Description
心臓弁置換術は、生体本来の心臓弁が損傷した状態、あるいは機能喪失や機能不全の状態にある場合に必要とされる処置である。心臓では、心臓弁の両側の圧力差に応じて弁が開閉することにより、血液が一方向に流れるように維持されている。哺乳動物の心臓は、4つの部屋、すなわち血液が貯留される部屋である2つの心房と、血液を送り出す部屋である2つの心室で構成されている。哺乳動物の心臓には、4つの心臓弁が存在し、これらの弁によって、血液が心臓内を通常一方向にのみに流れるようになっており、各心臓弁は、弁の両側の血圧の差に応じて開閉する。
Claims (14)
- 機能喪失または機能不全の状態にある生体本来の心臓弁を補助または置換するための、圧縮状態から拡張状態へと拡張可能な人工心臓弁(10)であって、
近位端(13)と、遠位端(14)と、近位端(13)から遠位端(14)へと延びる長軸(15)とを有する、拡張可能で概して管状のステント支持体(12);
外面(21)と、内面(22)と、複数の弁尖(23)と、長軸(24)と、外周(25)とを有する弁要素(20);および
複数の封止要素(30)
を含み、
前記管状のステント支持体(12)が、内面(16)と外面(17)とを有すること;
前記封止要素(30)が、それぞれ間隔を空けて、互いに接することなく前記弁要素(20)の外面(21)に周方向に取り付けられており、拡張時には、各封止要素(30)が、該弁要素(20)の外面(21)から径方向に突出する唇状要素(31)を形成すること;ならびに
前記封止要素(30)が取り付けられている前記弁要素(20)が、前記ステント支持体(12)の内面(16)に取り付けられていること
を特徴とする人工心臓弁(10)。 - 前記封止要素(30)が、ストリップ(32)の形態で、長さ(33)と幅(34)を有し、かつ該封止要素(30)が、弁要素(20)の長軸(24)に垂直な面内で、その幅(34)より弁要素(20)の外周(25)に沿う方向に長く延びて、該弁要素(20)の外面(21)に設けられている、請求項1に記載の人工心臓弁(10)。
- 前記封止要素(30)が、前記弁要素(20)の外周(25)に沿って、それぞれ規則的または不規則的に間隔を空けて配置されている、請求項1または2に記載の人工心臓弁(10)。
- 前記封止要素(30)が、前記弁要素(20)の長軸(24)に沿って、それぞれ規則的または不規則的に間隔を空けて配置されている、請求項1から3のいずれかに記載の人工心臓弁(10)。
- 少なくとも2つの前記封止要素(30)の長さ(33)および/または幅(34)が同一または異なる、請求項1から4のいずれかに記載の人工心臓弁(10)。
- 前記封止要素(30)が、その長さ方向が前記弁要素(20)の外周(25)に沿うように、該弁要素(20)の外面(21)に取り付けられている、請求項1から5のいずれかに記載の人工心臓弁(10)。
- 前記複数の封止要素(30)が、前記弁要素(20)の外面(21)に縫い付けられている、請求項1から6のいずれかに記載の人工心臓弁(10)。
- 前記ステント支持体(12)が、相互に連結された実質的に菱形状の開放セル構造(18)で構成された隣接する複数の列を含み、拡張時には、前記唇状要素(31)が、少なくとも部分的に該セル構造(18)から突出する、請求項1から7のいずれかに記載の人工心臓弁(10)。
- 2~20個の封止要素(30)を含む、請求項1から8のいずれかに記載の人工心臓弁(10)。
- 前記複数の封止要素(30)が、互いに接していない複数の唇状要素(31)から構成される列が複数形成されるように、前記弁要素(20)の外面(21)に配置されており、各列の封止要素(30)が、該弁要素(30)の外周(25)に沿って、それぞれ距離Rの間隔を空けて配置されており、該列同士が、該弁要素(20)の長軸(24)に沿って、それぞれ距離Sの間隔を空けて配置されている、請求項1から9のいずれかに記載の人工心臓弁(10)。
- 前記弁要素(20)の外面(21)に配置された封止要素(30)から構成される第1の列(30a)および第2の列(30b)を含み、該第1の列(30a)および該第2の列(30b)の各列の封止要素(30)が、該弁要素(20)の外周(25)に沿って、それぞれ距離Rの間隔を空けて配置されており、該第1の列(30a)の封止要素(30)が、該弁要素(20)の長軸(24)に沿って、該第2の列(30b)の封止要素(30)からオフセットされた位置に配置されている、請求項1から10のいずれかに記載の人工心臓弁(10)。
- 前記弁要素(20)の外面(21)に配置された封止要素(30)から構成される第1の列(30c)、第2の列(30d)、および第3の列(30e)を含み、該第1の列(30c)、該第2の列(30d)、および該第3の列(30e)の各列の封止要素(30)が、該弁要素(20)の外周(25)に沿って、それぞれ距離Aの間隔を空けて配置されており、該第1の列(30c)の封止要素(30)が、該弁要素(20)の長軸(24)に沿って、該第2の列(30d)および該第3の列(30e)の封止要素(30)からオフセットされた位置に配置されている、請求項1から11のいずれかに記載の人工心臓弁(10)。
- 前記封止要素(30)が、生体組織もしくは生体適合性を有する合成材料を含むか、または生体組織もしくは生体適合性を有する合成材料からなる、請求項1から12のいずれかに記載の人工心臓弁(10)。
- 前記封止要素(30)が、心膜組織、ポリエステル、ポリウレタン、ポリスチレン、ポリテトラフルオロエチレン、超高分子量ポリエチレン(UHMPE)、およびこれらの混合物から選択される組織もしくは材料を含むか、該組織もしくは材料からなる、請求項1から13のいずれかに記載の人工心臓弁(10)。
