・機構:折り畳まれた電子機器基板及び蛇行相互接続部の使用は、電子機器部品を組み込んだ最小の表面積を有する基板の高い伸縮性を提供する。ECG及びPPGの両方を柔らかいシリコーンシェルに封止することにより、ワイヤ及び電子機器が露出されない。より正確には、封止処理は、接着剤層を用いて電子機器が積層されるシリコーン(例としては、Elkem社のSilbione RTV4420が挙げられる)の平坦層、及びこの電子機器を覆う同じシリコーンのシェル層の使用を含む。平坦層及びシェルは、未硬化シリコーン層を介して接合される。封止の前に、柔らかいシリコーンゲルの層もシリコーン基板(例としては、Silbione RT GEL4717又はEcoflex Gelが挙げられる)上に加えられ、デバイスの全体的な柔らかさを増強し、歪み絶縁層として作用する。全てのシリコーンの硬化ステップは、オーブン(温度範囲70~100℃)中で行われる。結果として得られるデバイスは、皮膚適合性と、身体に配置されたときに受ける通常の変形に適合するレベルでの伸縮能力とを組み合わせる。
・電極防水:ECGデバイスは、身体からのECG電気信号を捕捉するための2つの電極を備える。防水性デバイスを提供するために、2つの測定点における電気的接触は、メインのシリコーン封止シェルに(例えば、コロナ処理を使用して)完全に結合されたカーボンブラックPDMS(CB-PDMS)パッドを通して行われ、その結果、開口部のないシリコーン封止が得られる。CB-PDMSパッドから電子機器基板上の金電極への接触は、導電性接着剤を介して行われる。
・出力:より良いモジュール性を可能にするために、設計は、(i)埋め込み型バッテリ、(ii)モジュール式バッテリ、(iii)モジュール式コイルの3つの異なる電力供給スキームを含む。(ii)及び(iii)については、バッテリ及びコイルモジュールは交換可能であり、磁石接続を介してメインモジュール(電力供給部分を除くすべての電子機器を含む)と接続される。脆弱な皮膚からデバイスを取り外すことなくバッテリの交換を可能にすることは、限られた数の接着剤剥離事象を可能にする。さらに、メインモジュール内にバッテリがないので、この部品は、より広範な消毒実施の適合性のためにオートクレーブ可能である。交換可能なバッテリユニットは、センサ基板上の磁石相互接続部に結合するバッテリユニットの容量及びサイズの変動に関連する様々な寿命を含む。CR1216コインセルバッテリを使用して最も薄い外形を実現し、最大厚さ3mmのバッテリモジュールとなっている。
この実施例は、本発明の一態様に関連しており、新生児及び小児の患者において最大24時間にわたって生理学的信号を連続的かつ非侵襲的に監視するバイノーダル、無線、及び機械的にソフトな電子プラットフォームに関する。このウェアラブルプラットフォームの技術的な進歩には、マルチモーダルな電源オプション、ソフト機構、並びに新生児及び小児患者のケアを強化することを目的とした高度な臨床診断機能:治療的な皮膚と皮膚とのケア又はいわゆるカンガルーマザーケア(KMC)の定量化、啼泣信号パターン及び持続時間、非侵襲的で連続的な血圧評価、並びに心拍数、呼吸数、体温及びパルス酸素化を含む臨床バイタルサインの無線捕捉、が含まれる。プラットフォームは、新生児集中治療室(NICU)に入院している在胎期間23~41週(GA)の新生児患者40名及び小児集中治療室(PICU)に入院している慢性期間(CA)3歳までの小児患者10名を対象とした臨床試験によって検証された。臨床研究によれば、このプラットフォームは、病院の臨床基準と比較して、24時間まで連続して正確なバイタルサイン測定を行うと同時に、カンガルーマザーケア及び啼泣行為の追跡など、バイタルサインを測定することを超えた、より高度な機能を確実に提供することが示されている。
無線バイタルサイン監視システムセンサ設計
図6Aは、ECG処理のためのアナログフロントエンド、3軸加速度計、温度計IC、及び、胸部EESとパルスオキシメータICとのためのBLE SoC、温度計、及び、四肢EESのためのBLE SoCを含む2つの時間同期されたEESを含む装置のコア構成要素の機能ブロック図を概略的に示す。具体的には、図6Aに示される装置は、胸部EES及び四肢EESを含む。各EESに関して動作するために必要な電力を供給する3つの異なる電源。胸部EESは、ECGセンシングユニット、3軸加速度計(BMI160、Bosch Sensortec)を介する運動センシングユニット、及び臨床グレード温度計(MAX30205、Maxim Integrated)を含む。ECGセンシングユニットは、2つの金メッキ電極、計装用増幅器、アナログフィルタ及び増幅器、並びにBLE SoC(nRF52832、Nordic Semiconductor)を含む。黒のPDMS用に残された。加速度計による運動センシングのデータ取得は、Serial Peripheral Interface(SPI)通信プロトコルを介してBLE SoCにより制御される一方で、温度計による温度データは、Inter-integrated Circuit(I2C)通信プロトコルを介して取得される。四肢EESには、血液酸素化(SpO2)を測定するための統合パルスオキシメトリモジュール(MAX30101、Maxim Integrated)、及び温度計(MAX30205、Maxim Integrated)が含まれている。両ICはI2Cプロトコルを介してBLE SoCにより制御される。埋め込み型及び取り外し可能バッテリ動作のための電力管理回路は、電圧を3.3V又は1.8Vのいずれかで様々なICの供給電圧まで低下させる電圧調整器を含む。無線電力伝送プラットフォームは、13.56MHzで調整された誘導コイル、全波整流器、2段電圧調整器、及びヒートシンクを含む。
リアルタイムのデータ分析をサポートするコンピュータシステムへの連続的な無線データ送信は、看護師、医師及び親に対して直感的な方法でグラフ表示することができる結果をもたらす。動作のBLEモードでの無線及びリアルタイムストリーミングにより、患者中心で正確なバイタルサインの測定が可能である。胸部EESは、それぞれ504Hz、100Hz及び5Hzで各々サンプリングされたECG、加速度計を通る胸部の動き及び皮膚温度を測定する。四肢EESは、100Hz及び5HzでそれぞれサンプリングされたPPG及び皮膚温度を測定する。図7Aは、基地局(Surface Pro)にストリーミングされる新生児のリアルタイムで測定されたECG、PPG、及び胸部の動きの波形を示す。Pythonで開発されたプログラムは、データを受け取り、リアルタイムの信号処理を実行してバイタルサインを生成する。フィルタリング及びRピーク検出プロセスから構成される合理化されたPan-Tompkinsアルゴリズムにより、HRが得られる。Z軸から測定された胸部の動きのデータは、帯域通過フィルタリング(fc1=0.1Hz、fc2=1Hz)及び合理化された自動マルチスケールベースのピーク検出(AMPD)によって処理されて、RRが得られる。
EESセンサは、既存のゴールドスタンダードと同等の精度で中核的なバイタルサインを測定できるだけでなく、より高度な機能を提供する。図8Aは、カンガルーマザーケア(KMC)追跡及びバイタルサイン監視を示し、図8Bは、本発明の特定の実施形態による新生児患者の啼泣信号分析を示す。
血圧は血行動態状態及び心臓血管の健康を反映しており、その障害は新生児及び小児集中治療室に入院した新生児及び子供によくみられるため、監視すべき重要なバイタルサインの1つである。現在の臨床診療における測定には、動脈管への侵襲的カテーテルが関与しており、親及び介護者の双方に対して重大な障壁となっている。この例において、本発明者らは、多数の文献によって血圧の有望な代用物として注目されているパルス到達時間(PAT)によって血圧を計算する非侵襲的方法を提示する。PATは、圧脈波が心臓から遠位先端まで伝わるのに必要な時間として定義され、血管系の幾何学的形状及び弾性並びに血圧に依存する。物理的に離れた2つのEESセンサ間の時間同期は、正確なPAT読み取りを達成するための鍵である。これは、タイムスロットAPIによって許容されるBLE SoCのマルチプロトコル能力によって達成される。各EESがBLE動作モードで基地局(Surface Pro)と独立に通信している間、1秒ごとに、四肢EESは、そのローカルクロック情報を胸部EESに送信し、2つのEESのローカルクロック間の時間差を同期させる(図8D)。その結果、24時間動作の連続運転(図8D)にわたって3.6msの平均標準偏差で1ms未満の時間遅延が達成され、EESはECG RピークとPPG信号の足との間の正確なPAT読み取り値を計算することができる。PATから導出されたSBP間の検証の2つの測定値は、2サイクルの安静及び運動中に健常成人で測定されたゴールドスタンダード(BP742N5 Series, Omron)と一致しており、傾向において同等の差は示されていない。図8Eは、PICU内の2つの異なる乳児について測定されたPAT由来のSBPを示す。
製造
製造は、レーザープロセスによって得られた可撓性の電子機器基板上に電子機器部品を半田付けすることを含む。組み立てられた回路基板を柔らかいシリコーンエラストマーシェル内に埋め込むことにより、電子機器部分の望ましくない露出が回避される。デバイスの埋め込み型バッテリバージョンに対しては、アルミニウムモールド内で鋳造されたSilbione RTV 4420(Silc-Pigシリコーン不透明染料5%と混合されたPart A & Part B)層が上部シェルを提供する。250rpmでスピンコートされたSilbione 4420RTV底部層は、平坦な底部層をもたらす。両方の層を100℃のオーブンで20分間完全に硬化させる。ECGデバイスの封止に関しては、平坦な底部Silbione RTV 4420層上に積層された3M 96042の二重コーティングテープの層は、電子機器部分と底部側との良好な接触を可能にする。PPGデバイスの封止に関しては、平坦なSilbione RTV 4420の底部層は裸のままである。CO2 Universalレーザーカッターを使用して、底部層上で切断された開口部は、PPGのLEDモジュールのためのECG電極の電気的接触及び光透過性を可能にする。ECGデバイスに関しては、可撓性の回路基板は3M 96042の二重コーティングテープ層に接着し、デバイスの左、中、右の部分にSilbione RT GEL 4717を加えると、折り畳まれた電子機器基板部分のための柔らかいクッションとなる。未硬化Silbione RTV 4420を使用して最終的に組み立てられた上部層及び底部層は、100℃のオーブン内に50分間配置され、その結果、デバイスのシール封止が得られる。PPGデバイスについては、透明なSilbione RTV 4420の薄層を、底面に250rpmでスピンコーティングし、100℃のオーブン中で20分間硬化させて、LEDモジュールの完全なシールを提供する。レーザー切断は最終的に両方のデバイスの輪郭に対してきれいな切断を与える。
Ease Release 200(Mann Release Technologies)でコーティングされたガラススライドにテープマスクを適用して、厚さ250μmのCB-PDMSフィルムを生成した。撹拌棒を備えた200mL丸底フラスコに、カーボンブラック4.5g及びSylgard 184ベース15.0gを秤量した。両成分をn-ヘキサン(約100mL)に溶解し、室温で10分間激しく撹拌した。混合溶液に、ヘキサンで10倍に希釈したSylgard184硬化剤1.5gを加え、反応物を2~3分間撹拌した。溶媒を迅速に除去し、滑らかな展延性ペーストが達成されるまで、40℃での回転蒸発によってポリマーを同時に脱気した。ポリマーをガラスモールド上に広げ、平坦なエッジを有する過剰なn-ヘキサン蒸発からのクラックがないことを確実にした。試料を一晩70℃で硬化させ、CB-PMDSフィルムを生成した。
上部シェルは、上記のように調製した。簡単に説明すると、CB-PDMSシール底部層は、Silbione 4420 RTVをスピンコーティングし、上述のように硬化させることによって調製された。CO2 Universalレーザーカッターを使用して、同じ寸法のセンサ開口部を生成した。CB-PDMS電極パッドを、全てのエッジ上に2mmの過剰なオーバーラップを有する同じ形状に切断した。Silbione底部層及びCB-PDMSパッドの両方を、BD-20A High Frequency Generator(Electro-Technic Products,Inc.)で40秒間コロナ処理し、15秒間一緒にプレスし、70℃で一晩硬化させた。硬化した底部層に、パッド用の孔を有するデバイスの形に切断された3M 96042の二重コーティングテープの層を積層した。両面3M電気テープ接着剤を使用して、CB-PDMSをAu電極に接着した。デバイス部品及び上部層と底部層との間のシールは、上述のように行った。
非機能性デバイスCB-PDMSシールデバイスは、水透過性を監視するために電子部品をDrieriteで置き換えることによって調製された。非機能性デバイスを1×DPBS中に37℃で浸漬し、重量変化を測定した。3つの水分インジケーターで内部裏打ちされた機能性CB-PDMSシールデバイスを、1×DPBS中、70℃で連続的にインキュベートした。ECGデバイスの毎日の測定値は、装置が故障するまで測定された。
本発明者らは、ベンチトップ検証実験を通して、2つのノード(ECG及びPPG)間の時間同期を特徴付けた:2チャネル関数発生器は、2つのチャネル間の制御された時間遅延を有する周期的信号(1秒間隔で20msの3.5Vの矩形パルス)を提供した。第1のチャネルを適切なECGレイアウトピンに接続し、第2のチャネルをPPG光モジュールの上部で点滅する赤色LEDに接続することによって、本発明者らは、関数発生器に接続されたオシロスコープを介して測定された時間遅延が、平均遅延が1ms未満で平均標準偏差が3.6msのバイノーダルシステムによって測定された時間遅延と一致することを成功裏に実証した(図8D)。
Heidolph Tuttnauer 3545E Autoclave Sterilizer Electronic Model AE-Kを用いて、センサモジュール式部品(バッテリなしを含む)と磁石のオートクレーブ可否試験を行った。滅菌には、121℃までの温度上昇とそれに続く15分間の滅菌時間、その後の乾燥が含まれた。このプロセスはデバイス性能の変化をもたらさず、プラットフォームのオートクレーブ滅菌の実現可能性を成功裏に実証した。
温度センサの精度は、基準温度計(FisherbrandTM 13202376、Fisher Scientific)測定を標準として用いて決定した。EESの温度計と基準温度計は共に42℃に加熱した湯浴中に配され、室温まで冷却された。冷却期間中、EESと温度計間の温度測定値を記録し、30℃~41℃の温度範囲でEESの温度センサの精度を特性評価した。
KMC分析は、100Hzのサンプリングレートでの加速度計測定に基づいていた。加速度計は、x軸、y軸及びz軸を重力ベクトルと整列させ、加速度計信号を重力と相関させることによって較正された。加速度信号は0.1Hzのカットオフ周波数のButterworth低域フィルタで処理し、重力に対するデバイス軸の角度は三角法処理で計算した。x,y,z軸の加速度計信号を三次元的にプロットし、臨床的に記録した体位と相関させた。
この実施例は、本発明の一態様に関連しており、新生児及び小児の集中治療室における高度な無線生理学的監視のための皮膚インターフェースバイオセンサ及びパイロット研究の適用に関する。
デバイス及びシステム設計
この実施例は、薄いヒドロゲル結合層を介して胸部(胸部ユニット)の湾曲した皮膚上に穏やかに配置して、心電図(ECG)、発声及び心臓/呼吸活動の音響信号、身体の向き及び動き、並びに皮膚温度を記録することを可能にする設計における電源のためのモジュール式バッテリユニットを使用し、これらは全て、BLE SoC及び関連するセンサの集合によって可能になる。全体的なレイアウトは、薄い可撓性プリント回路基板(PCB)と、蛇行相互接続部トレースを有する開放設計で構成された実装部品とを含む。この構造は、組み立て及びパッケージングにおける重要なステップとして、別個であるが接続されたプラットフォームの折りたたみを含む。システムレベルの機構の3次元有限要素解析(FEA)からの定量的洞察は、デバイスの横方向寸法を約250%減少させるために、有効部品の最適分布を画定するのに役立った。組み立てにおける予備圧縮プロセスは、蛇行構成の座屈レイアウトを形成して、柔軟性及び伸縮性を高める。内側シリコーンゲルライナー(~300μm厚、~4kPa)を有するエラストマーエンクロージャは、デバイスの柔らかさを増強し、最も低い在胎齢で生まれた新生児の脆弱な皮膚及び高度に湾曲した解剖学的特徴との適合性を確保する。ドープシリコーン材料(「CB PDMS」と略称されるポリジメチルシロキサン中のカーボンブラック;4.2Ω・cmのバルク抵抗率)を使用して形成された一対の薄い導電性要素は、可撓性プリント回路基板上の対応する金電極及びECG測定のための導電性ヒドロゲル皮膚インターフェースへの柔らかな電気接続として役立つ。その結果、NICUとPICUに限定されるものではないが、広範囲の設定に適用できる、柔らかく完全にシールされた防水性システムが得られた。
モジュール式バッテリユニットは、一対の対応する組の埋め込み型磁石を介して機械的及び電気的にデバイスに結合され、それによって以下のことが行われる。(1)臨床ケアの中断を最小限に抑え、臨床スタッフの負担を軽減し、結果的に皮膚損傷のリスクを軽減することを目的として、デバイスを患者から取り外すことなくバッテリの交換を可能にすること、(2)バッテリを取り外してデバイスのオートクレーブ滅菌を可能にすること、及び、(3)バッテリをデバイスから機械的に切り離すことによって、曲げ加工性を向上させ、したがって皮膚インターフェースにおける準拠を向上させること。磁気スキームはまた、バッテリサイズ、形状及び蓄積容量(したがって動作寿命)の選択だけでなく、無線電力転送に依存するバッテリフリースキームを含む代替モダリティにおいても、電源のための他のオプションを可能にする。この後者の可能性の例として、磁気的に結合されたハーベスティングユニットは、ベッドの下に配置され、生体組織における吸収が無視できる13.56MHzの無線周波数で動作するように設計された送信アンテナから電力を受信するように構成することができる。
図9Bは、胸部ユニット内で使用される蛇行相互接続部の概略図を示し、図9Cは、本発明の特定の実施形態による胸部ユニットの機械的特性の計算による実証を示す。3つのサブシステムは、柔らかな蛇行相互接続部によって機械的及び電気的にリンクされており、この蛇行相互接続部は、高い伸縮性を提供し、デバイスが人体に装着されたときの生理学的変形に従順に適合する。柔らかい蛇行相互接続部は、ポリイミド(PI)中に封止され、面外方向に25μm離れた2つの12μm厚さの銅層から成る。各銅層は幅W=75μmの三つの蛇行トレースを特徴とし、トレース間の面内間隔は75μmであった。蛇行相互接続部の総厚さは99μmである。胸部ユニットにおいて、ポリイミド(PI)で封止された蛇行相互接続部、折り畳まれた構成、及びゲルライナーを有する柔らかなエンクロージャは、デバイスレベルで約33%を超える一軸弾性伸縮性をもたらし、これは、外側シリコーンシェルにおける封止前の相互接続部における約500%の伸縮性に対応する(図9B及び9C)。ゲル(厚さ約300μm、弾性率約4kPa)は、折り畳まれたアイランドの間の歪み隔離を提供し、皮膚インターフェースにおける応力を、最大20%の一軸伸縮に対する感覚知覚の閾値(20kPa)未満のレベルに低下させる。この値は、実用において期待される範囲の上限である。図9Cのパネル(c)に示すように、結果として得られる弾性曲げ半径及び等価曲げ剛性は、それぞれ~20mm及び~9.6Nmm2である。これらの機械的特性は、小さい乳児及び/又は低い在胎期間で遭遇する極端な曲率の場合であっても、柔らかく、刺激のない皮膚インターフェースを確実にする。
柔らかな機械的特性及び無線動作モードは特に、限定された表面領域上の解剖学的構造の高度に湾曲した領域に位置する場合、新生児及び小児ICUの患者に効果的に使用するために極めて重要である。NICUでよくみられるように、未熟児には足首から足底までを包むことが効果的である。他のオプションには、足からつま先又は手首から手の周りに取り付けることが含まれ、典型的には、暦年齢が12ヵ月を超える乳児に最も適している。これらの取り付けオプションは、KMC中の密な接触や親の抱っこから授乳、おむつの交換、乳児への入浴に至るまで、臨床ケアにおける日常的で専門的な手順のほぼすべての態様を強化する。
リアルタイムのデータ分析をサポートするコンピュータシステムへの連続的な無線データ送信は、看護師、医師及び親に対して直感的な方法でグラフ表示することができる結果をもたらす。胸部ユニットは、ECGと皮膚温度を測定するとともに、サンプリング周波数が504Hz(ECG)、0.2Hz(温度)、100Hz(SCG)のSCG、呼吸数、その他を含む高帯域幅3軸加速度計で収集されたデータから推測できる豊富な情報を測定する。SCGはHRだけでなく、収縮期間隔、前駆出期(PEP)及び左室駆出時間に関する情報も提供する。四肢ユニットは、100Hz及び0.2Hzでそれぞれサンプリングされた赤(660nm)及び赤外(IR、880nm)の波長、並びに皮膚温度でPPGを測定する。
パルス到達時間(PAT)及びパルス通過時間(PTT)は、収縮期BP(SBP)との相関関係が確立されている2つの、関連しているが異なる測定値である。PATは、胸部ユニットにおけるECGのRピークと四肢ユニットにおけるPPGの谷領域との間の時間差から計算され、各心臓周期において大動脈から末梢の四肢の位置まで伝わる圧脈波の時間遅延を表す。PATからPEPを除外することによりSBPとの相関が改善される可能性があることを示唆する研究もある。SCG波形とPPG波形との間のピークから足までの時間遅延から計算されたPTTは、大動脈弁が開いたときの残留ピークを捕捉することによってこの除外を達成する。最終的には、PAT及びPTTはいずれも血管系の幾何学的形状、弾性、SBP、及びその他の因子に依存する。成人被験者に関する広範な研究は、経験的及び理論的モデルの両方を用いて、PAT、PTT及びSBPの間の較正された相関関係を確立し、それらのいくつかは、特定のシナリオ(例えば、Sotera ViSi Mobile(登録商標)System)における監視のために臨床的に承認されている。乳児におけるPATとSBPとの相関を報告した研究はほとんどなく、主に睡眠研究との関連で、また中核的な臨床ツールではなくスクリーニング方法として報告されている。この救命救急集団におけるPTTの測定値を報告するものはない。
SCG及びPTTに加えて、高帯域幅3軸加速度計からのデータをさらに使用することによって、いくつかの重要な動作モードが追加される。例としては、モーション/動き(KMC及び乳児の保持の追跡を含む)、並びに、啼泣の声調、動態及び周波数などの音声バイオマーカーの測定が挙げられる。世界保健機関(WHO)のガイドラインによると、KMCでは新生児を親の胸部に直立した位置に保持し、新生児の腹部を親の上腹部のレベルに置き、新生児の頭部を片側に向けて親とアイコンタクトさせる。胸部ユニットの加速度計からの低域フィルタ処理された(0~0.1Hz)データを使用して正確かつ連続的に監視することができるこの体位は、他のほとんどの活動及びケアの形態の間に生じるものとは異なる。
胸部及び四肢デバイスの製造及び組み立て
製造には、レーザーアブレーションプロセスを使用してパターン化された可撓性プリント回路基板上に電子部品を半田付けすることが含まれる。組み立てて折り畳んだシステムを柔らかいシリコーンエンクロージャの中に埋め込むことで、このプロセスは完了した。電源のモジュール式電源オプションを備えた胸部ユニットに関しては、250rpmでのスピンキャスト及びガラススライド上での熱硬化(オーブン中で100℃で20分間)によって形成された柔らかなシリコーン材料(Silbione RTV 4420; Part A及びPart B、5%のSilc-Pigシリコーン不透明染料と混合)のフィルムが、封止プロセスのための上部層及び底部層として供された。両層の硬化は、オーブン中で100℃で20分間加熱することを含んだ。CO2レーザー(ULS)を用いた切断プロセスにより、底部層上のECG電極及び上部層上の磁石のための開口部が画定された。シリコーン系接着剤(3M 96042)が、電子機器を最下層に接着した。このステップ中の蛇行の予備圧縮により、高いレベルの伸縮性が確保され、それに伴って曲げ加工性が向上した。100℃で20分間硬化させたシリコーンゲル(Ecoflex、Smooth-On)は、電子機器の下(中央部分)及び上(全体の被覆)の両方に、柔らかな、歪みを遮断するインターフェース層を提供した。Silbioneのオーバーレイヤーを接着することによって封止プロセスが完成した。ドロップキャスティング技術により、電源用の様々なモジュールの上にSilbioneのコーティングを形成した。
この処方は、n-ヘキサン(100mL)を含有する200mL丸底フラスコ中の15.0gのシリコーンプレポリマー(Sylgard 184ベース)への4.5gのカーボンブラックの添加を含み、室温で10分間、撹拌棒で激しく撹拌した。1mLのヘキサンで予め希釈した1.5gのシリコーン硬化剤(Sylgard 184硬化剤)を添加し、2~3分間連続攪拌して重合を誘導した。40℃での回転蒸発により、溶媒の迅速除去と重合体の脱ガスを同時に行い、滑らかなペーストを得た。モールドリリーススプレー(Ease Release200、Mann Release Technologies)で被覆されたレベルガイドを含むスライドガラス上に平坦なエッジで広げられた未硬化のCB-PDMSは、70℃のオーブン中で一晩硬化後、CB-PDMSの薄い固体フィルム(厚さ250μm)を生じた。胸部ユニットの底部表面上のECG電極のための開口部の全てのエッジに沿って2mm大きい横方向形状にCO2レーザーで切断された電極パッドは、結合のための重複領域を提供した。両方のエラストマー表面をコロナガン(BD-20A High Frequency Generator, Electro-Technic Products,Inc.)で40秒間処理し、直ちに続いて圧力誘導積層(15秒)し、オーブン中で70℃で一晩硬化させると、優れた接着性が得られた。両面導電性テープ(3M 9719)が、CB-PDMSパッドを可撓性プリント回路基板上のAu電極に結合した。
電子部品を乾燥剤(Drierite)(n=4)で置き換えたプラットフォームを使用した浸透テスト。試験では、1×DPBS(Dulbecco's Phosphate Buffered Saline)に37℃で連続浸漬した後の毎日の重量測定を行った。CB-PDMSとSilbioneとの間のシールとは対照的に、上部と底部のSilbione層の間の外周シールの部分的剥離から、約19~28日でデバイス重量の急速な増加(24時間で>1000mg)が続いた。機能的胸部ユニットを70℃で1×DPBSに連続的に浸漬した追加試験では、18日間にわたって毎日評価される安定した動作が実証された。
時間同期の特性評価は、2チャネル関数発生器を使用して、2つの間の制御された時間遅延を有する1対の周期的信号(1秒間に分離された20ms、3.5Vの平方パルス)を提供した。一方のチャネルをECGモジュールに接続し、他方をPPGモジュールの上部に配置された赤色LEDに接続すると、同期が1ms未満の平均遅延及び3.6msの標準偏差に対して検証されるデータが得られた。
試験は、モジュール式一次バッテリを備えた胸部ユニットとHeidolph Tuttnauer 3545E Autoclave Sterilizer(Electronic Model AE-K)に焦点を当てた。このプロセスには、121℃までの温度上昇、15分の滅菌時間、及び60分の乾燥時間が含まれ、バッテリを取り外したデバイスを使用して実行された。滅菌前後の機能試験では、性能に変化はなかった。
温度センサの精度の測定は、水浴に浸漬し、42℃に加熱した後、室温まで冷却し、同時に基準温度計(FisherbrandTM 13202376、Fisher Scientific)を標準として測定した。
本研究プロトコルはAnn&Robert H.Lurie Children's Hospital of Chicago及びNorthwestern University's Institutional Review Board(STU00202449)によって承認され、ClinicalTrials.gov(NCT02865070)に登録された。参加者全員に対して少なくとも1人の親からインフォームドコンセントを得た後、訓練を受けた研究スタッフが実験用センサを胸部及び四肢(足又は手)に装着した。センサは、既存のゴールドスタンダードの監視装置を妨害しないように配置された。センサの配置前又はセンサの取り外し時に皮膚の準備は行われなかった。このプロトコルにより、最大24時間の収集時間が可能になった。しかしながら、医療手順(例えば、手術)又は画像化は、センサの除去を必要とした。センサを取り外した後、有資格の皮膚科医が、刺激、発赤、又はただれの証拠がないか、下にある皮膚を評価した。データは、両親及び臨床スタッフから見えないところに置かれたタブレットPC(Surface Pro4、Microsoft)上へ送信され、収集され、さらなるデータ分析のために保存された。Northwestern Prentice Women's Hospital及びLurie Children's Hospitalの全被験者が新生児集中治療室及び小児集中治療室に入院し、在胎週数にかかわらず適格であった。
KMC解析は、100Hzのサンプリングレートで捕捉された加速度計測定値に依存していた。較正では、デバイスのx、y、及びz軸を重力ベクトルに位置合わせした。信号処理には0.1Hzでのカットオフ周波数を有するButterworth低域フィルタ(3次)を使用した。単純な三角関数は、加速度値からの向き角度を画定する。三次元でプロットされた結果は、手動で記録された体位と相関していた。1~10Hzの間のButterworth帯域通過フィルタ(3次)を通して加速度信号を処理し、続いてx、y及びz軸に沿った加速度値の二乗平均平方根を計算することにより、新生児活動レベルのメトリックが得られ、毎秒決定された。

前記PWVから前記哺乳動物対象の前記血圧Pを決定するステップであって、P=αPWV2+β、並びにα及びβは、前記哺乳動物対象の動脈形状及び動脈材料特性に依存して経験的に決定される定数である、ステップと;によって測定される、請求項7に記載の装置。 - 5kPaから20kPaの間の血圧範囲において、
0.13kPa×s2/m2≦α≦0.23kPa×s2/m2;かつ
2.2kPa≦β≦3.2kPa、である、請求項8に記載の装置。 - 哺乳動物対象の生理学的パラメータを非侵襲的に測定する方法であって:
前記哺乳動物対象上の複数のセンサシステムを利用するステップであって、前記センサシステムは、時間同期され、互いに無線で双方向に通信し、前記センサシステムの各々は、前記生理学的パラメータのうちの1つを監視するための少なくとも1つのセンサを含む、ステップと;
前記センサシステムによって、前記哺乳動物対象の前記生理学的パラメータを測定するステップと;
前記センサシステムに遠隔通信可能に接続されたマイクロコントローラにおいて、前記哺乳動物対象の前記生理学的パラメータを受信するステップと;
前記マイクロコントローラにおいて、前記哺乳動物対象の前記生理学的パラメータを表示するステップと;を含み、
前記複数のセンサシステムの各々は:
複数の電子部品、及び、異なる電子部品に電気的に接続された複数の可撓性かつ伸縮性の相互接続部と;
前記哺乳動物対象に取り付けられた、組織に面する表面及び環境に面する表面を形成するために、前記電子部品及び前記可撓性かつ伸縮性の相互接続部を少なくとも部分的に囲むエラストマー封止層と;を含む表皮電子システム(EES)であり、
前記複数のセンサシステムは:
前記哺乳動物対象の胴体領域に配置された第1のEESと;
前記哺乳動物対象の四肢領域に配置された第2のEESと;を含み、
前記第1のEESは、心電図(ECG)EESであり、ECG生成のために互いに空間的に離れた少なくとも2つの電極を含む;及び/又は
前記第2のEESは、フォトプレチスモグラフィ(PPG)EESであり、センサのフットプリント内に配置された光源及び光検出器を含むPPGセンサを含む;方法。 - 前記センサシステムは:
前記哺乳動物対象の胴体領域に配置された第1のセンサシステムであって、慣性運動センサシステム又は加速度計システムである第1のセンサシステムと;
前記哺乳動物対象の四肢領域に配置された第2のセンサシステムであって、フォトプレチスモグラフィ(PPG)表皮電子システム(EES)である第2のセンサシステムと、を含む、請求項10に記載の方法。 - 前記哺乳動物対象の前記生理学的パラメータが、心拍数、心拍変動、心音、血圧、胸壁変位、筋電図、心電図、血液酸素化、呼吸数、呼吸努力、呼吸周期、1回換気量、咳、いびき、くしゃみ、咳払い、喘鳴、無呼吸、低呼吸、身体活動、体幹位置、末梢四肢位置、引っ掻き、発声、擦り、歩行、睡眠の質、睡眠時間、睡眠時覚醒時間、皮膚温度、中核体温、及びそれらの組み合わせのうちの1つ以上を含む、請求項10または11に記載の方法。
- 前記血圧が:
前記哺乳動物対象の第1の位置に配置された第1のセンサと、前記哺乳動物対象の第2の位置に配置された第2のセンサとの出力信号を受信する受信ステップと;
前記出力信号を処理して、前記第1のセンサによる第1の信号の検出と前記第2のセンサによる第2の信号の検出との間の時間遅延Δtとして、パルス到達時間(PAT)を決定するステップと;
前記PATと、前記第1の位置と前記第2の位置との間のパルス到達距離Lとに基づいて、脈波伝播速度(PWV)を決定するステップであって、
前記PWVから前記哺乳動物対象の前記血圧Pを決定するステップであって、P=αPWV2+β、並びにα及びβは、前記哺乳動物対象の動脈形状及び動脈材料特性に依存して経験的に決定される定数である、ステップと;によって測定される、請求項12に記載の方法。 - 5kPaから20kPaの間の血圧範囲において、
0.13kPa×s2/m2≦α≦0.23kPa×s2/m2;かつ
2.2kPa≦β≦3.2kPa、である、請求項13に記載の方法。
