Description

本発明は、細胞、特にマイクロ流体デバイスまたはチップを含むがこれらに限定されない、流体デバイスにおける疾患モデリングのための細胞を培養する分野に関する。
筋萎縮性側索硬化症(ALS)及びパーキンソン病(PD)の根底にある生理学的、分子的、及び細胞的変化は複雑である。死後の脳組織では、疾患の根底にある運動ニューロン及びドーパミンニューロンが明らかに失われているが、神経細胞死への経路は、いくつかの既存の理論があるものの憶測のままである。しかしながら、これらの突然変異体固有の表現型は、現在の培養システムでは明白な細胞死を示さない孤発性の起源の成人発症疾患をまだ説明していない。特定の分子標的のためにALS及びPDの遺伝的形態に関する研究は興味深いが、ALS及びPDの症例の約90%には既知の遺伝子変異がなく、孤発性と呼ばれている。孤発性ALS(sALS)または孤発性パーキンソン病(sPD)について発表されているものは比較的少数であり、運動ニューロンまたはドーパミンニューロンに分化した少数の患者由来のiPSCを使用した論文はほとんどない。これらの研究は、ミトコンドリア機能を含む遺伝子発現パターンのいくつかの変化と、sALS症例のサブセットにおけるカスパーゼ活性またはTDP凝集の増加を示している。孤発性PDの場合、一部のレポートでは表現型が示されないが、DAニューロンプロセスの減少と、in vitroで後の時点において細胞死の表現型を示すレポートもある。疾患の遺伝的形態よりも挑戦的ではあるが、本発明者らは、sALS及びsPDに関する研究は孤発性形態が優勢であるためにはるかに重要であり、これらのモデルを開発するには早急な作業が必要であると考えている。さらに、孤発性疾患の複雑な性質により、疾患の病因全般をよりよく示す一般的な疾患の表現型を誘発するためのニューロン生理学のより完全なモデルが必要となる可能性がある。孤発性系統に明白な遺伝的「確証」がないという事実は、複雑な遺伝学が疾患に寄与する可能性があることを排除するものではない。したがって、基礎疾患の複雑さを説明することができる細胞の発達及び疾患病理学のモデルが当技術分野で非常に必要とされている。孤発性症例の交絡問題は、機能的に関連する生きているヒト組織を用いた生体模倣(MPS)モデルを使用することによって簡潔に調べることができる。

予備データ
臓器チップは、肺胞と小気道、腸、肝臓、腎臓、及び血液脳関門、及び他の臓器を含む、いくつかの臓器及び組織システムのMPSモデルを可能にする。臓器チップは、並列動作中にスケールアップでき、特別に開発された機器とペアにして、ユーザーがチューブを接続及び切断する必要をなくし、流体のデッドボリュームを減らしてサンプリングを改善し、不要な気泡を取り除くことができる。これらの利点により、実験スループットが向上し、ばらつきが減少する(図1)。
対照iPSC由来のMN及びDANの信頼できるMPSモデルの開発
上述のMPSデバイスで神経細胞及び内皮細胞を播種する本発明者らによる以前の研究では、spMNとDANの両方が、反対側のチャネルに播種されたBMECとの共培養において分化できることが示されている。特定の理論に縛られるものではないが、アストロサイト及びミクログリアの追加はモデルをさらに強化すると考えられている。例えば、ミクログリアは神経変性疾患に関与している細胞型でもあり、従来の培養形態では機能を失うことが知られている。したがって、MPSは、この細胞型がin vitroで初めて新たな生物学的及び疾患関連生理機能を示すことを可能にし得る。
アストロサイト及びミクログリアを使用して、spMN及びDANの成熟を通じて潜在的なバイオマーカーを強化する
spMNまたはDANのいずれかは、次の4つの異なる条件下でチップ内で培養される:(i)上部チャネルのニューロンのみ、(ii)上部チャネルのニューロンと下部チャネルのBMEC(図2のように)、(iii)上部チャネルにアストロサイト、下部チャネルにBMECでプレコーティングしたチップに播種したニューロン、及び(iv)上部チャネルにアストロサイト、下部チャネルにBMECでプレコーティングしたチップに播種したニューロン及びミクログリア(図5)。構成的プロモーターの下で細胞質蛍光レポーター系統を使用してミクログリアを生成することができる結果、培養物におけるそれらの持続性を決定することができる。このことは、特定のマーカーによってこれらの細胞を特性決定する問題を克服する。両チャネルは10μlの流量を有することができ、神経細胞及び内皮細胞の生存に最適であると思われる。
MPS培養物が複数の実験にわたって信頼できるバイオマーカー発現を示すかどうかを判断
最高スコアの播種の組み合わせに基づいて、より広範な表現型のアウトカム指標が適用される。これには、MPSの主要な利点と多領域の細胞生理学的リードアウトを組み合わせた細胞アッセイの包括的なマトリックスが含まれる(表2)。
10のiPSC系統全体で再現可能なバイオマーカーを検証
前述の研究は、MPSデバイスにおける表現型のリードアウトを確立する。単一の対照iPSC系統は、プロトコルを介して少なくとも3回実行される。目的は、このシステムを使用して疾患特異的な変化を検出する前に、様々な患者間にどのようなばらつきが存在するかを確立することである。その後、10の個別の対照系統により、患者間の違いを比較できる(表3)。後の実験において生物学的変動の可能性のあるソースを減らし、本発明者らは、単一の対照系統由来の支持組織(BMEC、アストロサイト、ミクログリア)の標準セットを利用する。4週間後、本発明者らはニューロン集団と形態分析を行う。チップ内の合計spMNまたはDANの最小しきい値に到達した対照系統が含まれ、その後、表現型マーカーの完全なセットが適用される。ヒト系統間のバイオマーカー発現にどの程度のばらつきがあるかについてデータが分析される。ANOVA試験を使用して系統間で有意差を示さない表現型アッセイの結果(P<0.05)がさらに評価される。
さらなる調整
本発明者らは、細胞産物を組み合わせたMPSデバイスにより、ニューロン活動が強化され、ニューロンがより成熟することを期待している。さらなる調整には、各細胞型の比率、接続時間、またはMPSデバイス内で細胞が一緒に生存できる条件を確立する他のパラメータのいずれかを調整することが含まれる。この最適なプロトコルを1つの対照系統で3回繰り返すが、全範囲のバイオマーカーの読み取りを取り込むことで、技術の信頼性を判断できる。現在、MPSチップとそれらを収容する培養装置は、許容レベルが極めて低く、デバイスから一貫した性能を提供する高品質の制御で、生物学的ソースに起因すると思われるばらつきを伴い、製造されている。
疾患のバイオマーカーを特定するための検証済みのspMN及びDAN MPSモデルにおける4つのsALS、4つのsPD、4つの対照、及び4つのSMA系統の比較
本発明者らによる以前の研究は、iPSCに由来する2次元spMN培養において、本発明者らがRNAseq分析及びプロテオミクスに基づいて、対照、SMA、及びALSの遺伝的形態を分離できることを示した(図6)。興味深いことに、本発明者らは、TGF-βが、RNAseq及びプロテオミクスデータの両方に基づいてALS試料において増加する信頼できるバイオマーカーであることを発見した。本明細書では、チップの改善された生理学的環境、ならびに電気生理機能及びメタボロミクスの包含により、疾患関連バイオマーカーの数がさらに増える。
20のsALS、sPD、及び対照系統を試験して、複数の孤発性系統全体にまたがるシグネチャーの存在を確認
4つの系統の初期の特性決定により、疾患特異的な表現型のアイデアがもたらされるが、本発明者らの対象疾患としてのsALS及びsPDのより大きな群全体にわたる信頼性を理解することはが関心対照である。これらの系統は、本発明者らが現在作製しているLothian対照コホート(25系統)及びAnswer ALSを介して利用可能であり、ALSコホート系統(現在1,000のiPSC系統を作製中)を生成する。本発明者らは、観察される表現型の重症度の可能性を高めるために急速に進行する患者に焦点を当てる。同様に、本発明者らは早期発症患者に焦点を当てた20のsPD iPSC系統も作製する。本発明者らは、少なくとも3つの主要なバイオマーカーに直接焦点を当てて、複数の疾患系統にわたるシグネチャーを比較する。
代謝バイオマーカー
本発明者らは、カスパーゼ3の増加、TDP43の誤局在化/凝集、またはDAニューロン線維の長さの減少などの家族性疾患モデルの文献において予測されるsALS及びsPDに特異的ないくつかのバイオマーカーを見る可能性がある。最も重要なのは、バイオマーカーが患者及び系統全体で信頼できることであり、孤発性疾患患者の系統と関連していることがわかっている。Lothian対照コホートを使用すると、これにより、この群(すなわち、ALSまたはPDを発症したであろう対照患者)のバイオマーカー信号が交絡することがなくなる。本発明者らは、最も重要なバイオマーカーは、後述するスクリーニングのためのスケールアップに役立つ性質のメタボロミクスであると考えている。
対照支持細胞の役割、環境ストレス
本発明者らは、支持細胞型を標準化して潜在的ばらつきを制限し、生理学的環境を反映するシステムを提供することを選択したため、本発明者らは、対照支持細胞を使用して疾患の表現型が保護されることを発見し得る。表現型が見られない場合、本発明者らは、ALSまたはPD患者系統に由来する支持細胞(BMEC、アストロサイト、ミクログリア)を使用して実験を繰り返し、潜在的な疾患の表現型を再評価する。
運動ニューロンまたはドーパミンニューロンにおける機能的表現型の検出
カルシウム蛍光色素の使用は、本発明者らのMPSモデルにおいてニューロン機能を決定するための貴重なツールであることが証明されている。全体的な一時的な活性のリードアウト(図3)に加えて、混合ニューラルチャネル内のニューロンの相互接続性の増加は、経時的に成熟する動的な集団レベルの活性シグネチャーを有するニューラルネットワークを形成する(図8)。カルシウム染料の投与はエンドポイントで行われるアッセイであるため、培養における経時的な神経生理機能の変化は、実験ごとにさらに多くのチップを必要とする。ネットワーク形成と薬物治療への反応における疾患関連のダイナミクスは、病因への新たな洞察をもたらし得る。
NCATSライブラリを使用したMPSベースの薬物スクリーニング
記載されているように、バイオマーカーは、ライブ電気生理機能(ニューロンの発火パターンの変化、すなわち図9)、細胞死アッセイ、溶出液での神経伝達物質及びメタボロミクススクリーニング、トランスクリプトーム及びプロテオミクス分析から見つかった非常に不一致な個々の遺伝子のq-PCRの組み合わせで検出される。バイオマーカーには、全ての試料でバイオマーカーパネルを簡単に実行できるメタボロームで見つかったものが含まれる。
臨床的に価値のあるバイオマーカーのアノテーションリスト
本発明者らの最も検証されたマーカーは、既存の文献の広範なマイニングに基づいて、血液、脳脊髄液(CSF)、及び脳/脊髄組織の患者(孤発性及び遺伝的形態のALS及びPDの両方)試料の既知のレベル/活性と比較される。これらの検索から、本発明者らは3つの可能性のある結果を予期している:(i)バイオマーカーは、ALSまたはPDにおいて変化することがすでに知られているが、これは、チップ内の生きているニューロンが患者の既知の疾患病変を再現していることを示す。(ii)バイオマーカーは、アッセイされたが、ALSまたはPDに関連することは示していない。これは、疾患の病因の初期にのみ見られ(本発明者らの細胞モデルが「若い」iPSCを使用しているため)、患者の臨床症状または後期段階で見落とされたかもしれないことを示している可能性がある。(iii)MPSモデルから決定されたバイオマーカーに関する既知のデータはない:この可能性は、患者において試験できる真に新しい疾患特有の特性である可能性がある。この場合、本発明者らは、sALS及びsPD患者の生体液中の選択されたバイオマーカーのアッセイを開発しようと試みるであろう。本発明者らの臨床共同PIのDrs.Baloh及びTagliatiは、MPSの新規バイオマーカー検証を中心とした臨床研究を実施するのに適した立場にある。本発明者らは、最も翻訳可能なバイオマーカーが末梢血及びCSFにあることを予期している。3つの可能性全てに明確な臨床的意義があるため、バイオマーカーは、有望なトランスレーショナルデータと、チップ上及び患者の生体液の両方での大規模なバイオマーカー検出の実現可能性の両方に基づいてランク付けされる。次に、本発明者らは、この短いリストを使用して、以下に説明する実験におけるこれらの新しいバイオマーカーを使用する新しい創薬プラットフォームを設計する。
BBBを通過してMPSの神経側に入る小分子
本発明者らは、最初の混合物中の活性分子のバリア特性または機能性疾患の表現型効果を交絡することがないように、最初にBMEC、MN、及びDAN毒性の全ての薬物候補を試験する。リソースを節約するために、本発明者らは、標準的な384ウェル形式の薬物スクリーニングプラットフォームで対照MN及びDAN培養物を試験する。非細胞毒性化合物は、MPSでのその後の組み合わせスクリーニングのために識別される。経験に基づいて、本発明者らは2~5%の細胞毒性の割合を予期している。次に、本発明者らは、候補分子が血液チャネルを通過し、BMEC層を通って神経コンパートメントに入る能力によって候補ライブラリをフィルタリングしようとする。未発表の大規模な研究では、本発明者らは、このシステムがバリアを通過する及び通過しない既知の薬物に関してBBBを模倣することを示した(図11)。さらに、MPSチップを収納する独自の培養デバイスには、薬物を血液側にロードし、脳側から薬物を収集するために使用できる入力リザーバがある。本発明者らは、これらの化合物がそれらのBBB透過性のために臨床に迅速に翻訳される可能性が最も高いと期待している。本発明者らは、最初に一連の100の化合物混合物を下部チャネルに流し、質量分析によって神経コンパートメントからの溶出液上の同じ分子を調べることにより、この透過性を決定する。これは、2,816の薬物全ての透過性が試験されるまで29回繰り返される。in vivoで観察された不十分なBBB浸透に基づいて、本発明者らはNCATのコレクションのサブセット(約20%)のみが、陰性対照として含まれているデキストランよりも高い閾値内で検出されることを予期する。これらのろ過された化合物は、本発明者らの一次スクリーニングで使用される。
一次スクリーニングを実施して、新規バイオマーカー調節化合物を特定する
次に、陽性の候補混合物が個々の化合物に分離され、新しいチップセットで再実行されて、この助成金の最終年に特性決定される個々の候補を決定する。最終候補は、バイオマーカーの全範囲を使用して検証され、メタボロミクスの変化に加えて、表現型の変化が各薬剤によってどれだけうまく逆転するかを決定する。本発明者らは、用量反応実験を実施して、改変された表現型に基づいて最適濃度も決定する。本発明者らはまた、新しい治療標的経路を解明し得る特定の疾患の表現型を悪化させる薬物を発見し得る。これらの薬物はFDA承認済みであるため、sALS及びsPDの臨床試験に迅速に移行できる独自の新薬セットが生まれる。
共培養iBMEC及びヒト神経細胞は、細胞相互作用及びBBB機能を示す。
アストロサイト及びiBMECはそれぞれのチャネルにとどまることができるが、アストロサイトプロセスは細孔から突出して、iBMECと直接細胞間接触を形成し、in vivoにおいて観察されるアストロサイトエンドフィートを模倣する。さらに、ヒト周皮細胞及びアストロサイトとの共培養は、臓器チップ上で単独で培養されたiBMECと比較して、蛍光デキストラン-FITC(3kDa)の血液から脳への漏出を有意に減少させ、層流に加えて、初代ヒトアストロサイト及び周皮細胞が、臓器チップ上で機能的なBBBの成熟をさらに促進することができることを示す。まとめると、これらの結果は、BBBを形成するNVUのインターフェースに似た臓器レベルの構造を示している。血液コンパートメントを10または100ng/mlのTNFαで一晩灌流すると、対照条件と比較して、アストロサイトの形態が変化し、血管表面を覆うエンドフィートのような構造が減少した。興味深いことに、血管のエンドフィートカバレッジの減少に続いて、デキストランの血液から脳への漏出が増加し、これは血管刺激がバリア機能に大きな影響を及ぼし、脳と血液コンパートメントの間の機能的連結が再現されたことを示している。
アストロサイトのエンドフィートカバレッジ分析
アストロサイトの突出部(エンドフィートのような構造)で覆われた血管表面の割合を定量化するために、その血管表面をDMEM(FBSなし)で洗浄し、血管グリコカリックスに結合できるレプチンである蛍光標識コムギ胚芽凝集素(WGA-647 Invitrogen)で染色する(DOI:10.1002/jemt.22602)。室温で15分後、チップを新鮮なDMEMで2回すすぎ、2% PFAを室温で20分間行い、アストロサイトマーカーGFAP(Abcam)で染色した。抗マウス二次抗体(Invitrogen,Alexa Fluor 488)及びHOECHS(Abcam)とのインキュベーション後、PBSで洗浄し、共焦点顕微鏡(Zeiss LSM850)で画像化した。

筋萎縮性側索硬化症(ALS)及びパーキンソン病(PD)の根底にある生理学的、分子的、及び細胞的変化は複雑である。本発明者らは、Cedars-Sinaiの人工多能性幹細胞(iPSC)コアを活用して、ALS及びPD系統の大規模なコホートを生成した。ALS(ALSチップ)及びパーキンソン病(PDチップ)を研究するために、疾患関連組織の共培養物を組み合わせた高度に生理学的なモデルを開発した。さらに、この微小生理学的プラットフォームには、脳特異的な血管チャネルが含まれており、ヒトの疾患システムにおける血液脳関門(BBB)の特性の研究を可能にする。疾患特異的な病態生理学を理解するために、包括的なゲノム、メタボロミクス、及び電気生理学的アッセイが開発されており、両方のシステムにおいて新たなバイオマーカーを生成するために使用される。このプロジェクトの主な結果は、孤発性ALS及びPDの両方の再現性のある疾患特異的表現型の確立であり、脳側または血液側のいずれかで新規表現型低下薬をスクリーニングするためのバイオマーカーとして使用され、血液脳関門を通過する化合物の能力をシミュレートした。本発明の2年目に、本発明者らは早期発症孤発性(EOS)PD及びALS患者系統のコホートを確立し、中脳及び脊髄チップモデルを使用してロバストなバイオマーカーを決定するための継続的な研究を行っている。本発明者らの最初の完全なPDトライアルでは、本発明者らは、5つのEOSPD系統を利用し、それらを本発明者らの「スーパー対照」Lothianコホートの5つの対照と比較した。本発明者らは、生細胞イメージング及びメタボロミクスシグネチャーの両方において、PD患者系統における初期の疾患特異的シグネチャーを決定した。PDチップパラダイムは、ヒトドーパミン作動性ニューロンに対するオピエートの効果を研究するためのプラットフォームとして使用するようにも適合されており、モルヒネ及び他のオピエートアゴニストへの応答に関する予備研究の結果も提示される。
BBBチップ間の個人間のばらつきは、罹患患者のBBB変化を検出できる
個別化医療に関連するシステムとしてiPSCベースのBBBチップを検証するには、臓器特異的疾患のモデリングを実証することが重要である。本発明者らは最近、ハンチントン病(HD)のiPSCベースのモデルを報告し、そこではHD-iBMECが改変されたバリア機能を示した。候補分子の患者特異的脳透過性の予測因子としてパーソナライズされたBBBチップを使用する可能性を評価するために、本発明者らは、3人の健康なドナー(CS617iCTR、CS172iCTR、及びCS188iCTR)及びHUNTINGTIN遺伝子(CS81iHD)内に71個のCAGリピートを有するHD患者に由来するiPSCベースのBBBチップ全体の様々な分子量の蛍光標識デキストランの透過性を調べた。特に、健康な個人間で有意なばらつきは観察されなかったが、HD-BBBチップではデキストラン-FITC分子の透過性が有意に増加した(図45F)。さらに、健康な対照由来のBBBチップを横切るデキストラン分子の透過性は、以前に報告されたin vivoでのげっ歯類の脳への取り込みと相関し(Yuan et al.,2009)(R2=0.96)、iPSCベースのBBBチップが、サイズに基づいて分子を選択的に分離できるバリアを形成し、維持することを示す(図45G)。本発明者らはまた、最近、精神運動遅滞の重篤な形態であるモノカルボン酸トランスポーター8(MCT8)欠損症のiPSCベースのモデルを報告し、iBMECを横断する甲状腺ホルモン(トリヨードサイロニン、T3)の輸送には機能的なMCT8が必要であることを示した。臓器特異的疾患モデリングをさらに評価するために、本発明者らは、MCT8欠損症用のiPSCベースのBBBチップを生成し、これは上記の健康な対照系統及び以下の公開されたiPSC系統を使用した:(i)対照系統CS03iCTR;(ii)CRISPR/Cas9を介したMCT8変異CS03iCTRmut;(iii)患者系統CS01iMCT8;(iv)CRISPR/Cas9を介した相同組換えCS01iMCT8corを使用して変異が修正されたMCT8変異系統。液体クロマトグラフィータンデム質量分析(LC-MS/MS)は、T3輸送が健康な対照BBBチップ全体で安定している一方で、MCT8欠損BBBチップは有意に低い透過性を示し(図45H)、BBBを横断するT3の血液から脳への輸送へのMCT8の必要性を確認した。概して、これらの結果は、患者特異的なiPSCベースのBBBチップを使用して、BBB機能の患者間のばらつきを予期できることを示唆し得る。
BBBチップを介して灌流されるヒト全血は「血液側」に限定され、これは血液に起因する毒性から保護し、疾患モデリングを強化する
ほとんどの培養システムでは、神経細胞は栄養及び成長因子を含む培地で維持されており、血管の血流は考慮されていない。しかしながら、神経細胞をろ過されていない血液に曝すと、脳出血で観察されるように、細胞毒性を引き起こす可能性がある。したがって、神経細胞に支持栄養素及び成長因子を安全に提供するために、血液をろ過する機能的なBBBが必要である。抗凝固剤のクエン酸ナトリウムで処理されたヒト全血は、「血液側」を介して生理学的に適切なせん断速度(3,600μl/hr,5dyn/cm2に相当)で灌流し、iPSC由来の神経細胞を含む脳コンパートメントは神経媒体で灌流した(図46A)。血液灌流中のデキストラン-FITC(3kDa)透過性の測定は、iBMECが血液を「血液側」に閉じ込める機能的障壁を維持できることを示した(図46B,C)。対照的に、iBMECを含まない、またはTNFαで処理されたiBMECを含むBBBチップには機能的な障壁がなかったため、血液が「脳側」に漏れた(図46B,C)。免疫細胞化学分析及び乳酸脱水素酵素(LDH)生存率アッセイは、iPSCベースのBBBチップを含む神経細胞が血液に起因する毒性を示さなかったが、iBMECを含まない、またはTNF-αで処理された神経培養物は有意な毒性を示した(図46D,E)。これらの結果は、iPSCベースのBBBチップのiBMECが「脳側」の神経細胞を血液に起因する細胞毒性から保護し、それによりBBBの重要なin vivo機能の1つを再現することを示している。ヒト全血をiPSCベースのBBBチップに組み込むと、このプラットフォームに別の重要なin vivoBBBインターフェースが導入される。
チップ上のアルツハイマー病
チップ上のアルツハイマー病前頭側頭型認知症(FTD)は、65歳未満の認知症患者に特によく見られる臨床症候群である。これは、アルツハイマー病及びレビー小体型認知症に次いで、全ての年齢層で3番目に一般的である。患者は、行動、実行機能、及び言語において進行性の欠陥を示し、FTDは顕著な行動要素のために様々な精神障害を模倣することができる。FTDの病変は、前頭側頭葉変性症(FTLD)と呼ばれ、前頭葉、前側頭葉、前帯状皮質、及び島皮質のニューロン喪失、神経膠症、及び微小空胞の変化を特徴とする。これらの領域は、皮質のレイヤー5に特殊なニューロン(フォン・エコノモ及びフォークニューロン)を持っており、皮質と皮質下のネットワークの統合に役割を果たし、行動障害型前頭側頭型認知症の非常に早い段階で退化すると仮定されている。RNA結合タンパク質TDP-43の凝集体は、FTLD(FTLD-TDPと呼ばれる)の約50%に見られ、残りの症例は凝集したMAPT(FTLD-tau)またはFUS(FTLD-FUS)のいずれかである。認知症の家族歴は対象の最大40%で報告されており、強力な遺伝的要素を裏付けている。FTLD-TDPの根底にある一般的な遺伝子変異には、C9orf72のヘキサヌクレオチドリピート伸長、GRN及びTBK1遺伝子の機能喪失変異、TARDBP(TDP-43自体をコードする)、VCP、及びCHMP2Bのミスセンス変異が含まれる。FTDの最も一般的な病理学的及び遺伝的変異であることに加えて、関連する神経変性症候群筋萎縮性側索硬化症(ALS)との有意な重複を示し、共通の遺伝的要因及び病変を有する。さらに、FTLD-TDPの非細胞自律性及びさらに非CNS自律性の病因についての強力な証拠があり、本明細書で提案する多細胞3D前脳オンチップモデリングに理想的に適している。
iPSC由来組織を備えたEmulateプラットフォーム
EmulateのMPSテクノロジーには、肺胞及び小気道、腸、肝臓、腎臓、血液脳関門(BBB)及び他の臓器の主要なモデルが含まれている。本発明者らは、ロバストな脳オンチップモデルの確立に加えて、iPSC由来の脳微小血管内皮細胞及び非特異的混合ニューロン培養を使用したBBBのモデリングにMPSプラットフォームを適用することに成功した(図50)。本発明者らは、患者の末梢血または線維芽細胞を使用し、リプログラミング因子の一過性の異所性発現を可能にする非統合技術を使用して直接リプログラミングされる。数回の継代にわたって、iPSCコロニーはエピジェネティックなマークを取り除き、ドナー患者の遺伝子構成を保持する多能性細胞の安定した供給源であり続ける。これらのiPSCは、以下に概説するように、関心のある様々な組織タイプのその後の疾患モデリング研究のために拡張して極低温で保存することができる。
多細胞3Dヒト前脳MPS及び患者iPSC系統の開発
興味深いのは、FTLD-TDPを研究するための多細胞前脳MPSモデル(fMPS)の開発である。これには、スコットランドのLothianコホートの対照iPSC系統が含まれ、ここでは、60~80歳の患者が長年追跡され、神経学的に正常であることが示されている。これらの系統の1つは、CN、BMEC、MGの産生、及びfMPSの確立のための標準的な対照として使用される。本発明者らは、アウトカム指標のパネルを使用して、通常の対照系統でシステムを検証し、次いで追加の4つの対照iPSC系統でロバスト性を調べる。並行して、本発明者らは、C9orf72及びGRN患者のiPSC系統と同質遺伝子対照のセットを開発し、その後、SA2及びこの提案のUH3部分におけるFTLD-TDPの疾患バイオマーカーを特定するために使用される。
fMPSの測定におけるバイオマーカー/アッセイのロバスト性の決定。
最高スコアの共培養パラダイムに基づいて、本発明者らは、Cedars-Sinaiで確立された多領域の細胞生理学的リードアウトを利用する細胞アッセイの包括的なマトリックスを含むように、本発明者らの表現型のアウトカム指標を拡張する。以下は、CN後、または最終分析のために、共培養の4週間後に、週間隔で評価される:溶出液は、(i)質量分析を使用した神経伝達物質及び代謝物のメタボロミクスバイオマーカー発現について、及び(ii)ELISAを使用したサイトカイン産生について、プロファイルされる、(iii)神経生理機能はライブカルシウムイメージングを使用して分析される、(iv)MPSデバイスは実験後に分析され、ハイコンテントイメージングを使用して細胞型の形態及び生存が評価される。細胞毒性及びBBB透過性は、神経コンパートメントにおいてLDH及びデキストランのクロスオーバーを使用して測定される。本発明者らは、全体的なチップ要件を低減するために、同じセットのチップに対する補完的なアッセイを注意深く検討した。このシステムの信頼性を確立するために、これらの実験は、SA1.1で一緒に生成された同じ対照iPSC系統由来の細胞ロットを使用して3回繰り返され、合計18fMPSが比較される。各アッセイにおいて、本発明者らは結果を比較し、測定あたり25%未満の変動係数(CV)に基づいてモデルの有効性を判断する。例えば、LDH値が1回の実行あたり5チップ間で25%の分散内にある場合、対照fMPSは細胞毒性のモデル化に信頼性があり(または有意な毒性が観察されないことを示すことにも信頼性がある)、3つの実験のそれぞれの平均は有意差がない。本発明者らは、繰り返し測定を説明するために適切な統計的検定を適用し、検証のために全てのデータをCedars-Sinaiの生物統計コアに提出する。本発明者らは、これらのアッセイが3回の実行間で有意差のない結果を生成すると判断した場合(実行ごとに5回の繰り返し)、本発明者らは5人の別々の対照個体から得られたfMPSモデルの評価に進む。
5人の対照個体からfMPS全体で再現可能なバイオマーカーを特定する。
このシステムを使用してFTLD-TDPの疾患特異的変化を検出する前に、本発明者らは、異なるヒト対象間にどのような差異が存在するかを確立する必要がある。ここで、本発明者らはSA1.2aの最もロバストなアッセイに焦点を当て、5つの対照fMPS間の変動性を調べる(表5)。さらに、これらのデバイスにおいて一貫して発現している細胞型を探索するために、系統ごとに1つのfMPSが単一細胞RNAトランスクリプトームプロファイリング(scRNA)に割り当てられる。MPSでMG及びBMECと共培養された中脳ニューロンに関する本発明者らの研究の予備データは、scRNAの適用、及び共培養された細胞型の回復を示している(図51)。本発明者らは、モデル内の細胞型の変動の評価を支援するために、本発明者らのfMPSに同じアプローチを適用することを計画している。4週間後に細胞生存の最小閾値を満たすfMPSチップは、系統間変動の分析に含まれる。5つの対照全体でCVが25%以下である、再現性のあるバイオマーカー及びリードアウトをもたらすアッセイが、対照手段としてのアプリケーションとして選択される。後の目的で生物学的変動の潜在的な原因を減らすために、本発明者らは、5つの対照MPSの中で最高のバリアスコアを生成する対照-BMEC系統を選択する。この系統は疾患チップのBBBチャンバーに使用され、CN及びMGは両方とも各FTLD-TDP系統由来である。本発明者らが将来の疾患分析のために前進するために選択したバイオマーカーは、実行間で25%CVを超える変動を示す必要がある。対照fMPSモデル対象間で、特に系統内で25%以下、系統全体で30%以下の低い変動係数を確立することは、本発明者らの進展のための第2の合格/不合格基準になる。これは、アプリケーションをスクリーニングするための本発明者らの多細胞fMPS設計の有用性を表している。
C9orf72(8)及びGRN変異を有するFTLD-TDP対象からのiPSC系統の生成。
UCSF記憶・加齢センター(UCSF Memory and Aging Center)は、iPSCリプログラミング用の細胞に加えて、高度に特性決定されたFTLD-TDP患者試料を収集した。これらには、臨床スケール、シリアルニューロイメージング、脳の病理学、及び生体試料などの疾患の表現型の測定が含まれる。Cedars-SinaiのiPSCコアを利用して、本発明者らはFTLD-TDP対象からiPSC系統を生成し、そこから臨床、生体試料、及び剖検材料の完全なコレクションを入手できる。したがって、本発明者らは、FTLD-TDP fMPSの結果を個々の臨床及び病理学的データで検証する独自の機会を得る。これには、血液/脳fMPS相互作用のためにこれらの患者から収集された血清、及びSeeley研究所による単一核RNAシークエンシングにすでに使用されている死後の前脳組織が含まれる。さらに、これらのFTLD-TDP iPSC系統はiPSCコアによって産生されるため、CN及びMGの分化にすぐに入ることができ、凍結保存して直接使用するために保存することができる。性別を生物学的変数として説明するために、男性及び女性の両方から系統が生成されるが、本発明者らはfMPS表現型に関しては有意差を予測していない。
CRISPR遺伝子編集を使用してC9orf72及びGRN同質遺伝子修正iPSC系統を生成する。
本発明者らは、3つのALS患者由来iPSCにおけるC9orf72遺伝子の反復増殖をすでに除去し、正常なC9orf72遺伝子発現を回復し、iPSC由来ニューロンにおけるRNA病巣及びRANジペプチドの産生を停止している(図52)。同様に、CRISPR/Cas9技術を使用して、本発明者らは通常のコード配列を、GRNで見られるような遺伝子のミスセンス及びナンセンス対立遺伝子に復元した。本発明者らは、これらのすでに確立されたアプローチを使用して、生成された3つのC9orf72及び3つのGRNiPSC系統の同質遺伝子対照を開発する。系統は、増殖するときに核型が決定され、継代され、親系統と一致する。前脳ニューロンへの予備的な分化は、チップにおいて使用する前に確立される。
FTLD-TDPの2つの一般的な遺伝的形態における疾患関連バイオマーカーのfMPSモデルの特徴
FTLD-TDP系統に起因する違い。本明細書では、本発明者らは、FTLD-TDP由来組織に関連する潜在的なバイオマーカー及び/または表現型の検出を含むように、本発明者らのアッセイを拡張する。本発明者らは、最初に、本発明者らのアッセイにおけるバイオマーカー及び表現型の個体間変動を最小限に抑えるために、疾患及び同質遺伝子修正された系統を使用することを提案する。次に、本発明者らは、fMPS及びアッセイをFTLD-TDP患者系統のより大きなコホートに適用して、バイオマーカーが再現性があり、疾患の不均一性を表すかどうかを判断する。バイオマーカーの同定及びバイオマーカーの確認により、本発明者らはシステムをヒトデータと相互検証し、同定されたバイオマーカーを使用して候補治療薬の前臨床有効性試験を実施し、潜在的な診断用途のためにfMPSに対する患者血清の流れの影響を調べる。
C9orf72に対するアンチセンスオリゴヌクレオチドを含む遺伝子型特異的修飾因子に対するバイオマーカーの治療反応、及びAAV-GRNによる遺伝子置換を評価する
バイオマーカーの発見は、特定の遺伝的バリアントを標的とするFTD/ALSスペクトラム障害の治療法が臨床試験に入っていることを示唆し得、現在、FTDではなくALSに焦点が当てられている。これは主に、ALSの進行速度が速いため、機能的評価尺度を使用した臨床試験がバイオマーカーなしで実行可能であるのに対し、FTDでは不可能であるためである。このことは、この提案の重要性、特にFTLD-TDPの新規バイオマーカーの特定及び検証を強調している。fMPSモデルを使用して特定されたバイオマーカーは、FTD患者の遺伝子治療の試験を設計及び監視するために使用でき、孤発性FTDの治療試験にも役立つことが期待されるため、本発明者らの発見は即座に翻訳に影響を与える。したがって、本目的において、本発明者らは、現在開発中のFTLD-TDPに対する2つの治療アプローチの効果を調査し、C9orf72及びGRN関連FTDのfMPSモデルにおけるバイオマーカーを変更する能力を決定する。
fMPSモデルにおいて流れるFTLD-TDP血清の効果の特性決定
全ての遺伝子型にわたるFTLD-TDP患者は、自己免疫疾患の発生率が高いことを示しており、GRNレベルは、肥満及びインスリン抵抗性に関連しており、GRN変異を有するFTLD-TDP対象は、循環IL-6のレベルが上昇している。C9orf72及びGRNタンパク質は、自然免疫系の末梢骨髄細胞においても高度に発現しており、本発明者らのグループなどの研究は、これらのタンパク質の機能の喪失が免疫細胞機能の変化につながることを強く支持している。これにより、FTLD-TDPにおける神経変性が、末梢血及び前脳組織の代謝因子及び/または免疫因子間の相互作用を伴う可能性が高まる。毒性試験または治療効率の事前スクリーニングに加えて、本発明者らのfMPSは、患者試料中のこれらの循環因子を評価するための生物学的リードアウトとして、独自に使用することができる。これはデバイスの野心的なアプリケーションであるため、本明細書において、本発明者らは、FTLD-TDP血清に曝露されたときにfMPSのアプリケーションをまず確立することを提案する。適切な対照実験を並行して使用して、本発明者らは、FTLD-TDP患者の血清が対照fMPSモデルにおいて疾患表現型を誘発するかどうかを試験する。本発明者らは、血清由来の因子が、BMEC、ミクログリア、アストロサイトの機能、及びiPSCが生成され、fMPSが播種されたのと同じ患者の血清を検査する前例のない能力に影響を与えると仮定している。これにより、fMPSモデルを使用して、皮質機能に対する末梢因子の影響について多数の患者の血清をスクリーニングし、BBB全体での薬物送達を調べるための段階が設定される。
Claims (7)
- 細胞を培養する方法であって、
(i)アストロサイト、
(ii)脳微小血管内皮細胞(BMEC)、
(iii)脊髄運動ニューロン(spMNs)及びドーパミン作動性ニューロン(DANs)、
(iv)ミクログリア、および、
(v)上面及び底面を含む膜を含む、マイクロ流体デバイス
を提供する工程と、
前記底面に前記BMECを播種して播種された内皮細胞を作製し、かつ前記上面に前記アストロサイトを播種して播種されたアストロサイトを作製する工程と、
前記BMECの播種後1日以上後、前記上面に前記spMNs及びDANsを播種して播種されたニューロンを作製する工程と、
前記spMNs及びDANsを播種した後1週間で、前記上面に前記ミクログリアを播種して、播種されたミクログリアを作製する工程と、及び、
前記播種された内皮細胞、播種されたアストロサイト、播種されたニューロン、及び播種されたミクログリアのうちの1つまたは複数を、一定の流量で一定期間培養する工程と
を含む、前記方法。 - 前記アストロサイト、BMEC、ニューロン、及びミクログリアが、幹細胞からそれぞれ分化されたものであるか、または、前記アストロサイト、BMEC、ニューロン、及びミクログリアが、一次組織から単離された初代細胞であり、
