Description
本出願は、2019年8月12日出願の米国仮特許出願第62/885,530号の利益を主張し、その優先権の利益はここに主張され、参照によりその全体が本明細書に組み込まれる。
微生物は一般的に関節インプラント上およびその近傍に認められる。関節置換術の修正は、感染症のために頻繁に行われる。しかし、現在のところ、微生物は、例えば抗生物質と比較して、ヨウ素に対する耐性を持たない。ヨウ素は、防腐剤として医学分野で歴史があり、甲状腺機能のために人体にすでに存在している。
様々な実施形態において、インプラントは、未架橋のまたは架橋した、ヨウ素注入ポリエチレンを含むことができる。ヨウ素は、例えば、ポリエチレン中約5~約3000μg/cm3(例えば、約200~約1000μg/cm3)の濃度を有することが可能である。いくつかの実施形態では、ヨウ素はポリエチレン全体に均一に分布している。いくつかの実施形態では、ポリエチレンはヨウ素で飽和されている。いくつかの実施形態では、ポリエチレンの一部分がヨウ素で選択的に処理され、他の部分はヨウ素で処理されない。
本明細書に開示されるのは、様々な実施形態において、ポリエチレンインプラントにヨウ素を注入する方法である。この方法は、ポリエチレンにヨウ素が注入されるように、ポリエチレンをヨウ素溶液などのヨウ素源に曝露することを含みうる。溶液は、例えば、ヨードフォア(例えば、ポビドンヨード)などのヨウ素溶液、または溶剤含有溶液などの他のヨウ素水溶液を含むことができる。一例では、ルゴール溶液を使用できる。ポリエチレンは、例えば、樹脂形態、部分的に圧密された形態、または完全に圧密された形態であり得る。
様々な実施形態において、本発明は、ヨウ素を注入する前に、ビタミンEなどの1または複数の酸化防止剤をポリエチレンに添加する方法を提供する。適切な酸化防止剤は上記に詳細に記載されている。

様々な実施形態において、この方法は、方法の任意の適切な段階で(例えば、照射工程の後)、酸化防止剤を脱保護することを含むことができる。脱保護は、加水分解(例えば、水溶液としての水でまたは空気中での水への曝露)などの任意の適切な手段を介して行うことができる。
圧密されたポリエチレンインプラントは、注入の前または後に照射することができる。照射は、圧密後にポリエチレンインプラントに架橋を誘導する照射を含むことができる。代替的または追加的に、注入後にインプラントに照射を行うことができる。照射は、例えば、電子線またはガンマ線照射によって行うことができる。一般に、照射は、例えば、約60℃~約300℃の温度で行うことができ、その結果、架橋が誘導される。場合により、架橋を誘導する照射は、ポリエチレンが溶融しない温度で行うことができる。照射は、例えば、約1 kGy~約100,000 kGyの総照射線量で行うことができる。
ポリエチレンインプラントにヨウ素を注入することが可能である(または代わりに、成形前にヨウ素をポリエチレンに混合することもできる)。いくつかの実施形態において、ポリエチレンは、ヨウ素で飽和させることができる。遊離型ヨウ素は、インビボでのヨウ素の抗菌性放出を促進するのに魅力的である。ポビドンヨードは、ヨウ素を水溶性にすることができるヨウ素含有ポリビニルポリマーである。ポビドンヨードは、ポビドン(PVP)と、ヨウ化水素と、元素型ヨウ素との化学複合体であり、化学構造は次のとおりである:
ヨウ素注入ポリエチレンサンプルは、以下の表1に要約される材料を使って調製した。
実施例1からのサンプルを50℃に維持し、経時的なヨウ素の放出について試験した。放出実験は、人体の場合と比較して誇張された条件のもとで実施した。サンプルを約50℃の水浴に入れた。サンプル条件を以下の表3に要約する。
サンプルは、酸化的分解、摩耗分析、および機械的特性についても試験した。サンプルを、ASTM F2003-02(2015)に従って、高温および高酸素圧を使用してエージングした。エージングしたサンプルを、UHMWPE(入手した時のまま)、エージングしたUHMWPE、および他のエージングしたポリマーサンプルと比較した。試験したサンプルは、以下の表4に要約される。
ヨウ素注入ポリエチレンサンプルは、図2に示すピン・オン・プレート(pin-on-plate)分析を使用する摩耗分析についても試験した。サンプルの重量を測定して、ピン・オン・プレート摩耗分析の回数あたりの平均累積質量損失を示した。図2に示されるように、ヨウ素注入されたがエージングしていないUHMWPEのサンプルを、未使用UHMWPEと比較した。ここで、未使用(virgin)UHMWPEは、1,000,000回数での試験で最大0.0072 gの平均累積質量損失を示した。これに対して、ヨウ素注入サンプルの場合、1,000,000回数の試験で約0.0028 gの平均累積質量損失を示した。
以下の表5に要約されている材料を使用して、ヨウ素注入ポリエチレンサンプルの第二のセットを調製した。

Claims (8)
- コールドシンタリングされ、架橋した、ヨウ素注入ポリエチレンを含むインプラントであって、ポリエチレンインプラント中のヨウ素の平均濃度が約5~約3000μg/cm3であるインプラント。
- 前記ヨウ素が、ポリエチレンインプラント中約200~約1000μg/cm3の平均濃度を有する、請求項1に記載のインプラント。
- 前記ポリエチレンが、超高分子量ポリエチレン、超低分子量ポリエチレン、高密度ポリエチレン、高分子量ポリエチレン、高密度架橋ポリエチレン、中密度ポリエチレン、低密度ポリエチレン、線状低密度ポリエチレン、超低密度ポリエチレン、分岐ポリエチレン、またはそれらの組み合わせである、請求項1に記載のインプラント。
- 前記ポリエチレンが、トコフェロール、トコフェロール亜リン酸エステル、トコトリエノール、ビタミンE、ビタミンE酢酸エステル、ビタミンE亜リン酸エステル、ローズマリーオイル、ペンタエリスリトールテトラキス(3-(3,5-ジ-tert-ブチル-4-ヒドロキシフェニル)プロピオネート)、ブタン二酸ジメチルエステル/4-ヒドロキシ-2,2,6,6-テトラメチル-1-ピペリジンエタノール共重合体、タンニン酸、ビルベリー抽出物、ビタミンC、カロテン、フラボノイド、イソフラボノイド、ネオフラボノイド、リグニン、キニン、ユビキノン、ビタミンK1、金属、グルタチオン、没食子酸プロピル、没食子酸オクチル、没食子酸ラウリル、レスベラトロール、ロスマリン酸、ルチン、5-アミノサリチル酸、ブチル化ヒドロキシアニソール、ブチル化ヒドロキシトルエン、フェノール系化合物、および単量体形または重合体形のヒンダードアミン安定剤のうちの少なくとも1つを含む酸化防止剤を含んでなる、請求項1に記載のインプラント。
- 前記酸化防止剤が、ポリエチレンの約0.01重量%~約5.0重量%である、請求項4に記載のインプラント。
- 前記ポリエチレンが、ピン・オン・プレート分析における摩耗分析の約10,000回数~約1,300,000回数にわたって約0.001g~約0.005gの平均累積質量損失を有する、請求項1に記載のインプラント。
- 前記ポリエチレンが約250MPa~約400MPaの弾性率を有する、請求項1に記載のインプラント。
- 前記ポリエチレンが、約475%~約515%の破断点伸びを有する、請求項1に記載のインプラント。
