Description
特許文献1には数種類の抗PCSK9抗体がPCSK9生理活性を拮抗することで血中LDL濃度を明らかに低下させることが開示されており、高コレステロール血症などの関連疾患の治療分野において重要な将来性を示している。治療性抗PCSK9抗体は、全てのタンパク質治療剤と同様に、製造又は保存において、例えば、凝集、変性、架橋、脱アミド化、異性化、酸化、及び剪断などの物理的及び化学的不安定性の影響を受けている(非特許文献1)。そのため、抗体の物理的及び化学的性質を安定的に保持することができる抗体製剤の開発は大きな課題である。
通常、高コレステロール血症などの関連疾患が慢性疾患であるため、患者が病院外又は自分で投与できる利便性を提供することは非常に重要である。通常、タンパク質治療剤が胃腸以外の経路だけで投与できるが、皮下注射(SC)又は筋肉注射(IM)の投与経路は治療コストの低下、及び投与期間における患者と医療従事者の利便性の改善を図ることができる。また、SC又はIM注射剤に必要とされる小体積(一般的に0.5~2mL)に関して、その他の製剤の課題が提示された。治療レベルを図るために、一般的に100mg~1gのタンパク質/剤の高濃度抗体製剤で投与する必要があるためである。ところが、高度に濃縮されたタンパク質製剤は、通常、タンパク質の凝集、沈殿、及び粘度を増加させることから、加工、製造、及び保存期間においてデメリットが発生する。粘度増加により製剤の投与に、例えば、疼痛、灼熱症状、及び薬物送達装置選定の制限などの悪い影響が与えられる(非特許文献2)
そのため、本分野において、特に患者の疼痛を軽減させる低粘度且つ高濃度タンパク質製剤を必要とされている。
一方、本発明は、(1)緩衝液と、(2)安定剤と、(3)抗PCSK9抗体又は抗原結合フラグメントと、を含む安定的な抗体製剤を提供する。
好ましい一実施形態として、前記安定的な抗体製剤は、非イオン性界面活性剤をさらに含んでもよい。
一実施形態において、当該ヒスチジン緩衝液の濃度は約20nMである。
一実施形態において、本発明に係る抗体製剤は、40℃で28日間保存した後に、少なくとも45%の抗体が主要帯電バリアントを有する。
一実施形態において、本発明に係る抗体製剤は、2~8℃で12ヶ月保存した後に、少なくとも98%の抗体が天然立体配座を有する。
一実施形態において、本発明に係る抗体製剤は、2~8℃で12ヶ月保存した後に、少なくとも87%の抗体が主要帯電バリアントを有する。
一方、本発明は、本発明に係る何れか1種の抗体製剤を含む薬剤充填済み注射器を提供する。
一方、本発明は、本発明に係る何れか1種の抗体製剤を含む送達装置又は薬剤充填済み注射器を用いて、任意のPCSK9活性に関連する疾患又は障害を治療、予防及び改善するための方法を提供する。
本発明に係る製剤は、ヒトPCSK9と特異的に結合する抗体又はその抗原結合フラグメントを含む。本明細書で使用される用語「PCSK9」はヒトプロタンパク質転換酵素であり、サブチリシン族を分泌するプロテアーゼK亜族に属される。既存文献では、PCSK9による低密度リポタンパク質粒子受容体との結合及びその分解の促進により、血漿LDLレベルが上昇される。
本発明に係る製剤は高濃度の活性抗体を含む、高安定性、低粘度の液体性製剤である。特に、本発明において、アルギニン塩を含む製剤の粘度が張化剤を含む調製液より明らかに低いことは発見された。また、調製液に含まれるアルギニン塩により張化剤を含む調製液が明らかに低下される。
本発明に係る薬物製剤は、薬物及びその他の医薬組成物の保存に適用できる任意の容器に保存されてもよい。例えば、本発明に係る薬物製剤は、小瓶、バイアル、注射器、カートリッジ、又は瓶などの、一定の体積を持つ、密封される殺菌済みのプラスチック製又はガラス製容器に保存される。各種類の小瓶は本発明に係る製剤の保存に使用されてもよいが、例えば、透明及び不透明(例えば、琥珀色)のガラス製又はプラスチック性小瓶を含む。同様に、各種類の注射器を用いて、本発明に係る薬物製剤を収納及び/又は投与することができる。
その他の用途以外に、本発明に係る医薬製剤は、PCSK9により仲介される疾患又は障害を含む、何れかのPCSK9活性に関連する疾患又は障害の治療、予防又は改善に使用される。本発明に係る医薬製剤により治療又は予防される代表的、非限定的な疾患及び障害は各種類の血中脂質異常症を含むが、例えば、高コレステロール血症、家族性高コレステロール血症、1 3/4脂血症、家族性1 3/4脂血症、異常III型リポタンパク血症、家族性異常III型リポタンパク血症、1 3/4トリグリセリド血症、及び家族性高トリグリセリド血症である。
本発明は本発明に係る何れかのPCSK9活性に関連する疾患又は障害の治療、予防及び改善に使用される薬剤の調製における本発明に係る抗体製剤の適用を含む。本発明に係る何れかのPCSK9活性に関連する疾患又は障害の治療、予防又は改善に使用される本明細書に係る抗体製剤をさらに含む。
液体型抗体製剤において、緩衝液系及びpH値が抗体の安定性に大きな影響を与えており、特別な物理的及び化学的性質を有する抗体はそれぞれ最適な緩衝液の種類及びpH値を持っている。最適な緩衝液系及びpH値を選定することで、臨床適用に適用できるように本発明に開示された抗PCSK9抗体が最適な安定性を持つようにする。
抗体安定性と粘度への各賦形剤による影響をさらに研究するために、ショ糖、アルギニン塩酸、ソルビトール又はマンニトールの1種類又はその組合せの製剤を使って対比試験を実施した。即ち、上記各賦形剤又はその組合せを、約150mg/mL JS002が含まれる20mMヒスチジン緩衝液(pH値5.5又は6.0)にそれぞれ入れるが、具体的な処方情報が表3に示される。各処方製剤を分注してから40℃で静置し、それぞれ2週目及び4週目で取り出して、抗体の安定性及び調製液の粘度の分析と測定を行った。分子排除高速液体クロマトグラフィー(SEC‐HPLC)によりJS002単体含有量の変化が測定され、弱カチオン高速液体クロマトグラフィー(CEX‐HPLC)によりJS002電荷主ピーク含有量が測定され、さらに4週目で標準方法により製剤の粘度が測定される。表4に示されるように、処方にショ糖、アルギニン塩酸、ソルビトール又はマンニトールの1種類又はその組合せが含まれる場合、抗体が比較的に強い熱安定性を有し、即ち、製剤が高温40℃で4週間静置された後、サンプル単体含有量が98%以上であり、主要電荷が51%以上占める。
液体製剤に添加される界面活性剤は、一般的に、例えば、抗体凝集の低減又は製剤における粒子状物質形成の最小化を図るために、抗体が保存中に空気/溶液界面の張力、溶液/表面の張力の影響を受けないようにすることで、タンパク質を保護するための試薬であり、抗体の物理的及び化学的性質の安定に繋げる。20mMヒスチジン緩衝液(pH値6.0)、130mMアルギニン塩酸と150mg/mlのJS002が含まれる製剤に、異なる濃度(0~0.5%)のポリソルベート20又はポリソルベート80をそれぞれ入れて、40℃で2週間静置後、分析と測定を行った。表6に示されるように、SEC‐HPLC単体含有量とCEX‐HPLC主ピーク含有量の測定結果によれば、異なる濃度(0~0.5%)のポリソルベート20又はポリソルベート80が含まれる製剤において、抗体JS002の熱安定性は明らかな変化がなく、高安定性を保持している。
通常、治療性抗体が含まれる液体薬物製品が2~8℃の条件で保存されるため、製剤が長時間保存において高安定性を保持することは、非常に重要である。上記の選定結果に基づき、4つの処方で製剤の長時間安定性について検討が行われた。
表7に示される4つの処方製剤を調製して透明な小瓶にそれぞれ保存し、2~8℃の条件で数ヶ月静置した後、各サンプルに対して分析と測定を行った。(a)肉眼観察による外観;(b)光遮蔽法により測定される不溶性微粒子(OD405nm);(c)pH値;(d)CE‐SDS法により測定される抗体の分子量;(d)SEC‐HPLCにより測定される抗体単体(品質標準:≧97.0%)、ポリマー(品質標準:≦3.0%)、又はフラグメントの含有量(品質標準:≦1.0%);(e)CEX‐HPLCにより測定される抗体の主要電荷(品質標準:≧70.0%)、酸性電荷(品質標準:≦30.0%)、又は塩基性電荷の含有量(品質標準:≦15.0%);(f)ELISA法により測定される抗体の結合活性(品質標準:対比品の70%~130%);及び、(g)抗体の生物活性(HepG2細胞へのLDL摂取実験、品質標準:対比品の70%~130%)などの、パラメータにより、安定性を評価した。表8に示されるように、4種類の製剤処方を2~8℃の条件で1~12ヶ月保存する間に、非常に良好な安定性を有する。
通常、治療性抗体が含まれる液体薬物製品が2~8℃で保存期間満了まで保存される。従って、患者が薬品の購入から使用までの期間において薬品を冷蔵する必要がある。提示された投与計画によるが、患者自身で薬物を投与する場合、患者による保存期間が数週間まで延びる可能性がある。そのため、冷蔵条件で保存する必要がない薬物は、家庭介護製品として、患者利便性の明らかな向上、及び薬物品質への間違った保存条件による影響の低下を示していることから、クレーム比率の低減及び温度偏差に対する監視の免除を実現した。
表9に示されるように、本発明に開示された製剤(処方番号:20)はタンパク質分解に対して更なる高安定性を有し、25℃で測定される分解動力学パラメータが6ヶ月保存の要求を満たした。
ForteBio親和力は既存の方法により測定される。簡単に言えば、4mg JS002(処方番号:29)とEvolocumab(140mg/ml/本、AMGENにより購入、ロット番号:1063135)を取り、それぞれ10KD限外濾過デバイスでリン酸塩緩衝液により100倍液交換を行ってから、280nmの吸収値でたんぱく質含有量を測定し、濃度を2mg/mLに調整した。ビチオンを取って室温まで平衡させ、2mgSulfo‐NHS‐Biotinを量って300μL超純水に入れ、10mMのビチオン母液を調製した。2mg/mL JS002とEvolocumabをそれぞれ1mL取って新しいEP管に入れ、たんぱく質:ビチオン=1:6で8μLのビチオン母液を入れ、均一に混ぜ、室温で1~2時間反応させた。ビチオンの反応が終了してから、10KD限外濾過デバイスでリン酸塩緩衝液により100倍液交換を行ってから、280nmの吸収値でたんぱく質含有量を測定し、濃度を1~2mg/mL範囲内に調整した。ビチオン化されたたんぱく質を1管あたり0.1mLで分注し、‐80℃で保存し、1回以内で凍結融解させた。ビチオン化されたJS002とEvolocumab抗体(5μg/mL)をストレプトアビジン(SA)バイオプローブに結合し、実験緩衝液(0.1%BSA、0.02%Tween‐20及び1×PBS)を300秒間平衡させてから、JS002プレートとEvolocumabプレートのそれぞれに異なる濃度まで希釈されたPCSK9を順に注入し、300秒間結合して、解離時間を1800秒間とした。計算式=koff/konにより親和力定数が計算された。
本実験は、JS002(処方番号:29)による、ヒトPCSK9‐D347Yに暴露されたHepG2細胞へのLDL摂取状況を細胞レベルから評価して、市販される同じターゲットの薬物Evolocumab(140mg/ml/本、AMGENにより購入、ロット番号:1063135)と比較した。簡単に言えば、ヒト肝臓癌細胞系(HepG2)細胞(ATCC、ロット番号:62591368)を2.0×104個/ウェルの密度で播種し(80μL/ウェル)、37℃、7%CO2で一晩培養した。JS002抗体とEvolocumabをそれぞれ濃度勾配希釈し(開始濃度20μg/mL、2倍で濃度勾配希釈)、10μLの抗体希釈液を取ってHepG2細胞に入れ、30分間インキュベートした。それと同時に、抗原を1μg/mLまで希釈し、10μLの抗原希釈液を取ってHepG2細胞に入れ、抗原、抗体と細胞を同時に4~6時間インキュベートした。蛍光標識が加えられたLDL(3μg/mL)と細胞を同時に16~18時間インキュベートしてから、マイクロプレートリーダにより細胞内へ摂取された蛍光量を測定した。
本試験に選ばれた19匹高脂血症アカゲザル(LDL≧l.3mmol/L)は、アトルバスタチンカルシウム群(1~4週目1.2mg/kgで投与、5~8週目2.4mg/kgで投与、4匹)と、JS002高用量群(12mg/kg、5匹)と、JS002低用量群(4mg/kg、5匹)と、プラセボ群(5匹)と、計4群に分けられた。アトルバスタチンカルシウム群は、経口投与で56日間連続的に投与し、溶出期間が21日間である。JS002高用量群とJS002低用量群は、4週間で1回、計2回投与し、78日間連続的に観察した。投与期間及び溶出期間で主要治療効果の分析指標(LDL‐C、TC、HDL‐C、ApoAL、ApoB、TG)、副次治療効果の指標(体重)、及び安全性指標(血液生化学、血液一般検査指標、臨床観察)の変化を定期的に観察した。上記指標を総合的に考えて、高脂血症アカゲザルに対するJS002(処方番号:20)の血中脂質低下効果について分析と評価を行った。
本実験系において、供試品JS002高用量群12mg/kg(臨床計画用量420mg/70kgヒト、4週間で1回、皮下注射計2回、に相当)は、高脂血症アカゲザルLDL‐Cに対して非常に明らかな低下効果を有し、ベースラインと比べれば、LDL‐C投与後、D2‐D71低下レベルが40%~70%に維持される。JS002低用量群4mg/kg(臨床計画用量140mg/70kgヒト、4週間で1回、皮下注射計2回、に相当)は、ベースラインと比べれば、LDL‐C投与後、D2‐D57低下レベルが20%~70%に維持され、D57‐D78の薬効が明らかに低下し、リバウンドが現れた。JS002高用量群はJS002低用量群より、効果の強度及び持続時間が優れた(図2)。全体の投与期間において、TG、FPG、ApoAlとHDL‐Cは明らかな変化を示さず、各安全性指標は投与に関連する変化を示していない。
試験により健康なカニクイザルに異なる用量のJS002(処方番号:20)を1回又は複数回で投与した後の薬物動態学の性質を観察した。カニクイザル実験の群れ分けと投与状況は表11に示される。A、B、C、D群は1回投与であり、投与量がそれぞれ2mg/kg、10mg/kg、50mg/kgと10mg/kgであり、A、BとC群の投与方式は皮下注射であり、D群の投与方式は静脈注射である。E群は連続投与であり、投与量が10mg/kgであり、週に1回投与で、連続的に4回投与した。
カニクイザルに10mg/kgの受検薬物JS002を皮下注射で1回投与した後、生物学的利用率が89.15%である。
カニクイザルに10mg/kg(週に1回、連続的に4回)のJS002を皮下注射で連続的に複数回投与した後、生体内における薬物の暴露量が当該用量で1回投与した群より明らかに高く、薬物が生体内に明らかに蓄積した。
本試験はカニクイザルに受検薬物JS002(処方番号:20)を皮下注射で投与する4週間繰返投与毒性試験と共に考察した。CFDAによるGLP制度の要求に基づき、組換えヒト化抗PCSK9モノクローナル抗体の注射液による免疫毒性と免疫原性を評価した。本実験は、賦形剤対照群(0mg/kg)、低用量群(30mg/kg)、中用量群(100mg/kg)、高用量群(300mg/kg)などの4つの用量群を設けて、1群につき動物10匹、雌雄それぞれ半数である。週に1回投与で、4週間連続的に投与し、回復期を4週間とする。検討の用量範囲内で、カニクイザルに賦形剤及び異なる用量(30、100、300mg/kg)の受検薬物JS002を皮下注射で複数回投与した後、全個体の異なるタイミングの血液サンプルは抗薬物抗体が測定されず、各群のサンプル陽性率と個体陽性率が共に0.0%である。結果によれば、異なる用量(30、100、300mg/kg)の受検薬物JS002を皮下注射で複数回投与した後、そのカニクイザル生体内における免疫原性が比較的に低い。各群の動物腎臓組織に対して免疫複合体の測定を行ったところ、免疫複合体の沈積が現れてない。その他の免疫関連指標によれば、各群動物の白血球絶対数と分類数、免疫グロブリンレベルとAIG比、リンパ器官/組織の肉眼解剖学観察、胸腺や脾臓の臓器重量/係数などは投与に関連する異常変化が見られず、リンパ球亜群の分布は投与に関連する規律的な変化が見られていない。それと同時に、組織病理学の測定は正常に示されている。
Claims (12)
- (a)緩衝液と、
(b)安定剤と、
(c)アミノ酸配列SEQ ID NO:1を有するHCDR1と、
アミノ酸配列SEQ ID NO:2を有するHCDR2と、
アミノ酸配列SEQ ID NO:3を有するHCDR3と、
アミノ酸配列SEQ ID NO:4を有するLCDR1と、
アミノ酸配列SEQ ID NO:5を有するLCDR2と、
アミノ酸配列SEQ ID NO:6を有するLCDR3と、を含む、ヒトPCSK9と特異的に結合する抗体又はその抗原結合フラグメントと、
を含み、pH値が5.5~6.5である、安定的な抗体製剤であって、
前記緩衝液はヒスチジン緩衝液であり、前記緩衝液の濃度は15~25mMであって、
前記安定剤はアルギニン若しくはその塩であって、前記アルギニン若しくはその塩の濃度が50~200mMである、又は、前記安定剤はアルギニンの塩とマンニトールの組合せであって、前記製剤における前記アルギニン塩の濃度が50~150mMであり、前記マンニトールの濃度が50~200mMであり、2種類の前記安定剤の総濃度が50mM~250mMの範囲内であって、
濃度0.01%~0.05%(w/v)のポリソルベート20をさらに含む、抗体製剤であって、
前記抗体又はその抗原結合フラグメントはアミノ酸配列SEQ ID NO:9を有する重鎖(HC)と、アミノ酸配列SEQ ID NO:10を有する軽鎖(LC)と、を含むこと、を特徴とする抗体製剤。 - 前記ヒスチジン緩衝液はL‐ヒスチジンとL‐ヒスチジン一塩酸塩により調製されたこと、を特徴とする請求項1に記載の抗体製剤。

- 前記ヒスチジンと前記ヒスチジン塩のモル比が1:1~1:4の範囲内であること、を特徴とする請求項3に記載の抗体製剤。
- 前記安定剤は130mM~160mMのアルギニン塩であること、を特徴とする請求項1に記載の抗体製剤。
- 前記抗体又はその抗原結合フラグメントはアミノ酸配列SEQ ID NO:7を有する重鎖可変領域(VH)と、アミノ酸配列SEQ ID NO:8を有する軽鎖可変領域(VL)と、を含むこと、を特徴とする請求項1に記載の抗体製剤。
- (a)20mMの前記ヒスチジン緩衝液と、
(b)100mM~200mMのアルギニン塩と、
(c)0.02%ポリソルベート20と、
(d)100mg/mL~200mg/mLの前記ヒトPCSK9と特異的に結合する抗体又は抗原結合フラグメントと、
を含むこと、を特徴とする請求項1に記載の抗体製剤。 - 前記製剤における前記アルギニン塩の濃度が130mM~160mMであること、を特徴とする請求項7に記載の抗体製剤。
