リウマチ様関節炎の処置法

本発明は、自己免疫疾患を予防および処置するための免疫系の調節に関する。詳
細には、ヒトのリウマチ様関節炎の特徴である免疫応答を予防および処置する方
法に関する。

90%を超えるリウマチ様関節炎血清陽性の患者の8978球およびマクロファ
ージは、血清学的定義によるクラスIIHLAタンパク質DR4あるいはDRI
を発現する。DR4特異性は、いくつかの異なるハブロタイブに存在することが
知られており、そのうちの3つ、Dw4、Dwl4およびDwl5はリウマチ様
関節炎患者の8978球およびマクロファージに4関与し、そのうちの2 ′つ
、DwlOおよびDW13は関与しない。従って、リウマチ様関節炎を持つ人々
の8978球およびマクロファージは、クラス11ハブロタイブDw4、Dw
14、Dwl5あるいはDRIを発現する。リウマチ様関節炎(RA)に関与す
るハブロタイブは、DR−β−1鎖の第3超可変部の70−74位のアミノ酸配
列によって、RAに関与しないハブロタイブと区別されることが既に知られてい
る(Gregersen、P、 、ら、Proc Natl Acad Sci

AA (Dw4)か、あるいはQRRAA (Dwl4、Dwl5およびDRI
)のいずれがである。R(アルギニン)によるK(リジン)の置換は保存的であ
る。従って、配列Q(K/R)RAAは、リウマチ様関節炎の発生および症状に
重要であり得る。
免疫系が自己および非自己を区別することを可能にすることにおける、主要組織
適合抗原遺伝子複合体(MHC)にフードされるタンパク質の役割に関する最近
の考えは、MHCにコードされるクラス11タンパク質によって、外来性抗原が
種々のクラスのT細胞に「提示」されることを仮定している。クラス11タンパ
ク質は、外来性抗原(あるいはその断片)に結合可能な部位およびT細胞レセプ
ターを認識する他の部位を有する。
他の研究者による構造的な研究は、クラスIIMHCタンパク質上のQ (K/
R) RAA配列が配置される位置は、T細胞レセプター認識部位に相当するこ
とを示した。
リウマチ様関節炎は、関節液のある種のタンパク質成分が、免疫系によって異物
と認識される、自己免疫疾患と考えられている。おそらくそれらは、Ml(Cク
ラスI+ タンパク質と関連してT細胞に提示されると推定され、そのMHCク
ラス1■のキャリヤーはT細胞によって自己と認識される。前述に基づいて、こ
の自己認識部位はQ(K/R)RAAを含むと考えられ得る。適切なワクチンを
投与することにより、関節液関連抗原に対する1978球の応答を、ヘルパーT
細胞クラスから転換するか、あるいは防ぐことができることが、現在見い出され
ている。従って、本発明のワクチンは、クラス11認識部位Q (K/R) R
AAに結合される関節液抗原に対するヘルパーT細胞の増殖を予防する。

本発明は、それ自身がリウマチ様関節炎症状として現れる、関節液タンパク質に
対する自己免疫応答を予防および処置するためのワクチンを提供する。本発明の
ワクチンは、その認識部位にQ(K/R)RAA配列を有するMHCクラスII
組織適合性タンパク質と関連して提示される、これらのタンパク質に対スるヘル
パーT細胞の応答を効果的に予防する。
従って、1つの観点では、本発明はリウマチ様関節炎を予防あるいは改善するた
めに免疫する方法を目指している。ここで、予防接種法のプロトコールには、サ
プレッサーおよび/あるいは細胞障害性T細胞の増殖を優先的に促進し、および
/あるいは、サプレッサー/細胞障害性T細胞群の増大に影響することなく、ヘ
ルパーT細胞群の拡大を抑制する、1種あるいはそれ以上の免疫促進あるいは免
疫抑制物質の有効量と共に、ペプチドQ(K/R)RAAの有効量を免疫原的形
態で投与することが包含される。このような免疫作用性物質には、インターロイ
キン6 <IL−5) 、サイクロスポリンA、 CD4抗原に対する抗体、ト
ランスフォーミング成長因子−β(TGF−β)およびそれらの組み合わせが含
まれる。本発明はさらに、この方法に有用なワクチンを目指している。
別の観点では、本発明は、クラスI組織適合性配列の不変部分に連結するQ(K
/R)RAAアミノ酸配列配列有する、リウマチ様関節炎の予防に有用なワクチ
ン、およびこのワクチンを用いるリウマチ様関節炎の予防および改善法を目指し
ている。

本発明のワクチンは、免疫原性形態のアミノ酸配列Q(K/R)RAA、 ある
いはこのエピトープに結合するT細胞のレセプターの部分を形成する免疫原性形
態のペプチド配列を、主要な成分として使用する。従って、特異的なペプチドハ
プテン配列は、大きなポリペプチドあるいはその一部であり得、および/あるい
は、必要に応じて、リンカ−を介して担体に連結され得る。大きなタンパク質的
にペプチドを含める場合は、都合のよい大きなタンパク質には、他のクラスII
DR−β−1鎖にあるアミノ酸配列、あるいは他のエプスタイン・バーウィルス
に関連する糖タンパク質gp110中のこのペプチドに隣接するアミノ酸残基が
含まれる;クラスl MHCタンパク質定常領域と関連してQ(K/R)RAA
配列を提示するペプチドには、これらの領域の延長上の部位もさらに使用され得
る。しかし、個々のペプチドも使用され得る。両者の場合に任意的に、妨害しな
い延長路もさらに使用され得る。
ハブテンの免疫原性を増強する、あるいはそれに免疫原性を付与するのに適当な
担体には、一般に用いられるキーポールリンベットヘモ/アニン(H,)I)タ
ンパク質、破傷風トキソイド、あるいはヒト血清アルブミンのような、抗原性に
ついて中性である他の物質が含まれる。破傷風トキソイド、KLHあるいはHS
Aのような独立した担体を用いる場合、このペプチドは、多くがPierce
Chemical Company、 Rockford、IL、 から市販さ
れているリンカ−の使用を含む、当該分野の標準的な方法を用いて担体に連結さ
れ得る。典型的なリンカ−には、例えば、5)IICCおよび5PDPが含まれ
る。このような結合に効果的な方法は、公知である。
前述は、本発明のQ (K/R) RAAのような配列の短いペプチドに免疫原
性を付与する、認められている技術の典型である;好ましくはこの配列を反1夏
して有する、1. O−100個のアミノ酸のような、大きなペプチドの合成を
含む、この配列に免疫原性をイ」与するいかなる方法も使用され得る。
本発明のワクチンは、上述のハブロタイブによってリウマチ様関節炎になる恐れ
がある人々に投与される。従って、Dw4、Dw 14.0w15あるいはDR
Iに相当するハブロタイブを有する人々がワクチンの接種を受ける。
RAの予防あるいは改善の1つの方法では、免疫原性形態のQ(K/R) RA
Aが、ヘルパーT細胞を減らしてサプレッサーT細胞および/あるいは細胞障害
性T細胞群を優先的に増大させる免疫促進剤あるいは免疫抑制剤とともに投与さ
れる。従って、サプレッサー/細胞障害性細胞群を拡大させるが、ヘルパー細胞
群には影響しない促進剤が使用され得る;あるいは、ヘルパー細胞群を活性的に
抑制するが、サプレッサー/細胞障害性T細胞群には影響しない物質が使用され
得る。あるいは、これらの因子の組み合わせが使用され得る。この区別的な増大
をもたらす、ある種の物質は当該分野で知られている;しかじ、本発明には、こ
の作用を有することを示し得る、あらゆる物質の使用が含まれる。インターロイ
キン6は、サプレッサー/細胞障害性子細胞に対する適当な免疫促進剤である。
この刺激を増強するためにIL−2も含めることは、しばしば望ましい。
サイクロスポリンA、 TGF−β、および抗CD4抗体(Carteron。

「〜と共に」投与されるとは、同じワクチン処方(免疫原性形態のQ(K/R)
RAAおよび、免疫促進剤および/あるいは免疫抑制剤の両方を含む)での投与
、あるいは、協調したそれらの作用を得られるようにこれらの成分を別々に同じ
期間に投与することを意味する。適当な同じ期間とは、4時間から2日の期間で
あり、好ましくは、4時間から1日である。免疫促進剤/免疫抑制剤および免疫
原性ペプチドは、どのような順序でも投与され得、さらに各々の、あるいは両方
の反復投与のプロトコールも使用され得る。
適当な投与経路は全身性であり、静脈内、筋肉内、腹腔内、あるいは皮下注入の
ような注射による投与が含まれる。皮下あるいは筋肉的投与か好ましい。免疫原
性ペプチドおよび免疫促進性/免疫抑制性物質を、緩衝液、リノガー溶液、ある
いはハンクス液のような適当な賦形剤を含み、さらに加湿剤、安定剤などを含み
得る、ペプチド投与用の標準的処方を用いて注射用に処方する。適当なアジ−バ
ントもさらに含み得る。
これらの経路に適当な処方を使用する、経膜的あるいは経皮的投与のような他の
投与形態もさらに使用され得る。例えば最新版の肱1三旦肛二と二二μ立山IL
江1匹且、 Mack Publishing Company、 Easto
n、 Penn5ylvaniaに、広い範囲の処方が記載されている。
免疫原性ペプチドの適当量は、患者当り約100μg−100mgであり、好ま
しくは10−100mgである。免疫促進剤/免疫抑制剤あるいはその組み合わ
せは、選ばれた物質の効力に応する量で投与されるへきである;例えば、IL−
6の場合、適当な投与量の範囲は1〜100μg/kgであり、好ましくは20
−50μg/kgである。
もちろん、投与量のレベルは、使用されるペプチドの特定の免疫原性形態、選択
された免疫促進剤/免疫抑制剤、選択された投与経路、製剤の性質、および投与
される人の個々の反応に依存する。適正投与量のプロトコールの確立および個人
の管理は、前述のパラメーターを考慮に入れながらルーチンに行われ、当業者に
は公知である。
ワクチンの他の形態では、ペプチドQ(K/R)l?AAあるいはこの配列を有
する大きなペプチドをクラスI組織適合性配列の不変部分に結合して1.活性の
あるワクチン成分を作成する。クラスI組織適合性配列の不変部分は既知であり
、関連データがシーンバンクおよび当分野で一般に知られている、同等の他のデ

および他の動物の既知のクラスI MHC抗原の1−3H位を占める配列が列記
されている。これらのページを総置すると、ヒトM)ICタンパク質のなかで、
不変部分は1−8位; 13−31位、 46−53位; 84−93位; 1
17−136位;157−176位; 200−238位: 241−268位
である。概して、10−20個、好ましくは12−15個程度の配列を有する、
これらの領域からのペプチドは、関連のQ(K/R)RAA配列に、通常この配
列を囲む部分からの延長を付加して、あるいは付加しないで、上述のように連結
し得る。
上述のようにクラス1不変配列(標準法を用いて合成し得る)および抗原を結合
する方法は、当該分野で知られている。クラス1部分およびQ(K/R)I?A
Aペプチドの結合もまた融合タンパク質の組み換え技術による生産によって可能
である。投与経路および処方も上述のそれと同様である。

と、それに続くほとんど同一の2番目のコピーである、QRAAを有する、エプ
スタイン・バーウィルス糖タンパク質gpHOの807−816位のセグメント
のDNA配列のシーンバンクのデータベースと照合した。ChouおよびFas
sman (Adv Enth mol(1978) 47:45−148)の
方法によるgpLloの760−860位の配列の2次構造のコンピューター分
析によると、関連のQKRAAQRAA配列は、T細胞への結合に責任のあるエ
ピトープを構成すると推定される構造型を表している、2つの疎水性部分の間の
αヘワックスの一部であると予想される。
この相同性の結果、エプスタイン・バーウィルス糖タンパク質のこの配列はりウ
マチ様関節炎を媒介するT細胞の増殖を刺激し得、それらのT細胞が感受性ハブ
ロタイブ、Dw4/Dw14/Dw15/DR1のクラスIIMHcタンパク質
に連結した関節液タンパク質抗原を認識する。
EBVに感染した人々のRAに対する感受性の増加が知られている;しかじ、そ
の関係はわかっていない。T細胞群を刺激して、gl)110ペプチドおよび関
連のDw4ペプチドの両方に反応するようにする、エプスタイン・バーウィルス
(EBV)感染の能力をRAに関連するクラスII MHCタンパク質ハブロタ
イブの関連部を提示する4つのペプチド、gpHoペプチドおよび対照のDwl

EBV感染「ノQ(K/R)RAA!l激によるT細胞の感作実施例2て合成し
たペプチドを、インビトロでのT細胞の反応を決定するのに次のように使用した
:その方法は、ここで参考文献として引用する、Ford、 D、、 Ce1l
Immunol (1983)79:334−344; Thorley−L
awson、D、A、ら、Proc Natl Acad Sci」録(198
7) 84:5384−5388に開示されている方法の変法であった。簡単に
説明すると、末梢血単核球を、ウィルスキャブジッド抗原(VCA)に対する抗
体の欠如により示される、EBV感染歴のない提供者、および、EBVに以前に
感染していないことを示す高い抗VCA抗体価を有し、そのこと以外は正常な人
の等密度遠心法で単離し、RPM l−1640培地で3回洗浄した。次に細胞
を106細胞/mlの密度でプールされた10%のヒトAB血清、1%L−グル
タミン、100U/mlペニシリンおよび100μg/ml ストレプトマイシ
ンを補充した同培地中に、上述のリストから選択された刺激用ペプチドの存在下
で、懸濁した。37°Cで1週間培養後、増殖的1次反応を評価するために10
5細胞のサンプルを培養物から取った。
2次培養では、残っている細胞を数え、試験される同ペプチドを補充した新鮮な
培地で、あるいは対照として前記ペプチドを含まない培地で、連続希釈した。次
に、96ウエルの丸底培養プレートでの1次培養からの細胞を、so、ooo、
25.000.10.000あるいはs、 ooo個の生存細胞を含む100μ
mの分注液に分配し、放射線照射(3,000rad)された、同提供者からの
末梢血単核球を加えて細胞数を105に調整した。各々の細胞密度に対して、ペ
プチドを含む18ウエルおよびペプチドを含まない6個の対照ウェルをアッセイ
した。
この2次培養4日後、ガラス繊維フィルター上で細胞を採集する前に、1μCi
のトリチウムラベルされたチミジンを各ウェルに加えた。各細胞希釈に対して、
6個の対照ウェルから得たcpmの(平均+2標準偏差)より高いcpmを有す
るものを陽性ウェルと定義した。T前駆細胞の頻度を、Fordら(上述)の記
載のように、lウェル当りの細胞数に対する各希釈の陰性ウェルの細胞数の割合
をプロ、+−することにより評価した。
結果を図1に示す。パネルaおよびCは、VCA陽性の提供者由来の細胞の異な
る希釈における反応(cpm/ウェルで示す)を示す。パネルaおよびbは、g
p110ペプチドで処置されたもの:パネルCおよびdは、Dw4ペプチドで処
置されたものである。
図1の結果は、VCA陽性提供者由来のT細胞に対してgpHoペプチドあるい
はDW4ペプチドは、増殖を刺激し得た; VCA陰性提供者由来の細胞に対し
ては、どの希釈においても刺激は起こらなかった。これに相当する、1次刺激を
受けた培養細胞における結果を以下の表1に示す。

1976+162上述の結果は、以前にEBVに感染していることが、次にg
pHOペプチドの関連部分あるいはDw4ペプチドの関連部分に接触するとT細
胞群を感作して増大することを、明かに示す。
これらの結果はさらに、EBV感染前の被検体のT前駆細胞濃度が、gll)1
10および0w4の両方に関して約70.000個のT細胞に1個であることを
示す。
尖血王土
抗 110抗体b 、k O’ FE RA感受性ハブロタイブ抗体の 差【皿
型実施例2で調製され、KLHに結合されたペプチドをウサギでの抗体産生に使
用した。ニューシーラント白ウサギに、1mgの結合物を完全フロインドアジュ
バントに乳化させたものを皮下投与し、次いで、不完全フロインドアジュバント
中の1mgの結合物を3週間の間隔をおいて3回ブースターをかけた。ウサギは
、最後の投与後4日に採血され、その血清を一20’Cに保存した。
得られた血清の抗体価をELISAアッセイあるいはイムノプロットを用いてテ
ストした。ELISAアッセイはここに参考文献として引用する、Luka、
J、ら、J 1mmunol Methods (1984) 67:145−
156の方法によった。簡単に説明すると、抗原ペプチドをELISAプレート
をコートするのに使用し、等張のホウ酸緩衝化生理食塩水(BBS)で希釈した
免疫前あるいは免疫後のウサギ血清の種々の溶液100μmをウェルに加え、4
°Cて一晩インキコベートした。BBS、 0.2%Tween−20で洗浄後
、アルカワホスファターゼ標識ヤギ抗つサギIgG抗体(Tago、 Burl
ingame、 CA)を用いて、結合した抗体を検出した。結果を、免疫血清
の4゜5nmにおけるO、Dを免疫前血清のそれで割った、吸光度比として表に
しめす。各血清に対する抗体価を、少なくとも吸光度比2を示す最高希釈倍数と
した。阻害実験では、抑制ペプチドの候補を抗血清に加え、EL I SA測測
定前に4°Cて一晩反応させた。
ELISA測定は、EBV gpHoに対する抗体の反応性をテストする際に行
われた。gl)110は、K15hishita、 Mら、■匡包■(1984
) 133:393−395ノ方法により、P 3 HR1リンパ芽球からアフ
ィニティークロマトグラフィーにより精製した。
このアッセイを用いて、gpiioペプチドで免疫されたウサギがgpHoペプ
チドおよび、上述のように単離されたgptioタンパク質の両方を認識する抗
体を産生ずることを証明した。これらの結果を図2に示す。
明かに、glloタンパク質上の認識されるエピトープはペプチドのそれである
。なぜならば、100μg/mlのgl)110ペプチドとともに先にインキュ
ベートされた血清の20倍希釈物はもはやgI)110タンパク質に結合できな
いからである。これらの結果は、関連配列がそのタンパク質に対するB細胞応答
に関与するエピトープであることを示唆する。
クラスII MHCタンパク質との抗血清の反応を、Mo1t 4、T細胞(対
照)あるいは、HLA 0w4あるいはHLA Dw2ホモ接合の提供者(各々
、GMO3161およびGMO66821A、 Human Genetic
MutantCell Repository、 Camden、 NJより取
得)から確立したリンパ芽球様細胞から抽出した膜タンパク質に対するイム/プ
ロットによってテストした。タンパク質は、Bordfer、C,、J Bio

泳動後、還元され変性した膜ポリペプチドを電気泳動的にニトロセルロース紙に
移した。次に、フィルターを、0.05Mホウ酸塩、015M NaC1、pH
3、および3%の粉ミルクを含む溶液中で1時間ブレインキュベートし、続いて
同希釈比の抗血清と4°Cで一晩インキユベートした。(阻害測定用には、ニト
ロセルロース紙とともにインキュベートする前に、候補のペプチドを抗体溶液に
加えた。)ニトロセルロース紙をホウ酸緩衝液でよく洗浄後、結合した抗体を1
251−プロティンA (l mCi/ml ICN、1rvine、 CA)
を用いて検出した。ニトロセルロース紙を検出用試薬で1時間、インキュベート
し、BBSで洗浄し、乾燥して、増感用スクリーンを用いてKodak XAR
フィルムに一70°Cで露光した。
また、Dw4ペプチドの特異抗体を引き起こす能力を、上述のイムノプロットに
よりウサギ抗血清をアッセイすることによっても示した。このアッセイの結果を
図3に示す。パネルAのイムノプロットは、1 : 1000に希釈された抗D
R−βモノクローナル抗体の、Mo1t 4 (M4)細胞、HLA Dw4ホ
モ接合リンパ芽球様細胞(D4)、あるいはHLA Dw2ホモ接合リンパ芽球
様細胞(D2)の抽出物との反応をテストする、対、唄である。両方のDRβタ
ンパク質は反応するが、M4はしない。
パネルBでは、ゲルをDw4ペプチドで免疫したウサギの抗血清とともにインキ
ュベートした。
レーンセット1は、膜抽出物のみを使用して電気泳動を行ったときの結果を表す
。D4抽出物のみが抗体に免疫反応し、抗血清の特異性が確認される。
レーンセット2は、抗血清を10μg/mlのDw4ペプチドとともにブレイン
キュベートしたときの結果を示す。ゲル中の膜由来D4ペプチドへの結合は完全
に除かれる。
レーンセット3は、抗血清を100μg/mlの対照ペプチドDwlOとともに
ブレインキュベートしたときの結果を示す。抗体のD4抽出物への結合には影響
しないことかわかった。
レーンセット4は、10μg/mlの関連ペプチドDw14とのブレインキュベ
ーションがD4への結合を除くのに部分的に有効であったことを示す。DW14
の濃度を100μg/m lに上げると、結合はほとんど除かれた(レーンセッ
ト5)。
0w4、Dw2、Dw14およびDwlOの配列の比較(図4)は、67.70
、及び71位の残基の存在か、Dw4抗体の2 Saに重要であることを示す。
DW4で免疫された、同ウサギの血清が表2に示される抗体価でDw4ペプチド
、gl)110ペプチドおよびgI)110タンパク質を認識することをELI
SAによって示した。

g/mlとともに、ブレインキュベートすることにより阻害された(実施例2)

上述の結果は、gplloおよびHLA DW4に共有されるEQKRAA配列
が血清学的交差反応の基であることを示す。

Claims (9)
- 1.リウマチ様関節炎に対して被検体を免疫する方法であって、そのような免疫 化を必要とする該被検体に、T免疫系を刺激するのに有効な量の、免疫原性形態 のQ(K/R)RAA配列を有するペプチドを、 ヘルパーT細胞に比較して、サプレッサーおよび/あるいは細胞障害性T細胞群 を優先的に増大させ得る、少なくとも1つの免疫促進剤および/あるいは免疫抑 制剤とともに、投与することを包含する、方法。
- 2.免疫促進剤が,IL−2を含むか、あるいは含まない、インターロイキン6 (IL−6)である、請求項1に記載の方法。
- 3.免疫抑制剤が、サイクロスポリンA、TGF−β、および、CD4表面抗原 に対して免疫反応性である抗体から選択される、請求項1に記載の方法。
- 4.免疫原性形態のQ(K/R)RAA配列を有するペプチドと、ヘルパーT細 胞に比較して、サプレッサーおよび/あるいは細胞障害性T細胞群を優先的に増 大させ得る、免疫促進剤および/あるいは免疫抑制剤とを混合して含有する、薬 学的組成物。
- 5.免疫促進剤がIL−2を含むか、あるいは含まない、インターロイキン6( IL−6)である、請求項4に記載の組成物。
- 6.免疫抑制剤が、サイクロスポリンA、TGF−β、および、CD4表面抗原 に対して免疫反応性である抗体から選択される、請求項4に記載の組成物。
- 7.リウマチ様関節炎に対して被検体を免疫する方法であって、そのような免疫 化を必要とする該被検体に、特異的なサプレッサーおよび/あるいは細胞障害性 T細胞の生産を刺激するのに有効な童の、クラスI組織適合性配列の不変部分に 結合したQ(K/R)RAA配列を有するペプチドを、投与することを包含する 、方法。
- 8.クラスI組織適合性配列の不変部分に結合されたアミノ酸配列Q(K/R) RAAを有するペプチドを活性要素として含有する、リウマチ様関節炎に対して 被検体を免疫するのに有用なワクチン。
- 9.本質的にクラスI組織適合性配列の不変部分に結合されたQ(K/R)RA A配列を含有するペプチドからなる、組成物。
