本発明は、ヒトまたは動物における加齢性老視、黄斑変性症(AMD)、糖尿病網膜症(DR)および糖尿病黄斑浮腫(DME)などの最終糖化産物(AGE)関連眼疾患による視力障害または失明の処置に関する。AGEの蓄積により毛様体および水晶体の弾性が著しく減少するときに、加齢性老視は40~50歳前後の人に発症する。その結果、適応(accommodation)およびすぐ近くを読む能力は徐々に減少する。加齢性老視の現在の唯一の解決策は、老眼鏡である。AMDは、西欧諸国で>65歳の人の不可逆的な視力喪失の最も一般的な原因である。現時点では、AMDの乾燥型に対して利用可能な処置はない。AMDの乾燥型は、最終糖化産物およびフルオロフォアの網膜(下)蓄積である、視力を脅かすドルーゼン(Druesen)によって特徴付けられる。他方、DRおよびDMEは、西欧諸国で<65歳の人の不可逆的な視力喪失の最も一般的な原因である。DRおよびDMEの実際の処置の選択肢は、局所および全身ステロイド、抗炎症剤、レーザー光凝固、毛様体扁平部硝子体切除、および/または抗血管内皮増殖因子(VEGF)抗体からなるが、多くの患者は不十分な応答を示す。DRおよびDMEは、高血糖によるブルッフ膜および網膜におけるAGEの蓄積によって特徴付けられる。本発明は、特に、フルクトサミン-3-キナーゼおよびその補因子(単数または複数)の投与に関する。これは、AGEおよびフルオロフォアの脱糖化および不活性化をもたらす。
老視は、誰もが40~50歳で直面する老化状態であり、近方視力が減少する。水晶体は、毛様体によって合成された水晶体毛様小体(lenszonule)の複雑な3次元システムによって所定の位置に保持される。適応中、毛様体の収縮は水晶体毛様小体の弛緩を引き起こし、水晶体嚢および外側皮質水晶体層の弾性により、水晶体の増大した曲率および屈折力の増加をもたらす。加齢とともに、AGEは硝子体のコラーゲン物質と密接に関連する毛様体、水晶体、および水晶体毛様小体において蓄積し、適応力が減少する(1)。現在、加齢性老視の唯一の処置は、老眼鏡をかけることである。
本発明は、フルクトサミン-3-キナーゼおよびその補因子(単数または複数)の投与が、AMDにおいてより少ない/より少ない密度(less/less dens)のドルーゼンをもたらすという驚くべき知見に関する。換言すれば、後者の投与は、AMDを有する患者において光透過率を回復させ、よって視力を回復させる。
本発明はまた、フルクトサミン-3-キナーゼおよびその補因子(単数または複数)による処置が、網膜、ブルッフ膜、および網膜下におけるAGEを低下させるという驚くべき知見に関する。換言すれば、FN3Kおよびアデノシン三リン酸を含む組成物は、AMD、DR、およびDMEなどのAGE依存性眼疾患を有する患者において光透過率を回復させ、よって視力を回復させる。
本発明はまた、フルクトサミン-3-キナーゼおよびその補因子(単数または複数)による処置が毛様体におけるAGEを低下させるという知見に関する。換言すれば、FN3Kおよびその補因子を含む組成物は、加齢性老視を有する個人において適応を回復させ、よって近方視力を回復させる。
本発明はさらに、上記のとおりの使用のための組成物に関し、当該組成物は、硝子体内注射によって投与される。
本発明はさらに、マグネシウムイオンおよび/またはアデノシン三リン酸再生系をさらに含む、上記のとおりの使用のための組成物に関する。
本発明はさらに、マグネシウムイオンをさらに含む、上記のとおりの使用のための、フルクトサミン-3-キナーゼおよびアデノシン三リン酸再生系を含む組成物に関する。
「アデノシン三リン酸」(ATP)および「マグネシウムイオン」という用語は、後者の酵素のよく知られた補因子に関する。
「アデノシン三リン酸生成系」という用語は、ADPまたはAMPからATPを再生するためのいくつかの酵素的および全細胞ベースの方法に関し、例えば、Woodyer R. D. et al. 2006に記載されている(15,16)。特に、後者の用語は、ADPからのATPの再生に一般的に使用される以下の4つの酵素を指す:1)ピルビン酸キナーゼによって触媒される結合反応におけるホスホエノールピルビン酸の使用、2)酢酸キナーゼと結合したアセチルリン酸、3)クレアチンキナーゼと結合したクレアチンリン酸、および4)ポリリン酸キナーゼと結合したポリリン酸。好ましくは、「ATP生成系」という用語は、二次エネルギー源としてのホスホクレアチンおよびそのリン酸基をADPに移してATPを再生するためのクレアチンキナーゼの利用を指す。よって、後者のATP生成系の利用は、さらに記載されるように、硝子体に注射された混合物中に存在するATPの濃度を制限する。
「加齢性老視を処置する」という用語は、処置対象の近方視力の安定化および/または改善に関する。
「動物」という用語は、任意の動物に関し得る。
「硝子体内注射による投与」という用語は、目の硝子体への本発明の化合物の注射に関する。硝子体内注射技法は、制御された無菌条件下で使用される。注射の前に適切な麻酔が与えられる。動物の目の処置には、(例えば、イソフルレン5%の吸入麻酔による)全身麻酔が使用される。ヒトの処置には、局所麻酔点眼薬が使用され得る。32ゲージの針は、より小さな動物(小さい齧歯類の動物など)の目における注射のために使用され得、30ゲージの針は、ヒトの目およびウマおよびブタなどのより大きな動物の目における注射のために使用され得る。すべての種において、強膜は45°~90°の角度で貫通される。例えば、マウスでは、強膜は角膜輪部から1~1.5ミリメートルで貫通され得、ヒトでは、強膜は角膜輪部から3~5ミリメートルで貫通され得る。針は、硝子体に到達するまで強膜および脈絡膜を通過する。針は水晶体にも網膜にも触れない。本発明の組成物は、そのまま送達され得、針は直ちに引き抜かれる。
動物の目(マウスではない)およびヒトの目において>5μlの量が(硝子体内に)投与されるとき、0.1mol/Lのチオスルファートおよび5U/mlのヒアルロニダーゼが混合物に加えられる。
以下の例は、本発明をよりよく説明するために提供され、本発明の範囲を限定するものと考えられるべきではない。
例1:フルクトサミン-3-キナーゼの組換え産生
ヒトフルクトサミン-3-キナーゼをコードする遺伝子(アクセッション番号または国立バイオテクノロジー情報センター(NCBI)参照配列番号:NP_071441.1(https://www.ncbi.nlm.nih.gov/protein/NP_071441を参照)、Pichia pastoris発現についてコドン最適化(配列番号1)、を有する)を、Claes, K. et al. (“Modular Integrated Secretory System Engineering in Pichia Pastoris To Enhance G-Protein Coupled Receptor Expression,” ACS Synthetic Biology 5, no. 10 (October 21, 2016): 1070-75)にしたがって、pKai61 P. Pastoris発現ベクターにクローニングした。コードされた遺伝子は、カスパーゼ3切断部位(DEVD)とともにフレーム内にN末端His6タグ(MHHHHHH)を含み、発現はメタノール誘導性AOX1プロモーターの制御下にある。プラスミドは、細菌および酵母細胞の選択のためのゼオシン耐性マーカーを含む。ベクターは、P. Pastoris(NRRL Y-11430株)に形質転換する前にAOX1プロモーターで線形化され、ゲノムへの安定した組み込みのために内因性AOX1遺伝子座での相同組換えを促進した。
ペレットを解凍し、タンパク質抽出のために洗浄緩衝液に再懸濁した。Pichia pastoris細胞を0.5mmガラスまたはシリカ/ジルコニウム(silicia/zirkonium)ビーズを使用して機械的に破壊した。清澄化した上清を、His6タグ付きフルクトサミン-3-キナーゼのNi2+アフィニティークロマトグラフィー、続いてゲル濾過により精製した。FN3K試料緩衝液(20mM Tris-HCl pH8.0、150mM NaCl、1mM DTT)で溶出したタンパク質を、SDS-PAGEおよびHis6タグおよびヒトFN3K(ThermoFisher)に対する抗体を用いたウェスタンブロッティングにより組換えヒトフルクトサミン-3-キナーゼとして同定した。酵素活性は、1デオキシ1モルホリノDフルクトース基質(R&D Systems)を用いたキナーゼ活性アッセイで確認した。フルクトサミン-3-キナーゼのアリコートを液体窒素で瞬間冷凍し、-20℃で保存した。
ドルーゼンを有するヒト網膜の5マイクロメートルの切片(D)を、生理食塩水+ATP+MgCl2で処置するか、またはFN3K+ATP+MgCl2で処置する。使用した用量は、約4,17μg/mlと12.5μg/mlとの間のフルクトサミン-3-キナーゼ、2.50mMと4,17mMとの間のATP、および1.00mMと1.67mMとの間のMgCl2の範囲であった。ドルーゼンを、好酸球物質の完全性および存在について光学顕微鏡で評価する(図2AおよびB)。
図3Dは、生理食塩水+ATP+MgCl2(対照)で処置またはFN3K+ATP+MgCl2で処置したときのブルッフ膜(実線)および網膜下ドルーゼン(点線)のAGEの測定したNIRスペクトルの平均スペクトルを示す。
試験を、典型的なAMD病変をドルーゼンとして示す老齢のC57/Bl6マウスで行う。硝子体内注射による一方の目のFN3K処置に続いて、近赤外(NIR)および蛍光分光法を使用してマウスの網膜を研究する。
典型的なドルーゼンの有無を評価するために組織切片を行う(図5)。生理食塩水+ATP+MgCl2の硝子体内注射で目を処置したとき、ドルーゼンは存在したが、FN3K+ATP+MgCl2で目(同じ動物の反対側の目)を処置したときは存在しなかった。
ヒト死体の目(角膜移植のために除去された)を氷上で輸送し、プレレベーション後24時間以内に光干渉断層撮影によってドルーゼンの存在について評価した。ドルーゼンが存在するとき、ドルーゼンをFN3K+ATP+MgCl2+0.1mol/Lチオスルファート+5U/mlヒアルロニダーゼを目に硝子体内注射することによって処置した。大きな網膜下ドルーゼンの周りのカルシウムヒドロキシアパタイトの最適な浸透のために、硝子体内注射の前にチオスルファートを混合物に加えた。硝子体の最適な浸透のためにヒアルロニダーゼを混合物に加えた。目を37℃で2時間維持し、ドルーゼンを再び光干渉断層撮影によって記録した(図6)。ドルーゼンが存在した4つの死体の目では、FN3K+ATP+MgCl2の硝子体内注射が明らかなサイズの低下を引き起こす。
ブタ網膜をプレレベーション後24時間以内に解剖した。ブタ網膜におけるAGEのUV蛍光分光法を、ベースライン時と、ヒト網膜で最も一般的な2つのAGE(メチルグリオキサール(methyglyxoal)(MG)、グリコールアルデヒド(GA))で24時間処置した後に行った。次いで、ブタ網膜をPBSで洗浄し、生理食塩水+MgCl2+ATPまたはFN3K+MgCl2+ATPで24時間処置し、UV蛍光分光法を繰り返した。
FN3K処置は、in vitroでブタ網膜の網膜内AGEの蛍光を減少させた。
試験は、典型的な糖尿病病変を示す老齢のob/obマウスで行う。硝子体内注射による一方の目のFN3K処置に続いて、近赤外(NIR)および蛍光分光法を使用してマウス網膜を研究する。
組織切片を、大きな漏出血管の増加および内境界膜のコラーゲン線維の厚さなど、DRおよびDMEの典型的な病変を評価するために行う。図7は、大きな漏出血管(大きな矢印)および極めて厚いコラーゲン性内境界膜(三角形)を有する、生理食塩水+ATP+MgCl2で処置したob/obマウスの糖尿病網膜症の兆候を示す。FN3K+ATP+MgCl2で処置したob/obマウスの網膜は、wtマウスと同等の網膜の正常化および正常な微小血管系(小さな矢印)を示した。
毛様体をヒト死体の目(角膜移植のために除去された廃棄物)から解剖し、3mLのFN3K(41.6μg/mL)+ATP 2.5mmol/L+MgCl2(1mmol/L)でex vivoで3時間処置した。蛍光定量法(図8)は、FN3K処置の1時間、2時間、および3時間後に、ミニチュア分光計システム(Flame-S-VIS-NIR、Ocean Optics、ラルゴ、フロリダ)を使用して固定距離および90°の角度で行った。QR400-7-VIS-BX Premium 400ミクロン反射プローブを使用した。
ヒト死体の目(角膜移植のために除去された廃棄物)は、プレレベーション(prelevation)後24時間以内に処置した。クロスオーバー実験では、常に同じドナーからの2つの目を使用する。
FN3K点滴薬または生理食塩水点滴薬を無傷のヒト死体の目に適用する技法は、以下からなる:FN3K(25μg/mL)+ATP(5mmol/L)+MgCl2(2mmol/L)溶液の6~7の点滴薬を、一方の目に1時間ごとに6時間適用し、生理食塩水点滴薬を、同じドナーからの他方の目に1時間ごとに6時間適用した。蛍光定量法は、処置前のベースライン時および処置の6時間後に、ミニチュア分光計システム(Flame-S-VIS-NIR、Ocean Optics、ラルゴ、フロリダ)を使用して固定距離および90°の角度で行った。QR400-7-VIS-BX Premium 400ミクロン反射プローブを使用した。まず、一方の目をFN3K点滴薬で処置し、同じドナーの他方の目を生理食塩水点滴薬で処置する。クロスオーバー実験では、次いで処置を切り替え、FN3Kで処置した目をさらに生理食塩水点滴薬で処置し、最初にFN3Kで処置した目をさらに生理食塩水点滴薬で6時間処置する。蛍光定量法は、ベースライン時(実験開始、t=0時間)、最初の処置の6時間後、および他の処置の6時間後に行う。
Claims (8)
- ヒトまたは動物において加齢性黄斑変性症および/または糖尿病網膜症を処置するための使用のための、フルクトサミン-3-キナーゼおよびアデノシン三リン酸(ATP)を含む組成物。
- 硝子体内注射によって投与される、請求項1に記載の使用のための組成物。
- マグネシウムイオンをさらに含む、請求項1または2に記載の使用のための組成物。
- 前記フルクトサミン-3-キナーゼが、組換えフルクトサミン-3-キナーゼである、請求項1~3のいずれか一項に記載の使用のための組成物。
- 前記組換えフルクトサミン-3-キナーゼが、Pichia pastorisにおける組換え産生によって得られる、請求項4に記載の使用のための組成物。
- Pichia pastorisにおける組換え産生によって得られる前記組換えフルクトサミン-3-キナーゼが、配列番号1または配列番号2によって与えられるアミノ酸配列を有する、請求項5に記載の使用のための組成物。
- 加齢性黄斑変性症および/または糖尿病網膜症の前記処置が、網膜(下)最終糖化産物およびフルオロフォアの脱糖化を伴う、請求項1~6のいずれか一項に記載の使用のための組成物。
- 前記組成物の1つの目当たり5μlに等しい総量が投与されるとき、または、前記組成物の1つの目当たり5μlよりも多い総量が投与されるとき、0.1mol/Lのチオスルファートおよび5U/mlのヒアルロニダーゼが、前記組成物に加えられる、請求項1~7のいずれか一項に記載の使用のための組成物。
