【目的】 骨折部の固定圧着にゆるみが生じない固定用
スクリュー装置を提供することである。 【構成】 一端部にらせん突条１１を有するラグスクリ
ュー１０と、このラグスクリュー１０の他端部が挿入さ
れたスリーブ２０より成り、前記ラグスクリュー１０の
カラー１２とワッシャー１５との間に圧縮コイルバネ１
７を取り付け、ラグスクリュー１０がスリーブ２０内に
引き込まれる方向に弾性を付与したのである。

【産業上の利用分野】この発明は、骨折部を固定するス
クリュー装置に関する。

【従来の技術】例えば大腿骨の大転子が骨折した場合、
図４に示すようなスクリュー装置が用いられる。この装
置は、ラグスクリュー１、支持アーム２、スリーブ３、
締付スクリュー４、セットビス５、固定スクリュー６か
ら成り、図５に示すように、大腿骨７に支持アーム２を
縦に挿入し、ラグスクリュー１を支持アーム２の斜孔２
ａに通して大転子７から骨折部９を介して骨頭８にねじ
込み、さらにスリーブ３をラグスクリュー１の後端部に
外挿し、セットビス５を支持アーム２の上端からねじ込
んでスリーブ３を固定する。そして締付スクリュー４を
ラグスクリュー１のねじ孔１ａ（図４）にねじ込み、ラ
グスクリュー１をスリーブ３内に引き込み、骨折部９を
密着させる。最後に固定スクリュー６をねじ込んでアー
ム２を固定する。
図４に示すようなスクリュー装置が用いられる。この装
置は、ラグスクリュー１、支持アーム２、スリーブ３、
締付スクリュー４、セットビス５、固定スクリュー６か
ら成り、図５に示すように、大腿骨７に支持アーム２を
縦に挿入し、ラグスクリュー１を支持アーム２の斜孔２
ａに通して大転子７から骨折部９を介して骨頭８にねじ
込み、さらにスリーブ３をラグスクリュー１の後端部に
外挿し、セットビス５を支持アーム２の上端からねじ込
んでスリーブ３を固定する。そして締付スクリュー４を
ラグスクリュー１のねじ孔１ａ（図４）にねじ込み、ラ
グスクリュー１をスリーブ３内に引き込み、骨折部９を
密着させる。最後に固定スクリュー６をねじ込んでアー
ム２を固定する。

【発明の課題】ところが、上記骨折部９をラグスクリュ
ー１と締付スクリュー４によって圧着させる際に、不正
確な固定や強すぎる固定などが生じると、骨折部周辺に
血行障害や圧迫による骨壊死が生じ、固定のゆるみの原
因となり、骨癒合遅延、骨癒合不全、偽関節などが生じ
る。

発明の固定装置は、一端部にらせん突条を有するラグス
クリューと、このラグスクリューの他端部が挿入された

向に弾性を付与する弾性部材を介在させたことを特徴と
する。

じない。

【実施例】以下、この発明の実施例を添付図面に基づい
て説明する。

装置は、ラグスクリュー１０とスリーブ２０より成り、
ラグスクリュー１０の先端部には、比較的高いらせん突
条１１が設けられ、後端にはカラー１２が設けられてお
り、このカラー１２の端面には、六角レンチやスクリュ
ードライバを係合できる六角孔やプラス孔などの凹部１
３が設けられている。また、ラグスクリュー１０のシャ
フト１４には、ワッシャー１５、１６が嵌め合され、こ
のワッシャー１５、１６間に圧縮コイルバネ１７が取り
付けられている。このコイルバネ１７に代えて、複数の
皿バネやゴムスリーブなどの弾性を有する部材を用いる
ことができる。
装置は、ラグスクリュー１０とスリーブ２０より成り、
ラグスクリュー１０の先端部には、比較的高いらせん突
条１１が設けられ、後端にはカラー１２が設けられてお

【０００９】前記スリーブ２０には、外周にらせん突条
２１が設けられており、内部の貫通孔２２は、前記ラグ
スクリュー１０の突条１１、ワッシャー１５、１６、カ
ラー１２を挿入できる径を有するが、先端には内側方向
のフランジ２３が設けられ、このフランジ２３の内径
は、らせん突条２１より大でワッシャー１５よりも小に
なっている。なお、スリーブ２０の後端にカラー２４を
設け、このカラー２４を例えば六角形にして工具でスリ
ーブ２０を回転し易くしておくことができる。

【００１０】図２は、前記ラグスクリュー１０をスリー
ブ２０に挿入した状態を示す。図示のように、ラグスク
リュー１０に嵌挿したワッシャー１５がスリーブ２０の
フランジ２３よりも大径になっているため、フランジ２
３がストッパーとなってラグスクリュー１０の右側方向
に抜け止めされる。

【００１１】図３に示すように、上記のようなスクリュ
ー装置を例えば大腿骨頸部３０の骨折部３１に使用する
場合には、まず骨折部３１にまたがって下穴を設け、ス
リーブ２０を固定した後、その中にラグスクリュー１０
を挿入し、回転させながららせん突条１１によって所定
位置まで進入させる。このとき同時にコイルバネ１７を
圧縮させる。これによって骨折部３１の右側の骨片は、
骨折部３１に圧着される。そのため、骨折部３１に圧壊
などが生じても骨折部３１がゆるむことがない。また、
ワッシャー１５、１６とスリーブ２０の貫通孔２２との
間、シャフト１４とワッシャー１５、１６との間、カラ
ー１２と貫通孔２２との間等のいずれか又は二者以上の
間に多少の間隙を設けておくと、ラグスクリュー１０が
スリーブ２０に対して半径方向のガタを有するため、骨
折部３１に部分的なゆるみの原因が生じてもコイルバネ
１７の弾性が作用して、部分的なゆるみも吸収すること
ができる。

【効果】この発明によれば、以上のように、ラグスクリ
ューをスリーブ方向に引き込む弾性部材を設けたので、
骨折部にまたがってラグスクリューを取り付けると、骨
折部は弾性的に圧着されるため、骨折部にゆるみが生じ
るような原因があっても、このゆるみを防止することが
できる。

Claims (2)
- 【請求項１】 一端部にらせん突条を有するラグスクリ
ューと、このラグスクリューの他端部が挿入されたスリ
ーブより成り、前記ラグスクリューとスリーブとの間
に、ラグスクリューをスリーブ内に引き込む方向の弾性
を付与する弾性部材を介在させた骨折部固定用スクリュ
ー装置。 - 【請求項２】 前記ラグスクリューが前記スリーブに挿
入・引き出し自在となっており、前記スリーブの他端部
には弾性部材が外嵌され、この弾性部材の一端はスリー
ブの一端に、弾性部材の他端はラグスクリューの他端に
限定されて、ラグスクリューをスリーブ内に引き込む方
向に弾性を付与するようになっていることを特徴とする
請求項１記載の骨折部固定用スクリュー装置。
