合される摺動スリ
ーブへ所謂入れ子式に延びていて、ステムがスリーブ内
で縦方向に移動できるようにしてあることが定められて
いる。このことによって、ステムは長さの変化の補償を
すべく摺動スリーブ内で移動可能であり、補償運動を引
き起こすことができる。
【００１３】摺動スリーブがポリエチレンからなること
は好ましい。摺動スリーブは第２の中空シャフトと錐形
の締付結合部材によって永続的に結合され得る。
【００１４】摺動スリーブはボールケージに向いた自ら
の端部にカラーを有し、該カラーは継手の組み合わされ
た状態ではボールケージの外面に接触している。これに
よって、スリットの少なくとも部分的な被覆はボールケ
ージの壁部によって達成される。勿論これが最善に達成
されるのは、ボールケージ自体も外側がボールである場
合である。そのとき、摺動スリーブのカラーは部分球欠
を有し、スリットを実際に完全に覆うことができる。従
って、ボールケージの内部は継手の良好な支持性を長期
に亘って損なうであろう生理的液体から著しく離れてい
る。

【実施の形態】以下、図面を参照して本発明の実施の形
態を詳述する。本発明の指関節１の中心部分は単軸ヒン
ジ継手４のボールケージ５である。この場合、ボールケ
ージ５自体は球形に形成されており、極冠１０によって
補完されている。この目的のために、極冠１０に形成さ
れたねじ１６を、ボールケージ５の雌ねじと螺着するこ
とができる。
【００１６】極冠１０からは半径方向にテーパが形成さ
れた錐体１５が突出しており、該テーパが形成された錐
体１５は、錐形の締付結合部材によって、第１の中空シ
ャフト２内に形成された錐形の締付スリーブ１４と永久
的に、もしく着脱可能に結合されている。この場合、中
空シャフト２は目の荒いメッシュの網目格子状組織から
なる。装着(Implantation)後にシャフト２の周りの海綿
質の骨小柱は網目格子状組織を通って成長することがで
きて、装着物(Implantat) は永続的に固定される。

からその赤道Ｍの領域へガイドスリット８によって貫か
れている。該ガイドスリット８は、図１の（Ａ）から明

領域から赤道の領域Ｍに向かって連続的に広くなってい
る。これによって、指関節の屈曲位置においては、指関
節を伸展した場合（遊びが実際にゼロに近い）よりも大
きな遊びが、図１（Ａ）の矢印方向で、継手の球体（以
下、単に球体と言う）６に取着された（図１（Ｂ））ス
テム７に与えられる。このことは自然の指関節の生理的

【００１８】この場合には、球体６はポリエチレン層９
で被覆されている。該ポリエチレン層９は、突出するス
テム７の下方領域をも、スリット８の領域において（金
属製の）ステム７と（金属製の）ボールケージ５との間
で何等接触が生じないようにして、被覆している。この
ことによって、金属磨耗(Metallose) は効果的に防止さ
れる。
【００１９】第２の中空シャフト３が設けられている。
該中空シャフト３は、この場合、第１の中空シャフト２
と全く同様に形成されており、従って目の荒いメッシュ
の網目格子状組織からなり、錐形の締付スリーブ１３を
有する。該錐形の締付スリーブ１３と錐形の締付結合を
行なうことができる摺動スリーブ１１は、この目的のた
めに、テーパ状錐体の外郭を有する。しかし、摺動スリ
ーブ１１の内部は円筒状に形成されており、ステム７が
摺動スリーブ１１内で縦方向移動可能に取り付けられて
いる。これによって、摺動スリーブ１１内でのステム７
の所謂入れ子式の運動は指関節の装着後にも図１（Ｂ）
の矢印の方向で可能である。このことによって、縦方向
補償運動を実行することができる。
【００２０】この場合、摺動スリーブ１１は単軸ヒンジ
継手４の球状に形成されたボールケージ５に向いた自ら
の端部にカラー１２を有する。該カラー１２は指関節１
が組み合わされた状態（図２）ではボールケージ５の外
面に接触している。ここでは、カラー１２は球欠のー部
として見做され得る。カラー１２はステム７が貫通する
スリット８を少なくとも部分的に被覆して、血液等のよ
うな生理的液体がボールケージ５の内部に浸入するのを
防ぐために用いられる。カラー１２は具体的な実施の形
態では図面に示されたのより著しく大きいこともある。
【００２１】図示した実施の形態では、図２から見て取
れるように、９０度までの屈曲運動が可能である。しか
し、本発明の指関節はこの限度に定められない。スリッ
トの延長によって、例えば１００度の屈曲範囲も可能で
ある。
【００２２】図１（Ｂ）の部品の組立は、見る如くに、
ステム７を取着した球体６をボールケージ５へ挿入し、
それからステム７がスリット８を貫通することによって
なされる。その後、極冠１０を残余のボールケージ５に
螺着する。カラー１２がボールケージ５の外壁に接触す
るまで、摺動スリーブ１１をステム７に通す。次に、一
方では中空シャフト２内に形成された締付スリーブ１４
と、極冠１０に取着されたテーパ状錐体１５との間で、
他方では中空シャフト３内に形成された錐形の締付スリ
ーブ１３と、摺動スリーブ１１の外面に形成されたテー
パ状錐体との間で錐形の締付結合を形成する。
【００２３】上記実施の形態の広いの発明思想から逸脱
することなく、該実施の形態に対して変形を行なうこと
ができるだろうことは、通常の当業者によって理解され
るだろう。従って、本発明が開示された特定な実施の形
態に限定されず、請求項によって定義された本発明の精
神及び範囲の以内で本発明が変形の実施の形態を包含て
いることは理解される。

Claims (8)

    【特許請求の範囲】
  1. 【請求項１】 指管状骨に装着可能な第１及び第２の中
    空の固定用シャフト（２，３）を有し、該固定用シャフ
    ト（２，３）の間には玉継手の形態をなす単軸ヒンジ継
    手（４）が設けられ、前記第１の中空シャフト（２）は
    前記単軸ヒンジ継手（４）のボールケージ（５）と結合
    され、該ボールケージ（５）にはステム（７）を具備す
    る継手の球体（６）が取り付けられており、前記ステム
    （７）は前記ボールケージ（５）のスリット（８）を通
    り、前記第２の中空シャフト（３）と結合しており、前
    記ボールケージ（５）に形成された前記スリット（８）
    は、前記ステム（７）が前記単軸ヒンジ継手（４）の伸
    展位置で前記ボールケージ（５）を貫通するときの地点
    （Ｐ）から、前記ステム（７）が前記単軸ヒンジ継手
    （４）の屈曲位置で前記ボールケージ（５）を貫通する
    ときの地点（Ｍ）へと連続的に拡大してなる人工指関
    節。
  2. 【請求項２】 前記ボールケージ（５）自体は球体であ
    り、前記スリット（８）は子午線のー部上で極（Ｐ）か
    らほぼ赤道（Ｍ）まで球形の前記ボールケージ（５）の
    壁部を貫通してなる請求項１に記載の人工指関節。
  3. 【請求項３】 前記継手の球体（６）は前記ステム
    （７）と一体形成されており、ポリエチレン層（９）を
    具備してなる請求項１又は２に記載の人工指関節。
  4. 【請求項４】 少なくとも前記球体（６，９）の寸法の
    極冠（１０）が具備されており、該極冠（１０）は覆い
    として前記ボールケージ（５）に螺着可能であり、該ボ
    ールケージ（５）の球形の外形を補完してなる請求項２
    又は３に記載の人工指関節。
  5. 【請求項５】 前記継手の球体（６）に取着された前記
    ステム（７）は、前記第２の中空シャフト（３）と結合
    可能である摺動スリーブ（１１）に延びていて、前記ス
    テム（７）は縦方向補償運動を実行すべく前記摺動スリ
    ーブ（１１）内で縦方向移動可能であるようにしてなる
    請求項１乃至４のいずれか１に記載の人工指関節。
  6. 【請求項６】 前記摺動スリーブ（１１）はポリエチレ
    ンからなる請求項５に記載の人工指関節。
  7. 【請求項７】 前記摺動スリーブ（１１）は前記ボール
    ケージ（５）に向いた自らの端部にカラー（１２）を有
    し、該カラー（１２）は継手の組み合わされた状態では
    前記ボールケージ（５）の外面に接触してなる請求項５
    又は６に記載の人工指関節。
  8. 【請求項８】 前記カラー（１２）は球形の前記ボール
    ケージ（５）に対応して部分球欠状に形成されてなる請
    求項２又は７に記載の人工指関節。
