Description
本明細書に記載されるTAG72標的CARは、scFvを標的化するTAG72(例えば、以下のアミノ酸配列:
本明細書に記載されるCARは、スペーサーを、TAG72標的標的ドメイン(すなわち、TAG72標的ScFv又はその変異体)と膜貫通ドメインとの間に位置させて含みうる。様々な異なるスペーサーを用いうる。それらのいくつかは、ヒトFc領域の少なくとも部分、例えば、当該ヒトFc領域のヒンジ部分、又はCH3ドメイン若しくはその変異体を含む。以下の表1に、本明細書で提供されるCARにおいて用いられうる様々なスペーサーを記載する。

様々な膜貫通ドメインを用いうる。表2には、適当な膜貫通ドメインを例示する。スペーサー領域が存在する場合、膜貫通ドメインは、スペーサー領域のカルボキシ末端にある。

共刺激ドメインは、CD3ζシグナル伝達ドメインと共に用いるのに適するいかなるドメインでありうる。ある場合、共シグナル伝達ドメインは、KRGRKKLLYIFKQPMRPVQTQEEDGCRFEEEGCEL(配列番号24)と、少なくとも90%、少なくとも95%、少なくとも98%又はと同一である配列があげられる4-1BB共シグナル伝達ドメインである。ある場合、当該4-1BB共シグナル伝達ドメインは、配列番号24と比較して、(好ましくは、保存的)1、2、3、4又は5個のアミノ酸変化がある。
当該共刺激ドメインは、膜貫通ドメインとCD3ζシグナル伝達ドメインの間に位置する。表3に、CD3ζシグナル伝達ドメインの配列と共に、適当な共刺激ドメインを例示する。

CD3ζシグナル伝達ドメインは、CD3ζシグナル伝達ドメインと共に用いるのに適するいかなるドメインでありうる。ある場合、CD3ζシグナル伝達ドメインは、以下の配列:RVKFSRSADAPAYQQGQNQLYNELNLGRREEYDVLDKRRGRDPEMGGKPRRKNPQEGLYNELQKDKMAEAYSEIGMKGERRRGKGHDGLYQGLSTATKDTYDALHMQALPPR(配列番号21)と少なくとも90%、少なくとも95%、又は少なくとも98%同一又は同一である配列を含む。ある場合では、当該CD3ζシグナル伝達は、配列番号21と比較して、1、2、3、4又は5個(好ましくは保存的)がアミノ酸変化する。

当該配列では、EGFRtの共発現により、遺伝子修飾細胞が正確に測定されえ、遺伝子修飾細胞の正の選択、並びに養子移入後の治療用T細胞のインビボで効率的に細胞追跡しうる不活性な非免疫原性表面マーカーが提供される。サイトカインストーム及び標的外毒性を回避するための効率的な増殖制御は、T細胞免疫療法の成功にとって重要なハードルである。TAG72 CARレンチウイルスベクターに組み込まれたEGFRtは、自殺遺伝子として作用し、治療関連毒性の場合には、CAR+ T細胞を除去しうる。
本発明は、特許請求の範囲に記載された本発明の範囲を限定しない以下の実施例においてさらに説明される。
以下の材料及び方法を、本明細書に記載した実施例で用いた。
上皮性卵巣がん株OVCAR3(ATCC HTB-161)を、20%ウシ胎児血清(FBS、Hyclone)及び1X抗生物質抗真菌剤(1XAA、Gibco)(完全RPMI)含有RPMI-1640(Lonza)で培養した。転移性腹水由来の上皮性卵巣がん株OV90(CRL11732)を、水酸化ナトリウム(Sigma)及び最終濃度20%FBS及び1×AAを用いてpH7.0に調整したMCDB105培地(Sigma)及び培地199(Thermo)の1:1混合液中で培養した。上皮性類子宮類卵巣がん株COV362.4(Sigma)を、10%FBS、1XAA、25mM HEPES(Irvine Scientific)及び2mM L-グルタミン(Fisher Scientific)(完全DMEM)含有ダルベッコ改変イーグル培地(DMEM、LifeTechnologies)で培養した。上皮性卵巣がん株OVCAR8は、寛大にもシティオブホープのCarlotta Glackin博士から供与されたものであり、完全RPMI-1640で培養した。上皮性卵巣がん株SKOV3(ATCC HTB‐77)と結腸上皮がん株LS174T(ATCC CL‐188)を完全DMEMで培養した。すべての細胞を5% CO237℃で培養した。
腫瘍細胞を、既報(22;Priceman SJ, Gerdts EA, Tilakawardane D,Kennewick KT, Murad JP, Park AK, Jeang B, Yamaguchi Y, Yang X, Urak R, Weng L, Chang WC, Wright S, Pal S, Reiter RE, Wu AM, Brown CE, Forman SJ. Co-stimulatory signaling determines tumor antigen sensitivity and persistence of CAR T cells targeting PSCA+ metastatic prostate cancer. Oncoimmunology. 2018; 7(2):e1380764)のEF1αプロモーターの制御下でeGFP/fluc融合を担持するeHIV7レンチウイルスを用いた形質導入により、増強緑色蛍光タンパク質及びホタルルシフェラーゼ(eGFP/fluc)を発現するように改変した。当該CAR構築物に用いたヒト化scFv配列は、TAG72を標的とするモノクローナル抗体クローンhuCC49から得られた(17)。細胞外スペーサードメインは、IgG4 Fcスペーサーの129アミノ酸の中長CH2欠失型(ΔCH2)を含んでいた(Jonnalagadda M, Mardiros A, Urak R, Wang X, Hoffman LJ, Bernanke A, et al. Chimeric antigen receptors with mutated IgG4 Fc spacer avoid fc receptor binding and improve T cell persistence and antitumor efficacy. Mol Ther. (2015) 23:757-68)。含まれる細胞内の補助刺激シグナル伝達ドメインは、CD4膜貫通ドメインを有する4-1BBであった。CD3ζ細胞溶解ドメインは、既報である(22)。当該CAR配列を、T2Aリボソームスキップ配列により切断CD19遺伝子(CD19t)から分離し、EF1αプロモーターの制御下で、epHIV7レンチウイルス骨格中でクローニングした。
白血球除去輸血製品は、シティオブホープ内部審査会(IRB)により承認されたプロトコルに基づき、同意を得た研究参加者(健康なドナー)から入手した。白血球除去輸血の日に、末梢血単核細胞(PBMC)を、Ficoll-Paque (GEヘルスケア)上での密度勾配遠心分離、続いてPBS/EDTA (MiltenyiBiotec)で複数洗浄して単離した。細胞を室温(RT)で回転子上に一晩静置し、次いで洗浄し、10% FBS(完全X-VIVO)を含むX-VIVO T細胞培地(Lonza)に再懸濁した。最大5.0×109個のPBMCを、抗CD14及び抗CD25マイクロビーズ(MiltenyiBiotec)と共に室温で30分間インキュベートし、製造業者のプロトコルに従ってCliniMACS(登録商標)システム(MiltenyiBiotec)を用いて磁気的に除去し、これらを枯渇PBMC(dPBMC)とした。dPBMCをCryoStor(登録商標)CS5(StemCell Technologies)で、次の処理まで凍結した。
フローサイトメトリー分析のため、細胞をFACS緩衝液(Ca2+、Mg2+又は2%FBS及び1×AA含有フェノールレッド(HBSS-/-、LifeTechnologies)非含有Hank平衡食塩溶液)に再懸濁した。細胞を一次抗体と共に暗所4℃で30分間インキュベートした。二次染色には、Brilliant Violet 510(BV510)、フルオレセインイソチオシアネート(FITC)、フィコエリトリン(PE)、ペリジニンクロロフィルタンパク質複合体(PerCP)、PerCP-Cy5.5、PE-Cy7、アロフィコシアニン(APC)又はAPC-Cy7(又はAPC-eFluor780)結合抗体を用いて、暗所で4℃30分間インキュベートする前に細胞を2回洗浄した。CD3(BDBiosciences、クローン:SK7)、CD4(BDBiosciences、クローン:SK3)、CD8(BDBiosciences、クローン:SK1)、CD14(BDBiosciences、クローン:MΦP9)、CD19(BDBiosciences、クローン:SJ25C1)、CD25(BDBiosciences、クローン:2A3)、マウスCD45(BioLegend、クローン:30-F11)、CD45(BDBiosciences、クローン:2D1)、CD69(BDBiosciences、クローン:L78)、CD137(BDBiosciences、クローン:4B4-1)、MUC1(BioLegend、クローン16A)、MUC16(Abcam、クローンX75又はEPSISR23)、ビオチン化Protein-L(GenscriptUSA)(Zheng Z, Chinnasamy N, Morgan RA. Protein L: a novel reagent for the detection of chimeric antigen receptor (CAR) expression by flow cytometry. J Transl Med. (2012) 10:29)、TAG72(クローン、(Invitrogen)、ヤギ抗マウス免疫グロブリン(BDBiosciences)、及びストレプトアビジン(BDBiosciences)を用いた。細胞生存率は、4’、6-ジアミジノ-2-フェニルインドール(DAPI、Sigma)を用いて測定した。フローサイトメトリーをMACSQuantAnalyzer10(MiltenyiBiotec)で行い、データをFlowJoソフトウェア(v10、TreeStar)で分析した。
腫瘍殺傷アッセイのため、CAR T細胞及び腫瘍標的を、96穴プレート中の外因性サイトカインの非存在下で、完全X-VIVO中の示されたエフェクター:腫瘍(E:T)比で24~72時間共培養し、上記のようにフローサイトメトリーで分析した。CAR T細胞による腫瘍殺傷は、標的をMock(非形質導入)T細胞との共培養で観察されたものと比較してCD45陰性細胞数を比較して、算出した。T細胞活性化アッセイのため、CAR T細胞及び腫瘍標的を、所定の時点のため、96穴プレート中の外因性サイトカインの非存在下で、完全X-VIVO中で示されたE:T比で共培養し、そしてT細胞活性化の特異的マーカーについてフローサイトメトリーで分析した。凍結培養されていない患者の原発性卵巣がん腹水(OAS3、OAS4、及びOAS7)を解凍し、TAG72発現について直接分析し、T細胞機能アッセイで評価した。簡潔には、シティオブホープ国立医療センター(COH)の外科スタッフから、COH内部審査会(IRB)及び検体保護局の承認を得て、滅菌真空容器で腹水を採取した。COH IRBは、既報のとおり(26)、すべての検体は識別されず、腹水材料を廃棄したため、書面によるインフォームドコンセントの必要性を撤回した。
プレート結合TAG72抗原上の腫瘍殺傷アッセイ又はCAR T細胞活性化アッセイ由来の上清を、所定の時間に収集し、さらなる使用のために-20℃で凍結した。次に、分泌されたヒトIFNγ及びIL-2について各々、ヒトIFNγ及びIL-2 ELISA Ready-SET-GO!(登録商標)ELISAキット製造業者のプロトコルに従って上清を分析した。WallacVictor3 1420 Counter(Perkin-Elmer)及びWallac 1420 Workstationソフトウェアを用いて、450nmでプレートを読み取った。
すべての動物実験は、シティオブホープ施設動物ケア・使用委員会が承認したプロトコルに基づいて実施した。インビボ腫瘍研究のため、OVCAR3及びOV90細胞(5.0×106)を最終容量の500μlのHBSS-/-で調製し、6~8週齢の雌NSGマウスに腹腔内(i.p.)注射により生着させた。腫瘍の増殖は、少なくとも週1回、バイオホトニック画像(Xenogen、LagoX)により追跡し、流束シグナルを生体画像ソフトウェア(Xenogen)で分析した。画像化のために、マウスに、4.29mg/マウスのPBS中に懸濁した150μLのD-ルシフェリンカリウム塩(PerkinElmer)を腹腔内注射した。いったん流束シグナルが所望のレベルに達すると、OV90については8日目、OVCAR3については14日目に、T細胞を1XPBS中で調製し、マウスに5.0×106Mock又はTAG72-BBζ CAR T細胞を500μL又は200μL静脈内(i.v.)注射した。OV90腫瘍モデルでは、腹腔内投与による反復処理の効果を試験した。TAG72-BBζ CAR T細胞を8日目に開始し、その後、腫瘍生着後追加の指示日に投与した場合の影響を検討した。人道的終点を用いて生存期間を決定した。腹水による腹部膨満、努力性又は呼吸困難、明確な体重減少、可動性障害、瀕死状態のエビデンス等の苦痛の徴候が認められた場合、マウスを安楽死させた。所定の時点又は瀕死状態で、マウスを安楽死させ、組織及び/又は腹水を収集し、以下に記載するようにフローサイトメトリー及び免疫組織化学用に処理した。
腫瘍組織を4%パラホルムアルデヒド(4%PFA、BostonBioProducts)中で3日間まで固定し、次の処理まで70%エタノール中で保存した。免疫組織化学はシティオブホープ病理学コアが行った。簡潔には、パラフィン包埋切片(10μm)を、ヘマトキシリン・エオシン(H&E、Sigma-Aldrich)、マウス抗ヒトCD3(DAKO)、マウス抗ヒトTAG72(AB16838、Abcam)、ウサギ抗ヒトMUC1(AB45167、Abcam)、MUC16(AB1107、Abcam)で染色した。画像は、Nanozoomer 2.0HTデジタルスライドスキャナ及び関連するNDP.view2ソフトウェア(Hamamatzu)を用いて獲得した。
データは、特断の限り、平均値±標準誤差で示す。別段の記載がない限り、対のない両側スチューデントのt検定を用いて群間の統計的比較を行い、p値を算出した。*p<0.05、**p<0.01、***p<0.001;NS(有意ではない)。
4-1BB細胞内補助刺激ドメインを含むTAG72-CAR T細胞が精製されたTAG72に対して効果的に活性化を示すか否かの決定のため、上記細胞を、可溶性TAG72又はプレート結合TAG72のいずれかで漸増量の存在下で増殖させ、活性化の指標であるCD137発現を測定した。
既報22のように、TAG72-BBζ CARレンチウイルスを用いて、CD14+及びCD25+細胞(dPBMC)を除去したヒト健常ドナー由来の末梢血単核細胞を形質導入した。TAG72-BBζ CAR T細胞は、調製工程中(CD19t+選択に基づく)に濃縮され、T細胞表面に安定に発現した(図1B)。CAR T細胞はex vivoで増殖し、その動態は類似し、Mock(非形質導入)T細胞に対するCD4:CD8比は同等であった(図1C)。重要なことに、TAG72に対するCAR T細胞の活性化の最初の尺度として、TAG72-BBζ CAR T細胞は、プレート結合したが非可溶性精製TAG72と共に培養した場合、表面上で用量依存性のCD137発現を示した(図1D)。さらに、TAG72-BBζ CAR T細胞は、血小板結合TAG72(非可溶性精製TAG72)と共に培養した場合、他の活性因子指標、特に細胞表面CD69発現及びIFNγ放出の用量依存的誘導を示した(図2)。
TAG72-BBζ CAR T細胞がTAG72陽性がん細胞に対して選択的活性を示すか否かの決定のため、TAG72-BBζ CAR T細胞をTAG72陽性又はTAG72陰性卵巣がん細胞の存在下で増殖させ、卵巣がん細胞の死滅率を定量した。
TAG72陽性がん細胞に対するTAG72-BBζ CAR T細胞の選択的活性(標的細胞の標的化及び細胞死の付与を含む)の評価の第一段階として、SKOV3、OVCAR8、COV362.4、OVCAR3、OV90を含むヒト卵巣がん細胞株及びTAG72+結腸がん細胞株LS174TにおけるTAG72発現を評価し、TAG72陽性がん細胞株を同定した。以前の研究では、卵巣腫瘍患者サンプルの免疫組織化学及びヒト卵巣がん細胞株のウェスタンブロット法により、TAG72の発現が示された(非特許文献11;Ponnusamy MP, Venkatraman G,Singh AP, Chauhan SC, Johansson SL, Jain M, Smith L, Davis JS, Remmenga SW, Batra SK. Expression of TAG-72 in ovarian cancer and its correlation with tumor stage and patient prognosis. Cancer letters.2007;251(2):247-57)。フローサイトメトリーで、TAG72はOVCAR3細胞(約42%)及びOV90細胞(約90%)でより多く発現され、COV362.4細胞では非常に低いレベルで検出された(図3A)。TAG72は、SKOV3及びOVCAR8細胞上に存在しなかった。腫瘍細胞の免疫蛍光染色により、TAG72の発現及び細胞表面及び細胞内での細胞局在が確認された。特に、腫瘍動物の腹水から採取したOVCAR3及びOV90細胞上のTAG72の発現は、インビトロ培養細胞と比較して高かった(図4)。
さらに、TAG72が卵巣がんCAR標的であり、本発明者らのTAG72‐BBζCAR T細胞の抗腫瘍活性を確認するため、TAG72‐BBζCAR T細胞を3名の患者(OAS3、OAS4、OAS7)のヒト卵巣がん腹水の存在下で増殖させた。
OAS3、OAS4、及びOAS7由来の、解凍されたばかりの腹水は、フローサイトメトリーで各々、62%、80%、及び67%のTAG72を発現したが、培養72時間後には各々2%、53%、及び19%に低下した(図3F)。特定の理論に縛られることを望まないが、TAG72発現低下は、TAG72発現の維持に対するex vivo培養条件の影響を反映しうる(Horan Hand P, Colcher D, Salomon D, Ridge J, Noguchi P, Schlom J. Influence of spatial configuration of carcinoma cell populations on the expression of a tumor-associated glycoprotein. Cancer research. 1985; 45(2):833-40)。TAG72-BBζ CAR T細胞は、腹水との共培養72時間後に細胞溶解活性を示し、TAG72-陰性OAS3細胞に対して検出可能な抗腫瘍活性を示さず、TAG72-陽性OAS4及びOAS7細胞の強力かつ選択的なCAR媒介性殺傷を示した(図3G)。TAG72-BBζ CAR T細胞は、OAS4に対してIFNγ及びIL-2を産生したが、OAS3及びOAS7細胞に対しては産生しなかった(図3H)。これらのデータは、TAG72‐CAR T細胞がインビトロで卵巣がん腹水由来のTAG72陽性細胞を選択的に標的化することを示唆する。
TAG72-BBζ CAR T細胞のインビボにおける治療可能性を評価するため、免疫不全NSGマウスでTAG72-BBζ CAR T細胞がTAG72陽性OVCAR3腫瘍を選択的に標的とする能力を試験した。このマウスモデルは、後期ヒト疾患で観察される腹膜卵巣腫瘍に類似する。TAG72‐BBζCAR T細胞を腹腔内(i.p.)注射により送達した。
OVCAR3細胞をレンチウイルスで形質導入してeGFP/fflucを発現させ、非侵襲的光学イメージングを介して腫瘍増殖を追跡させえた。腫瘍腹腔内投与注射の14日後、マウスをMock又はTAG72-BBζ CAR T細胞(5.0×106)で全身静脈内(i.v.)又は局所腹腔内投与送達により処理した(図6A)。局所腹腔内投与送達を介してTAG72-BBζ CAR T細胞で処置したマウスでは、迅速な抗腫瘍効果が観察され、処置の1~2週間後に最大の抗腫瘍反応に達した(図6B及び6C)。局所送達と比較して、TAG72‐BBζCAR T細胞の経静脈投与送達の抗腫瘍応答は限定された。マウスの抗腫瘍反応は3~4週間持続したが、最終的にはマウスで腫瘍再発が観察された。TAG72-BBζ CAR T細胞の局所腹腔内投与送達は、マウスの生存を有意に延び、経静脈投与送達により観察された利益は限定されていた(図6D)。
OV90腹腔内投与モデルにおける選択的標的TAG72陽性細胞に対するTAG72-BBζ CAR T細胞の効能を評価するため、TAG72-BBζ CAR T細胞を単回又は複数回反復投与し、経時的に腫瘍サイズを評価した。
特に、OV90腹腔内投与モデルは、OVCAR3と比較して、インビトロでより均一なTAG72発現を示す(図3A)。OV90腹腔内投与モデルにおける局所CAR T細胞送達は、OVCAR3モデル腹腔内投与と比較してTAG72細胞の選択的標的化を示した。対照的に、静脈内投与のTAG72-BBζ CAR T細胞処理は、OV90モデルでは抗腫瘍効果を示さなかった(図8)。このモデルでは、おそらくこのモデルの積極的な性質により、TAG72-BBζ CAR T細胞の腹腔内投与により、全生存期間が約25日延びただけであった(図8)。この観察結果を踏まえて、単回投与と比較したTAG72-BBζ CAR T細胞の反復投与の効能が評価され、治療応答が明らかに改善された(図9A)。TAG72-BBζ CAR T細胞の単回投与と比較して、1ヵ月にわたる反復投与は、OV90モデルにおいてより持続的な抗腫瘍反応を示した(図9B及び9C)。相対腫瘍増殖動態としてプロットすると、反復投与は単回投与と比較して腫瘍のより広範な退縮を促進し、腫瘍のより持続的な制御を促進した(図9D)。
上記実施例でTAG72-BBζ CAR T細胞が腫瘍再発前に低下することが観察されたことを考慮し、腫瘍におけるTAG72の発現を経時的に定量化し、TAG72発現の消失がTAG72-BBζ CAR T細胞数の低下と相関するかを判定した。
CAR T細胞療法に対する主要な耐性機序の一つは、最終的な抗原喪失又は逃避を促進する固形腫瘍に存在する腫瘍抗原の不均一性である(非特許文献4)。2つのインビボモデル(上記実施例の)におけるCAR T細胞の消失が腫瘍再発前におこったことを考えると、腫瘍におけるTAG72発現の潜在的消失は、CAR T細胞の消失と相関する。前者を評価するため、Mock及びTAG72-BBζ CAR T細胞処理マウス由来の腫瘍におけるTAG72の発現を、治療前及び治療後で経時的に測定した。TAG72、MUC1、及びMUC16は全て卵巣がんの潜在的標的として同定されたので、TAG72陰性OVCAR8、及びTAG72陽性OVCAR3及びOV90細胞上の当該細胞表面抗原の発現を定量した。OVCAR8によるMUC1の発現はわずかに低レベルであり、TAG72及びMUC16では存在しなかったが、OVCAR3は3種類の抗原すべてを様々なレベルで発現し、OV90によるMUC1の発現は低く、MUC16では発現しなかった(図11A)。したがって、Mock又はTAG72-BBζ CAR T細胞で処理したマウス由来のOVCAR3腫瘍における当該抗原の発現を定量した。T細胞注入の12週間後に、Mock処理マウス由来の腫瘍は、TAG72(細胞系のフローサイトメトリー分析に類似)、MUC16、及びMUC1の発現は不均一であった(図11B)。しかし、TAG72-BBζ CAR T細胞で処理したマウスからの初期時点での腫瘍再発は、MUC16及びMUC1の発現を維持しつつ、TAG72発現が劇的に低下した。同様に、OV90腫瘍モデルにおけるTAG72-BBζ CAR T細胞の反復処理によってもまた、治療後の早期再発腫瘍におけるTAG72発現が低下した(図11E)。特に、TAG72の発現は、後の時点での腫瘍再発、固形腫瘍及び腹水において高レベルで検出された(図11C及び11D)。この所見をさらにインビトロで確認するため、CAR T細胞共培養後の腫瘍細胞におけるTAG72の発現を定量化し、CAR T細胞との共培養の非存在下で増殖した腫瘍細胞と比較して低下していることが見いだされた(図11E)。全体として、当該データは、抗原逃避がTAG72‐BBζCAR T細胞療法後の腫瘍再発に重要な機能を担いうることを示唆する。
ヒト化TAG72-BBζ CAR T細胞もまたTAG72陽性がん細胞を効果的に殺傷するかを判定するため、ヒト化TAG72-BBζ CAR T細胞をTAG72陽性卵巣がん細胞の存在下で増殖させ、死滅した卵巣がん細胞の比率を定量した。
抗TAG-72抗体クローンCC49のヒト化変異体(IDEC-TAG72-BBz、V15-TAG72-BBz、又はV59/15-TAG72-BBz)から、IDEC、V15、又は複合V59/V15抗原結合ドメイン(scFv)のいずれかを発現する一連の代表的な4-1BBB共刺激CAR T細胞;図19-21(カルボキシ末端に存在するT2A及びCD1t配列なしで示される)を作製した。当該CARはすべて、同じ細胞外ドメイン(ヒンジのアミノ酸10におけるPへの変異を伴うIgG4ヒンジ;配列GGGSSGGGSG及びヒトIgG CH3ドメインを有するリンカー)、CD4膜貫通ドメイン、及び4-1BB細胞内補助刺激シグナル伝達ドメインを利用する。これらのヒト化TAG72-BBζ CAR T細胞を、OV90又はOVCAR3卵巣がん細胞のいずれかの存在下で増殖させ、死滅した卵巣がん細胞の比率を定量した。インビトロでは、IDEC及びV15 TAG72-BBz CAR T細胞は、TAG72を発現する卵巣がん細胞株への曝露後に、同等の強力なT細胞媒介性抗原依存性細胞傷害、活性化及びT細胞増殖を示す(図13A-13C)。V59/15 TAG72-BBz CARは、このアッセイではほとんど活性を示さかったため、次の実験から除した。
OV90腹腔内投与モデルにおける選択的標的TAG72陽性細胞に対するヒト化TAG72-BBζ CAR T細胞の効能評価のため、ヒト化TAG72-BBζ CAR T細胞を単回又は複数回反復投与して、経時的に腫瘍サイズを評価した。
OV90腫瘍細胞株上のTAG72抗原の内因性発現をフローサイトメトリーで測定した。OV90-ffluc細胞をNSGマウスの腹腔(i.p.)内腔に注入し、生物発光イメージングにより追跡し、流束(写真/秒)として報告した。腫瘍注射後8日目に、腫瘍マウスの腹腔内にTAG72 CAR T細胞の5.0×106 Mock、IDEC、又はV15変異体を局所的に投与した(図14A)。単一又は反復T細胞処理マウスの腫瘍量を生物発光イメージングにより定量した。破線はT細胞による初回治療と反復治療の時点を示す。興味深いことに、発明者らにより、インビボ卵巣腫瘍モデルを用いた、V15-TAG72-BBzによる局所腹腔内投与により、IDEC-TAG72-BBz CARよりも抗原陽性標的(OV90移植腫瘍)の腫瘍量が大幅に低下したことが示された(図14A-14B)。この観察結果を踏まえて、単回投与と比較したTAG72-BBζ CAR T細胞の反復投与の効能を評価したところ、治療応答が明らかに改善された(図14B)。TAG72-BBζ CAR T細胞の単回投与と比較して、50日間以上にわたる反復投与により、OV90モデルにおける抗腫瘍反応は持続された(図14B)。
OVCAR3腹腔内投与モデルにおける選択的標的TAG72陽性細胞に対するヒト化TAG72-BBζ CAR T細胞の効能を評価するため、ヒト化TAG72-BBζ CAR T細胞を単回投与し、経時的に腫瘍サイズを評価した。
内因性表面TAG72発現を、OVCAR3腫瘍細胞上のフローサイトメトリーで分析した。次に、OVCAR3-ffluc腫瘍をNSGマウスの腹腔内に注入し、5.0×106Mock、IDEC又はV15変異型TAG72 CAR T細胞を単回投与して静脈内投与した(図15A)。単回投与マウスの腫瘍量を生物発光イメージングにより定量化し、流束(写真/秒)として報告した。破線はT細胞による治療の時点を示す。興味深いことに、V15-TAG72-BBz CAR T細胞を静脈内投与(i.v.)し、IDEC-TAG72-BBz CAR T細胞は投与しない場合、OVCAR3担がんマウスに対する強力な抗腫瘍反応を媒介することができる(図15B)。V15-TAG72-BBz CARのインビボにおけるこの抗腫瘍反応は、ある部分、IDEC-TAG72-BBzと比較した、増殖増加が介在し、その結果、反応寿命が延びた(図15C)。(図15C)。
ヒト化TAG72 CAR T細胞の設計評価のため、V15 scFvを特徴とし、リンカー、膜貫通及び共刺激ドメインを変化させた、一連の代表的なTAG72 CAR T細胞を作製した。
V15 scFvクローンを有する7つの代表的なヒト化TAG72-CAR T細胞変異体は全て、CAR発現安定性を示した(図16A)。インビトロ腫瘍殺傷活性では、ヒト化TAG72CAR T細胞を、TAG72陽性(OVCAR3、OV90、及びOVCAR8-sTn)又はTAG72陰性(DU145、OVCAR8)卵巣がん細胞のいずれかの存在下で増殖させ、死滅した卵巣がん細胞の比率を定量した。7つの代表的なヒト化TAG72-CAR T細胞変異体は全て、TAG72-陽性OVCAR3、OV90、及びOVCAR8-sTn細胞の強力で選択的なCAR介在性の殺傷を示し、TAG72-陰性DU145及びOVCAR8細胞に対する検出可能な抗腫瘍活性は示されなかった(図16B)。T細胞の増殖は様々で、TAG72陰性DU145及びOVCAR8細胞よりもTAG72陽性OVCAR3、OV90、及びOVCAR8-sTn細胞の方が高かった(図16C)。CD137+活性化指標は、代表的なヒト化TAG72-CAR T細胞変異体が、TAG72-陰性DU145及びOVCAR8細胞よりも、TAG72-陽性OVCAR3、OV90、及びOVCAR8-sTn細胞において多様であり、より高いことが示された(図16D)。腫瘍細胞を発現するTAG72陰性(DU145、OVCAR8)及びTAG72陽性(OVCAR3、OV90、及びOVCAR8-sTn)に対するCAR T細胞のPD-1+枯渇標識(72時間)。
Claims (28)
- キメラ抗原受容体(CAR)をコードするヌクレオチド配列を含む核酸分子であって、前記キメラ抗原受容体が、配列番号1、33又は34のいずれか1つのアミノ酸配列を含むscFv標的Tag-72、スペーサー、膜貫通ドメイン、CD28又は4-1BB共刺激ドメイン、及びCD3ζシグナル伝達ドメインを含む、核酸分子。
- 膜貫通ドメインは、CD4膜貫通ドメイン又はCD28膜貫通ドメインである、請求項1に記載の核酸分子。
