【０００１】
【産業上の利用分野】本発明は、股関節部全置換外科手
術において使用される補てつ学上の(prosthetic)寛骨臼
杯状くぼみに関し、そして非球状の空間をもつ補てつ学
上の寛骨臼杯状くぼみに特定の用途をもつ。
【０００２】
【従来の技術及び発明が解決しようとする課題】1950年
代までは、米国内における股関節部の補てつは、主に大
腿骨の内部補てつから成り、そして幾つか杯状くぼみが
壊死性大腿骨頭の関節形成表面置換物として断頭された
大腿骨頸の上に合うように設計されていた。ほとんどの
場合、寛骨臼は、股関節部の置換において非接触で置か
れていた。患った寛骨臼を置換する必要性は、内部補て

本発明の他の目的は、非球状の内部空間をもつ補てつ学
上の寛骨臼杯状くぼみを提供することである。本発明の
他の目的は、金属股関節部軸補てつ物とつがう(mating)
ための金属の寛骨臼杯状くぼみを提供することである。
本発明のさらに他の目的は、添付図面と共に行われる以
下の記載を読むことで明らかになるであろう。
【０００６】 図面の簡単な説明 図1 は、本発明に係る寛骨臼杯状くぼみ幾何の略図であ
る。図2 は、説明の目的のために示した大腿骨頭部の位
置をもつ寛骨臼杯状くぼみの組み立て物の略図である。
図3 は、本発明に係る寛骨臼杯状くぼみ組み立て物の断
面図である。図4 は、本発明に係る寛骨臼杯状くぼみ組
み立て物の第二態様の断面図である。図5 は、本発明に
係る寛骨臼杯状くぼみ組み立て物の第三態様の断面図で
ある。
【０００７】本明細書中に記載する好ましい態様は、本
発明を、網羅し、又は開示したまさにその形態に限定す
ることを意図されていない。むしろ、それらは、当業者
が彼らの技術を使用することができるように本発明を最
良に説明するために選ばれ、そして記載されている。
【０００８】図1 を参照すると、座標軸xyz が寛骨臼杯
状くぼみ(10)の球中心において交差する。杯状くぼみ(1
0)を、図示するが、本発明の限定された立体形状として
考えてはならない。点"A" は、Δy 及びΔz のある値に
よりyz平面内の第四象限内にある。弧(12)は、yz平面内
にあり、一末端"B" においてy 軸と、そしてその他の末
端"C" においてz 軸と交差する。図1 中に示すように、
弧(12)が軸z の回りを角度α回転するとき、それは、本
発明の関節形成表面の母面(generatrix)となる。その母

大腿骨頭部(14)及び寛骨臼杯状くぼみ(10)を説明してい

いて、半径は、杯状くぼみ及び大腿骨の球中心(16)を通
り、杯状くぼみ- 頭部の接点(18)において交差する。こ
の接点(18)は、本発明の立体態様においては、潤滑リザ
ーバーとして作用する上部空間(20)を定め、そして定位
に単離する線である。接線に向かい、そしてそれから離
れる杯状くぼみ(10)と頭部(14)の間の隙間(22)は、等角
度である。これは、2 つの表面間のより大きく又はより
小さく安定した潤滑フィルムの確立においての助けとな
る。弧(12)を作り出す杯状くぼみの半径及びその中心の
変位の両方は、選ばれた補てつ頭部の直径と、頭部(14)
と杯状くぼみ内部(11)との間の正接線接触(18)に向か
う、そしてそれから離れる選ばれた隙間角度との関数で

くぼみは、関節形成頭部と同じ直径をもつ円筒に等しい
無限円錐の形態である。
【００１０】先に記載した杯状くぼみ及び頭部の幾何
は、様々な、Co-Cr-Mo合金(ASTM F-75、ASTM F-90 、AS
TM F-799、Haynes Stellite 6B、Ultimet Alloy 、等)
、Titanium Alloys(ASTM F-136、Betaチタニウム合
金、等) 、Stainless Steels(ASTMF-138 、ASTM F131
4、等) 、Zirconium Base Alloys 、Tungsten Base All
oys、Tantalum及びTantalum Base Alloys、Niobium 及
びNiobium Base Alloys 、金属マトリックス・コンポジ
ット、セラミックス・マトリックス・コンポジット、ポ
リマー・マトリックス・コンポジット、並びにポリマー
により作られた。本明細書中で使用した金属は、その成
分の関節形成表面の磨耗性能を改善するために、好まし
くは、イオン注入、窒化、又は他の商業的に利用可能な
表面硬化法、例えば、表面仕上げ(surfacing) 、窒化チ
タン・コーティング、ダイアモンド様炭素コーティン
グ、酸化ジルコニウム・コーティング及び他のセラミッ
ク・コーティングにより表面硬化される。本杯状くぼみ
の外部は、好ましくは、骨の非成長適用又は機械的固定
のためにコートされたファイバー金属、ビーズ又はプラ
ズマ・スプレーであることができる細孔性コーティング
を含む。この細孔性コーティング(24)は、線図形態にお
いてのみ例示する。
【００１１】図3 は、図2 中に記載した寛骨臼杯状くぼ
み(10)の他の態様を示す。薄い殻(shell 26)が、殻(28)
に付着されている。この殻(26)は、冶金学的結合又はポ
リマー結合により殻(28)に付着されている。図3 は、上
記殻を一緒に接着剤により結合する殻(26)と(28)間のポ
リマー(30)の薄い層を例示している。殻(26)及び(28)の
ために使用される材料は、本明細書の先の段落中に使用
した材料のいずれか1つ又はその組み合わせである。杯
状くぼみの外部は、先に記載したような骨非成長適用の
ための細孔性表面層(24') をも含むことができる。寛骨
臼杯状くぼみの他の態様を、図4 中に記載する。ここ
で、杯状くぼみ(10") は、リベット(34)又はリベット(3
4)と冶金学的結合により殻(28') に機械的に結合された
殻(26') を含む。この場合にも、殻(26') 及び(28') の
ために使用される材料は、本説明の先のセクション中に
示した材料のいずれか1 つ又はその組み合わせであるこ
とができる。
【００１２】寛骨臼杯状くぼみのさらに他の態様を、図
5 中に示す。例示するように、杯状くぼみ(10''') は、
ポリマー又はコンポジット殻(36)に付着された金属殻(2
6")及び(28") を含む。得られたコンポジット構造は、
杯状くぼみ(10''') により低い弾性率及び適当な柔軟性
を提供するであろう。この設計は、杯状くぼみの関節形
成表面と大腿骨頭部( 図示せず）との適当な潤滑を維持
することにおける助けとなるであろう。この杯状くぼみ
の製作において使用される金属は、先のセクション中に
記載した材料のいずれか1 つ又はその組み合わせである
ことができる。ポリマー又はコンポジット殻のために使
用される材料は、Ultra High MolecularWeight Polyeth
ylene(UHMWPE)、Ultrapek、PEEK、Bone Cement(Polymet
hyl methacrylate)、又は整形外科用途のために使用さ
れるいずれかの他のポリマー又はコンポジットであるこ
とができる。様々な合金、非金属材料及びコンポジット
において先に記載した一般的な幾何は、関節形成表面の
上の潤滑液の流れを強化するための特定の杯状くぼみ柔
軟性をもつように設計される。杯状くぼみの曲げ弾性率
は、潤滑フィルムの確立及び維持における役割を演じ
る。正常な歩行ステップの2 モードの力曲線の影響の
下、大腿骨頭部は、杯状くぼみ内で上方に押される。そ
の頭部の上の上部空間内の潤滑剤のリザーバーに対する
頭部の生成圧縮力は、その頭部と杯状くぼみの壁との間
にその潤滑剤を押し出す。比較的非圧縮性液体の圧力
は、その頭部から杯状くぼみの壁を遠ざけ、そしてそれ
らの間の潤滑フィルムの確立を許容する。杯状くぼみの
材料の曲げ弾性率が低ければ低い程、潤滑フィルムは、
より厚くなる。
【００１３】一般的な幾何、製作材料、設計柔軟性、表
面柔軟性、及び修飾方法は、関節形成表面に課され、そ
して杯状くぼみと頭部の表面上の安定した又は半安定性
の潤滑フィルムの確立のための不連続性及びチャンネル
並びに他の手段を提供するために幾何学的に変更され
る。これらの表面の特徴は、本発明の明細書中に記載し
た一般的な杯状くぼみ幾何において使用される。それ
は、関節形成頭部と同じ直径をもつ杯状くぼみを作るた
めに変位Δy 及びΔz=0 である本発明の限定球の場合に
おいても使用されることができる。

Claims (10)

    【特許請求の範囲】
  1. 【請求項１】 その表面に対する関節形成動きのため
    の、その内の球状の玉に合うように改良された非球表面
    を含む補てつ学上の杯状くぼみ補てつ物であって、その
    杯状くぼみの表面が、その杯状くぼみの赤道面と極軸と
    の間にある弧の回転により定められ、その弧が、その杯
    状くぼみの球中心から平行移動された曲率中心をもち、
    その弧が、その杯状くぼみのについての上記極軸の回り
    に回転されるような補てつ物。
  2. 【請求項２】 杯状くぼみの非球上関節形成表面に対す
    る関節形成動きのための、その内の球状の玉に合うよう
    に形作られた補てつ学上の杯状くぼみ補てつ物であっ
    て、その杯状くぼみが、その杯状くぼみの球中心を定め
    るために交差するx 、y 、及びz 座標軸により定めら
    れ、その関節形成表面が、yz平面内に引かれ、そしてy
    軸とz 軸との間にある弧により定められ、上記関節形成
    表面を定めるためにその弧がz 軸の回りを360 度回転さ
    れ、その弧がその杯状くぼみの球中心から平行移動され
    た曲率中心をもつような補てつ物。
  3. 【請求項３】 空間が、杯状くぼみの非球上関節形成表
    面と球状玉との間に形成され、その空間がその内に所定
    量の潤滑性物質を収容するような、請求項２に記載の補
    てつ杯状くぼみ補てつ物。
  4. 【請求項４】 実質的に球状の外部殻及び実質的に非球
    状の内部殻をもつ補てつ学上の杯状くぼみ補てつ物であ
    って、その内部殻が、その内の関節形成動きのための実
    質的に球状の頭部に合うように形作られている補てつ
    物。
  5. 【請求項５】 外部殻が、x 、y 、及びz 軸の交差によ
    り定められた球中心を含み、内部殻が、y 軸とz 軸との
    間にあり、そしてz 軸の回りに回転された弧により定め
    られる非球状関節形成表面を含む、請求項４に記載の補
    てつ学上の杯状くぼみ補てつ物。
  6. 【請求項６】 杯状くぼみの内部殻及び外部殻が、金属
    から形成され、そして冶金学的に一緒に結合されてい
    る、請求項４に記載の補てつ学上の杯状くぼみ補てつ
    物。
  7. 【請求項７】 杯状くぼみの内部殻及び外部殻が、金属
    から形成され、そして接着剤ポリマーにより一緒に接着
    剤により結合されている、請求項４に記載の補てつ学上
    の杯状くぼみ補てつ物。
  8. 【請求項８】 杯状くぼみの内部殻及び外部殻が、金属
    から形成され、そして一緒にリベット結合されている、
    請求項４に記載の補てつ学上の杯状くぼみ補てつ物。
  9. 【請求項９】 杯状くぼみの内部殻及び外部殻が、金属
    から形成され、そしてポリマー殻により分かれて置かれ
    ている、請求項４に記載の補てつ学上の杯状くぼみ補て
    つ物。
  10. 【請求項１０】 所定量の潤滑性材料が、非球状内部殻
    と実質的に球状の頭部との間に定められた空間内に担持
    される、請求項４に記載の補てつ学上の杯状くぼみ補て
    つ物。
