Description
本明細書に記載されるのは、非神経学的及び/又は神経学的状態の疾患管理におけるIGF-1の機能及びその適用の改善である。より詳細には、非神経学的及び/又は神経学的状態を治療するための有機又は植物ベースの材料、例えばブラックカラントアントシアニン(BCA)の濃縮抽出物を含有するcGPの使用に関する。
インスリン様増殖因子-1(IGF-1)は、内分泌ホルモンとして生成されるタンパク質である。それは、成長ホルモンの効果の一次メディエーターであり、身体内のほぼすべての細胞における体成長を刺激する。成長効果に加えて、IGF-1はまた、神経細胞及び細胞DNA合成を調節しうる。
非神経学的及び/又は神経学的状態の治療及び管理のためのブラックカラントアントシアニン(BCA)の濃縮抽出物などの環状グリシン-プロリン(cGP)を含有する有機又は植物ベースの材料の使用の臨床適用に関する方法が本明細書に記載される。この方法は、ベースラインデータの標準セットと比べて不十分なIGF-1機能を有する個体に対する具体的治療方法である。
a)動物から生物学的検体を得るステップと;
b)インスリン様増殖因子1(IGF-1)のバイオアベイラビリティを決定するためのcGP/IGF-1のモル比を計算するために環状グリシン-プロリン(cGP)及びIGF-1の濃度を測定するステップと;
c)生物学的検体中のcGP/IGF-1のモル比を標準と比較することで、結果の連続体の中で、測定されたcGP濃度及び/又はcGPのIGF-1総測定量に対するモル比が、疾患経過及び治療応答における個体のIGF-1機能変化を評価するための相対標準(標準としてのベースラインと比べての差異)を確認するステップと;
d)cGPを含有する有機又は植物ベースの材料から製剤化されたcGPを治療有効量で動物に投与し、
動物におけるcGPの濃度及び/又はcGPのIGF-1総測定量に対するモル比における減少を阻止し;及び/又は
動物における既存のcGPの濃度及び/又はcGPのIGF-1総測定量に対するモル比を維持し;及び/又は
動物におけるcGPの濃度及び/又はcGPのIGF-1総測定量に対するモル比を増加させるステップと、
を含む、方法が提供される。
a)動物から生物学的検体を得るステップと;
b)動物の第1設定年齢時、又は神経学的状態の初期段階、又はcGPを含有する有機又は植物ベースの材料から製剤化されたcGPの治療有効量での動物に対する処置前に、生物学的検体中でのインスリン様増殖因子1(IGF-1)のバイオアベイラビリティを決定するためのcGP/IGF-1のモル比を計算するために環状グリシン-プロリン(cGP)及びIGF-1の濃度を測定するステップと;
c)動物のさらなる設定年齢間隔又は非神経学的及び/又は神経学的状態のさらなる段階、又はcGPを含有する有機又は植物ベースの材料から製剤化されたcGPの治療有効量での動物に対する処置後に、生物学的検体中でのインスリン様増殖因子1(IGF-1)のバイオアベイラビリティを決定するために環状グリシン-プロリン(cGP)プラズマバイオマーカー及びIGF-1の濃度を再測定するステップと;
d)第1の設定年齢に対する設定年齢間隔、又はIGF-1機能障害を伴う非神経学的及び/又は神経学的状態の初期段階、又はcGPを含有する有機又は植物ベースの材料から製剤化されたcGPの治療有効量での動物に対する処置前、生物学的検体中での測定されたcGP濃度及び/又はcGP濃度のIGF-1総測定量に対する比のいずれかを比較することで、結果の連続体の中で、測定されたcGP濃度及び/又はcGPのIGF-1総測定量に対するモル比において変化があるか否かを確認し、それにより動物における認知減退からの非神経学的及び/又は神経学的状態の発生リスクがベースラインデータの標準セットと比べて増加しているか否かを判定し、認知減退の経過をモニタリングするステップと、
を含み、ここで前記測定された比が、cGPを含有する有機又は植物、例えばそこに含まれるBCAから製剤化された治療薬の適した用量を選択するために用いられる、方法が提供される。
a)動物から生物学的検体を得るステップと;
b)非神経学的及び/又は神経学的状態の発症からの動物のベースライン時(<72時間)に生物学的検体中でのインスリン様増殖因子1(IGF-1)のバイオアベイラビリティを計算するために、環状グリシン-プロリン(cGP)及びIGF-1の濃度を測定するステップと、
c)動物の回復中のさらなる規則的間隔で、生物学的検体中での環状グリシン-プロリン(cGP)及びIGF-1の濃度を再測定し、インスリン様増殖因子1(IGF-1)のバイオアベイラビリティを計算するステップと、
d)さらなる設定間隔でのベースラインからの動物の機能的回復を評価するステップと、
を含み、ここで、動物におけるより高いベースラインcGP濃度、より陽性の予後が機能的回復の評価に基づくように、データの連続体からのcGPのベースライン濃度及び/又はcGP/IGF-1のモル比から、動物の非神経学的及び/又は神経学的状態の回復の能力及び臨床転帰が予測される、方法が提供される。
cGPを含有する有機又は植物ベースの材料から製剤化された天然cGPの治療計画及び用量は、IGF-1の機能及び神経学的及び/又は非神経学的状態からの回復の能力に基づいて決定される。
IGF-1の機能を維持するために、動物における既存のcGPの濃度及び/又はcGPのIGF-1総測定量に対する比を維持し;及び/又は
IGF-1の機能を改善するために、動物におけるcGPの濃度及び/又はcGPのIGF-1総測定量に対する比を増加させるための、製剤化される薬剤の製造におけるcGPを含有する有機又は植物ベースの材料、例えばブラックカラントアントシアニン(BCA)の濃縮抽出物である。
IGF-1の機能を維持するために、動物における既存のcGPの(正常/生理学的)濃度及び/又はcGPのIGF-1総測定量に対するモル比を維持し;及び/又は
IGF-1の機能を改善するために、動物におけるcGPの濃度及び/又はcGPのIGF-1総測定量に対するモル比を増加させる又は正常化するための、動物への投与用のcGPを治療有効量で含むcGPを含有する有機又は植物ベースの材料が提供される。
a)動物から生物学的検体を得るステップと;
b)生物学的検体中でのインスリン様増殖因子1(IGF-1)のバイオアベイラビリティを計算するために環状グリシン-プロリン(cGP)及びIGF-1の濃度を測定するステップと;
c)生物学的検体中での測定されたcGP濃度及び/又はcGPのIGF-1総測定量に対するモル比のいずれかを標準と比較することで、結果の連続体の中で、測定されたcGP濃度及び/又はcGPのIGF-1総測定量に対するモル比が、個体の疾患の予後を支援するためのIGF-1機能変化を評価するための相対標準(ベースライン)と比べての差異に一致するか否かを確認し、治療応答をモニタリングするステップと
d)cGPを含有する有機又は植物ベースの材料の濃縮抽出物を治療有効量で動物に投与し、

a)動物から生物学的検体を得るステップと;
b)動物の第1設定年齢時、又は非神経学的及び/又は神経学的状態の初期段階、又はcGPを含有する有機又は植物ベースの材料から製剤化されたcGPの治療有効量での動物に対する処置前に、生物学的検体中でのインスリン様増殖因子1(IGF-1)のバイオアベイラビリティを計算するために環状グリシン-プロリン(cGP)及びIGF-1の濃度を測定するステップと;
c)動物のさらなる設定年齢間隔又は非神経学的及び/又は神経学的状態のさらなる段階、又はcGPを含有する有機又は植物ベースの材料から製剤化されたcGPの治療有効量での動物に対する処置後に、生物学的検体中でのインスリン様増殖因子1(IGF-1)のバイオアベイラビリティを計算するために、環状グリシン-プロリン(cGP)及びIGF-1の濃度を再測定するステップと;
d)第1の設定年齢に対する設定年齢間隔、又は非神経学的及び/又は神経学的状態の初期段階、又はcGPを含有する有機又は植物ベースの材料の濃縮抽出物の治療有効量での動物に対する処置前、生物学的検体中での測定されたcGP濃度及び/又はcGPのIGF-1総測定量に対するモル比のいずれかを比較することで、結果の連続体の中で、測定されたcGP濃度及び/又はcGP/IGF-1のモル比において変化があるか否かを確認し、それにより動物における非神経学的及び/又は神経学的状態、例えば認知減退の発生リスク及び予後/経過がベースラインデータの標準セットと比べて増加しているか否かを判定するステップと、
を含み、ここで前記測定された比が、cGPを含有する有機又は植物ベースの材料での処置についての個別患者及びそこでのcGPを含有する有機又は植物ベースの材料での処置に適した用量を選択するために用いられる、方法が提供される。
IGF-1の機能を維持するために、動物における既存のcGPの濃度及び/又はcGPのIGF-1総測定量に対するモル比を維持し;及び/又は
IGF-1の機能を改善するために、動物におけるcGPの濃度及び/又はcGPのIGF-1総測定量に対するモル比を増加させるための、製剤化される薬剤の製造におけるcGPを含有する有機又は植物ベースの材料の濃縮抽出物の使用が提供される。
IGF-1の機能を維持するために、動物における既存のcGPの(正常/生理学的)濃度及び/又はcGPのIGF-1総測定量に対するモル比を維持し;及び/又は
IGF-1の機能を改善するために、動物におけるcGPの濃度及び/又はcGPのIGF-1総測定量に対するモル比を増加させる又は正常化するための、動物への投与用に製剤化されるcGPを含有する有機又は植物ベースの材料から処理された天然cGPを治療有効量で含む抽出物が提供される。
脳卒中に関連する症状を低減するための方法であって、cGPを含有する有機又は植物ベースの材料から処理された天然cGPを治療有効量で投与することを含む、方法が提供される。
個別動物の治療計画、及びIGF-1機能障害を伴う非神経学的及び神経学的状態の治療の効果的な介入の後の治療応答を設計するための、IGF-1機能を計算するための環状グリシン-プロリン(cGP)/IGF-1モル比の臨床適用、並びにそれらを治療するためのブラックカラントアントシアニン(BCA)の濃縮抽出物などのcGPを含有する有機又は植物ベースの材料の使用に関する方法のさらなる態様は、以下の添付の図面に対する参照とともに、あくまで例として示される以下の説明から明白になるであろう。
上記の通り、非神経学的及び神経学的状態の回復の能力、並びに経過/予後を予測するための環状グリシン-プロリン(cGP)/IGF-1モル比の利用の臨床適用に関する方法、並びにそれらを治療するためのブラックカラントアントシアニン(BCA)の濃縮抽出物などのcGPを含有する有機又は植物ベースの材料の使用が本明細書に記載される。この方法では、cGP及びcGP/IGF-1モル比(ひいては活性IGF-1濃度)を調節するための手段、及びベースラインデータ又は非疾患群の標準セットと比べて不十分なcGPレベルを有する個体に対する特定の治療方法とともに、cGP及びcGP/IGF-1比を間接的に用いて、インビボでIGF-1がより正確に測定される。
cGPを含有する有機又は植物ベースの材料という用語は、本明細書及び特許請求の範囲の全体を通じて加工/製剤化された材料であると理解されるべきである。
a)動物から生物学的検体を得るステップと;
b)生物学的検体中での活性濃度依存性のインスリン様増殖因子1(IGF-1)のバイオアベイラビリティに対する環状グリシン-プロリン(cGP)プラズマバイオマーカーの濃度を測定するステップと;

c)動物のさらなる設定年齢間隔又は非神経学的及び/又は神経学的状態のさらなる段階、又はcGPを含有する有機又は植物ベースの材料の治療有効量での動物に対する処置後に、生物学的検体中での活性濃度依存性のインスリン様増殖因子1(IGF-1)のバイオアベイラビリティに対する環状グリシン-プロリン(cGP)プラズマバイオマーカーの濃度を再測定するステップと;
d)第1の設定年齢に対する設定年齢間隔、又は神経学的状態の初期段階、又はcGPを含有する有機又は植物ベースの材料の治療有効量での動物に対する処置前、生物学的検体中での測定されたcGP濃度及び/又はcGPのIGF-1総測定量に対するモル比のいずれかを比較することで、結果の連続体の中で、測定されたcGP濃度及び/又はcGPのIGF-1総測定量に対するモル比において変化があるか否かを確認し、それにより動物における認知減退からの非神経学的及び/又は神経学的状態の発生リスク及び/又は非神経学的及び/又は神経学的状態の疾患経過/予後のモニタリングがベースラインデータの標準セットと比べて増加しているか否かを判定するステップと、
を含み、ここで前記測定された比が、個別患者及びそこでのcGPを含有する有機又は植物ベースの材料を使用する処置に適した用量を選択するために用いられる、方法が提供される。
a)動物から生物学的検体を得るステップと;
b)非神経学的及び/又は神経学的状態の発症からの動物のベースライン時(<48時間)の生物学的検体中での活性濃度依存性のインスリン様増殖因子1(IGF-1)のバイオアベイラビリティに対する環状グリシン-プロリン(cGP)プラズマバイオマーカーの濃度を測定するステップと、

d)ベースラインからさらなる設定間隔での動物の機能的回復を評価するステップと、
を含み、ここで、動物におけるより高いベースラインcGP濃度、より陽性の予後が機能的回復の評価に基づくように、データの連続体からのCGPのベースライン濃度から動物の神経学的状態の回復の短期転帰が予測される、方法が提供される。
cGPの増加が神経学的状態のリスクを示し、及びcGPの減少が神経学的状態の段階と相関することも見出されている。それ故、cGPを含有する有機又は植物ベースの材料による処置が認知減退の発生を阻止する又は遅延させることがあると予想される。
動物における既存のcGPの濃度及び/又はcGPのIGF-1総測定量に対するモル比を維持し;及び/又は
動物におけるcGPの濃度及び/又はcGPのIGF-1総測定量に対するモル比を増加させるための、製剤化される薬剤の製造におけるcGPを含有する有機又は植物ベースの材料の使用が提供される。
動物における既存のcGPの(正常/生理学的)濃度及び/又はcGPのIGF-1総測定量に対するモル比を維持し;及び/又は
動物におけるcGPの濃度及び/又はcGPのIGF-1総測定量に対するモル比を増加させる又は正常化するための、動物への投与用に製剤化されるcGPを含有する有機又は植物ベースの材料を治療有効量で含む抽出物が提供される。
ヨーロッパ、及びニュージーランド内を含む、国際的に育成された任意のブラックカラント品種に由来してもよい。
非神経学的及び神経学的状態のリスク及び回復を予測するための環状グリシン-プロリン(cGP)の臨床適用に関する上記方法、並びにそれらを治療するためのブラックカラントアントシアニン(BCA)の濃縮抽出物などのcGPを含有する有機又は植物ベースの材料の使用が、ここで具体的実施例の参照により説明される。
cGPに富んだ有機植物から製剤化された天然cGPが経口的に生体利用可能か否かを知るため、発明者により試験を完了した。発明者は、ブラックカラントアントシアニン(BCA)の添加により、パーキンソン患者の脳脊髄液(CSF)中で環状グリシン-プロリン(cGP)が増加することを発見した。そのデータは、天然cGPが経口的に生体利用可能であることを裏付けた。
CSFサンプル
CSFサンプルを腰椎穿刺により得た。各来診中に約8mLのCSFを得た。平滑管内に収集したCSFサンプルは、収集から15分以内に、濡れた氷上でEndolab, Christchurch, New Zealandに輸送した。次に、サンプル受け取りから30分以内に、サンプルを室温、3000rpmで15分間遠心分離し、上清を2つの平滑管の間で等しくアリコートし、-80℃未満で凍結させた。
血液サンプルを肘前窩の静脈穿刺を介して得て;20mLをヘパリン及びEDTA管の間で等しく分割した。サンプルは、収集から45分以内に、濡れた氷上でEndolabに直ちに輸送した。次に、サンプル受け取りから30分以内に、サンプルを15分間遠心分離し(3000rpm)、血漿を平滑管内に吸引し、-80℃未満で凍結させた。
BCA中のcGPの存在可能性を分析するため、BCAを水に溶かし、5、50及び100mg/mlの3つの異なる濃度を得た。各濃度は5通りに分析している。
cGPアッセイにおける内部標準として提供されるcGP-d2。cGP-d2(500ng/mLの50μL)を血漿100μLに添加し、ボルテックス混合した。溶液を、4.5mLの管内に含まれる1mLのPhreeリン脂質除去カートリッジ(Phenomenex, Auckland, New Zealand)に移し;アセトニトリル(MeCN)中の1%ギ酸500μLをカートリッジに添加し、4℃、1000rpmで5分間遠心分離し、濾液を収集することができた。真空濃縮器を用いて濾液を乾燥させた(1.5mTorで1時間、次に0.7mTorで45分間、室温)。乾燥したサンプルを100μLの10%メタノール/水で再構成し、定量のために超高圧液体クロマトグラフィーバイアルに移し、次に4℃、500rpmで5分間遠心分離し、任意の残存する微粒子を沈降させた。cGPを木炭除去ヒト血漿に添加することによって調製した標準である品質管理サンプルを、2つの異なる濃度のcGPとともに利用し、次にサンプルと同じ抽出手順を施した。
手短に言うと、クロマトグラフィー条件は、35℃のカラム温度で10%メタノール/90%水の初期移動相組成物が200μL/分で流れる、Synergy Hydroの2.5μmカラム(Phenomenex)(100×2mm)から構成した。質量分析条件は、4000Vの電圧、30psiのシースガスフロー、2psiの補助ガスフロー、及び250℃のキャピラリー温度で正モードのエレクトロスプレーイオン化から構成した。断片化は、衝突ガスとしての1.2mTorrのアルゴン及び35Vの解離電圧を用いて行った。質量分析計は、cGP及びcGP-d2各々に対して155.1→70.2m/z及び157.1→70.2m/zといった2つのトランジションを利用して、選択的反応モニタリング(SRM)モードで稼動させた。両ピークに対する保持時間は3.6分であった。未知のサンプルは、既知濃度の検量線と比べてのcGP/cGP-d2のピーク面積比を用いて定量化した。
総IGF-1、IGFBP-1、-2及び-3の血漿及びCSF濃度は、市販のELISAキット(Crystal Chem, Chicago, IL, USA)を用い、製造業者の使用説明書に従って測定した。アッセイは、血漿サンプル中で4回繰り返したが、利用可能なCSF量が限られたため、CSFサンプル中では繰り返さなかった。
ブラックカラント添加後、cGP、IGF-1、IGFBP-1、IGFBP-2及びIGFBP-3における変化を分析するため、対応t検定を用いた。生物学的変化間の相関は、ピアソン検定を用いて計算した。0.05未満のP値を有意であるとみなす。
BCA中のcGP濃度のインビトロ分析
一元配置分散分析によると、cGPの濃度がBCAの異なる用量間で有意に異なることが示唆された(p<0.0001、n=5、図1)。cGPの濃度は、50mg/mlのBCAを用いる群において、低いBCA(5mg/ml)を用いる群と比べて有意に増加し(p=0.0001、n=5)、BCA用量が100mg/mlまで増加したとき、さらに増加した(p<0.0001、n=5)。
CSFのcGP濃度は、BCAの添加後、有意に増加した(7.27±0.67から12.12±0.94へ、p<0.001、n=6、図2A)。cGP濃度における平均百分率変化は、添加後、74.36%増加した(p<0.05、t(5)=3.989。全部で7対のサンプルの中で、添加後、それらの6つが増加を示した。患者1名が、CSF中で16.9倍のcGPの増加を示し(11.30ng/mlから191.80ng/mlへ)、それは異常値(平均の15.7倍)として統計分析から排除している。患者1名が添加に応答せず、添加後のcGP濃度は一定を維持した(8.8~8.7ng/ml、図2D)。IGF-1、cGP/IGF-1比(図2B及びC)、IGFBP-2及びIGFBP-3の濃度における変化はなかった(表2)。
BCAの添加後、cGP、IGF-1、cGP/IGF-1比(図3A~C)、IGFBP-1、2及び-3の濃度における統計学的変化はなかった(表2)。
表2は、添加前及び添加後のCSF及び血漿中でのcGP、IGF-1及びIGFBPにおける値を示す。IGFBP-1の濃度は、血漿中で低かった。IGFBP-3(3038~3029ng/mL)は、血漿中の主要なIGFBPであり、IGFBP-2(83.7~85.5ng/mL)は、CSF中の主要なIGFBPであった。CSF/血漿比は、IGF-1及びIGFBP-3双方において<1%であり;IGFBP-2において1.3~2%であり、またcGPにおいて52~71%であった(表2)。
ピアソン検定によると、CSF中のcGP濃度と血漿中のcGP濃度との間に有意な相関(R=0.68、p=0.01、図4A、n=12)、並びに血漿中のcGP/IGF-1の比とCSF中のcGP濃度との間に有意な相関(R=0.66、p=0.01、図4E、n=12)が示された。IGF-1濃度においてCSFと血漿との間に相関がなく(R=0.09、p=0.75、図4B)、且つCSFと血漿の双方におけるcGP濃度とIGF-1濃度との間に相関がなかった(R=0.04、p=0.85、図4D、R=-0.12、p=0.69、図4C)。
先に見出されているように、BCAは、天然栄養素であるcGPを含む。BCAの添加により、PD患者のCSF中でIGF-1ではなくcGPの増加がもたらされた。CSF中のcGP濃度は、血漿中のcGP濃度及びcGP/IGF-1のモル比と、血漿中での変化が有意でなかったとしても相関した。CSF中でのcGPの増加は、天然cGPの経口バイオアベイラビリティを裏付けている。血漿cGPの非統計学的変化がその原因である可能性が高かった。その機構の解明により、BCAがIGF-1機能を正常化するための介入であってもよいことのさらなる科学的証拠が得られることになる。
結論として、cGPはBCAの栄養素であり、ヒトにおいて経口的に生体利用可能であり、血漿中及びCSF中でのcGPの増加はIGF-1の総量を変えなかったが、IGF-1のバイオアベイラビリティを変えた。
本実施例において、cGP濃度及びcGP/IGF-1モル比における、年齢に関する変化が高齢者において調査された。試験のため、全部で29名をリクルートし、認知機能が正常である参加者が、2つの異なる年齢のグループ(70歳超又は70歳未満)に分けられた。生物学的検体を血漿の形態で参加者から採取した。血漿中のIGF-1及びIGFBP-3の濃度をELISAにより測定し、血漿cGPを高性能液体クロマトグラフィー質量分析アッセイにより測定した。
本実施例において、発明者は、cGPの血漿濃度及びcGP/IGF-1モル比と、運動機能の回復及び臨床成績との関連を調査した。
上記試験では、尿であっても有用な生物学的検体となるか否かを試験するため、試験に登録した患者から尿サンプルも採取した。試験中、全部で71の尿サンプルを採取し(対照30名、脳卒中患者33名)、これらのサンプルを上記方法と同様の様式で分析した。相関が存在することを確認するため、より早期の試験から脳卒中患者及び高齢対照におけるさらなる37の血漿サンプルも比較した。
本実施例において、試験は、レット症候群(RTT)と遺伝子診断された6歳少女の、IGF-1、メラトニン(MT)、ブラックカラント抽出物(BCA)、及びリハビリテーションで6か月間処置されてからの良好な進化を説明する。患者は、1歳時から正常な発達を停止した。
本実施例において、脳卒中後3日以内の環状グリシン-プロリン(cGP)の血漿濃度と、いかに血漿cGPが短期的な脳卒中回復を予測することがあるかを評価するため、実施例3に対するさらなる更新試験を完了した。
年齢適合対照対象50名(女性/男性が35名/15名)及び脳卒中を有する者34名(女性/男性が15名/19名)を脳卒中の3日以内にリクルートした。臨床的評価は、ベースライン(<3日)時、次いで7日目及び90日目のNIH脳卒中スケール(National Institutes of Health Stroke Scale)(NIHSS)、並びに7日目及び90日目の修正ランキンスケール(modified Rankin Scale)(mRS)及びFugl-Meyer Upper Limb Assessmentスケール(FM)を含んだ。
対照参加者は、有意な神経障害(NIHSS中央値(範囲):0(0-2))も全身体障害(global disability)(mRS中央値(範囲)0(0-1))も有しておらず、正常な上肢機能(FM中央値(範囲):66(65-66))を有した。脳卒中患者では、経時的に脳卒中重症度(NIHSS)及び身体障害(mRS)が低下し、上肢機能(FM)が改善した。例えば、NIHSSスコアは経時的に改善した(F(2,20)=27.48、p<.001、n=21、図14A)。ベースラインと比べて、NIHSSスコアは、7日(中央値:2対4、p=0.01)及び90日(中央値:1対4、p<0.001)までに低下した。FMスコアが改善したように(中央値:64対65、p=0.001、n=21、図14C)、mRSスコアも改善した(中央値:3対2、p=0.003、n=21、図14)。
この試験は、脳卒中発症後、短期的に(<3日)、cGP、IGFBP-3の血漿濃度及びcGP/IGF-1モル比が減少することを示しており、IGF-1の自己分泌調節の障害を示唆している。次の90日にわたり、脳卒中患者は、神経学的機能及び全身体障害における改善を示した。これは血漿cGP及びcGP/IGF-1モル比の漸増と並行している。ベースライン時により高いcGP/IGF-1モル比を有する患者は、脳卒中後90日目、神経学的機能におけるより良好な回復を来した。これらの結果は、脳卒中回復におけるIGF-1の自己分泌調節の役割を示唆してもよい。cGP/IGF-1モル比における急激な変化は、脳卒中からの回復の能力を予測してもよい。
いて低酸素性-虚血性脳損傷後の最初の2時間で、>100%まで増強される。したがって、我々は、脳損傷が血漿からのcGPの中枢取り込みを促進し、それ故、脳卒中の急性期の間、血漿cGPレベルを低下させたという可能性を排除することができなかった。
低いcGP濃度及びモルcGP/IGF-1比は、脳卒中におけるIGF-1の自己分泌調節の障害を示唆する。cGP濃度及びモルcGP/IGF-1比の増加は、3か月後の臨床的回復に関連してもよい。入院時のcGP/IGF-1モル比は、より大規模な試験においてさらに確認される場合、脳卒中患者における回復を予測するのに臨床的に役に立ってもよい。さらに、高血圧患者におけるcGP濃度の漸減もまた、脳卒中リスクを示してもよい。
Claims (33)
- 動物におけるIGF-1機能障害に関連した非神経学的及び/又は神経学的状態を治療する方法における使用のための、環状グリシン-プロリン(cGP)を含有する有機又は植物ベースの材料を含む、薬剤であって、前記方法が、
a)前記動物から生物学的検体を得るステップと;
b)前記生物学的検体中でのインスリン様増殖因子1(IGF-1)のバイオアベイラビリティ、すなわち機能を計算するためにIGF-1及びcGPの濃度を測定するステップと;
c)前記生物学的検体中の前記cGP濃度及び/又はcGP/IGF-1のモル比のいずれかを、1週~3~6か月~2年にわたり連続した時点での、病状のない個別動物について収集されたデータセット又はベースラインとして早期/初期段階から収集されたデータセットに基づく標準と、病状のある個別動物との間で比較することで、結果の連続体の中で、又は病状が相対標準と比較して前記cGP濃度及び/又はcGP/IGF-1のモル比を変えたかどうかを確認し、この差が、疾患経過及び治療応答における個体のIGF-1機能変化を評価するために役立つステップと;
d)cGPを含有する有機又は植物ベースの材料を治療有効量で前記動物に投与し、
IGF-1の機能低下を阻止するために、動物におけるcGPの濃度及び/又はcGP/IGF-1のモル比における減少を阻止し;及び/又は
IGF-1の機能を維持するために、動物における既存のcGPの濃度及び/又はcGPのIGF-1総測定量に対するモル比を維持し;及び/又は
IGF-1の機能を改善するために、動物におけるcGPの前記濃度及び/又はcGPのIGF-1総測定量に対するモル比を増加させるステップと、
を含み、
前記非神経学的及び/又は神経学的状態又は疾患が、脳血管発作若しくは脳卒中、軽度認知障害(MCI)、アルツハイマー病、血管性認知症、レット症候群(他の発達神経学的状態を含む)、脳震盪、高血圧症及びそれに伴う脳合併症、パーキンソン病の認識機能障害及び/又は任意の他の加齢関連状態若しくはIGF-1欠損関連状態である、
薬剤。 - 投与により、疾患又は状態の結果としてのcGP濃度における減少が停止する、請求項1に記載の薬剤。
- 投与により、cGP濃度が投与のない前記患者において測定される場合よりも少なくとも1%高く増加する、請求項1に記載の薬剤。

c)前記生物学的検体中での前記測定されたcGP濃度及び/又はcGP/IGF-1のモル比のいずれかを、1週~6か月~2年にわたり、病状のない個別動物について収集されたデータセット又はベースラインとして早期/初期段階から収集されたデータセットに基づく標準と比較し、結果の連続体の中で、前記測定されたcGP濃度及び/又はcGPのIGF-1総測定量に対するモル比の、前記個体の有効な治療計画を設計するためのIGF-1機能変化を評価するための相対標準(ベースライン)と比べての差異を確認するステップと;
